Dyslipidaemia is associated with chronic low-grade inflammation and immune dysfunction, but the immunological effects of dietary phytosterols in humans remain unclear. We conducted a secondary analysis of an outcome-assessor-blinded, randomised controlled feeding trial to evaluate the effects of a high-phytosterol (HPS) diet, using phytosterol-enriched corn–wheat germ blended oil (CWGO), compared with a low-phytosterol (LPS) diet using peanut oil, on systemic inflammatory markers, humoural immune markers and peripheral blood lymphocyte subsets in Chinese adults with dyslipidaemia. After a 2-week run-in period, 104 participants were randomised to the HPS group (n 52) or the LPS group (n 52) for 12 weeks. In intention-to-treat analyses, the HPS group had a higher CD4+:CD8+ ratio at 12 weeks than the LPS group (adjusted mean difference: 0·561; 95 % CI 0·060, 1·063; P = 0·03) and a lower CD8+ T-cell count (adjusted mean difference: −116·315 cells/μl; 95 % CI −215·781, –16·849; P = 0·02). No significant between-group differences were observed for systemic inflammatory markers, humoral immune markers or most other lymphocyte subset outcomes. In per-protocol analyses, the difference in CD4+:CD8+ ratio remained significant, whereas the reduction in CD8+ T-cell count was attenuated. These exploratory findings suggest that a 12-week phytosterol-enriched CWGO intervention may be associated with changes in T-cell subset balance in adults with dyslipidaemia, although the results should be interpreted cautiously given the exploratory nature of this secondary analysis.
Increasing evidences show that inflammation might be involved in bipolar disorder (BD), but the association between abnormal brain function and inflammation in BD is still unclear. In this study, we tried to explore the disrupted brain functional network topology, peripheral inflammatory cytokine levels, and their correlations in unmedicated bipolar II depression (BDII-D).
Methods
In this study, 65 individuals with unmedicated BDII-D and 50 healthy controls (HCs) underwent resting-state magnetic resonance imaging scans. Graph theory analysis was performed to investigate the topological properties of the whole-brain functional connectome at both global and nodal levels. Besides, serum levels of 17 inflammatory cytokines were measured in both BDII-D and HCs. Correlations between topological properties, clinical variables, and peripheral inflammatory cytokine levels in BDII-D were calculated.
Results
Compared with HCs, at the global level, BDII-D showed significantly higher $ \lambda $, decreased $ \gamma $, $ \sigma $, Eglo, and Eloc; at the nodal level, BDII-D showed decreased Enodal in the right olfactory cortex, left pallidum, and vermis. Besides, BDII-D showed higher levels of interleukin-8 (IL-8), interleukin-10 (IL-10), and granulocyte colony-stimulating factor (G-CSF) compared with the HCs. In BDII-D, $ \gamma $ and $ \sigma $ were significantly negatively correlated with the Hamilton Depression Rating Scale (HDRS) scores and number of episodes. Also, IL-8 level showed significant negative correlation with $ \gamma $, $ \sigma $, and Enodal of the left pallidum in BDII-D.
Conclusions
Reduced information segregation and integration, and lower nodal efficiency in the left pallidum were associated with proinflammatory cytokine IL-8 level and might contribute to severe depressive symptoms in unmedicated BDII-D.
To examine the association between dietary patterns and metabolic syndrome (MetS) in western China, which has not been previously reported.
Design:
A population based cross-sectional study design. Dietary intake was assessed using a semi-quantitative FFQ. Principal component analysis identified dietary patterns and multivariate logistic regression evaluated their associations with MetS.
Setting:
Population-based Cohort Study of Chronic Diseases in Xinjiang (PCCDX), conducted in 2022.
Participants:
A total of 3208 individuals from PCCDX (mean age: 53·1 (sd 10·8) years; 49·1 % male).
