It is acknowledged that health technology assessment (HTA) is an inherently value-based activity that makes use of normative reasoning alongside empirical evidence. But the language used to conceptualise and articulate HTA's normative aspects is demonstrably unnuanced, imprecise, and inconsistently employed, undermining transparency and preventing proper scrutiny of the rationales on which decisions are based. This paper – developed through a cross-disciplinary collaboration of 24 researchers with expertise in healthcare priority-setting – seeks to address this problem by offering a clear definition of key terms and distinguishing between the types of normative commitment invoked during HTA, thus providing a novel conceptual framework for the articulation of reasoning. Through application to a hypothetical case, it is illustrated how this framework can operate as a practical tool through which HTA practitioners and policymakers can enhance the transparency and coherence of their decision-making, while enabling others to hold them more easily to account. The framework is offered as a starting point for further discussion amongst those with a desire to enhance the legitimacy and fairness of HTA by facilitating practical public reasoning, in which decisions are made on behalf of the public, in public view, through a chain of reasoning that withstands ethical scrutiny.
Status of primary and secondary mental healthcare of people with severe mental illness: an epidemiological study from the UK PARTNERS2 programme

Background
There is global interest in the reconfiguration of community mental health services, including primary care, to improve clinical and cost effectiveness.
AimsThis study seeks to describe patterns of service use, continuity of care, health risks, physical healthcare monitoring and the balance between primary and secondary mental healthcare for people with severe mental illness in receipt of secondary mental healthcare in the UK.
MethodWe conducted an epidemiological medical records review in three UK sites. We identified 297 cases randomly selected from the three participating mental health services. Data were manually extracted from electronic patient medical records from both secondary and primary care, for a 2-year period (2012–2014). Continuous data were summarised by mean and s.d. or median and interquartile range (IQR). Categorical data were summarised as percentages.
ResultsThe majority of care was from secondary care practitioners: of the 18 210 direct contacts recorded, 76% were from secondary care (median, 36.5; IQR, 14–68) and 24% were from primary care (median, 10; IQR, 5–20). There was evidence of poor longitudinal continuity: in primary care, 31% of people had poor longitudinal continuity (Modified Modified Continuity Index ≤0.5), and 43% had a single named care coordinator in secondary care services over the 2 years.
ConclusionsThe study indicates scope for improvement in supporting mental health service delivery in primary care. Greater knowledge of how care is organised presents an opportunity to ensure some rebalancing of the care that all people with severe mental illness receive, when they need it. A future publication will examine differences between the three sites that participated in this study.

Summary
BD is a 34-year-old semi-professional rugby union player and a part-time schoolteacher. Three years ago she suffered three concussions during one season and required a prolonged period of rest. Following an extended RTP protocol, which involved a month away from any rugby contact and a gradual increase in physical activity, she was integrated back into the team environment although she always thought she never reached her previous level of performance.

Buried surface hoar and near-surface faceted crystals are known to lead to deadly avalanches. Over the course of three winter seasons a field investigation detailing the environmental conditions leading to the formation of these crystals was performed. Weather stations on north- and south-facing aspects were established. The weather data were accompanied by detailed daily observations and grain-scale photographs of the snow surface. During the three seasons, 35 surface hoar and 47 near-surface facets events were recorded. The mean weather conditions for the entire dataset (all three seasons and both stations) were compared to the nights when surface hoar formed. The comparison yielded five parameters that were statistically linked to the formation of surface hoar: incoming longwave radiation, snow surface temperature, wind velocity, relative humidity and the air/snow temperature difference. A similar comparison between the daytime mean values for all days with near-surface facet events revealed three parameters with statistically significant differences. Thus, these parameters (short- and longwave radiation and relative humidity) could be statistically linked to facet formation. This research also suggests that environmental conditions in the daytime hours before and after surface hoar formation are statistically similar to the conditions causing near-surface facet formation.

Background: Behavioral symptoms of dementia are common among residents in mainstream aged care settings, and have a substantial impact on residents and professional caregivers. This study evaluated the impact of individualized psychosocial interventions for behavioral symptoms through a small preliminary study.
Method: Interventions were delivered to a patient group of 31 psychogeriatric aged care residents who presented with behavioral symptoms of dementia that had failed to respond to pharmacological treatment approaches. Outcome data on severity of behaviors, health service utilization and staff burden of care were collected.
Results: A modest but significant reduction in staff ratings of the severity of aggressive and verbally agitated behavioral symptoms was found, with an associated reduction in their perceptions of the burden of caring for these patients. Reduced behavioral disturbance was associated with a reduction in the requirement for primary care consultations, and all participants were able to continue to reside in mainstream aged care facilities, despite an increase in the severity of dementia.
Conclusions: This study supported the use of individualized psychological strategies for behavioral symptoms at all stages of dementia. Methodological limitations of this preliminary study are discussed.
