Diabetes Mellitus (DM) represents a significant public health concern with a global impact(1,2). According to the International Diabetes Federation, in 2021 the global prevalence of this pathology was 10.5% and 12.7% in Chile(3). Of these cases, 90% are cases of type 2 Diabetes Mellitus (DM2). One of the risk factors for its management and prognosis, once diagnosed, is a sedentary lifestyle(3). A sedentary lifestyle rate of 86.7% has been reported in Chile(4). However, to our knowledge, there is no data on sedentary lifestyles in the diabetic population of the commune of Copiapó. Therefore, this study aimed to describe and compare the level of sedentary lifestyle in women and men with DM2 in the Family Health Centres (CESFAM) of Copiapó (Chile).

A cross-sectional study was conducted using data obtained from participants in the CODIACO cohort. Twenty-five individuals, comprising both sexes and aged between 30 and 65 years, diagnosed with DM2 and users of the CESFAMs of Copiapó, were included. These individuals also had mobile devices capable of installing the accelerometry application ‘Physics Toolbox Sensor Suite’. Pregnant and lactating women, patients with diabetic neuropathy or nephropathy, cancer, and severe inflammatory or infectious diseases were excluded. They were asked to activate and deactivate the application for 7 to 12 days, at waking and bedtime, respectively. Physical activity and sedentarism were determined when the accelerometer was over or below 0.6 mm/sec2 respectively from the total daily activity (%). WhatsApp transmitted the information recorded to estimate each participant’s sedentary lifestyle level. A Student’s t-test for independent samples was employed to compare the percentage of sedentary behaviour between women and men. The results were expressed as the mean and standard deviation. A P-value of less than 0.05 was considered statistically significant. The CODIACO study was approved by the Scientific Research Ethics Committee of the Universidad de Atacama. The number of women in the sample was 19 (76%) and the number of men was 6 (24%). The values of sedentary lifestyle for women were 85.32±9.5%, which was similar to those found in men, 82.5±10.31%. Although women show a higher tendency to increase sedentary lifestyle behaviour, The T-test did not determine differences between sexes (p>0.05), maybe, due to the high number of women compared with men in the sample.

The level of sedentary lifestyle in patients diagnosed with DM2 in the CESFAM of Copiapó is high and is in line with that observed in the general population in Chile. Furthermore, no differences are observed between men and women. Further studies are required to confirm this result and to investigate its cause to generate strategies to help reduce sedentary lifestyles in this population.

Changing practice patterns caused by the pandemic have created an urgent need for guidance in prescribing stimulants using telepsychiatry for attention-deficit hyperactivity disorder (ADHD). A notable spike in the prescribing of stimulants accompanied the suspension of the Ryan Haight Act, allowing the prescribing of stimulants without a face-to-face meeting. Competing forces both for and against prescribing ADHD stimulants by telepsychiatry have emerged, requiring guidelines to balance these factors. On the one hand, factors weighing in favor of increasing the availability of treatment for ADHD via telepsychiatry include enhanced access to care, reduction in the large number of untreated cases, and prevention of the known adverse outcomes of untreated ADHD. On the other hand, factors in favor of limiting telepsychiatry for ADHD include mitigating the possibility of exploiting telepsychiatry for profit or for misuse, abuse, and diversion of stimulants. This Expert Consensus Group has developed numerous specific guidelines and advocates for some flexibility in allowing telepsychiatry evaluations and treatment without an in-person evaluation to continue. These guidelines also recognize the need to give greater scrutiny to certain subpopulations, such as young adults without a prior diagnosis or treatment of ADHD who request immediate-release stimulants, which should increase the suspicion of possible medication diversion, misuse, or abuse. In such cases, nonstimulants, controlled-release stimulants, or psychosocial interventions should be prioritized. We encourage the use of outside informants to support the history, the use of rating scales, and having access to a hybrid model of both in-person and remote treatment.

Innovations in horse equipment during the early Middle Ages provided advantages to societies from the steppes, reshaping the social landscape of Eurasia. Comparatively little is known about the precise origin of these crucial advances, although the available evidence points to early adoption in East Asia. The authors present new archaeological discoveries from western and northern Mongolia, dating to the fourth and fifth centuries AD, including a wooden frame saddle with horse hide components from Urd Ulaan Uneet and an iron stirrup from Khukh Nuur. Together, these finds suggest that Mongolian groups were early adopters of stirrups and saddles, facilitating the expansion of nomadic hegemony across Eurasia and shaping the conduct of medieval mounted warfare.

