This study examines the presence of bacterial contamination on surgical gloves and suggests appropriate measures for an aseptic surgical environment. To prevent glove contamination during surgery, surgeons and assistants should change gloves periodically, and scrub nurses should be careful when opening packages and handing over implants.
The “Fast track” protocol is an early extubation strategy to reduce ventilator-associated complications and induce early recovery after open-heart surgery. This study compared clinical outcomes between operating room extubation and ICU extubation after open-heart surgery in patients with CHD.
Methods:
We retrospectively reviewed 215 patients who underwent open-heart surgery for CHDs under the scheduled “Fast track” protocol between September 2016 and April 2022. The clinical endpoints were post-operative complications, including bleeding, respiratory and neurological complications, and hospital/ICU stays.
Results:
The patients were divided into operating room extubation (group O, n = 124) and ICU extubation (group I, n=91) groups. The most frequently performed procedures were patch closures of the atrial septal (107/215, 49.8%) and ventricular septal (89/215, 41.4%) defects. There were no significant differences in major post-operative complications or ICU and hospital stay duration between the two groups; however, patients in group I showed longer mechanical ventilatory support (0.0 min vs. 59.0 min (interquartile range: 17.0–169.0), p < 0.001). Patients in Group O showed higher initial lactate levels (3.2 ± 1.7 mg/dL versus 2.5 ± 2.0 mg/dL, p = 0.007) and more frequently used additional sedatives and opioid analgesics (33.1% versus 19.8%, p = 0.031).
Conclusions:
Extubation in the operating room was not beneficial for patients during post-operative ICU or hospital stay. Early extubation in the ICU resulted in more stable hemodynamics in the immediate post-operative period and required less use of sedatives and analgesics.
Left aortic arch with right descending aorta associated with coarctation of the aorta is a rare congenital cardiac anomaly. Conventional aortic arch repair in this condition may cause airway compression by the abnormally coursing descending aorta. We present the case of a neonate with this anomaly who underwent successful descending aortic translocation to prevent postoperative left main bronchial stenosis.
Although disconnectivity among brain regions has been one of the main hypotheses for schizophrenia, the superficial white matter (SWM) has received less attention in schizophrenia research than the deep white matter (DWM) owing to the challenge of consistent reconstruction across subjects.
Methods:
We obtained the diffusion magnetic resonance imaging (dMRI) data of 223 healthy controls and 143 patients with schizophrenia. After harmonising the raw dMRIs from three different studies, we performed whole-brain two-tensor tractography and fibre clustering on the tractography data. We compared the fractional anisotropy (FA) of white matter tracts between healthy controls and patients with schizophrenia. Spearman’s rho was adopted for the associations with clinical symptoms measured by the Positive and Negative Syndrome Scale (PANSS). The Bonferroni correction was used to adjust multiple testing.
Results:
Among the 33 DWM and 8 SWM tracts, patients with schizophrenia had a lower FA in 14 DWM and 4 SWM tracts than healthy controls, with small effect sizes. In the patient group, the FA deviations of the corticospinal and superficial–occipital tracts were negatively correlated with the PANSS negative score; however, this correlation was not evident after adjusting for multiple testing.
Conclusion:
We observed the structural impairments of both the DWM and SWM tracts in patients with schizophrenia. The SWM could be a potential target of interest in future research on neural biomarkers for schizophrenia.
The bremsstrahlung process is studied in complex plasmas including the influence of ions and variable dust charge. The electron-dust particle bremsstrahlung radiation cross-section (e-D-BRCS) is obtained with the analytic expression for the effective dust charge in terms of the Debye length and the temperature ratio. The e-D-BRCS is found to be reduced with either the decrease of ion temperature or increase of electron temperature. The ion density effect is found to be more important in the small electron temperature domain. Interestingly, the influence of ion temperature and density is found to be independent of the bremsstrahlung emission energy. The effective dust charge is also found to decrease with an increase of the ratio of the electron temperature to the ion temperature. In addition, it is found that the effective dust charge increases with an increase of the ratio of the electron density to the ion density. Moreover, the e-D-BRCS is found to be increased with the decrease of ion density.
