This chapter explores Swift’s reception during the twentieth and twenty-first centuries. In the early twentieth century, Swift was reinterpreted as a proleptic modernist, complete with convoluted personal life and affinity for marginal spaces. But as the century wore on, his writings were often enlisted to express covert criticism of authoritarian regimes. This chapter is divided into five sections. The first looks at the various editions and biographies of Swift that have appeared since 1900, and the critical controversies that have risen from these scholarly projects. The second section charts the fluctuating reception of Swift’s writings on Ireland. The third provides an overview of recent work on Swift’s engagement with eighteenth-century science and medicine. The fourth focuses on gender and sexuality. And the fifth and final section explores Swift’s afterlives and future directions in Swift scholarship.
To examine trends in rates of self-harm among emergency department (ED) presenting older adults in Ireland over a 13-year period.
Design:
Population-based study using data from the National Self-Harm Registry Ireland.
Setting:
National hospital EDs.
Participants:
Older adults aged 60 years and over presenting with self-harm to hospital EDs in Ireland between January 1, 2007 and December 31, 2019.
Measurements:
ED self-harm presentations.
Results:
Between 2007 and 2019, there were 6931 presentations of self-harm in older adults. The average annual self-harm rate was 57.8 per 100,000 among older adults aged 60 years and over. Female rates were 1.1 times higher compared to their male counterparts (61.4 vs 53.9 per 100,000). Throughout the study time frame, females aged 60–69 years had the highest rates (88.1 per 100,000), while females aged 80 years and over had the lowest rates (18.7 per 100,000). Intentional drug overdose was the most commonly used method (75.5%), and alcohol was involved in 30.3% of presentations. Between the austerity and recession years (2007–2012), self-harm presentations were 7% higher compared to 2013–2019 (incidence rate ratio (IRR): 1.07 95% CI 1.02–1.13, p = 0.01).
Conclusions:
Findings indicate that self-harm in older adults remains a concern with approximately 533 presentations per year in Ireland. While in younger age groups, females report higher rates of self-harm, this gender difference was reversed in the oldest age group (80 years and over), with higher rates of self-harm among males. Austerity/recession years (2007–2012) had significantly higher rates of self-harm compared to subsequent years.
Following the end of universal testing in the UK, hospital admissions are a key measure of COVID-19 pandemic pressure. Understanding leading indicators of admissions at the National Health Service (NHS) Trust, regional and national geographies help health services plan for ongoing pressures. We explored the spatio-temporal relationships of leading indicators of hospitalisations across SARS-CoV-2 waves in England. This analysis includes an evaluation of internet search volumes from Google Trends, NHS triage calls and online queries, the NHS COVID-19 app, lateral flow devices (LFDs), and the ZOE app. Data sources were analysed for their feasibility as leading indicators using Granger causality, cross-correlation, and dynamic time warping at fine spatial scales. Google Trends and NHS triages consistently temporally led admissions in most locations, with lead times ranging from 5 to 20 days, whereas an inconsistent relationship was found for the ZOE app, NHS COVID-19 app, and LFD testing, which diminished with spatial resolution, showing cross-correlation of leads between –7 and 7 days. The results indicate that novel surveillance sources can be used effectively to understand the expected healthcare burden within hospital administrative areas though the temporal and spatial heterogeneity of these relationships is a key determinant of their operational public health utility.
The effectiveness of filtering facepiece respirators such as N95 respirators is heavily dependent on the fit. However, there have been limited efforts to discover the size of the gaps in the seal required to compromise filtering facepiece respirator performance, with prior studies estimating this size based on in vitro models. In this study, we measure the size of leak necessary to compromise the fit of N95 respirators.
Methods:
Two methods were used to create a gap of specific dimensions. A set of 3D-printed resin spacers and hollow steel rods were used to generate gaps in N95 respirators while worn on 2 participants. Occupational Safety and Health Administration (OSHA) quantitative fit testing methods were used to quantify mask performance with gaps between 0.4 and 2.9-mm diameters.
Results:
Gap size was regressed against fit factor, showing that overall, the minimum gap size to compromise N95 performance was between 1.5 mm2 and 3 mm2.
Conclusions:
These findings suggest the fit of a N95 respirator is compromised by gaps that may be difficult to visually detect. The study also adds to the body of evidence supporting the routine use of quantitative fit testing to ensure that masks are well-fitting.
Design, delivery and improvement processes in healthcare is of great challenge due to increasing pressures on healthcare systems alongside inherent complexities, resulting in limited benefits or unforeseen consequences. A recent Systems Approach framework has aimed at responding to the calls about implementing a more holistic approach. Though being an approach in its own right that can iteratively and systematically structure, guide, and/or facilitate design and improvement projects in healthcare, it can also augment existing approaches that have already place in healthcare designers' and improvement practitioners' agendas. Thus, the objective of this paper is to compare and contrast the questions, activities, and tools of the Systems Approach with processes of other major improvement approaches in healthcare to demonstrate their coverages, overlaps, and extended and new opportunities to pursue in the light of the Systems Approach.
