  • Introduction:

    Clinical trials are a vital component of translational science, providing crucial information on the efficacy and safety of new interventions and forming the basis for regulatory approval and/or clinical adoption. At the same time, they are complex to design, conduct, monitor, and report successfully. Concerns over the last two decades about the quality of the design and the lack of completion and reporting of clinical trials, characterized as a lack of “informativeness,” highlighted by the experience during the COVID-19 pandemic, have led to several initiatives to address the serious shortcomings of the United States clinical research enterprise.

    Methods and Results:

    Against this background, we detail the policies, procedures, and programs that we have developed in The Rockefeller University Center for Clinical and Translational Science (CCTS), supported by a Clinical and Translational Science Award (CTSA) program grant since 2006, to support the development, conduct, and reporting of informative clinical studies.

    Conclusions:

    We have focused on building a data-driven infrastructure to both assist individual investigators and bring translational science to each element of the clinical investigation process, with the goal of both generating new knowledge and accelerating the uptake of that knowledge into practice.

  • Summary

    Experimentation is central to the Irish literary tradition, so it is striking to see that many new forms of digital literature remain uncultivated on this island. Where Irish literature has engaged with the digital, it is usually in the form of film-poetry, fragments of text set to video and sound. Other national canons have long progressed to more technically sophisticated genres – literary games, walking simulators, interactive fiction set in immersive virtual worlds – but Irish digital literature remains dominated by the film-poem, and in many respects, seems stagnated. But the situation in Ireland is not entirely bleak: in the figure of Doireann Ní Ghríofa, now a household name after the success of A Ghost in the Throat (2020), Ireland has a high-profile author who has shown a willingness to embrace the digital, something which may encourage further support for multimodal writing among arts practitioners and policymakers. Through its emphasis on Ní Ghríofa’s e-lit works, this chapter explores the past, present, and future of digital literature in Ireland, its major impediments, and possible futures.

    In editing this volume of essays, Technology in Irish Literature and Culture, for the Cambridge Themes in Irish Literature and Culture series, we are pleased to accept the dual invitation delivered by series editor Ronan McDonald: in his words, ‘to choose a significant issue that animates or perplexes contemporary Irish culture, and use it as an aperture through which to examine the literature of previous eras’. Our choice of subject is technology, a subject of fundamental significance to our current condition, and central to very many of our contemporary concerns, vexations, pleasures, and opportunities.

  • Technology in Irish Literature and Culture shows how such significant technologies—typewriters, gramophones, print, radio, television, computers—have influenced Irish literary practices and cultural production, while also examining how technology has been embraced as a theme in Irish writing. Once a largely rural and agrarian society, contemporary Ireland has embraced the communicative, performative and consumption habits of a culture utterly reliant on the digital. This text plumbs the origins of the present moment, examining the longer history of literature's interactions with the technological and exploring how the transformative capacity of modern technology has been mediated throughout a diverse national canon. Comprising essays from some of the major figures of Irish literary and cultural studies, this volume offers a wide-ranging, comprehensive account of how Irish literature and culture have interacted with technology.

    The COVID-19 pandemic substantially impacted care of patients with schizophrenia treated with long-acting injectable antipsychotics (LAIs). This study examined how clinics adapted operations to maintain a standard of care for these patients after pandemic onset.

    Methods

    Online surveys were completed in October-November 2020 by one principal investigator (PI) or PI-appointed designee at 35 clinics participating in OASIS (NCT03919994). Items concerned pandemic impacts on clinic operations, particularly telepsychiatry, and on the care of patients with schizophrenia treated with LAIs.

    Results

    All 35 clinics reported using telepsychiatry; 20 (57%) implemented telepsychiatry after pandemic onset. Telepsychiatry visits increased from 12%-15% to 45%-69% across outpatient visit types after pandemic onset; frequency of no-show and/or canceled telepsychiatry visits decreased by approximately one-third. Nearly half of clinics increased the frequency of telepsychiatry visits for patients with schizophrenia treated with LAIs. Approximately one-third of participants each reported switching patients treated with LAIs to longer injection interval LAIs or to oral antipsychotics. The most common system/clinic- and patient-related barrier for telepsychiatry visits was lower reimbursement rate and access to technology/reliable internet, respectively. Almost all participants (94%) were satisfied with telepsychiatry for maintaining care of patients with schizophrenia treated with LAIs; most predicted a hybrid of telepsychiatry and office visits post-pandemic.

    Conclusions

    Changes made by clinics after pandemic onset were viewed by almost all participants as satisfactory for maintaining a standard of care for patients with schizophrenia treated with LAIs. Most participants predicted continuing telepsychiatry to support patient care post-pandemic; equitable access to telepsychiatry will be important in this regard.

  • Digital art is fundamentally digital; it is art which cannot happen without some contemporary media technology, some element of computation, some bit-based machine. Digital art goes by a lot of names – net art, electronic art, computational art, multimedia art, new media art, screen-based art – but generally, this is a domain in which the objects of discussion rely absolutely on modern and contemporary electronics to achieve their artistic purpose.

    This collection of essays explores digital art in Ireland, filling a major gap in the national media archaeology of Ireland by bringing together a collection of timely perspectives from scholars and practitioners engaged with screen-based expression. In no way is this book a true representative selection of forms and figures, but it is, hopefully, a small contribution to addressing what remains an intellectual void.

    Wonderfully creative things are happening with computers, screens and machines right across the spectrum of artistic practice, but through disciplinary isolation – by focusing only on fine art, literature or film – we are blinding ourselves to contemporary media art as a wider cultural upheaval. The intimate connections being formed between the digital and the expressive, and how such production is mediated through national contexts, will only be fully revealed when considered through an interdisciplinary gaze. This book, comprising contributions from EL Putnam, Anne Karhio, Ken Keating, Conor McGarrigle, Kieran Nolan, Claire Fitch, Kirstie North and Chris Clarke, attempts to do just that, treating what it means for art to be both digital and Irish.