Inspired by the intermittent locomotion of fish schools, numerical simulations are performed with two self-propelled flexible fins in a side-by-side configuration with anti-phase oscillation actuated by laterally constrained heaving motions. For an intermittent swimming gait, one type of the half-tail-beating mode (HT mode) and two types of multiple-tail-beating modes coasting at the smallest (MTS mode) and largest (MTL mode) lateral gap distances are applied. Similar to the continuous-tail-beating mode (CT mode), equilibrium lateral gap distances between two fins with HT and MTL modes exist, whereas two fins with MTS mode do not maintain a lateral equilibrium state. Although the cycle-averaged lateral force acting on two fins with CT and MTL modes is mostly determined by an outward deflected jet and enhanced positive pressure between two fins, an added-mass lateral force related to an asymmetric flapping kinematics by passive flexibility also plays an important role in MTL mode to achieve a stable state with a lateral gap distance smaller than that in CT mode. When the cruising speed or the cycle-averaged input power is identical in a stable state, the cost of transport (COT) for two fins with MTL mode is smaller than that with CT mode due to not only a benefit from the intermittent swimming gait but also an enhanced schooling benefit with a small equilibrium lateral gap distance. The COT for two fins with CT mode is reduced further when the bending rigidity increases, whereas it is opposite with MTL mode.
Birds in flight are prone to collide with various transparent or reflective structures. While bird–window collision has been recognised as a critical conservation issue, collision with other transparent structures has been less understood. Noise barriers made of transparent materials are considered critical hazards for birds; however, little is known about the bird mortality they cause. We conducted the first nationwide-scale estimates of bird-collision mortality caused by transparent noise barriers (TNBs) along roads in the Republic of Korea. The total length of existing roadside transparent noise barriers was estimated at 1,416 km nationwide (as of 2018), and it had been increasing exponentially. Based on carcass surveys at 25 sites, daily mortality at the observed barriers was 0.335 ± 1.132 birds/km on average, and no difference in observed mortality was detected between both sides of a single barrier and between road types (i.e. local roads and motorways). Finally, we estimated that approximately 186,000 birds (95% confidence interval: 162,465–204,812 birds) are killed annually by collisions with roadside TNBs. As privately installed barriers were not considered in this study, the actual mortality is likely be higher than our estimates. Thus, collision with TNBs could become an emerging threat to avian conservation, especially in developing and urbanising regions around the world. As such structures are not formally recognised as conservation issues of importance, more systematic surveys aided by citizen science, both for the status of TNBs and bird-collision mortality, are needed in addition to management and mitigation policies.
This study investigated the effect of the flavonoid-based compound isorhamnetin (ISO) on maturation and developmental competence in oxidative stress-exposed porcine oocytes in vitro. Treatment with 2 μM ISO (2 ISO) increases the developmental rate of oxidative stress-exposed porcine oocytes during in vitro maturation (IVM). The glutathione level and mRNA expression of antioxidant-related genes (NFE2L2 and SOD2) were increased in the 2 ISO-treated group, whereas the reactive oxygen species level was decreased. Treatment with 2 ISO increased mRNA expression of a cumulus cell expansion-related gene (SHAS2) and improved chromosomal alignment. mRNA expression of maternal genes (CCNB1, MOS, BMP15 and GDF9) and mitogen activated protein kinase (MAPK) activity were increased in the 2 ISO-treated group. The total cell number per blastocyst and percentage of apoptotic cells were increased and decreased in the 2 ISO-treated group, respectively. Treatment with 2 ISO increased mRNA expression of development-related genes (SOX2, NANOG, and POU5F1) and anti-apoptotic genes (BCL2L1 and BIRC5) and decreased that of pro-apoptotic genes (CASP3 and FAS). These results demonstrate that 2 ISO improves the quality of porcine oocytes by protecting them against oxidative stress during IVM and enhances subsequent embryo development in vitro. Therefore, we propose that ISO is a useful supplement for IVM of porcine oocytes.
