Finding ways of improving the health and wellbeing of older men is an important challenge for public health. This review aimed to assess evidence for the effects of Men's Sheds and other gendered social activities on the health and wellbeing of older men, and to consider their effective components and theoretical frameworks. A scoping review using standardised search criteria and terms identified 31 relevant papers of sufficient quality for inclusion. Analysis was informed by guidance on interpretative and narrative synthesis and a quality assessment tool designed for reviewing disparate data from different disciplines and research paradigms applied. The review found some limited evidence that Men's Sheds and other gendered social activities may have impact on the mental health and wellbeing of older men, but little evidence of the impact on physical health. Qualitative data provided valuable insights into how and why complex psycho-social activities can affect participants, but there was a lack of longitudinal evidence drawing on validated health and wellbeing measures. Key components of successful interventions included accessibility, range of activities, local support and skilled co-ordination. A variety of theoretical frameworks were employed. As yet, there is no conclusive evidence that Men's Sheds and other gendered interventions confer health and wellbeing benefits on older men. Studies in this field to date are few and of variable quality. Larger and more robust mixed-methods studies, including randomised designs, are needed.

This article examines work-related stress and rehabilitation in the light of the introduction of Stress Management Standards in the UK in 2004. The Management Standards are outlined and the relevant literature on good practice in stress and rehabilitation is reviewed to provide a context for empirical research conducted with human resource practitioners in organisations. This view from the workplace provides insights on the issues faced, especially in the public services, when managing stress and rehabilitation. These include the role played by stress in people's personal life, the limitations of line management and the role of General Practitioners.

Pancreatic polypeptide (PP) is a gut hormone released from the pancreas in response to food ingestion and remains elevated for up to 6 h postprandially. Plasma levels are elevated in patients with pancreatic tumours. An intravenous infusion of PP has been reported to reduce food intake in man, suggesting that PP is a satiety hormone. We investigated whether a lower infusion rate of PP would induce significant alterations in energy intake. The study was randomised and double-blinded. Fourteen lean fasted volunteers (five men and nine women) received 90 min infusions of PP (5 pmol/kg per min) and saline on two separate days. The dose chosen was half that used in a previous human study which reported a decrease in appetite but at supra-physiological levels of PP. One hour after the end of the infusion, a buffet lunch was served and energy intake measured. PP infusion was associated with a significant 11 % reduction in energy intake compared with saline (2440 (se 200) v. 2730 (se 180) kJ; P < 0·05). Preprandial hunger as assessed by a visual analogue score was decreased in the PP-treated group compared to saline. These effects were achieved with plasma levels of PP within the pathophysiological range of pancreatic tumours.

The study investigated longitudinal change in cognitive function in 87 patients with Huntington's disease (HD), using a range of neuropsychological tests, which tap mental manipulative abilities, memory, and frontal executive skills. Over a 1-year period the largest changes were noted in letter fluency, object recall, and Stroop Test performance, whereas no changes were noted over more than 3 years on the modified Wisconsin Card Sorting Test. Contrary to expectation, greater change was evident over 1 year for tasks with low compared to high cognitive demands. The differential sensitivity of tasks was attributed in part to inherent characteristics of the tests themselves: their capacity to detect minor gradations of change and their vulnerability to practice effects. However, the greater change for relatively automatic, speed-based tasks with low cognitive demands was interpreted as reflecting the evolution of HD, with a greater magnitude of change occurring in basal ganglia than cortical function. One purpose of the study was to identify tasks sensitive to the progression of HD and hence most suitable for the evaluation of therapies. Despite reaching statistical significance by virtue of the large group size, numerical differences in test scores over 1 year were very small, suggesting that the use of such tests to evaluate change in individuals or small groups of subjects would be problematic. The data highlight the slow progression of HD, the limitations of standard cognitive tests in detecting change over short periods, and the need for therapeutic studies that encompass a relatively prolonged time frame. Dementia is not a disease. It is the neuropsychological deficit which results from chronic brain disease or encephalopathy. Dementias may be nonprogressive, for example resulting from brain trauma or cerebral hypoxia, or progressive, resulting from intrinsic, extrinsic or metabolic cerebral disorder. The progressive encephalopathies, in particular those resulting from intrinsic brain disease, form the subject of this review.
