We report the discovery of an ancient forest bed near Stanley, on the Falkland Islands, the second such ancient deposit identified on the South Atlantic island archipelago that is today marked by the absence of native tree species. Fossil pollen, spores and wood fragments preserved in this buried deposit at Tussac House show that the source vegetation was characterized by a floristically diverse rainforest dominated by Nothofagus-Podocarpaceae communities, similar to cool temperate Nothofagus forests/woodlands and Magellanic evergreen Nothofagus rainforests. The age limit of the deposit is inferred from the stratigraphic distribution of fossil pollen species transported by wind, birds or ocean currents from southern Patagonia, as well as similar vegetation types observed across the broader region. The deposit is suggested to be between Late Oligocene and Early Miocene, making it slightly older than the previously analysed Neogene West Point Island forest bed (200 km west of Tussac House). The combined evidence adds to our current knowledge of the role of climate change and transoceanic dispersal of plant propagules in shaping high-latitude ecosystems in the Southern Hemisphere during the late Palaeogene and Neogene.
Patients with persistent post-concussion symptoms (PPCS) experience prolonged recovery (e.g., headache, fatigue, or dizziness) lasting >2 months post injury. These symptoms are thought to be maintained by several biopsychosocial factors including dysregulated stress responses, such as pain catastrophizing, that may drive behavioral avoidance and contribute to mood symptoms and cognitive difficulties. Conditions with similar symptomatology to PPCS (e.g., anxiety disorders, somatosensory disorders, chronic pain, etc.) also exhibit maladaptive thought patterns like pain catastrophizing as well as decrements in certain aspects of cognitive performance; however little is known about how pain catastrophizing might relate to neuropsychological performance in youth with PPCS. Therefore, the purpose of this study was to examine the relationship between pain catastrophizing and neuropsychological performance in youth participants with PPCS.
Participants and Methods:
A prospective case-control study design was used to examine 29 participants between the ages of 13 to 23. Participants were divided into two groups: 1) patients with PPCS (2-16 months post-injury; n = 15) and 2) age-matched, non-injured controls (n = 14). Participants completed the Pain Catastrophizing Scale (PCS) to determine degree of catastrophic thinking related to pain experience and the Beck Depression Inventory (BDI). Neuropsychological performance was assessed using the Repeatable Battery for the Assessment of Neuropsychological Status (RBANS) and a modified version of the Paced Auditory Serial Addition Test (PASAT) where performance was evaluated by total correct and error type (i.e., commission and omission) across 5 trials. ANCOVA was used to compare group differences in pain catastrophizing and neuropsychological tests scores while controlling for age and linear regressions examined the relationship between PCS total score and each neuropsychological test score while controlling for level of depression.
Results:
Overall, the PPCS group reported significantly higher levels of pain catastrophizing on the PCS compared to the control group (p < 0.01). For neuropsychological performance, the PPCS group scored significantly lower than the control group on List Learning (p < 0.01), Semantic Fluency (p < 0.05), and List Recall (p < 0.01) on the RBANS and made significantly higher omission errors (but not commission) on the PASAT(p <.01). Higher pain catastrophizing was also associated with poorer neuropsychological performance on the exact same tasks the PPCS group performed worse than controls. There was no significant interaction by group in the impact of PCS scores on neurocognitive performance.
Conclusions:
Compared to controls, youth PPCS patients reported higher levels of pain catastrophizing. Additionally, pain catastrophizing was associated with poorer neuropsychological performance. These findings suggest that increased pain catastrophizing after head injury could contribute to poorer cognitive performance in youth. As such, interventions that target maladaptive cognitive coping styles like pain catastrophizing may be especially helpful for patients with PPCS.
Disaster planning and preparedness for a burn mass casualty incident (BMCI) must consider the needs of those who will be directly involved and support the response to such an event. An aspect of developing a more comprehensive statewide burn disaster program included meeting (regionally) with healthcare coalitions (HCC) to identify gaps in care and deficiencies.
Method:
Regularly scheduled (quarterly) HCC meetings are held around the state linking stakeholders representing local hospitals, health departments, emergency medical services (EMS) agencies, and other interested parties. We were able to use the HCCs regional meetings to serve as a platform for conducting focus group research to identify gaps specific to a BMCI and to inform strategy development for a statewide approach. Additionally, we held engagement meetings with state emergency response network (a state agency that coordinates the movement of ambulances to appropriate destinations) and the Burn Medical Directors findings were vetted from the focus groups.
