  • Aims and method

    Serious incident management and organisational learning are international patient safety priorities. Little is known about the quality of suicide investigations and, in turn, the potential for organisational learning. Suicide risk assessment is acknowledged as a complex phenomenon, particularly in the context of adult community mental health services. Root cause analysis (RCA) is the dominant investigative approach, although the evidence base underpinning RCA is contested, with little attention paid to the patient in context and their cumulative risk over time.

    Results

    Recent literature proposes a Safety-II approach in response to the limitations of RCA. The importance of applying these approaches within a mental healthcare system that advocates a zero suicide framework, grounded in a restorative just culture, is highlighted.

    Clinical implications

    Although integrative reviews and syntheses have clear methodological limitations, this approach facilitates the management of a disparate body of work to advance a critical understanding of patient safety in adult community mental healthcare.

  • Summary

    This essay explores father figures who meditate on their relation to a dominant late nineteenth- early twentieth-century construction of active masculinity, or figures who often find themselves uncomfortably far from the era’s gender ideals. Seeing their own lives as “limited” and constrained, or akin to the era’s fears of masculine “overcivilization,” these figures find solace in an imagined trajectory of masculine accomplishment.  Their sons, however, appear to them as promising to restore the family’s depleted vigor, for the younger generation appears as more self-determined and more manly, especially in professional matters. Such meditations on manhood lead to a belief in the developmental potential of the next generation, which promises to supply a long-absent manliness. Not only are the young more active men, engaged in innovative fields that were not available to their elders, but they also seem poised to recover the kind of familial masculine distinction that their elders fear that they forfeited, both for themselves and for their families.

  • Up to 90% of people with severe traumatic brain injury (TBI) report dissatisfaction with the status of their leisure participation and/or social integration. Yet, there are virtually no studies that have investigated the benefits of interventions that target leisure specifically for this population. The purpose of this study was to determine whether the Leisure Education Program (Carbonneau, Fontaine, & Lussier, 2006) designed to assist people with stroke to engage in meaningful leisure activities, build leisure self-efficacy and promote general wellbeing would have similar benefits for survivors of TBI. We recruited three community-dwelling survivors of TBI to participate in a 10-week leisure program. All three participants reported some benefit in leisure satisfaction and self-efficacy. This extended to improvements in general wellbeing and health-related quality of life for two of the three. These findings suggest that further investigations into the benefits of leisure education for adults with TBI should be conducted.