The Eighth World Congress of Pediatric Cardiology and Cardiac Surgery (WCPCCS) will be held in Washington DC, USA, from Saturday, 26 August, 2023 to Friday, 1 September, 2023, inclusive. The Eighth World Congress of Pediatric Cardiology and Cardiac Surgery will be the largest and most comprehensive scientific meeting dedicated to paediatric and congenital cardiac care ever held. At the time of the writing of this manuscript, The Eighth World Congress of Pediatric Cardiology and Cardiac Surgery has 5,037 registered attendees (and rising) from 117 countries, a truly diverse and international faculty of over 925 individuals from 89 countries, over 2,000 individual abstracts and poster presenters from 101 countries, and a Best Abstract Competition featuring 153 oral abstracts from 34 countries. Acknowledging the need for urgent change, we wanted to take the opportunity to bring a common voice to the global community and issue the Washington DC WCPCCS Call to Action on Addressing the Global Burden of Pediatric and Congenital Heart Diseases. A copy of this Washington DC WCPCCS Call to Action is provided in the Appendix of this manuscript. This Washington DC WCPCCS Call to Action is an initiative aimed at increasing awareness of the global burden, promoting the development of sustainable care systems, and improving access to high quality and equitable healthcare for children with heart disease as well as adults with congenital heart disease worldwide.

One of the most important aspects of genetic evaluation (GE) is the definition of contemporary groups (CG), commonly defined as animals of the same sex born in the same herd, year and season. The objective of this study was to use an aridity index (AI) to classify season and evaluate the implications on the GE of Braunvieh cattle. A data set with 32 777 and 22 448 birth weight (BW) and weaning weight adjusted to 240 days (WW) records, respectively, was used to compare two methods of classification of climatic seasons to be used in the definition of CG for GE models. The first method considered rain season criterion (RC), and the second method is a proposed classification using an AI. Both methods were compared using two approaches. The first approach examined differences in mixed models using the RC and AI season to select the best model for BW and WW, evaluated by different goodness of fit measures. The second approach considered fitting a GE model including the season classifications into the CG structure. Lower probability values for season effect and better goodness of fit measures were obtained when the season was classified according to the AI. Results showed that although differences are small, the AI allows a better model fitting for live-weight traits than RC and revealed a re-ranking effect on expected progeny differences data. Further analysis with other traits would demonstrate the extended utility of AI indicators to be considered for fitting models under a climatic change environment.

The Mountain West Clinical Translational Research – Infrastructure Network (MW CTR-IN), established in 2013, is a research network of 13 university partners located among seven Institutional Development Award (IDeA) states targeting health disparities. This is an enormous undertaking because of the size of the infrastructure network (encompassing a third of the US landmass and spanning four time zones in predominantly rural and underserved areas, with populations that have major health disparities issues). In this paper, we apply the barriers, strategies, and metrics to an adapted educational conceptual model by Fink (2013). Applying this model, we used four tailored approaches across this regional infrastructure network to: (1) assess individual faculty specific needs, (2) reach out and engage with faculty, (3) provide customized services to meet the situational needs of faculty, and (4) utilize a “closed communication feedback loop” between Professional Development (PD) core and MW CTR-IN faculty within the context of their home institutional environment. Summary statement results from participating faculty show that these approaches were positive. Grounded in best educational practice approaches, we have an opportunity to refine and build from this sound foundation with implications for future use in other CTR-IN networks and institutions in the IDeA states.

Preterm infants show postnatal deficits of long-chain polyunsaturated fatty acids (LCPUFAs) which are essential for adequate growth and neurodevelopment. Human milk is a primary source of fatty acids (FAs) for the preterm infant, and therefore, knowledge about milk FA levels is required to design appropriate supplementation strategies. Here, we expanded on our previous study (Nilsson et al., 2018, Acta Paediatrica, 107, 1020–1027) determining FA composition in milk obtained from mothers of extremely low gestational age (<28 weeks) infants on three occasions during lactation. There was a clear difference in FA composition in milk collected at Day 7 and milk collected at postmenstrual weeks (PMW) 32 or PMW 40. Notably, the proportion of LCPUFAs was low and declined significantly during milk maturation. These results strengthen previous data that the content of FAs required by the preterm infant is not supplied in sufficient amounts when the mother’s own milk is the sole source of these essential nutrients.

The cosmic evolution of the chemical elements from the Big Bang to the present time is driven by nuclear fusion reactions inside stars and stellar explosions. A cycle of matter recurrently re-processes metal-enriched stellar ejecta into the next generation of stars. The study of cosmic nucleosynthesis and this matter cycle requires the understanding of the physics of nuclear reactions, of the conditions at which the nuclear reactions are activated inside the stars and stellar explosions, of the stellar ejection mechanisms through winds and explosions, and of the transport of the ejecta towards the next cycle, from hot plasma to cold, star-forming gas. Due to the long timescales of stellar evolution, and because of the infrequent occurrence of stellar explosions, observational studies are challenging, as they have biases in time and space as well as different sensitivities related to the various astronomical methods. Here, we describe in detail the astrophysical and nuclear-physical processes involved in creating two radioactive isotopes useful in such studies, $^{26}\mathrm{Al}$ and $^{60}\mathrm{Fe}$ . Due to their radioactive lifetime of the order of a million years, these isotopes are suitable to characterise simultaneously the processes of nuclear fusion reactions and of interstellar transport. We describe and discuss the nuclear reactions involved in the production and destruction of $^{26}\mathrm{Al}$ and $^{60}\mathrm{Fe}$ , the key characteristics of the stellar sites of their nucleosynthesis and their interstellar journey after ejection from the nucleosynthesis sites. This allows us to connect the theoretical astrophysical aspects to the variety of astronomical messengers presented here, from stardust and cosmic-ray composition measurements, through observation of $\gamma$ rays produced by radioactivity, to material deposited in deep-sea ocean crusts and to the inferred composition of the first solids that have formed in the Solar System. We show that considering measurements of the isotopic ratio of $^{26}\mathrm{Al}$ to $^{60}\mathrm{Fe}$ eliminate some of the unknowns when interpreting astronomical results, and discuss the lessons learned from these two isotopes on cosmic chemical evolution. This review paper has emerged from an ISSI-BJ Team project in 2017–2019, bringing together nuclear physicists, astronomers, and astrophysicists in this inter-disciplinary discussion.

