It is known that many psychopharmacological drugs have anti-inflammatory, as well as antibacterial and antiviral effects.

To investigate the association between the severity and duration criteria of COVID-19 with psychopharmacotherapy in double-diagnosed patients.

A total of 169 case histories from a specialized infectious psychiatric department (May 2020 to January 2021) were evaluated. Progression indicators of severe and mild COVID-19, along with the duration of persistent SARS-CoV-2 viral shedding, were assessed in correlation with the administration of antidepressants, antipsychotics, and acid sphingomyelinase inhibitors (FIASMA-active drugs).

The use of any psychotropic agents was associated with a 0.9% increase in the risk of severe course of COVID-19 for each unit increase in the systemic inflammation index PLR, specifically in patients with intellectual disability (ICD-10 codes F70-79), when compared to patients with schizophrenia (ICD-10 codes F20-29): R²McF=0.138; AIC=181; χ²=25.8; df=9; p=0.002. High PLR values and the use of FIASMA-active drugs were associated with prolonged COVID-19 duration, while antidepressant therapy and elevated C-reactive protein levels were associated with a reduced predicted duration of viral shedding in 13.8% of variance: R²=0.0864; AIC=1299; F=5.2(3), p=0.002. Including the nosology of psychiatric disorders in the regression model increased the proportion of explained variance to 22.8%.

Thymoanaleptic therapy for individuals with psychiatric disorders may act as a protective factor against COVID-19. There is no evidence suggesting adverse effects of antipsychotics on the severity and duration of COVID-19. Further research is necessary to investigate the effects of FIASMA-active psychopharmacological agents within nosologically homogeneous groups.

Because of different phenotypic approaches, data on depression prevalence is variable and controversial.

The aim was to evaluate the prevalence of different depressive phenotypes in the Russian population (DSM criteria based self-report vs HADS questionnaire).

The data was from the on-line survey of 5116 clients of Genotek Ltd. (males - 50,63%; age - Me=35 (Q1-30;Q3-42)). The survey included questions on sex, age; sel-report adapted major depression DSM-V criteria questionnaire and depression subscale of Hospital Anxiety and Depression Scale.

DSM Major depression phenotype was detected with moderately-high prevalence - 17,67% (N=904). The DSM depression phenotype was more prevalent in women (22,72%) compared to men (12,74%, p<0,001) and in younger individuals (10,18%, p<0,001) compared to older ones (6,16%). HADS-D clinical depression phenotype (score>11) was less prevalent (3,4%) with no significant differences for sex and age. However, the prevalence increased with HADS-D subclinical scores (>8) - 14,97%. HADS-D scores were higher in DSM-depression phenotype individuals compared to ones without DSM phenotype (5,822(3,221) vs. 3,893(2,437), p< 0,001).

Our results showed variable prevalence of depression with different phenotypic approaches. The differences could be associated with the clinical severity of the symptoms and the life-time evaluation in DSM compared to only current symptoms for HADS. Further research is needed to understand the factors affecting the phenotyping approaches and providing the most effective and valid instrument for depression prevalence evaluation. Examination of the relationships in population between vaccine attributes and COVID-19 personal experience, social and demographic characteristics

Cohort cross-sectional study of the population attitude to vaccination against coronavirus infection COVID-19 was performed online during the first 2 months of mass vaccination in Russia, using the special designed questionnaire assessing social demographic variables, COVID-19 related factors, and preferable sources of information about COVID-19 vaccines. 4977 participants in the age from 18 to 81 years were enrolled in the study to vaccination against coronavirus infection COVID-19 was performed online during the first 2 months of mass vaccination in Russia, using the special designed questionnaire assessing social demographic variables, COVID-19 related factors, and preferable sources of information about COVID-19 vaccines .

They indicated that they do not plan to be vaccinated. 32.3%. postpones their decision until more remote data on the results and effectiveness of vaccination are obtained ‑ 34.0%. were vaccinated at the time of the study ‑ 11.6%.

Attitude towards vaccination depends on age, gender, education, fear of possible complications, coronaphobia. Young people are less focused on vaccination than middle-aged and older people.

