Basal melt of ice shelves is not only an important part of Antarctica's ice sheet mass budget, but it is also the origin of platelet ice, one of the most distinctive types of sea ice. In many coastal Antarctic regions, ice crystals form and grow in supercooled plumes of Ice Shelf Water. They usually rise towards the surface, becoming trapped under an ice shelf as marine ice or forming a semi-consolidated layer, known as the sub-ice platelet layer, below an overlying sea ice cover. In the latter, sea ice growth consolidates loose crystals to form incorporated platelet ice. These phenomena have numerous and profound impacts on the physical properties, biological processes and biogeochemical cycles associated with Antarctic fast ice: platelet ice contributes to sea ice mass balance and may indicate the extent of ice-shelf basal melting. It can also host a highly productive and uniquely adapted ecosystem. This paper clarifies the terminology and reviews platelet ice formation, observational methods as well as the geographical and seasonal occurrence of this ice type. The physical properties and ecological implications are presented in a way understandable for physicists and biologists alike, thereby providing the background for much needed interdisciplinary research on this topic.
The article argues that the impact of law enforcement efforts against corruption deserves more scholarly attention. Drawing on a mixed-methods study from Malawi in southern Africa, where a large-scale law enforcement operation has been investigating and prosecuting those involved in a 2013 corruption scandal known as ‘Cashgate’, the article explores the potential for corruption deterrence from the perspective of government officials in the Malawi civil service. Malawi provides a challenging environment for deterrence due to limited state capacity, weak law enforcement agencies and widespread corruption. Nonetheless, the research findings show that Malawian government officials perceive prosecutions and convictions to deter corruption, both with regards to the law enforcement response to Cashgate specifically and law enforcement efforts in general. The findings from Malawi suggest that law enforcement and criminal justice have the potential to make an important contribution to anti-corruption strategies in Africa and the Global South at large.
In daily clinical practice, the smooth, timely and comprehensive transfer of information between care settings is important and reflects a cornerstone of high-quality patient care. The integration of nutritional information in the medical information transfer is currently not included in an evidence-based approach. It was, therefore, the aim of this study to develop a nutritional documentation tool (NDoc) on the basis of evidence and test it for its usability in daily clinical practice. Based on the results of a literature review, the authors collected core content using a modified Delphi survey from experts across Europe and included the information collected in a structured, NDoc. The subsequently developed tool included thirty items and was tested for its usability on a daily basis among primary care physicians and clinical physicians. The new NDoc can be introduced for use in any computer-based hospital information system regionally and adapted for worldwide use.
Stars with masses between ∼0.7 and 8 M⊙ end their lives as Planetary Nebulae (PNe). With the MegaCam at CFHT, we have carried out a survey of the central 16 sq. degrees of Andromeda (M31) reaching the outer disk and halo, using a narrow-band [OIII]5007 and a broad-band g filter. This survey extends previous PN samples both in uniform area coverage and depth. We identify ∼4000 PNe in M31, of which ∼3000 are new discoveries. We detect PNe down to ∼6 mag below the bright cut-off of the PN luminosity function (PNLF), ∼2 mag deeper than in previous works. We detect a steep rise in the number of PNe at ∼4.5 mag fainter than the bright cut-off. It persists as we go radially outwards and is steeper than that seen in the Magellanic clouds. We explore possible reasons for this rise, which give insights into the stellar population of M31.
To investigate whether the use of long-acting benzodiazepines, in individuals aged 65 and over is mediated by physical or psychological factors.
Background
Long-acting benzodiazepine consumption among older people has implications for mortality, morbidity and cost-effective prescribing. Two models explain benzodiazepine use in this age group, one linked to physical illness and disability and one to psychological factors.
Methods
Secondary analysis of baseline data from a study of 1059 community-dwelling non-disabled people aged 65 years and over recruited from three general practices in London. For this analysis, use of long-acting benzodiazepines was defined as any self-reported use of diazepam or nitrazepam in the last four weeks. Associations between demographic factors, health service use, and physical and psychological characteristics and benzodiazepine use were investigated.
Findings
The prevalence of benzodiazepine use in this sample was 3.3% (35/1059). In univariate analyses, benzodiazepine use was associated with female gender, low income, high consultation rates, physical factors (medication for arthritis or joint pain, polypharmacy, difficulties in instrumental activities of daily living, recent pain) and psychological factors (poor self-perceived health, social isolation, and symptoms of anxiety or agitation). In a multivariate logistic regression analysis only two factors retained statistically significant independent associations with benzodiazepine use: receiving only the state pension (OR=4.0, 95% CI: 1.70, 9.80) and pain in the past four weeks (OR=3.79, 95% CI: 1.36, 10.54).