Results:
MetS was diagnosed in 1762 participants (54·9 %). Four distinct dietary patterns were identified, with the refined grain–animal products dietary pattern being the dominant one. After adjusting for general demographic and lifestyle factors, a higher score in the refined grain–animal product pattern was associated with an increased risk of MetS. The OR for the second, third and fourth quartiles of the dietary score were 1·07 (95 % CI: 0·860, 1·322), 1·14 (0·923, 1·413) and 1·48 (1·189, 1·853), with a statistically significant trend (P = 0·003). Higher dietary scores in this pattern were also associated with increased risks of elevated waist circumference, high TAG and low HDL cholesterol (P < 0·05). Mediation analysis showed that visceral fat percentage partially mediated the association between the refined grain–animal product dietary pattern and low HDL-cholesterol, accounting for 17·2 % of the total effect (indirect effect = 0·005, P = 0·006). The other three dietary patterns showed no significant associations with MetS or its components.
Conclusions:
This study highlights the high prevalence of MetS in western China and links a refined grains-animal products diet to poorer metabolic health, emphasising the need for region-specific dietary strategies.
Empathy involves communicating and understanding others’ emotion in multisensory contexts, including visual and auditory modalities. Schizophrenia (SCZ) patients have impaired empathy, but whether the impact of visual/auditory context would be altered in SCZ patients and people with high social anhedonia (HSoA) remained unclear.
Methods
We administered the modified Chinese version of the Empathic Accuracy Task (EAT) to clinical (50 SCZ patients and 50 healthy controls) and subclinical samples (59 HSoA and 60 low social anhedonia [LSoA] participants). The EAT employed audio-only, audiovisual, and audioavatar visual conditions to assess the impact of multimodal information on empathy during positive and negative emotional events.
Results
In positive-valenced context, SCZ patients performed worse than controls in cognitive and affective empathy. The Modality-by-Group interaction on empathic accuracy was significant, that is, SCZ patients performed worse than controls in both audiovisual and audioavatar visual conditions, but comparable to controls in audio-only condition. In negative-valenced context, SCZ patients performed worse than controls in cognitive empathy. The Modality-by-Group interaction on empathic accuracy was significant, that is, SCZ patients performed worse than controls in audio-only and audiovisual conditions. Moreover, HSoA participants exhibited lower cognitive empathy than controls in positive-valenced context; and lower cognitive empathy and empathic motivation in negative-valenced context. No significant Modality-by-Group interaction was found in the HSoA–LSoA sample.
Conclusions
SCZ patients have generalized impairments of cognitive and affective empathy across positive and negative contexts, particularly in multimodal conditions. HSoA individuals are primarily impaired in cognitive empathy and empathic motivation.
The traditional design of laser drivers for inertial confinement fusion (ICF) is highly dependent on coherent laser light fields, which have significant advantages in achieving harmonic conversion and enhancing amplification efficiency. However, they also bring the core challenge of achieving uniform irradiation. This paper investigates the dynamic evolution process of uniform irradiation of multi-mode spatiotemporal light fields and analyzes the influence mechanism of spatiotemporal coherence on irradiation uniformity with different integration times. By balancing the relationship between the spatiotemporal coherence of the light field and uniform irradiation, we explore a possible scheme to alleviate the beam smoothing problem while satisfying the basic requirements of laser amplification and high-efficiency harmonic conversion. Based on this scheme, the overall architecture of the ICF laser driver is constructed.
Orthopaedic inpatients have distinct clinical traits. This study aimed to quantify the burden of nosocomial infections (NIs) on orthopaedic patients. A nested case–control study (2022–2024) at the China National Orthopaedic Medical Center compared orthopaedic inpatients with and without NIs and matched cases and controls 1:3 to evaluate the burden of NIs. A national economic burden analysis was subsequently conducted under various scenarios. Among 120,764 eligible patients, 338 (0.28%) developed NIs. A total of 321 cases were matched with 916 controls. The economic and temporal burdens of NIs are US$2,100 and 5 days per case respectively. Haematologic NIs had the highest additional cost (US$4,295) and the second longest extended stay (9 days). In terms of initial hospitalisations and readmissions, surgical site infections extended hospital stays by 20 days and increased costs by US$4,881. The top three diagnosis-related groups (DRGs) with high burdens are ZC11, ZJ15, and IE21 for costs and ZJ15, IE21, and IB19 for duration. In the mixed-region scenario, orthopaedic specialty hospitals nationwide incur US$5.23 million in direct medical costs annually because of NIs. These findings indicate that NIs significantly affect orthopaedic patients both individually and nationally, necessitating focused prevention and control for high-burden DRGs and specific infections.