Depression is associated with a high risk of suicidal thoughts (ST) and behaviour (SB). Suicidality and depression have partially shared genetic underpinnings and family history of mood disorders (FH) can reflect genetic impact on specific features of depression. Thus, in depressed patients, FH may affect suicidality and its associations with other risk factors, such as personality traits.

We conducted a cross-sectional study to test the impact of FH on the association between suicidality and personality traits in depressed patients.

200 depression in- and outpatients (64% (n=128) women, mean age (M(SD):36,21(15,09)) were enrolled. 28% (n=56) reported FH (“FH+” cohort), other patients comprised the “FH-” cohort. Columbia-Suicide Severity Rating Scale (C-SSRS) was used to assess ST and SB during the most suicidal period of life. Personality traits were assessed by Cloninger Temperament and Character Inventory (TCI-125). Information about FH and history of suicide attempts (SA) was obtained during the clinical interview.

Personality traits and suicidality characteristics (ST, SB, SA) did not differ between FH+ and FH- patients. In FH+, no differences in TCI-125 scores between suicide attempters and non-attempters were found, while in FH-, attempters had higher scores of TCI-125 “Novelty seeking” (p=0.002) and “Self-transcendence” (p=0.031) subscales. Multiple correlations between ST, SB and TCI-125 subscales were found only in FH-, In FH+, only one correlation (between ST and TCI-125 “Persistence” subscale (r=-0.288, p=0.038) was found.

Our results showed a weakened association between personality traits and suicidality in depressed patients with FH of mood disorders, although more data on larger samples are needed.

The age at onset of depression is not only an important clinical predictor of the further disease course, but also a robust marker, reflecting the genetic impact on depression risk.

This study aimed to find whether early-onset depression had an association with specific clinical symptoms, comorbid psychiatric disorders and family history of mood disorders.

This pilot cross-sectional, multicenter study was performed under the supervision of the Russian National Consortium for Psychiatric Genetics. Early-onset depression was defined as the first depressive episode before the median age of onset in the sample (Me=29 years). Logistic regression models were used to determine the independent association of early-onset depression, after adjusting for the effects of sex and age, with binary characteristics.

A total of 172 patients with depression were enrolled in the study (64.5% women; age - 40.9 (15.9) years). Early-onset depression was associated with psychomotor retardation (p=0,025; OR=2,3; 95%CI [1,1 - 4,9]), decreased libido (p=0,014; OR=2,8; 95%CI [1,2 - 6,2]), and lower prevalence of weight loss/decreased appetite (p=0,011; OR=0,4; 95%CI [0,2 - 0,8]). No associations were found with the history of comorbid psychiatric disorders and the family history of mood disorders.

Early-onset depression is associated with specific neurovegetative symptoms. Further clinical and genetic studies are needed to evaluate the specific effects of age at onset of depression on its clinical course.

Depression is a chronic, recurrent mental disorder with a moderate level of genetic impact. Modern GWAS of depression require extra-large sample sizes and new effective, clinically sensitive, objective and simple to fill online phenotyping tools for population studies are necessary today.

Aim: to test online phenotyping tools based on clinical and psychometric instruments to evaluate depressive symptoms in population cohort for using in GWAS

Participants: 2610 Russian-speaking respondents- clients of Genotek Ltd., provider of genetic testing services in Russian Federation. The online survey included HADS-D (Hospital Anxiety and Depression Scale - depression subscale), original questionnaire adapted for self-report from major depression DSM-5 criteria, questions about sex and age. Three research phenotypes were defined: quantitative “HADS-D score” and two categorical “HADS-D depression” (cut-off 8 points) - 20.63 %, “DSM depression” 16.73 %. DNA samples obtained from saliva were genotyped on Illumina Infinium GSA v1.0/v2.0/v3.0 microarrays. GWAS analysis was performed independently for each of the research phenotypes.