Despite the significant impact of malnutrition in hospitalised patients, it is often not identified by clinical staff in daily practice. To improve nutritional support in hospitals, standardised routine nutritional screening is essential. The Graz Malnutrition Screening (GMS) tool was developed for the purpose of malnutrition risk screening in a large hospital setting involving different departments. It was the aim of the present study to validate the GMS against Nutritional Risk Screening (NRS) and Mini Nutritional Assessment-short form (MNA-sf) in a randomised blinded manner. A total of 404 randomly selected patients admitted to the internal, surgical and orthopaedic wards of the University Hospital Graz were screened in a blinded manner by different raters. Concurrent validity was determined by comparing the GMS with the NRS and in older patients (70+ years) with the MNA-sf additionally. According to GMS, 31·9 or 28·5 % of the admitted patients were categorised as at ‘risk of malnutrition’ (depending on the rater). According to the reference standard of NRS, 24·5 % of the patients suffered from malnutrition. Pearson’s r values of 0·78 compared with the NRS and 0·84 compared with the MNA showed strong positive correlations. Results of accuracy (0·85), sensitivity (0·94), specificity (0·77), positive predictive value (0·76) and negative predictive value (0·95) of GMS were also very high. Cohen’s κ for internal consistency of the GMS was 0·82. GMS proves to be a valid and reliable instrument for the detection of malnutrition in adult patients in acute-care hospitals.
All antipsychotic medications carry warnings of increased mortality for older adults, but little is known about comparative mortality risks between individual agents.
Aims
To estimate the comparative mortality risks of commonly prescribed antipsychotic agents in older people living in the community.
Method
A retrospective, claims-based cohort study was conducted of people over 65 years old living in the community who had been newly prescribed risperidone, olanzapine, quetiapine, haloperidol, aripiprazole or ziprasidone (n = 136 393). Propensity score-adjusted Cox proportional hazards models assessed the 180-day mortality risk of each antipsychotic compared with risperidone.
Results
Risperidone, olanzapine and haloperidol showed a dose–response relation in mortality risk. After controlling for propensity score and dose, mortality risk was found to be increased for haloperidol (hazard ratio (HR) = 1.18, 95% CI 1.06–1.33) and decreased for quetiapine (HR = 0.81, 95% CI 0.73–0.89) and olanzapine (HR = 0.82, 95% CI 0.74–0.90).
Conclusions
Significant variation in mortality risk across commonly prescribed antipsychotics suggests that antipsychotic selection and dosing may affect survival of older people living in the community.
Secondary metabolites of herbs and spices are widely used as an alternative strategy in the therapy of various diseases. The polyphenols naringenin, quercetin and curcumin have been characterised as anti-diabetic agents. Conversely, in vitro, naringenin and quercetin are described to inhibit phosphoinositide-3-kinase (PI3K), an enzyme that is essential for the neuronal control of whole body glucose homoeostasis. Using both in vitro and in vivo experiments, we tested whether the inhibitory effect on PI3K occurs in neurons and if it might affect whole body glucose homoeostasis. Quercetin was found to inhibit basal and insulin-induced phosphorylation of Akt (Ser473), a downstream target of PI3K, in HT-22 cells, whereas naringenin and curcumin had no effect. In Djungarian hamsters (Phodopus sungorus) naringenin and quercetin (10 mg/kg administered orally) diminished insulin-induced phosphorylation of Akt (Ser473) in the arcuate nucleus, indicating a reduction in hypothalamic PI3K activity. In agreement with this finding, glucose tolerance in naringenin-treated hamsters (oral) and mice (oral and intracerebroventricular) was reduced compared with controls. Dietary quercetin also impaired glucose tolerance, whereas curcumin was ineffective. Circulating levels of insulin and insulin-like growth factor-binding protein were not affected by the polyphenols. Oral quercetin reduced the respiratory quotient, suggesting that glucose utilisation was impaired after treatment. These data demonstrate that low doses of naringenin and quercetin acutely and potently impair glucose homoeostasis. This effect may be mediated by inhibition of hypothalamic PI3K signalling. Whether chronic impairments in glucose homoeostasis occur after long-term application remains to be identified.
Many species in the genus Andrena have been described from one sex only. During studies of this genus in Ontario, the authors have associated the males of four species with the appropriate females. The males of Andrena (Trachandrena) alleghaniensis Viereck, A. (Leucandrena) chippewaensis Mitchell, A. (Conandrena) rufosignata Cockerell and A. (Gonandrena) dreisbachi Mitchell, all previously unknown, are described in this paper.
Enhancing self-efficacy is central to programmes promoting self-care and self-management. However, little is known about older people's self-efficacy in doctor–patient interactions. This paper investigates lifestyle, medical and demographic factors associated with self-efficacy in doctor–patient interactions in older people in general practice.
Methods
A cross-sectional analysis of data from a randomised controlled trial of older people was conducted in a health risk appraisal study in London. Self-efficacy was measured using the Perceived Efficacy in Patient–Physician Interactions Questionnaire.