Creativity appears to be an important part of cognitive capacities and problem solving. Creativity is one’s ability to generate ideas that are novel, surprising, and compelling (Kaufman and Sternberg, 2010). This chapter will focus on the creative-cognitive approach, which seeks to further understand how human minds produce creative ideas.
Qualitative fit testing is a popular method of ensuring the fit of sealing face masks such as N95 and FFP3 masks. Increased demand due to the coronavirus disease 2019 (COVID-19) pandemic has led to shortages in testing equipment and has forced many institutions to abandon fit testing. Three key materials are required for qualitative fit testing: the test solution, nebulizer, and testing hood. Accessible alternatives to the testing solution have been studied. This exploratory qualitative study evaluates alternatives to the nebulizer and hoods for performing qualitative fit testing.
Methods:
Four devices were trialed to replace the test kit nebulizer. Two enclosures were tested for their ability to replace the test hood. Three researchers evaluated promising replacements under multiple mask fit conditions to assess functionality and accuracy.
Results:
The aroma diffuser and smaller enclosures allowed participants to perform qualitative fit tests quickly and with high accuracy.
Conclusions:
Aroma diffusers show significant promise in their ability to allow individuals to quickly, easily, and inexpensively perform qualitative fit testing. Our findings indicate that aroma diffusers and homemade testing hoods may allow for qualitative fit testing when conventional apparatus is unavailable. Additional research is needed to evaluate the safety and reliability of these devices.
Hydrogen lithography has been used to template phosphine-based surface chemistry to fabricate atomic-scale devices, a process we abbreviate as atomic precision advanced manufacturing (APAM). Here, we use mid-infrared variable angle spectroscopic ellipsometry (IR-VASE) to characterize single-nanometer thickness phosphorus dopant layers (δ-layers) in silicon made using APAM compatible processes. A large Drude response is directly attributable to the δ-layer and can be used for nondestructive monitoring of the condition of the APAM layer when integrating additional processing steps. The carrier density and mobility extracted from our room temperature IR-VASE measurements are consistent with cryogenic magneto-transport measurements, showing that APAM δ-layers function at room temperature. Finally, the permittivity extracted from these measurements shows that the doping in the APAM δ-layers is so large that their low-frequency in-plane response is reminiscent of a silicide. However, there is no indication of a plasma resonance, likely due to reduced dimensionality and/or low scattering lifetime.
Oliver Goldsmith’s The Deserted Village (1770) and George Farquhar’s The Recruiting Officer (1706) embody the textual and ideological persistence of the Irish eighteenth century in our present. These texts inhabit contemporary culture as object of memory and as model of modernity. Eavan Boland’s poetry memorialises the eighteenth as Ireland’s ‘darkest century’, re-reading The Deserted Village as a front for a hostile colonial and capitalist modernity which took accelerated and influential shape in the Irish eighteenth century. Similarly, Farquhar’s play served throughout the eighteenth century to consolidate and extend the British fiscal-military state, an ideological function highlighted in Bertolt Brecht’s adaptation Trumpets and Drums (1955). The chapter focuses on two subsequent re-imaginings, Thomas Keneally’s 1987 novel The Playmaker and Timberlake Wertenbaker’s stage adaptation Our Country’s Good (1988). Both texts use metaphors of performance and rehearsal to illuminate the play’s function in propagating a political modernity grounded in the transitory and transitional cultures of eighteenth-century Ireland.
Between March and October of 1942, Slovakia deported the majority of its Jews to extermination camps in German-occupied Poland. Since then, critics and apologists of the nominally independent Nazi satellite state have argued bitterly over who was to blame. Did the Slovaks act voluntarily or under German pressure? If the latter, were they in any position to do otherwise? With equal vigor, the two sides have clashed over whether the Slovaks realized they were participating in genocide, whether they acted to limit or stop the deportations once the truth came out, and whether, compared with other German-occupied or German-allied countries, Slovakia succeeded in saving a relatively high percentage of its Jewry.
Ongoing, rapid innovations in fields ranging from microelectronics, aerospace, and automotive to defense, energy, and health demand new advanced materials at even greater rates and lower costs. Traditional materials R&D methods offer few paths to achieve both outcomes simultaneously. Materials informatics, while a nascent field, offers such a promise through screening, growing databases of materials for new applications, learning new relationships from existing data resources, and building fast predictive models. We highlight key materials informatics successes from the atomic-scale modeling community, and discuss the ecosystem of open data, software, services, and infrastructure that have led to broad adoption of materials informatics approaches. We then examine emerging opportunities for informatics in materials science and describe an ideal data ecosystem capable of supporting similar widespread adoption of materials informatics, which we believe will enable the faster design of materials.
There is strong evidence that people born in winter and in spring have a small increased risk of schizophrenia. As this ‘season of birth’ effect underpins some of the most influential hypotheses concerning potentially modifiable risk exposures, it is important to exclude other possible explanations for the phenomenon.