Although most couples share a bed, current interventions rarely consider dyadic sleep patterns.
Aims
We investigated whether bedtime alignment between partners affects longitudinal sleep outcomes in older couples, with particular attention to gender differences.
Method
Based on the temporal relationship between partners’ bedtimes and the earlier sleeper’s sleep onset latency, 859 couples (1718 individuals) aged ≥60 years were classified into 5 mutually exclusive bedtime alignment groups. Pittsburgh Sleep Quality Index (PSQI) scores, sleep onset latency and sleep efficiency were compared using analysis of variance and multivariate analysis of covariance. Both cross-sectional and 8-year longitudinal trajectory analyses were conducted.
Results
Bedtime alignment significantly affected sleep outcomes (P < 0.001, Pillai’s Trace = 0.37, F24, 3352 = 14.04, P < 0.001, η2P = 0.09). Couples with synchronised bedtimes demonstrated excellent sleep quality, whereas those with bedtime differences less than the earlier sleeper’s sleep onset latency exhibited the worst. The earlier sleepers in such couples experienced longer sleep onset latencies (53.4 ± 46.8 min) and greater sleep quality impairment (PSQI = 7.9 ± 4.1). The 8-year trajectory analysis revealed gender-specific vulnerability: only women in misaligned groups experienced progressive sleep deterioration over time (5.84 ± 8.42 min/year increase in sleep onset latency, P < 0.001; 1.27 ± 1.93%/year decrease in sleep efficiency, P < 0.001), whereas men maintained stable sleep parameters regardless of alignment.
Conclusions
Bedtime alignment represents a modifiable determinant of sleep health in older couples, with synchronised bedtimes providing optimal outcomes and partial sleep onset overlap creating disruption. This particularly benefits women, who show progressive deterioration with misalignment. These findings support the development of gender-informed, couple-based interventions for sleep disorders.
Unmanned aerial vehicles (UAVs) tracking one moving target in an oilfield environment with dynamic obstacles has been studied. Firstly, the UAV tracking one target is modeled mathematically, including the oilfield environment, the UAV kinematic model, the moving target model and so on. Considering the oilfield environment with dynamic obstacles, the chance-constraint method is incorporated into the distributed model predictive control (DMPC) to realise the target tracking and avoid the obstacles in the environment. Secondly, to obtain high-quality control-input sequences, the improved salp swarm algorithm (ISSA) is designed as the solver for the chance-constraint DMPC, integrating the sine-tent-cosine chaotic mapping in the initialisation, the differential mutation operation in the leader-position updating and the nonlinear adaptive inertia weights in the follower-position updating. Finally, the simulation results show that the proposed method has great performance in tracking the moving target in an oilfield environment with dynamic obstacles.
To determine values for the digestible indispensable amino acid score (DIAAS), it is recommended that ileal amino acid (AA) digestibility values obtained in growing pigs are used to characterise protein quality in different foods. Therefore, an experiment was conducted to determine the standardised ileal digestibility (SID) of AA in eight energy ingredients (barley, sorghum, wheat, brown rice, rice bran, wheat bran, cassava and paddy rice) fed to pigs, where SID values in pigs can be used to calculate approximate DIAAS values in humans. Among the data obtained for all energy ingredients, significant variations (P < 0·01) in CP and AA composition were observed. Rice bran and wheat bran had the highest CP (16·43 % and 18·16 %, respectively) and DIAAS scores of 81–88 for adult, qualifying as ‘good’ protein sources (> 75). Cassava, with the lowest CP (2·74 %), was limited by sulphur amino acid (54). Lysine (Lys) was the first-limiting AA in barley (74), sorghum (51) and wheat (49), with SID values lowest in wheat (71·04 %). Brown rice and paddy rice showed higher SIDLys (87·51 % and 78·13 %, respectively). These findings highlight the potential of bran-based ingredients and Lys fortification to improve protein quality in grain-dependent diets, providing the scientific basis to combat protein malnutrition.