Results:
One of the deficiencies identified, included a lack of burn-specific wound care dressings that could support the initial response. Relying on this same process, a consensus was attained for equipment types and quantities, including a kit for storage. Furthermore, a maintenance, supply replacement, and delivery to the scene processes were developed for these kits of supplies that could augment a BMCI response.
Conclusion:
Focus group feedback reminded us that outside of the world of burn care, many report an infrequent opportunity to provide care for patients with burn injuries. Several types of burn-specific dressings can be expensive, and with the occurrence being infrequent. EMS agencies and rural hospitals alike reported that it was unlikely their agency/hospital would have more than a minimal stock of burn injury supplies. Developing supply caches that can be quickly mobilized and deployed to the impacted area was one of the deficiencies we addressed.
In the USA, traumatic injuries are the leading cause of death before age 45 and have significantly lower mortality if treated in a verified trauma center. Burn injuries are included in trauma statistics and represent 1.1 million injured people annually seeking medical assistance. Routing of burn injuries to ABA-recognized burn centers has yet to be assessed as it has in trauma injury. Our goal was to examine the impact of prehospital routing of burn injuries on hospital length of stay, mortality, and potential costs-of-care through a statewide care coordination center.
Method:
Our study is a retrospective statewide analysis of burn injuries from 01/01/2017 thru 12/31/2019 using the Louisiana Hospital Inpatient Discharge Database. Routing of burn patients was implemented in 2018 using the ABA burn referral criteria. Data included: total admissions with primary burn diagnosis, region, discharge status, length of stay, and raw mortality by region and state. Descriptive and comparative statistics were performed to assess the impact of routing burn-injured patients. Cost analysis was performed using Louisiana Medicaid per diem rates from 2021 at $1,907.92/day.
Results:
1,288 patients were treated in Louisiana during the study period, with 855 post-routing and 433 pre-routing. The mean length of stay was reduced from 11.84 days in 2017 to 8.82 days in 2018 (p value=0.0988), with a potential savings of 761 inpatient care days or $2.17 million. Overall mortality across the state was unchanged except in the highest volume region, where it dropped from 7.9% in 2017 to 3.6% in 2019 (54%).
Conclusion:
Burn injuries are a time-sensitive trauma. This study marks the first analysis pre and post-implementation of routing for burn injuries by a statewide care coordination center. Our study demonstrates improvement in length of stay and mortality but a continued need to examine other contributing factors, such as injury severity and concomitant trauma.
We describe an ultra-wide-bandwidth, low-frequency receiver recently installed on the Parkes radio telescope. The receiver system provides continuous frequency coverage from 704 to 4032 MHz. For much of the band (
${\sim}60\%$
), the system temperature is approximately 22 K and the receiver system remains in a linear regime even in the presence of strong mobile phone transmissions. We discuss the scientific and technical aspects of the new receiver, including its astronomical objectives, as well as the feed, receiver, digitiser, and signal processor design. We describe the pipeline routines that form the archive-ready data products and how those data files can be accessed from the archives. The system performance is quantified, including the system noise and linearity, beam shape, antenna efficiency, polarisation calibration, and timing stability.
The Folkton ‘Drums’ constitute three of the most remarkable decorated objects from Neolithic Britain. New analysis using Reflectance Transformation Imaging and photogrammetry has revealed evidence for previously unrecorded motifs, erasure and reworking. Hence these chalk drums were not decorated according to a single, pre-ordained scheme, but were successively carved and recarved over time. Such practices may have been widespread in the making of artefacts in Neolithic Britain. The study of these drums also demonstrates the ability of these new techniques not only to record visible motifs, but to document erased and reworked motifs clearly.
Previous studies have suggested that antidepressant treatment of depression may potentiate dopamine transmission through increased sensitivity of postsynaptic D2 receptors.
Method:
D2 receptor function was assessed in 24 patients with major depression before and 16 patients after 16 weeks of treatment with cognitive behavioural therapy (CBT) using a challenge with a selective D2 antagonist, sulpiride. Four hundred milligrams of sulpiride was administered orally on two test days and response was measured by the change in prolactin levels and changes in self-rating scale measures of mood, anxiety and pleasure.
