Psychologists and psychological research have shaped sleep and circadian science for over a century. Yet, psychology has not fully embraced sleep as a core area of inquiry, and sleep medicine has not distinctly acknowledged the foundational role psychology plays in understanding sleep and circadian rhythms. This Question Paper invites submissions exploring psychology’s profound impact on the study, measurement and intervention strategies in sleep and circadian science, as well as reciprocal influences. Manuscripts may include historiographies of key contributors, laboratory milestones, theoretical advancements and methodological innovations within a historical context. We aim to capture the full scope of sleep psychology from its origins to a vision of its future.

Levofloxacin prophylaxis reduces bloodstream infections in neutropenic patients with acute myeloid leukemia or relapsed acute lymphoblastic leukemia. A retrospective, longitudinal cohort study compares incidence of bacteremia, multidrug-resistant organisms (MDRO), and Clostridioides difficile (CDI) between time periods of levofloxacin prophylaxis implementation. Benefits were sustained without increasing MDRO or CDI.

Food cravings are one of several important complexities between psychological and physiological triggers for food consumption. Cravings are commonly cited as contributing to over-consumption of hyperpalatable foods (sugary, salty, and fatty foods) and may be causal in obesity(1). The Mediterranean dietary pattern (MedDiet) is linked to reduced disease risk and improved health and wellbeing(2). Despite a lower intake of sugary and salty foods compared to a Western diet, free-living adults switching to the MedDiet find it satiating and achieve high adherence in Western countries. The MedDiet is known to improve mood and wellbeing, is high in fibre, monounsaturated fat and low in added sugar, and has a low glycaemic load, which could separately and synergistically reduce food cravings. The relationship between adherence to the MedDiet and food cravings has never been investigated. In the MedLey randomised controlled trial, we investigated the effects of a MedDiet on food cravings, compared with a habitual Australian diet (HabDiet)(3). Adherence to the MedDiet was scored out of 15 (maximum adherence). Participants completed three food cravings questionnaires at baseline and 6-months. The State questionnaire measures momentary cravings and has a maximum score of 75, indicating maximum food cravings. The Trait-reduced questionnaire measures general cravings and has a maximum score of score of 126, indicating more frequent and intense cravings for foods. The Food Cravings Inventory (FCI) measures cravings for four food domains: fatty foods, fast foods, sugary foods, and high carbohydrate (CHO) foods. MedDiet group (n = 58) responses were compared with the HabDiet group (n = 53) across visits using linear mixed effects modelling. Predicted differences were obtained for adherence scores of ≤8 (median adherence) and ≥9. Means ± SD or CIs are presented. Mean adherence increased from 7.1 ± 1.8 to 10.7 ± 1.48 in the MedDiet group (P<0.01), with no change in the HabDiet group (P = 1.00). Trait-reduced scores were not significantly different between groups at 6-months (P = 0.11), although there was a 5.57-point reduction within the MedDiet group (CI −12.56, −1.96, P = 0.04). State score was significantly lower in the MedDiet group than the HabDiet at 6-months (−4.4 (CI −7.53, −0.39), P = 0.03), and was significantly lower than at baseline (−5.9 (CI −9.33, −0.24,) P = 0.04). There were no differences between groups for the four domains of the FCI (P>0.05). Cravings for sugary foods was significantly reduced within the MedDiet group (−0.26 (CI −0.46, −0.05) P = 0.01). The predictive modelling suggested moving from an adherence score of 8 to 9 was associated with lower cravings for sugar (−0.03 ± 0.01, P = 0.03), fast food (−0.04 ± 0.02, P = 0.02) and CHO foods (−0.05 ± 0.02, P = 0.02). These results are suggestive that higher adherence to a MedDiet could reduce cravings compared to the Australian diet and suggest that the MedDiet may specifically reduce sugar cravings. Further investigation is warranted, through observational and intervention trials.

