  • Prevalence and treatment of perinatal anxiety: diagnostic interview study

    Anxiety affects around one in five women during pregnancy and after birth. However, there is no systematic information on the proportion of women with perinatal anxiety disorders who want or receive treatment.

    Aims

    To examine (a) the prevalence of anxiety disorders during pregnancy and after birth in a population-based sample, and (b) the proportion of women with anxiety disorders who want treatment and receive treatment.

    Method

    This study conducted 403 diagnostic interviews in early pregnancy (n = 102), mid-pregnancy (n = 99), late pregnancy (n = 102) or postpartum (n = 100). Participants also completed self-report measures of previous/current mental health problems and desire for treatment at every time point.

    Results

    The prevalence of anxiety disorders over all time points combined was 19.9% (95% CI 16.1–24.1), with greatest prevalence in early pregnancy (25.5%, 95% CI 17.4–35.1). The most prevalent disorders were obsessive–compulsive disorder (8.2%, 95% CI 5.7–11.3) and generalised anxiety disorder (5.7%, 95% CI 3.7–8.4). The majority of women with anxiety disorders did not want professional help or treatment (79.8%). Most women with anxiety disorders who did want treatment (20.2%) were receiving treatment. The majority of participants with anxiety disorders had a history of mental health problems (64.6%).

    Conclusions

    Prevalence rates overall are consistent with previous research, lending validity to the findings. However, findings challenge the assumption that everyone with a psychological disorder wants treatment. These findings highlight the importance of relationship-based care, where individual needs and contextual barriers to treatment can be explored.

  • Limited analyses based on national samples have assessed whether early attention-deficit/hyperactivity disorder (ADHD) symptoms predict later internalizing and externalizing symptoms in youth and the influence of sex and pubertal timing on subsequent psychiatric symptoms. This study analyzed data (n = 2818) from the Environmental influences on Child Health Outcomes Program national cohort. Analyses used data from early childhood (mean age = 5.3 years) utilizing parent-reported ADHD symptoms to predict rates of internalizing and externalizing symptoms from late childhood/adolescence (mean age = 11.9 years). Within a subsample age at peak height velocity (APHV) acted as a proxy to assess pubertal timing from early childhood (mean age = 5.4 years) to adolescence (mean age = 12.3 years). Early-childhood ADHD symptoms predicted later psychiatric symptoms, including anxiety, depression, aggressive behavior, conduct problems, oppositional defiant disorder, and rule-breaking behavior. Earlier APHV was associated with increased Conduct Disorder symptoms from late childhood to adolescence for females only. A stronger relation between ADHD symptoms and later aggression was observed in females with earlier APHV, whereas this same pattern with aggression, conduct problems and depression was observed in males with later APHV. Clinicians should consider that both young girls and boys with elevated ADHD symptoms, particularly with off-set pubertal timing, may be at risk for later psychiatric symptoms.

    Anxiety in pregnancy and after giving birth (the perinatal period) is highly prevalent but under-recognised. Robust methods of assessing perinatal anxiety are essential for services to identify and treat women appropriately.

    Aims

    To determine which assessment measures are most psychometrically robust and effective at identifying women with perinatal anxiety (primary objective) and depression (secondary objective).

    Method

    We conducted a prospective longitudinal cohort study of 2243 women who completed five measures of anxiety and depression (Generalized Anxiety Disorder scale (GAD) two- and seven-item versions; Whooley questions; Clinical Outcomes in Routine Evaluation (CORE-10); and Stirling Antenatal Anxiety Scale (SAAS)) during pregnancy (15 weeks, 22 weeks and 31 weeks) and after birth (6 weeks). To assess diagnostic accuracy a sample of 403 participants completed modules of the Mini-International Neuropsychiatric Interview (MINI).

    Results

    The best diagnostic accuracy for anxiety was shown by the CORE-10 and SAAS. The best diagnostic accuracy for depression was shown by the CORE-10, SAAS and Whooley questions, although the SAAS had lower specificity. The same cut-off scores for each measure were optimal for identifying anxiety or depression (SAAS ≥9; CORE-10 ≥9; Whooley ≥1). All measures were psychometrically robust, with good internal consistency, convergent validity and unidimensional factor structure.