In this review, we introduce our recent applications of deep learning to solar and space weather data. We have successfully applied novel deep learning methods to the following applications: (1) generation of solar farside/backside magnetograms and global field extrapolation based on them, (2) generation of solar UV/EUV images from other UV/EUV images and magnetograms, (3) denoising solar magnetograms using supervised learning, (4) generation of UV/EUV images and magnetograms from Galileo sunspot drawings, (5) improvement of global IRI TEC maps using IGS TEC ones, (6) one-day forecasting of global TEC maps through image translation, (7) generation of high-resolution magnetograms from Ca II K images, (8) super-resolution of solar magnetograms, (9) flare classification by CNN and visual explanation by attribution methods, and (10) forecasting GOES solar X-ray profiles. We present major results and discuss them. We also present future plans for integrated space weather models based on deep learning.
The genus Lilium contains a number of ornamental crop species, which are commercially important in many countries. As they are vegetatively propagated, maintaining genetic stability is essential for their efficient conservation. In this study, we investigated the genetic stability of regenerated plants of three cultivars (L. bolanderi ‘Lenora’, L. bolanderi ‘Mount Duckling’ and L. bolanderi ‘Mount Dragon’) and one variety (L. callosum var. flavum) after cryopreservation, compared with fresh (donor) and non-cryopreserved plants using morphological traits and ISSR markers. No differences in morphological parameters including flower, stigma and pollen colour, floral spots, floral direction or polymorphic bands were observed between control (fresh and non-cryopreserved) and cryopreserved plantlets. In addition, based on the resulting UPGMA dendrogram, the four taxa were divided into different clusters. All cryopreserved, non-cryopreserved and fresh plants in each group could be grouped together in a single cluster with more than 97 or 100% similarity. The results suggest a very low level or the absence of genetic variation in terms of morphological and genetic stability among the plants regenerated after cryopreservation.
Our previous studies have already revealed that β-cryptoxanthin (BCX), hesperetin (HES), and icariin (ICA) antioxidants are effective for in vitro maturation (IVM) of porcine oocytes. In this study, we investigated which of BCX, HES, or ICA was more effective for IVM of porcine oocytes. The antioxidant properties were assessed with aged porcine oocytes and embryos by comparing 2,2-diphenyl-1-(2,4,6-trinitrophenyl)hydrazyl (DPPH), reducing power, and H2O2 scavenging activity assays. The chemical assay results demonstrated that BCX had a greater DPPH scavenging activity and reducing power than HES and ICA, compared with controls. However, the H2O2 scavenging activity of the antioxidants was similar when tested at the optimal concentrations of 1 μM BCX (BCX-1), 100 μM HES (HES-100), and 5 μM ICA (ICA-5). The biological assay results showed that BCX-1 treatment was more effective in inducing a significant reduction in reactive oxygen species (ROS), improving glutathione levels, and increasing the expression of antioxidant genes. In addition, BCX-1 inhibited apoptosis by increasing the expression of anti-apoptotic genes and decreasing pro-apoptotic genes in porcine parthenogenetic blastocysts. BCX-1 also significantly increased the blastocyst formation rate compared with the ageing control group, HES-100 and ICA-5. This study demonstrates that damage from ROS produced during oocyte ageing can be prevented by supplementing antioxidants into the IVM medium, and BCX may be a potential candidate to improve assisted reproductive technologies.