Results:
The prolactin response to sulpiride (as measured by the maximum prolactin level) was significantly increased after CBT (z = −2.792, p = 0.005). Sulpiride resulted in an improvement on mood ratings on both test days, but after CBT, this effect was significantly diminished as measured by the Profile of Mood States score (t = −2.27, p = 0.038).
Conclusions:
After 16 weeks of CBT, we detected an enhanced prolactin response to sulpiride, suggesting an increased sensitivity of D2 receptor functioning.
The present study aimed to measure tissue protein synthesis in sea bream fed isonitrogenous diets that contained 63, 55 and 50 % fishmeal; in the latter two diets, 16 and 27 % of the fishmeal protein was replaced with plant protein. Over a 35 d period, there were no differences in feed intake, growth or feed efficiency among the three diets. Protein metabolism was then measured in the liver and white muscle tissue as rates of protein synthesis and as the capacity for protein synthesis before feeding (0 h) and at different times after feeding (4–48 h). Diet did not have a significant effect on protein synthesis or on the capacity for protein synthesis in either tissue. The capacity for protein synthesis was not affected by time after feeding, and overall mean values were 81·02 (se 1·68) and 4·07 (se 0·94) mg RNA/g protein for the liver and white muscle, respectively. Liver and white muscle fractional rates of protein synthesis were significantly higher at 4–8 h, intermediate at 12 h and were not different among pre-feeding (0 h), 24 and 48 h. Overall, the indices of protein metabolism measured at various times over 48 h following feeding were closely aligned with measurements of feeding, growth and growth efficiency established over a longer time scale.
Vegetable oils (VO) have become the predominant substitute for fish oil (FO) in aquafeeds; however, the resultant lower content of n-3 long-chain ( ≥ C20) PUFA (n-3 LC-PUFA) in fish has put their use under scrutiny. The need to investigate new oil sources exists. The present study tested the hypothesis that in Atlantic salmon (Salmo salar L.), a high intake of stearidonic acid (SDA) from Echium oil (EO) would result in increased n-3 LC-PUFA biosynthesis due to a lower requirement for Δ6 desaturase. Comparisons were made with fish fed on diets containing rapeseed oil (RO) and FO in freshwater for 112 d followed by 96 d in seawater. EO fish had higher whole-carcass SDA and eicosatetraenoic acid (ETA) in freshwater and prolonged feeding on the EO diet in seawater resulted in higher SDA, ETA, EPA and docosapentaenoic acid (DPA) compared with RO fish. Fatty acid mass balance of freshwater fish indicated higher biosynthesis of ETA and EPA in EO fish compared with fish fed on the other diets and a twofold increase in n-3 LC-PUFA synthesis compared with RO fish. In seawater, n-3 biosynthetic activity was low, with higher biosynthesis of ETA in EO fish and appearance of all desaturated and elongated products along the n-3 pathway. SDA-enriched VO are more suitable substitutes than conventional VO from a human consumer perspective due to the resulting higher SDA content, higher total n-3 and improved n-3:n-6 ratio obtained in fish, although both VO were not as effective as FO in maintaining EPA and DHA content in Atlantic salmon.
The Canadian Emergency Department Triage and Acuity Scale (CTAS) is a 5-level triage tool used to determine the priority by which patients should be treated in Canadian emergency departments (EDs). To determine emergency physician (EP) workload and staffing needs, many hospitals in Ontario use a case-mix formula based solely on patient volume at each triage level. The purpose of our study was to describe the distribution of EP time by activity during a shift in order to estimate the amount of time required by an EP to assess and treat patients in each triage category and to determine the variability in the distribution of CTAS scoring between hospital sites.
Methods:
Research assistants directly observed EPs for 592 shifts and electronically recorded their activities on a moment-by-moment basis. The duration of all activities associated with a given patient were summed to derive a directly observed estimate of the amount of EP time required to treat the patient.
Results:
We observed treatment times for 11 716 patients in 11 hospital-based EDs. The mean time for physicians to treat patients was 73.6 minutes (95% confidence interval [CI] 63.6–83.7) for CTAS level 1, 38.9 minutes (95% CI 36.0–41.8) for CTAS-2, 26.3 minutes (95% CI 25.4–27.2) for CTAS-3, 15.0 minutes (95% CI 14.6–15.4) for CTAS-4 and 10.9 minutes (95% CI 10.1–11.6) for CTAS-5. Physician time related to patient care activities accounted for 84.2% of physicians' ED shifts.
