A comprehensive approach of the determinants of use of care in dementia: the Recaredem (recourse to care in dementia) cross-sectional study

Background:
Given the rate of the undiagnosed cases of dementia and the consequences of inappropriate care, understanding the factors that explain the use of medical and health care in dementia is a critical concern. Our objective was to identify the psychosocial and medical determinants of use of care in dementia.
Methods:The study sample consisted of 308 participants: the persons with dementia (n = 99) selected from three French population-based cohorts (i.e. PAQUID, 3C, AMI), their family caregivers (primary, n = 96, and secondary, n = 51), and their general practitioners (n = 62). Use of care in dementia was considered according to two indicators: (1) recourse to secondary care, (2) number of community and health services used.
Results:Multiple logistic models including sociodemographics and psychosocial variables revealed that the determinants of nonuse of care are similar both for the recourse to secondary care and for the number of community and health services used: lack of education and the contribution of the people with dementia to the decisions regarding their own care and dementia care services in the community area. In addition, satisfaction of the primary caregiver with the services used by his/her relative is associated with non-recourse to secondary care.
Conclusions:Taken together, these results highlight the predominant role of psychosocial factors in the use of care in dementia and the importance of addressing this issue through an integrative approach including psychological, social, medical, and family dimensions.

The Protoplanetary Discussions conference—held in Edinburgh, UK, from 2016 March 7th–11th—included several open sessions led by participants. This paper reports on the discussions collectively concerned with the multi-physics modelling of protoplanetary discs, including the self-consistent calculation of gas and dust dynamics, radiative transfer, and chemistry. After a short introduction to each of these disciplines in isolation, we identify a series of burning questions and grand challenges associated with their continuing development and integration. We then discuss potential pathways towards solving these challenges, grouped by strategical, technical, and collaborative developments. This paper is not intended to be a review, but rather to motivate and direct future research and collaboration across typically distinct fields based on community-driven input, to encourage further progress in our understanding of circumstellar and protoplanetary discs.

The effects of turbulence on the dynamics of dust grains in protoplanetary discs is of relevant importance in the study of pre-planetesimal formation. The complex interplay between gas and dust and the modelling of turbulence require numerical simulations.
A statistical study of the noise in SPH simulations of gas-only protoplanetary accretion discs is performed in order to determine if it could mimic turbulence and to what extent.

To form meter-sized pre-planetesimals in protoplanetary discs, dust aggregates have to decouple from the gas at a distance far enough from the central star so they are not accreted. Dust grains are affected by gas drag, which results in a vertical settling towards the mid-plane, followed by radial migration. To have a better understanding of the influence of growth on the dust dynamics, we use a simple grain growth model to determine the dust distribution in observed discs. We implement a constant growth rate into a gas+dust hydrodynamics SPH code and vary the growh rate to study the resulting effect on dust distribution. The growth rate allows us to determine the relative importance between friction and growth.We show that depending on the growth rate, a range of dust distribution can result. For large enough growth rates, grains can decouple from the gas before being accreted onto the central star, thus contributing as planetary building rocks.

The science of extra-solar planets is one of the most rapidly changing areas of astrophysics and since 1995 the number of planets known has increased by almost two orders of magnitude. A combination of ground-based surveys and dedicated space missions has resulted in 560-plus planets being detected, and over 1200 that await confirmation. NASA's Kepler mission has opened up the possibility of discovering Earth-like planets in the habitable zone around some of the 100,000 stars it is surveying during its 3 to 4-year lifetime. The new ESA's Gaia mission is expected to discover thousands of new planets around stars within 200 parsecs of the Sun. The key challenge now is moving on from discovery, important though that remains, to characterisation: what are these planets actually like, and why are they as they are?
In the past ten years, we have learned how to obtain the first spectra of exoplanets using transit transmission and emission spectroscopy. With the high stability of Spitzer, Hubble, and large ground-based telescopes the spectra of bright close-in massive planets can be obtained and species like water vapour, methane, carbon monoxide and dioxide have been detected. With transit science came the first tangible remote sensing of these planetary bodies and so one can start to extrapolate from what has been learnt from Solar System probes to what one might plan to learn about their faraway siblings. As we learn more about the atmospheres, surfaces and near-surfaces of these remote bodies, we will begin to build up a clearer picture of their construction, history and suitability for life.
The Exoplanet Characterisation Observatory, EChO, will be the first dedicated mission to investigate the physics and chemistry of Exoplanetary Atmospheres. By characterising spectroscopically more bodies in different environments we will take detailed planetology out of the Solar System and into the Galaxy as a whole.
EChO has now been selected by the European Space Agency to be assessed as one of four M3 mission candidates.

We present the first results of the treatment of grain growth in our 3D, two-fluid (gas+dust) SPH code describing protoplanetary disks. We implement a scheme able to reproduce the variation of grain sizes caused by a variety of physical processes and test it with the analytical expression of grain growth given by Stepinski & Valageas (1997) in simulations of a typical T Tauri disk around a one solar mass star. The results are in agreement with a turbulent growing process and validate the method. We are now able to simulate the grain growth process in a protoplanetary disk given by a more realistic physical description, currently under development. We discuss the implications of the combined effect of grain growth and dust vertical settling and radial migration on subsequent planetesimal formation.

We investigate the behaviour of dust in protoplanetary disks under the action of gas drag using our 3D, two-fluid (gas+dust) SPH code. We present the evolution of the dust spatial distribution in global simulations of planetless disks as well as of disks containing an already formed planet. The resulting dust structures vary strongly with particle size and planetary gaps are much sharper than in the gas phase, making them easier to detect with ALMA than anticipated. We also find that there is a range of masses where a planet can open a gap in the dust layer whereas it doesn't in the gas disk. Our dust distributions are fed to the radiative transfer code MCFOST to compute synthetic images, in order to derive constraints on the settling and growth of dust grains in observed disks.

From the rotational variation of UV resonance lines of carbon, nitrogen, and oxygen, we study the irregular distribution of these elements upon the surface of two magnetic Ap stars: HD 18296 (21 Per) and HD 25823 (41 Tau).