African swine fever (ASF) is a highly contagious animal disease caused by African swine fever virus (ASFV). It is listed by the World Organization for Animal Health (WOAH) as an animal disease subject to statutory reporting. ASFV, a large, enveloped double-stranded DNA virus with high genomic complexity, exhibits a case fatality rate of up to 100%, posing a significant threat to the global pig industry and food safety. To date, the absence of a safe commercial ASFV vaccine primarily stems from challenges in identifying immunogenic viral antigens, insufficient characterization of ASFV pathogenesis, and limited understanding of the virus’s immune evasion mechanisms. Here, we review the pathogenic characteristics (morphological structure, clinical symptoms, and epidemiological characteristics), molecular biological characteristics, and infection mechanism of ASFV, as well as the immune response mechanism, vaccine research, and the latest information on ASFV in other areas. This review will be in favour of understanding the current state of knowledge of ASF and developing effective vaccines to control this disease.
Understanding the flow behaviour of wet granular materials is essential for comprehending the dynamics of numerous geological and physical phenomena, but remains a significant challenge, especially the transition of these flow regimes. In this study, we perform a series of rotating drum experiments to systematically investigate the dynamic observables and flow regimes of wet mono-dispersed particles. Two typical continuous flows including rolling and cascading regimes are identified and analysed, concentrating on the impact of fluid density and rotation speed. The probability density functions of surface angles, $\theta _{\textit{top}}$ and $\theta _{\textit{lo}w\textit{er}}$, reveal distinct patterns for these two flow regimes. A morphological parameter thus proposed, termed angle divergence, is used to characterise the rolling–cascading regime transition quantitatively. By integrating quantitative observables, we construct the flow phase diagram and flow curve to delineate the transition rules governing these regimes. Notably, the resulting nonlinear phase boundary demonstrates that higher fluid densities significantly enhance the likelihood of the system transitioning into the cascading regime. This finding is further supported by corresponding variations in flow fluctuations. Our results provide new insights into the fundamental dynamics of wet granular matter, offering valuable implications for understanding the complex rheology of underwater landslides and related phenomena.
This study reports potassium (K) isotope compositions of diamondiferous kimberlites. Altered kimberlite samples exhibit δ41K values ranging from −1.293 ± 0.052 (2SD) to −0.114 ± 0.029 ‰, showing covariations with chemical indicators of alteration. This is consistent with the geochemical dynamics of K isotopes in hydrothermal fluid-related processes. In contrast, pristine kimberlite samples display restricted K isotope compositions, with δ41K values between −0.494 ± 0.057 and −0.270 ± 0.048 ‰. Notably, the δ41K values of these pristine kimberlite samples correlate well with K2O and Rb contents, suggesting that approximately ∼0.2 ‰ of K isotope fractionation is induced by phlogopite crystallization, as indicated by quantitative modelling. The estimated δ41K values of −0.458 ‰ for the primary kimberlite melt and of −0.414 ‰ for the kimberlite source imply a potential link to the bulk silicate Earth. These new measurements, along with literature data from various rocks, indicate that the K isotope composition in the deep mantle (>150 km) is more homogenous than in shallow regions, likely reflecting the efficiency of convection flow and K behaviour during subduction. In addition, the K isotope data reveal temporal variations in mantle-derived magmas from the Palaeozoic to the Cenozoic, highlighting the geological history and lithospheric destruction of the North China Craton. This study underscores the significance of K isotopes in enhancing our understanding of mantle dynamics, crustal recycling and the geochemical evolution of the Earth’s interior.
Euthymic bipolar disorder (euBD) patients exhibit deficits in neurocognitive and social cognitive functioning compared to healthy controls (HCs). Our prior research has shown that the excitatory/inhibitory (E/I) imbalance in the default mode network (DMN) is linked to executive function in euBD. Neurocognitive impairments are associated with social cognition deficits in individuals with mental disorders. Given this connection, this study posits E/I imbalance within the DMN is associated with social cognition, with executive function as a mediator.
Methods
Seventy-five HCs and 49 euBD individuals were recruited. Using the emotion recognition task, Diagnostic Analysis of Nonverbal Accuracy 2-Taiwan version (DANVA-2-TW) and cognitive flexibility task, Wisconsin Card Sorting Test (WCST), we assessed emotion recognition and prefrontal function. Proton magnetic resonance spectroscopy (1H-MRS) measured metabolites in the posterior cingulate cortex (PCC) and medial prefrontal cortex/anterior cingulate cortex (mPFC/ACC), quantifying excitatory glutamate+glutamine (Glx) and inhibitory GABA to calculate the E/I ratio.