Rabies is endemic in Bangladesh. To identify risk factors, a case-control study was conducted based on hospital-reported rabid animal bite (RAB) cases in domestic ruminants, 2009 − 2018. RAB cases (n = 449) and three controls per case were selected. Dogs (87.8%) and jackals (12.2%) were most often identified as biting animals. In the final multivariable model, the risk of being a RAB case was significantly higher in cattle aged >0.5–2 years (odds ratio (OR) 2.89; 95% confidence interval (CI): 1.56–5.37), >2–5 years (OR 3.63; 95% CI: 1.97–6.67) and >5 years (OR 6.42; 95% CI: 3.39–12.17) compared to those aged <0.5 years. Crossbred cattle were at higher risk of being a RAB case (OR 5.48; 95% CI: 3.56–8.42) than indigenous. Similarly, female cattle were more likely to be a RAB case (OR 1.26; 95% CI: 1.15–2.29) than males. Cattle in rural areas (OR 39.48; 95% CI: 6.14–254.00) were at a much higher risk of being RAB cases than those in urban areas. Female, crossbred and older cattle, especially in rural areas should either be managed indoors during the dog breeding season (September and October) or vaccinated. A national rabies elimination program should prioritise rural dogs for mass vaccination. Jackals should also be immunised using oral bait vaccines. Prevention of rabies in rural dogs and jackals would also reduce rabies incidence in humans.

From November 2019 to April 2020, the prototypical red supergiant Betelgeuse experienced an unexpected and historic dimming. This event was observed worldwide by astrophysicists, and also by the general public with the naked eye. We present here the results of our observing campaign with ESO’s VLT and VLTI in the visible and infrared domains. The observations with VLT/SPHERE-ZIMPOL, VLT/SPHERE-IRDIS, VLTI/GRAVITY and VLTI/MATISSE provide spatially resolved diagnostics of this event. Using PHOENIX atmosphere models and RADMC3D dust radiative transfer simulations, we built a consistent model reproducing the images and the photometry.

Carbon-rich dust is known to form in the atmosphere of the semiregular variable star R Sculptoris. Such stardust, as well as the molecules and gas produced during the lifetime of the star, will be spread into the Galaxy via the mass-loss process. Probing this process is crucial to understand the chemical enrichment of the Galaxy. R Scl was observed using the ESO/VLTI MATISSE instrument in December 2018. Here we show the first images of the star between 3 and 10 R*. Using the complementary MIRA 3D image reconstruction and the RHAPSODY 1D intensity profile reconstruction code, we reveal the location of molecules and dust in the close environment of the star. Indeed, the C2H2 and HCN molecules are spatially located between 1 and 3.4 R* which is much closer to the star than the location of the dust. The R Scl spectrum is fitted by molecules and a dust mixture of 90% of amorphous carbon and 10% of silicone carbide. The inner boundary of the dust envelope is estimated by DUSTY at about 4.6 R*. We derive a mass-loss rate of 1.2 ± 0.4 × 10−6M⊙ yr−1however no clear SiC forming region has been detected in the MATISSE data.

Psychosocial stress in childhood and adolescence is linked to stress system dysregulation, although few studies have examined the relative impacts of parental harshness and parental disengagement. This study prospectively tested whether parental harshness and disengagement show differential associations with overall cortisol output in adolescence. Associations between overall cortisol output and adolescent mental health problems were tested concurrently. Adolescents from the Fragile Families and Child Wellbeing Study (FFCWS) provided hair samples for cortisol assay at 15 years (N = 171). Caregivers reported on parental harshness and disengagement experiences at 1, 3, 5, 9, and 15 years, and adolescents reported at 15 years. Both parent and adolescent reported depressive and anxiety symptoms and antisocial behaviors at 15. Greater parental harshness from 1–15 years, and harshness reported at 15 years in particular, was associated with higher overall cortisol output at 15. Greater parental disengagement from 1–15 years, and disengagement at 1 year specifically, was associated with lower cortisol output. There were no significant associations between cortisol output and depressive symptoms, anxiety symptoms, or antisocial behaviors. These results suggest that the unique variances of parental harshness and disengagement may have opposing associations with cortisol output at 15 years, with unclear implications for adolescent mental health.