Land-use change for crop production is one of the key drivers of habitat loss and fragmentation and consequently biodiversity loss and change in tropical regions. This may impact biodiversity-regulated ecosystem services; birds are important to crop health regulating services (e.g. seed dispersal, pest control) and disservices (e.g. seed predation, grain herbivory). However, knowledge is limited on how birds use heterogeneous agricultural landscapes and the consequences for spatial distribution and flow of services and disservices. We studied crop and non-crop–habitat associations of birds in forest–agricultural landscapes of the Kilombero Valley, Tanzania. We focused on dietary preference as a key trait impacting bird responses to land-use change, services, and disservices to crops. We surveyed birds across four main habitat types using repeated point counts, recording a total of 148 species. We found that crop habitats supported higher species richness and larger communities of potentially beneficial species to crop health, whereby 34.5% of invertebrate-feeding species were recorded in cropland. We found that habitat heterogeneity within the landscape supports bird functional diversity and that each habitat type supported unique communities of species. Furthermore, the number of species unique to forest habitats increased with increasing forest canopy closure. Our findings suggest that management strategies for maintaining trees and shrubs, and enhancing tree cover within the crop production landscape, can be effective approaches for maintaining bird diversity and services. However, in-depth studies on trade-offs with disservices need further exploration to mitigate negative impacts of birds on crop yields.

A pharmacist-driven protocol for methicillin-resistant Staphylococcus aureus nares screening and empiric vancomycin discontinuation was instituted in a community healthcare system utilizing a tele-antimicrobial stewardship program to reduce inappropriate use of vancomycin. The protocol and associated intervention resulted in a significant decrease in both vancomycin utilization and the rate of acute kidney injury.

The personalised oncology paradigm remains challenging to deliver despite technological advances in genomics-based identification of actionable variants combined with the increasing focus of drug development on these specific targets. To ensure we continue to build concerted momentum to improve outcomes across all cancer types, financial, technological and operational barriers need to be addressed. For example, complete integration and certification of the ‘molecular tumour board’ into ‘standard of care’ ensures a unified clinical decision pathway that both counteracts fragmentation and is the cornerstone of evidence-based delivery inside and outside of a research setting. Generally, integrated delivery has been restricted to specific (common) cancer types either within major cancer centres or small regional networks. Here, we focus on solutions in real-world integration of genomics, pathology, surgery, oncological treatments, data from clinical source systems and analysis of whole-body imaging as digital data that can facilitate cost-effectiveness analysis, clinical trial recruitment, and outcome assessment. This urgent imperative for cancer also extends across the early diagnosis and adjuvant treatment interventions, individualised cancer vaccines, immune cell therapies, personalised synthetic lethal therapeutics and cancer screening and prevention. Oncology care systems worldwide require proactive step-changes in solutions that include inter-operative digital working that can solve patient centred challenges to ensure inclusive, quality, sustainable, fair and cost-effective adoption and efficient delivery. Here we highlight workforce, technical, clinical, regulatory and economic challenges that prevent the implementation of precision oncology at scale, and offer a systematic roadmap of integrated solutions for standard of care based on minimal essential digital tools. These include unified decision support tools, quality control, data flows within an ethical and legal data framework, training and certification, monitoring and feedback. Bridging the technical, operational, regulatory and economic gaps demands the joint actions from public and industry stakeholders across national and global boundaries.

The late Pleistocene to Holocene subaerial pyroclastic deposits of the Quill stratovolcano on the Caribbean island of St Eustatius form seven stratigraphic divisions. New radiocarbon ages of charcoal are presented for the second, third and seventh divisions in order to better constrain the Quill’s eruption history. Three samples from the same layer of Division 2 at two localities on the northeast coast yield ages of 18,020 ± 40 (1σ), 18,310 ± 45 and 18,490 ± 45 14C yr BP (∼19,800–20,600 yr cal BC). These are considerably younger (∼4400 yr) than a previously published result for this division. A single sample of Division 3 gave an age of 8090 14C yr BP (∼7100 yr cal BC) and overlaps with previously published 14C ages for this division. A charred root in the pyroclastic unit deposited by the last eruption of the Quill (Division 7) gave an age of 919 14C yr BP (∼1100–1200 yr cal AD). This result is ∼600 years younger than a previously published age, and its origin is attributed to human activity. The timing of the last eruption of the Quill therefore remains poorly constrained but is older than 600 AD. Terrestrial gastropods found in paleosols and organic material found in small streams that developed in Division 3 indicate that Division 4 must be younger than 6100 ± 500 yr cal BC. The oxygen and carbon isotope composition of the terrestrial gastropods derived from Division 3 paleosols indicates that the C4 and CAM-type vegetation was dominant and that the climate subsequently changed to wetter conditions. The minimum eruption frequency for the Quill volcano is one eruption every ∼1400 years during the past 22,000 years. This eruption frequency of the Quill volcano is of the same order of magnitude as other recent northern Lesser Antilles volcanoes, Soufrière Hills (Montserrat, ∼5000 years) and Mt Liamuiga (St. Kitts, ∼2500 years).