The dissipation of ion-acoustic surface waves propagating in a semi-bounded and collisional plasma which has a boundary with vacuum is theoretically investigated and this result is used for the analysis of edge-relevant plasma simulated by Divertor Plasma Simulator-2 (DiPS-2). The collisional damping of the surface wave is investigated for weakly ionized plasmas by comparing the collisionless Landau damping with the collisional damping as follows: (1) the ratio of ion temperature $({T_i})$ to electron temperature $({T_e})$ should be very small for the weak collisionality $({T_i}/{T_e} \ll 1)$; (2) the effect of collisionless Landau damping is dominant for the small parallel wavenumber, and the decay constant is given as $\gamma \approx{-} \sqrt {\mathrm{\pi }/2} {k_\parallel }{\lambda _{De}}\omega _{pi}^2/{\omega _{pe}}$; and (3) the collisional damping dominates for the large parallel wavenumber, and the decay constant is given as $\gamma \approx{-} {\nu _{in}}/16$, where ${\nu _{in}}$ is the ion–neutral collisional frequency. An experimental simulation of the above theoretical prediction has been done in the argon plasma of DiPS-2, which has the following parameters: plasma density ${n_e} = (\textrm{2--9)} \times \textrm{1}{\textrm{0}^{11}}\;\textrm{c}{\textrm{m}^{ - 3}}$, ${T_e} = 3.7- 3.8\;\textrm{eV}$, ${T_i} = 0.2- 0.3\;\textrm{eV}$ and collision frequency ${\nu _{in}} = 23- 127\;\textrm{kHz}$. Although the wavelength should be specified with the given parameters of DiPS-2, the collisional damping is found to be $\gamma = ( - 0.9\;\textrm{to}\; - 5) \times {10^4}\;\textrm{rad}\;{\textrm{s}^{ - 1}}$ for ${k_\parallel }{\lambda _{De}} = 10$, while the Landau damping is found to be $\gamma = ( - 4\;\textrm{to}\; - 9) \times {10^4}\;\textrm{rad}\;{\textrm{s}^{ - 1}}$ for ${k_\parallel }{\lambda _{De}} = 0.1$.
Litter-dwelling arthropods play an important role in maintaining forest ecosystem function. This study was designed to understand seasonal variations and diversity of litter-dwelling adult beetles, one of the most diverse groups of arthropods. Sampling was conducted in mixed-wood forests of South Korea between March and December 2019, covering all seasons, including winter. We used a sifting method and a Berlese funnel to collect arthropods living in leaf litter and soil. We collected a total of 5820 invertebrates representing six orders, of which 1422 were beetles representing 24 families and minimum 141 species. Beetle species richness was highest in spring and lowest in summer based on rarefaction and extrapolation. However, beetle abundance was lowest in spring, but abundance was similar among the other seasons. Beetle assemblage composition was correlated significantly with soil surface and atmospheric temperature. The assemblage composition differed among seasons, except between spring and winter, which overlapped slightly. The combined sifting–Berlese funnel method showed great advantages for investigating the diversity of overwintering arthropods. Continued study of the relationship between arthropods and the leaf-litter environment is essential to understand this microecosystem and will increase the chance of discovering new beetle species.
The impact of a drop train, a series of identical liquid drops separated by a constant distance, on a liquid pool initially generates a long slender cavity. However, the cavity soon collapses and turns into a shallow funnel. Here we theoretically model the dynamic profile of the elongated cavity and the steady shape of the funnel, which are then shown to agree well with experiment. When the liquid inertia plays a dominant role, the cavity assumes a parabolic profile that depends only on the drop diameter and the centre-to-centre spacing of adjacent drops. We consider the capillary forces as well as the drop impact force to obtain the shape of the funnel that persists once the elongated cavity collapses. Our study allows for predicting the interfacial morphology by the impact of a drop train and designing impact conditions useful for semiconductor cleaning processes.
To evaluate the impact of a vancomycin-resistant Enterococcus (VRE) screening policy change on the incidence of healthcare-associated (HA)-VRE bacteremia in an endemic hospital setting.
Design:
A quasi-experimental before-and-after study.
Setting:
A 1,989-bed tertiary-care referral center in Seoul, Republic of Korea.
Methods:
Since May 2010, our hospital has diminished VRE screening for admitted patients transferred from other healthcare facilities. We assessed the impact of this policy change on the incidence of HA-VRE bacteremia using segmented autoregression analysis of interrupted time series from January 2006 to December 2014 at the hospital and unit levels. In addition, we compared the molecular characteristics of VRE blood isolates collected before and after the screening policy change using multilocus sequence typing and pulsed-field gel electrophoresis.