Results
Older people with higher self-efficacy were significantly more likely to report having had recent preventive care measures such as recent blood pressure measurement and influenza immunisation. Women were less likely to have higher self-efficacy than men. Older people were significantly less likely to have high self-efficacy if they reported having poor memory, low mood, limited activities due to fear of falling, basic education, difficulties with at least one activity of daily living, reduced physical activity, living alone, or risk of social isolation.
Conclusion
A third of people had low self-efficacy in doctor–patient interactions. They appear to be a vulnerable group. Low self-efficacy in interactions with doctors may be a symptom or a characteristic of older people who experience social isolation and depression. Policies that depend on enhancing self-care and self-management need to consider the large number of older people with low self-efficacy in using medical services, and understanding characteristics in older people associated with lower confidence in doctor–patient interactions may be useful in clinical practice and research.
The spectral power distribution of the light emitted by the Sun is almost constant. The variations in daylight (Figure 13.1) that we experience over the course of a day and with changes in seasons are due to the interaction of sunlight with the Earth's atmosphere (Henderson, 1977). The resulting spectral distribution of daylight across the sky is typically spatially inhomogeneous and constantly changing (Lee and Hernández-Andrés, 2005a,b). The light arriving at each small patch of surface in the scene depends in general on the patch's location and orientation in the scene. Furthermore, objects in the scene create shadows or act as secondary light sources, adding further complexity to the light field (Gershun, 1936/1939; see also Adelson and Bergen, 1991) that describes the spectral power distribution of the light arriving from every direction at every point in the scene. The light field captures what a radiospectrophotometer placed at each point in the scene, pointing in all possible directions, would record (Figure 13.2).
When the light field is inhomogeneous, the light absorbed and reradiated by a matte smooth surface patch can vary markedly with the orientation or location of the patch in the scene.
In Figure 13.3, for example, we illustrate the wide range of the light emitted by identical rectangular achromatic matte surfaces at many orientations, illuminated by a distant neutral, collimated light source.
Early intervention can help to reduce the burden of disability in the older population, but many do not access preventive care. There is uncertainty over what factors influence case finding in older patients in general practice.
Aim
To explore factors associated with case finding for hypertension, hyperlipidaemia and diabetes mellitus in older patients.
Method
Two thousand four hundred and ninety-one patients aged 65 years and above were recruited from three large practices in suburban London before the introduction of the Quality and Outcomes Framework (QOF) completed a questionnaire on health, functional status, health behaviours and preventive care.
Findings
Those not reporting heart disease, diabetes or hypertension were included in a secondary data analysis to explore factors influencing uptake of preventive care measures. Approximately one-third denied having had a blood pressure check in the previous year. They were more likely to have had little contact with doctors and to have an unhealthy lifestyle (smoking and a high-fat diet). One-third reported a cholesterol test in the previous five years. Cholesterol measurement was reported more often by men and those with a high body mass index. Those with unhealthy lifestyles (smoking and high-fat diet), those who had only received the state pension and those who limited their activities because of a fear of falling were less likely to report cholesterol measurement. About 10% reported a fasting blood glucose measurement and were more likely to consult more often and have more medications, but they were less likely to have a high-fat diet. Preventive care uptake was associated with frequent contacts with doctors, but overall the uptake of preventive care was low. Older people with healthier lifestyles were more likely to have primary preventative care interventions. These findings provide a baseline against which the effect of the QOF on the care of older people can be measured in future studies.
Commission 33 held its business session in the afternoon of Friday 7 August 2009. The president briefly described developments during the previous months. A short discussion followed about the activities of the Commission and whether it should play a more active role in view of the many on-going surveys relevant to the subject of the Galaxy.
Evidence about the cost-effectiveness and cost utility of computerised
cognitive–behavioural therapy (CCBT) is still limited. Recently, we
compared the clinical effectiveness of unsupported, online CCBT with
treatment as usual (TAU) and a combination of CCBT and TAU (CCBT plus
TAU) for depression. The study is registered at the Netherlands Trial
Register, part of the Dutch Cochrane Centre (ISRCTN47481236).
Aims
To assess the cost-effectiveness of CCBT compared with TAU and CCBT plus
TAU.
Method
Costs, depression severity and quality of life were measured for 12
months. Cost-effectiveness and cost-utility analyses were performed from
a societal perspective. Uncertainty was dealt with by bootstrap
replications and sensitivity analyses.
Results
Costs were lowest for the CCBT group. There are no significant group
differences in effectiveness or quality of life. Cost-utility and
cost-effectiveness analyses tend to be in favour of CCBT.
Conclusions
On balance, CCBT constitutes the most efficient treatment strategy,
although all treatments showed low adherence rates and modest
improvements in depression and quality of life.