The NIH National Center for Advancing Translational Science (NCATS) was established to support translational research that spans the entire TS Continuum, with the goal of bridging the gap between preclinical biomedical research and real-world applications to advance treatments to patients more quickly. In 2018, the Translational Science Training (TST) TL1 Program at the University of Texas Health Science Center at San Antonio implemented new strategies to better include and encourage research more broadly across the TS Continuum, including the addition of postdoctoral scientists and a clinically trained Program Co-Director, expansion of team science and community engagement programming, and targeted trainee recruitment from schools of nursing, dentistry, and allied health, in addition to medicine. The objective of this bibliometric analysis was to determine if the program exhibited a more diverse mix of T-types after the adjustments made in 2018. The TST/TL1 Program experienced a shift in T-type, from mostly T0 (preclinical) to more T3/T4 (clinical implementation/public health) research, after new strategies were implemented. This supports the conclusion that strategic programmatic adjustments by an NCATS-funded predoctoral training program resulted in outcomes that better align with NCATS priorities to develop Trainees who contribute across the entire TS Continuum.

As the scale of cosmological surveys increases, so does the complexity in the analyses. This complexity can often make it difficult to derive the underlying principles, necessitating statistically rigorous testing to ensure the results of an analysis are consistent and reasonable. This is particularly important in multi-probe cosmological analyses like those used in the Dark Energy Survey (DES) and the upcoming Legacy Survey of Space and Time, where accurate uncertainties are vital. In this paper, we present a statistically rigorous method to test the consistency of contours produced in these analyses and apply this method to the Pippin cosmological pipeline used for type Ia supernova cosmology with the DES. We make use of the Neyman construction, a frequentist methodology that leverages extensive simulations to calculate confidence intervals, to perform this consistency check. A true Neyman construction is too computationally expensive for supernova cosmology, so we develop a method for approximating a Neyman construction with far fewer simulations. We find that for a simulated dataset, the 68% contour reported by the Pippin pipeline and the 68% confidence region produced by our approximate Neyman construction differ by less than a percent near the input cosmology; however, they show more significant differences far from the input cosmology, with a maximal difference of 0.05 in $\Omega_{M}$ and 0.07 in w. This divergence is most impactful for analyses of cosmological tensions, but its impact is mitigated when combining supernovae with other cross-cutting cosmological probes, such as the cosmic microwave background.

Background: The Canadian Registry for Amyloidosis Research (CRAR) is a nationwide disease registry of transthyretin (ATTR) and light-chain (AL) amyloidosis. Recent advances in disease-modifying therapy have improved prognosis, however there is a critical need for real-world evidence to address knowledge gaps, particularly longer-term therapeutic outcomes and surveillance strategies. Methods: A multi-stakeholder process was undertaken to develop a consensus dataset for ATTR- and AL-amyloidosis. This process included surveys to rank the importance of potential data items, and a consensus meeting of the CRAR steering committee, (comprised of multidisciplinary clinical experts, and patient organization representatives). Patients and patient organizations supported the development and implementation of a patient-reported dataset. Results: Consensus data items include disease onset, progression, severity, treatments, and outcomes, as well as patient-reported outcomes. Both prospective and retrospective (including deceased) patient cohorts are included. Further baseline data will be presented on an initial cohort of patients. Conclusions: CRAR has been established to collect a longitudinal, multidisciplinary dataset that will evaluate amyloidosis care and outcomes. CRAR has launched at multiple specialty amyloidosis centers nationally and is continually expanding. The growth of this program will promote opportunities to assess real-world safety and efficacy and inform the cost-effectiveness of therapies while supporting patient recruitment for research.