Results
euBD patients showed poorer emotion recognition (p = 0.020) and poorer cognitive flexibility (fewer WCST categories completed, p = 0.002). A negative association was found between emotion recognition and the E/I ratio in the mPFC/ACC of the BD patients (r = −0.30, p = 0.034), which was significantly mediated by cognitive flexibility (Z = −2.657, p = 0.007).
Conclusion
The BD patients demonstrate deficits in emotion recognition, linked to an altered E/I balance in the prefrontal cortex, and the cognitive flexibility, a key aspect of executive function, mediates the impact of the E/I ratio on emotion recognition accuracy in euBD patients.
Objectives: Data on primary care antibiotic prescription practices for children in Southeast Asia, which are essential for policy, quality improvement and patient safety, are lacking. We aimed to describe this gap and to benchmark prescription practices against international standards. Methods: Antibiotic prescriptions for children (age <18 years) who visited six public primary care clinics in Singapore between 2018 and 2021 were extracted and categorized according to the World Health Organization Access, Watch, Reserve (WHO AWaRe) classification. Quality indicators from the European Surveillance of Antimicrobial Consumption Network (ESAC- Net) and National Institute for Health and Care Excellence (NICE) guidelines were used as a measure of appropriateness of antibiotic prescribing. Descriptive statistics and T-test was used to compare prescription rates pre- and post-COVID-19 pandemic. Results: 19,325 and 20,692 oral and topical antibiotics were prescribed for 831,669 visits, with a prescription rate of 2.3% and 2.5% respectively. Mean antibiotic prescriptions fell significantly post-pandemic (2020–2021), compared to pre-pandemic numbers (1062.8 to 604.5 prescriptions per month) (p <0.001). The majority (95.8%) of prescriptions belonged to the Access group. Watch group antibiotics constituted 6.1% of the total antibiotics prescribed for respiratory conditions (n = 562). While prescriptions were low (4.1%) and well within EASC-Net quality indicator limits of 0-20% for respiratory infections, prescriptions for otitis media were significantly high (56.6%). Approximately 1 in 2 children received antibiotics as recommended by NICE guidelines for both respiratory infections (n=4,622, 51.5%) and otitis media (n=204, 51.8%). Conclusions: Primary care antibiotic prescriptions for children in Singapore decreased post- COVID-19. However, high rates of otitis media prescriptions and only 50% appropriateness for respiratory infections and otitis media emphasize the need for targeted improvements in these areas.
Identifying key areas of brain dysfunction in mental illness is critical for developing precision diagnosis and treatment. This study aimed to develop region-specific brain aging trajectory prediction models using multimodal magnetic resonance imaging (MRI) to identify similarities and differences in abnormal aging between bipolar disorder (BD) and major depressive disorder (MDD) and pinpoint key brain regions of structural and functional change specific to each disorder.
Methods
Neuroimaging data from 340 healthy controls, 110 BD participants, and 68 MDD participants were included from the Taiwan Aging and Mental Illness cohort. We constructed 228 models using T1-weighted MRI, resting-state functional MRI, and diffusion tensor imaging data. Gaussian process regression was used to train models for estimating brain aging trajectories using structural and functional maps across various brain regions.
Results
Our models demonstrated robust performance, revealing accelerated aging in 66 gray matter regions in BD and 67 in MDD, with 13 regions common to both disorders. The BD group showed accelerated aging in 17 regions on functional maps, whereas no such regions were found in MDD. Fractional anisotropy analysis identified 43 aging white matter tracts in BD and 39 in MDD, with 16 tracts common to both disorders. Importantly, there were also unique brain regions with accelerated aging specific to each disorder.
Conclusions
These findings highlight the potential of brain aging trajectories as biomarkers for BD and MDD, offering insights into distinct and overlapping neuroanatomical changes. Incorporating region-specific changes in brain structure and function over time could enhance the understanding and treatment of mental illness.