Methods
Here we sought to determine whether the season of birth effect reflects gene-environment confounding rather than a pathogenic process indexing environmental exposure. We directly measured, in 136 538 participants from the UK Biobank (UKBB), the burdens of common schizophrenia risk alleles and of copy number variants known to increase the risk for the disorder, and tested whether these were correlated with a season of birth.
Results
Neither genetic measure was associated with season or month of birth within the UKBB sample.
Conclusions
As our study was highly powered to detect small effects, we conclude that the season of birth effect in schizophrenia reflects a true pathogenic effect of environmental exposure.
Maastrichtian sediments exposed in the Bay of Biscay region at coastal sections at Zumaya and Sopelana, northern Spain, and Hendaye and Bidart, France, yield the most diverse Upper Maastrichtian ammonite faunas yet recovered. Thirty-two species/subspecies referred to 21 genera are described, one of which, Anapachydiscus terminus, is new. The ranges of all taxa are fully documented and provide the basis for a four-fold division of the uppermost Campanian and Maastrichtian, with zones of Pseudokossmaticeras tercense (oldest), Pachydiscus (P.) epiplectus, Anapachydiscus fresvillensis, and A. terminus (youngest). This represents the most refined, and first properly documented, ammonite zonation for the Maastrichtian of western Europe, and can be directly correlated with the belemnite zonation developed for the white chalks of northern Europe. The extinction of the last ammonites, documented here, occurred at relatively high standing diversity and appears to have been sudden and catastrophic.
Objectives: Despite recent development of health technology assessment (HTA) methods, there are still methodological gaps for the assessment of complex health technologies. The INTEGRATE-HTA guidance for effectiveness, economic, ethical, socio-cultural, and legal aspects, deals with challenges when assessing complex technologies, such as heterogeneous study designs, multiple stakeholder perspectives, and unpredictable outcomes. The objective of this article is to outline this guidance and describe the added value of integrating these assessment aspects.
Methods: Different methods were used to develop the various parts of the guidance, but all draw on existing, published knowledge and were supported by stakeholder involvement. The guidance was modified after application in a case study and in response to feedback from internal and external reviewers.
Results: The guidance consists of five parts, addressing five core aspects of HTA, all presenting stepwise approaches based on the assessment of complexity, context, and stakeholder involvement. The guidance on effectiveness, health economics and ethics aspects focus on helping users choose appropriate, or further develop, existing methods. The recommendations are based on existing methods’ applicability for dealing with problems arising with complex interventions. The guidance offers new frameworks to identify socio-cultural and legal issues, along with overviews of relevant methods and sources.
Conclusions: The INTEGRATE-HTA guidance outlines a wide range of methods and facilitates appropriate choices among them. The guidance enables understanding of how complexity matters for HTA and brings together assessments from disciplines, such as epidemiology, economics, ethics, law, and social theory. This indicates relevance for a broad range of technologies.
Policy-makers and practitioners have a need to assess community resilience in disasters. Prior efforts conflated resilience with community functioning, combined resistance and recovery (the components of resilience), and relied on a static model for what is inherently a dynamic process. We sought to develop linked conceptual and computational models of community functioning and resilience after a disaster.
Methods
We developed a system dynamics computational model that predicts community functioning after a disaster. The computational model outputted the time course of community functioning before, during, and after a disaster, which was used to calculate resistance, recovery, and resilience for all US counties.
Results
The conceptual model explicitly separated resilience from community functioning and identified all key components for each, which were translated into a system dynamics computational model with connections and feedbacks. The components were represented by publicly available measures at the county level. Baseline community functioning, resistance, recovery, and resilience evidenced a range of values and geographic clustering, consistent with hypotheses based on the disaster literature.
Conclusions
The work is transparent, motivates ongoing refinements, and identifies areas for improved measurements. After validation, such a model can be used to identify effective investments to enhance community resilience. (Disaster Med Public Health Preparedness. 2018;12:127–137)
Efforts to address health disparities and achieve health equity are critically dependent on the development of a diverse research workforce. However, many researchers from underrepresented backgrounds face challenges in advancing their careers, securing independent funding, and finding the mentorship needed to expand their research.
Methods
Faculty from the University of Maryland at College Park and the University of Wisconsin-Madison developed and evaluated an intensive week-long research and career-development institute—the Health Equity Leadership Institute (HELI)—with the goal of increasing the number of underrepresented scholars who can sustain their ongoing commitment to health equity research.
Results
In 2010-2016, HELI brought 145 diverse scholars (78% from an underrepresented background; 81% female) together to engage with each other and learn from supportive faculty. Overall, scholar feedback was highly positive on all survey items, with average agreement ratings of 4.45-4.84 based on a 5-point Likert scale. Eighty-five percent of scholars remain in academic positions. In the first three cohorts, 73% of HELI participants have been promoted and 23% have secured independent federal funding.
Conclusions
HELI includes an evidence-based curriculum to develop a diverse workforce for health equity research. For those institutions interested in implementing such an institute to develop and support underrepresented early stage investigators, a resource toolbox is provided.