Results:
After the VRE screening policy change, the incidence of hospital-wide HA-VRE bacteremia increased, although no significant changes of level or slope were observed. In addition, a significant slope change in the incidence of HA-VRE bacteremia (change in slope, 0.007; 95% CI, 0.001–0.013; P = .02) was observed in the hemato-oncology department. Molecular analysis revealed that various VRE sequence types appeared after the policy change and that clonally related strains became more predominant (increasing from 26.1% to 59.3%).
Conclusions:
The incidence of HA-VRE bacteremia increased significantly after VRE screening policy change, and this increase was mainly driven by high-risk patient populations. When planning VRE control programs in hospitals, different approaches that consider risk for severe VRE infection in patients may be required.
Current evidence on antipsychotic treatment and risk of psychiatric hospitalization in first-episode schizophrenia (FES) is largely based on the findings from randomized clinical trials (RCTs). However, the generalization of the findings to real-world patients is limited due to inherent caveats of the RCT. We aimed to investigate the treatment discontinuation and risk of psychiatric hospitalization using a nationwide population database.
Methods
The Health Insurance Review Agency database in South Korea was obtained, and the observation period started from 1 January 2009 to 31 December 2016. We defined the maintenance period as the period from 6-month after the diagnosis of schizophrenia, which is utilized for the main results. For a total of 44 396 patients with FES, a within-individual Cox regression model was used to compare the risk of the treatment discontinuation and psychiatric hospitalization.
Results
In group comparison, a long-acting injectable (LAI) antipsychotic group was associated with the lowest risk of the treatment discontinuation (0.64, 0.55–0.75) and psychiatric hospitalization (0.29, 0.22–0.38) in comparison with a typical antipsychotic group and no use, respectively. Among individual antipsychotics, the lowest risk of the treatment discontinuation was observed in LAI paliperidone (0.46, 0.37–0.66) compared to olanzapine. Clozapine was found to be the most effective antipsychotic in lowering the risk of psychiatric hospitalization as monotherapy compared to no use (0.23, 0.18–0.31).
Conclusions
In real-world patients with FES, LAI paliperidone and clozapine were associated with low treatment discontinuation and better effectiveness in lowering the risk of psychiatric hospitalization.
It has been reported that the follower in a tandem configuration with no wall (0W) reduces the time-averaged input power by utilizing the vortex interception mode (Zhu et al., Phys. Rev. Lett., vol. 113, 2014, p. 238105). In the present study, a numerical simulation is conducted with two self-propelled flexible fins in the tandem configuration near a single wall (1W) and two parallel walls (2W). Contrary to the vortex interception for 0W, the follower employs spontaneously a mixed mode (i.e. a combination of the vortex interception mode and the slalom mode) for 1W and the slalom mode for 2W. Although the lateral motion of the follower for 0W, 1W and 2W is synchronized with the induced lateral flow generated by the leader, the time-averaged input power of the follower for 1W and 2W is reduced significantly due to the enhanced lateral flow by the vortex–vortex interaction near the wall. The jet-like flow opposite to the moving direction continuously hinders the movement of the follower for 0W, whereas the follower for 1W and 2W utilizes the negative horizontal flow when passing between the main vortex and the induced vortex near the wall, leading to a decrease of the thrust force acting on the follower allowing the follower to keep pace with the leader. The global efficiency of the schooling fins is optimized with a small heaving amplitude of the follower and a critical value of phase difference between the leader and follower when the values of the wall proximity and bending rigidity are moderate.
The objective of this study was to evaluate the feasibility and effects of education on complex congenital heart diseases using patient-specific three-dimensional printed heart models.
Methods:
Three-dimensional printed heart models were created using computed tomography data obtained from 11 patients with complex congenital heart disease. Fourteen kinds of heart models, encompassing nine kinds of complex congenital heart disease were printed. Using these models, a series of educational hands-on seminars, led by an experienced paediatric cardiac surgeon and a paediatric cardiologist, were conducted for medical personnel who were involved in the care of congenital heart disease patients. Contents of the seminars included anatomy, three-dimensional structure, pathophysiology, and surgery for each diagnosis. Likert-type (10-point scale) questionnaires were used before and after each seminar to evaluate the effects of education.