    Conclusions

    This study identified robust and effective methods of assessing perinatal anxiety and depression. We recommend using the CORE-10 or SAAS to assess perinatal anxiety and the CORE-10 or Whooley questions to assess depression. The GAD-2 and GAD-7 did not perform as well as other measures and optimal cut-offs were lower than currently recommended.

  • Summary

    Introduction

    Older adults have been thrown into the spotlight of the COVID-19 pandemic and the bright lights have exposed both societies’ admirable and deplorable traits. We have seen stories of heart-warming compassion and deep-rooted ageism. From the appalling #boomerremover hashtag to the calls for mandatory quarantines for those over 70 years of age, public responses to COVID-19 demonstrate the role of age and (dis)ability in amplifying social and spatial inequalities. Although these reactions are unfounded, unethical, and have not received widespread political support, they do highlight the distressing interrelation of several truths: society at large is aging; older adults are at higher risk for developing more serious complications from COVID-19; and the social and physical infrastructure of cities has not been built to support the needs of older adults. In addition to the risks of COVID-19, the confluence of these three realities has potentially exacerbated a second public health crisis: loneliness. And as in the case of COVID-19, older adults are particularly susceptible. In this chapter we examine the relationship between COVID-19, social distance, social isolation, and loneliness with a focus on the older adult experience in urban and suburban environments. In addition to outlining the risks faced by older adults in times of crisis, we explore opportunities to strengthen social bonds while physically distancing through the development of blended communities or virtual retirement villages. Using the experience of the Oakridge Seniors Association in suburban Calgary, we offer targeted recommendations for community leaders and policy makers on how to minimize risk and maximize social cohesion by embracing communication technology while remembering the importance of human interaction. (Chapters Eleven and Twelve also explore the theme of selforganization in the face of the pandemic, but from the perspective of different national contexts and social categories.)

    Loneliness, social isolation, and social distancing

    We should differentiate between loneliness, social isolation, and social distancing. Loneliness is a subjective description of a person's emotional state, whereas social isolation is an objective measure of a person's lack of interaction with other people. Social distancing, as we have experienced during the COVID-19 pandemic, refers to limiting human interaction to curb the spread of a virus. By taking part in social distancing and significantly reducing physical interactions with others, many people are becoming increasingly socially isolated.

Shakespeare and Quotation

  • Shakespeare is the most frequently quoted English author of all time. Quotations appear everywhere, from the epigraphs of novels to the mottoes on coffee cups. But Shakespeare was also a frequent quoter himself - of classical and contemporary literature, of the Bible, of snatches of popular songs and proverbs. This volume brings together an international team of scholars to trace the rich history of quotation from Shakespeare's own lifetime to the present day. Exploring a wide range of media, including Romantic poetry, theatre criticism, novels by Jane Austen, Thomas Hardy and Ian McEwan, political oratory, propaganda, advertising, drama, film and digital technology, the chapters draw fresh connections between Shakespeare's own practices of creative reworking and the quotation of his work in new and traditional forms. Richly illustrated and featuring an Afterword by Margreta de Grazia, the collection tells a new story of the making and remaking of Shakespeare's plays and poems.

  • A Student Teacher's Personal Pathway to Education for Sustainability

  • Increasing attention is being given to the inclusion of Education for Sustainability in pre-service teacher education. However, there is little research to explain why and how early-career teachers include Education for Sustainability in their work, or how teacher education providers can encourage and support them to do this. Through analysis of two interviews this paper examines the way in which a pre-service primary school teacher interprets her role in Education for Sustainability. Her personal reflections on the manner in which her life experiences, including her own schooling, have influenced her intentions as teacher are explored. Her university teacher education program in Education for Sustainability and her teaching internship experience also appear to have influenced her teaching goals. The profile presented in this paper outlines her intentions and sense of identity as teacher and how that has developed and is expressed. Conclusions centre on the relevance of her reflections for education for sustainability in pre-service teacher education. In particular, her example demonstrates how the development of pedagogical content knowledge relevant to Education for Sustainability during the pre-service years can play an integral part in an individual's decision-making when teaching.