OBJECTIVES/GOALS: Supported by the State of Alabama, the Alabama Genomic Health Initiative (AGHI) is aimed at preventing and treating common conditions with a genetic basis. This joint UAB Medicine-HudsonAlpha Institute for Biotechnology effort provides genomic testing, interpretation, and counseling free of charge to residents in each of Alabama’s 67 counties. METHODS/STUDY POPULATION: Launched in 2017, as a state-wide population cohort, AGHI (1.0) enrolled 6,331 Alabamians and returned individual risk of disease(s) related to the ACMG SF v2.0 medically actionable genes. In 2021, the cohort was expanded to include a primary care cohort. AGHI (2.0) has enrolled 750 primary care patients, returning individual risk of disease(s) related to the ACMG SF v3.1 gene list and pre-emptive pharmacogenetics (PGx) to guide medication therapy. Genotyping is done on the Illumina Global Diversity Array with Sanger sequencing to confirm likely pathogenic / pathogenic variants in medically actionable genes and CYP2D6 copy number variants using Taqman assays, resulting in a CLIA-grade report. Disease risk results are returned by genetic counselors and Pharmacogenetics results are returned by Pharmacists. RESULTS/ANTICIPATED RESULTS: We have engaged a statewide community (>7000 participants), returning 94 disease risk genetic reports and 500 PGx reports. Disease risk reports include increased predisposition to cancers (n=38), cardiac diseases (n=33), metabolic (n=12), other (n=11). 100% of participants harbor an actionable PGx variant, 70% are on medication with PGx guidance, 48% harbor PGx variants and are taking medications affected. In 10% of participants, pharmacists sent an active alert to the provider to consider/ recommend alternative medication. Most commonly impacted medications included antidepressants, NSAIDS, proton-pump inhibitors and tramadol. To enable the EMR integration of genomic information, we have developed an automated transfer of reports into the EMR with Genetics Reports and PGx reports viewable in Cerner. DISCUSSION/SIGNIFICANCE: We share our experience on pre-emptive implementation of genetic risk and pharmacogenetic actionability at a population and clinic level. Both patients and providers are actively engaged, providing feedback to refine the return of results. Real time alerts with guidance at the time of prescription are needed to ensure future actionability and value.

Gaps in the implementation of effective interventions impact nearly all cancer prevention and control strategies in the US including Massachusetts. To close these implementation gaps, evidence-based interventions must be rapidly and equitably implemented in settings serving racially, ethnically, socioeconomically, and geographically diverse populations. This paper provides a brief overview of The Implementation Science Center for Cancer Control Equity (ISCCCE) and describes how we have operationalized our commitment to a robust community-engaged center that aims to close these gaps. We describe how ISCCCE is organized and how the principles of community-engaged research are embedded across the center. Principles of community engagement have been operationalized across all components of ISCCCE. We have intentionally integrated these principles throughout all structures and processes and have developed evaluation strategies to assess whether the quality of our partnerships reflects the principles. ISCCCE is a comprehensive community-engaged infrastructure for studying efficient, pragmatic, and equity-focused implementation and adaptation strategies for cancer prevention in historically and currently disadvantaged communities with built-in methods to evaluate the quality of community engagement. This engaged research center is designed to maximize the impact and relevance of implementation research on cancer control in community health centers.

We examine the redshifts of a comprehensive set of published Type Ia supernovae, and provide a combined, improved catalogue with updated redshifts. We improve on the original catalogues by using the most up-to-date heliocentric redshift data available; ensuring all redshifts have uncertainty estimates; using the exact formulae to convert heliocentric redshifts into the Cosmic Microwave Background (CMB) frame; and utilising an improved peculiar velocity model that calculates local motions in redshift-space and more realistically accounts for the external bulk flow at high-redshifts. We review 2607 supernova redshifts; 2285 are from unique supernovae and 322 are from repeat-observations of the same supernova. In total, we updated 990 unique heliocentric redshifts, and found 5 cases of missing or incorrect heliocentric corrections, 44 incorrect or missing supernova coordinates, 230 missing heliocentric or CMB frame redshifts, and 1200 missing redshift uncertainties. The absolute corrections range between $10^{-8} \leq \Delta z \leq 0.038$ , and RMS $(\Delta z) \sim 3{\times 10^{-3}}$ . The sign of the correction was essentially random, so the mean and median corrections are small: $4{\times 10^{-4}}$ and $4{\times 10^{-6}}$ respectively. We examine the impact of these improvements for $H_0$ and the dark energy equation of state w and find that the cosmological results change by $\Delta H_0 = -0.12\,\mathrm{km\,s}^{-1}\mathrm{Mpc}^{-1}$ and $\Delta w = 0.003$ , both significantly smaller than previously reported uncertainties for $H_0$ of 1.0 $\mathrm{km\,s}^{-1}\mathrm{Mpc}^{-1}$ and w of 0.04 respectively.