Conclusion:
In our study, EPs had very limited downtime. There was significant variability in the distribution of CTAS scores between sites and also marked variation in EP time related to each triage category. This brings into question the appropriateness of using CTAS alone to determine physician staffing levels in EDs.
The anterior cingulate cortex is frequently implicated in the pathophysiology of bipolar disorder, but magnetic resonance imaging (MRI) studies have reported variable findings owing to a reliance on patient samples with chronic illness and to limited appreciation of the region's heterogeneity.
Aims
To characterise anterior cingulate cortex abnormalities in patients with bipolar disorder experiencing their first episode of psychosis while accounting for regional anatomical variability.
Method
Grey matter volume, surface area and cortical thickness were measured in six anterior cingulate cortex subregions per hemisphere using MRI scans acquired from 26 patients with bipolar I disorder experiencing first-episode psychosis and 26 healthy controls matched for age, gender and regional morphological variability.
Results
Relative to controls, male patients displayed increased thickness in the right subcallosal limbic anterior cingulate cortex. No significant differences were identified in females for grey matter volume or surface area measures. The findings were not attributable to medication effects.
Conclusions
These data suggest that first-episode psychosis in bipolar disorder is associated with a gender-specific, right-lateralised thickness increase in anterior cingulate cortex subregions known to play a role in regulating physiological stress responses.
The present review examines renewable sources of oils with n-3 long-chain ( ≥ C20) PUFA (n-3 LC-PUFA) as alternatives to oil from wild-caught fish in aquafeeds. Due to the increased demand for and price of wild-caught marine sources of n-3 LC-PUFA-rich oil, their effective and sustainable replacement in aquafeeds is an industry priority, especially because dietary n-3 LC-PUFA from eating fish are known to have health benefits in human beings. The benefits and challenges involved in changing dietary oil in aquaculture are highlighted and four major potential sources of n-3 LC-PUFA for aquafeeds, other than fish oil, are compared. These sources of oil, which contain n-3 LC-PUFA, specifically EPA (20 : 5n-3) and DHA (22 : 6n-3) or precursors to these key essential fatty acids, are: (1) other marine sources of oil; (2) vegetable oils that contain biosynthetic precursors, such as stearidonic acid, which may be used by fish to produce n-3 LC-PUFA; (3) single-cell oil sources of n-3 LC-PUFA; (4) vegetable oils derived from oil-seed crops that have undergone genetic modification to contain n-3 LC-PUFA. The review focuses on Atlantic salmon (Salmo salar L.), because it is the main intensively cultured finfish species and it both uses and stores large amounts of oil, in particular n-3 LC-PUFA, in the flesh.
We have shown recently that the degradation of AC electroluminescence (EL) in ZnS:Cu,Cl and similar phosphors can be significantly reversed by a short anneal near 200°C. To better understand the degradation/rejuvenation processes, we investigated EL degradation and rejuvenation under different conditions. To probe for changes in the local atomic structure about the Cu sites, we collected Extended X-ray Absorption Fine Structure (EXAFS) data at the Cu K-edge EXAFS data for several as-made, thermally degraded (240°C anneal), and EL degraded powders.
Measurement of the ion energy distributions (IED's) of N2+ ions produced in an inductively coupled plasma (ICP) source revealed that adding argon to the nitrogen gas before excitation in the source increased the nitrogen ion current. Adding helium resulted in an increase of the most probable ion energy, and a higher-energy component of the N2+ IED at about 20 eV. Preliminary calculations of the electron energy distributions (EED's) correlate well to the IED's for both inert gas additions. Experiments were performed to determine the effect of these modified energy distributions of the reactive species on titanium nitride thin film growth rates, microstructure and mechanical properties. Deposition experiments were conducted using the ICP source and an unbalanced magnetron with a titanium cathode. Results showed that the argon inert gas addition resulted in increased deposition rate and reduced 3-D features. Film roughness was decreased to 1.61 nm from 2.05 nm when helium was mixed with the nitrogen. Mixing both argon and helium with the nitrogen yielded combined effects of deposition rate increase, reduced 3-D features and increased smoothness. The inert gas additions increased hardness of the TiN films from 17.3 GPa to 21.1 GPa with the argon addition and to 25.0 GPa with the helium addition