Results:
Between November 2019 and June 2020, a total of 16 sessions of hands-on seminar were conducted. The total number of questionnaire responses was 75. Overall, participants reported subjective improvement in understanding anatomy (4.8 ± 2.1 versus 8.4 ± 1.1, p < 0.001), three-dimensional structure (4.6 ± 2.2 versus 8.9 ± 1.0, p < 0.001), pathophysiology (4.8 ± 2.2 versus 8.5 ± 1.0, p < 0.001), and surgery (4.9 ± 2.3 versus 8.8 ± 0.9, p < 0.001) of the congenital heart disease investigated.
Conclusions:
The utilisation of three-dimensional printed heart models for education on complex congenital heart disease was feasible and improved medical personnel’s understanding of complex congenital heart disease. This education tool may be an effective alternative to conventional education tools for complex congenital heart disease.
Background: Recently, healthcare-associated infections (HAIs) in long-term care hospitals (LTCHs) have markedly increased, but no infection control policy has been established in South Korea. We investigated the current HAI surveillance system and executed a point-prevalence pilot study in LTCHs. Methods: HAIs were defined by newly established surveillance manual based on McGeer criteria revised in 2012. Three LTCHs in Seoul and Gyeonggi province were voluntarily recruited, and data were collected from up to 50 patients who were hospitalized on August 1. The medical records from September to November 2018 were retrospectively reviewed by a charge nurse for infection control per each hospitals after 1 day of training specific for LTCH surveillance. All data were reviewed by a senior researcher visiting onsite. Results: The participating hospitals had 272.33 ± 111.01 beds. Only 1 hospital had an onsite microbiological laboratory. In total, 156 patients were enrolled and 5 HAIs were detected, for a prevalence rate of 3.2%. The average patient age was 79.04 ± 9.92 years. The HAIs included 2 urinary tract infections, skin and soft-tissue infection, low respiratory infection, and conjunctivitis. Conclusions: This is the first survey of HAI in LTCHs in South Korea. The 3.2% prevalence rate is lower than those from previous reports from the European Union or the United States. This study supports the development of a national HAI surveillance and infection control system in LTCHs, although implementation may be limited due to the lack of laboratory support and infection control infrastructure in Korea.
We report our experience with an emergency room (ER) shutdown related to an accidental exposure to a patient with coronavirus disease 2019 (COVID-19) who had not been isolated.
Setting:
A 635-bed, tertiary-care hospital in Daegu, South Korea.
Methods:
To prevent nosocomial transmission of the disease, we subsequently isolated patients with suspected symptoms, relevant radiographic findings, or epidemiology. Severe acute respiratory coronavirus 2 (SARS-CoV-2) reverse-transcriptase polymerase chain reaction assays (RT-PCR) were performed for most patients requiring hospitalization. A universal mask policy and comprehensive use of personal protective equipment (PPE) were implemented. We analyzed effects of these interventions.
Results:
From the pre-shutdown period (February 10–25, 2020) to the post-shutdown period (February 28 to March 16, 2020), the mean hourly turnaround time decreased from 23:31 ±6:43 hours to 9:27 ±3:41 hours (P < .001). As a result, the proportion of the patients tested increased from 5.8% (N=1,037) to 64.6% (N=690) (P < .001) and the average number of tests per day increased from 3.8±4.3 to 24.7±5.0 (P < .001). All 23 patients with COVID-19 in the post-shutdown period were isolated in the ER without any problematic accidental exposure or nosocomial transmission. After the shutdown, several metrics increased. The median duration of stay in the ER among hospitalized patients increased from 4:30 hours (interquartile range [IQR], 2:17–9:48) to 14:33 hours (IQR, 6:55–24:50) (P < .001). Rates of intensive care unit admissions increased from 1.4% to 2.9% (P = .023), and mortality increased from 0.9% to 3.0% (P = .001).
Conclusions:
Problematic accidental exposure and nosocomial transmission of COVID-19 can be successfully prevented through active isolation and surveillance policies and comprehensive PPE use despite longer ER stays and the presence of more severely ill patients during a severe COVID-19 outbreak.