None of the signals reached genome-wide significance (p value 10-8), but some signals with subthreshold significance were identified including four signals in the genes encoding proteins: “DSM_Depression”: rs2131596 in GRIP1 (p=3.682e-06,β=0.6965), rs11158021 in SAMD4A (p=2.841e-06, β=0.6762), “HADS-D Depression”: rs2425793 in CDH22 (p=4.408e-07, β=1.539), rs36006890 in PDIA6 (p = 6.529e-07, β=1.549).

Preliminary results of the first GWAS of depression symptoms in the Russian population are acceptable and confirm the accuracy of the research strategy using online phenotyping tools based on clinical and psychometric instruments and provide basis for further studies.

There is no consensus about whether late-onset schizophrenia(LOS) is a type of schizophrenia or a secondary psychotic disorder. One of the theories of the occurrence of late-onset psychoses is neurodegeneration caused by the imbalance of proteostasis.

To study the concentration and expression of alpha-synuclein in patients with LOS compared with controls.

The study involved 42 patients with the ICD-10 criteria of schizophrenia with the onset of the disease after 45 years and 104 controls with no dementia and severe somatic pathology, comparable in age and gender. The alpha-synuclein level was estimated in a lymphocytic cell fraction from patients with LOS N=42 and controls N=104 using the Human alpha-synuclein ELISA kit. The expression of the SNCA gene was studied in 22 LOS patients and 22 controls and determined by PCR using the SYBR Green Supermix kit and the CFX96 instrument in comparison with the referent genes using previously developed primers. LOS group underwent psychopathological examination including scales. Statistics: SPSS 12.0; the Mann–Whitney test; exponential regression analysis; data is given as median (min–max). The level of significance was set at p<0.05.

The alpha-synuclein level is higher in patients with LOS (9.21 (0.78-29.52)) compared to control (6,355 (0.46-35.44)), p=0.024. The mRNA level of the SNCA gene is higher in the LOS group (0.533 (0.089-1.406)) compared to controls (0.087 (0.016-0.266) , p<0.001.

Our study shows the relationship of alpha-synuclein with the manifestations of LOS. Obtained novel data can open up new targets for therapy and bring it closer to understanding the phenomenon.

The health care workers have extremely high risks of adverse psychological reactions from COVID-19 pandemic. On the other hand, patients with respiratory syndromes face stigmatization due to their possible contagiousness of SARS-Cov-2.

To study the association of behavior, psychological distress in health care workers, and their stigmatization attitudes to the patients.

The online-survey of 1800 health care workers performed during different lockdown periods in Russia: the first week and the last (30/Mar-5/Apr/20 and 4-10/May/20). The Psychological stress scale (PSM-25), modified Perceived devaluation-discrimination scale (Cronbach’s α=0.74) were used. Dispersion analysis with p-value=0.05 and Cohen’s d, Cramer’s V calculation (ES) performed.

In the 2nd phase medical stuff more often wore masks (64% vs. 89%; χ2=98.7, p=0.000, df=1; ES=0.23) and gloves (30% vs. 57%; χ2=57.6, p=0.000, df=1; ES=0.18), continued perform hand hygiene (94-95%) and physical distancing (73-74%), but was restricted in most effective protective measure: self-isolation (49% vs. 36%; χ2=16.0, p=0.000, df=1; ES=1.0). The psychological stress levels decreased in the 2nd phase (ES=0.13), while the stigma levels (ES=0.33) increased. Physicians experienced more stress compared with nurses and paramedical personnel (ES=0.34; 0.64) but were less likely to stigmatize SARS-CoV-2 infected individuals (ES=0.43; 0.41). The highest rates of contacts with COVID-19 patients (83%) were reported by physicians (χ2=123.0; p = 0.00, df=4; ES=0.28).

Direct contact with SARS-Cov-2 is associated with a significant increase in stress among medical personnel. However, the stigmatizing reactions are not directly associated with the risks of infection and are most prevalent among nurses and paramedical personnel.

Current epidemiological data on the pandemic prevalence of HIV in the world expand the boundaries of the problems associated with the threat of unpredictable spread of infection and the frequency of mental disorders that accompany HIV infection.

Somatogenic mental disorders are determined by the fact that the central nervous system (CNS) is one of the reservoirs for HIV. HIV is indirectly a neurotropic virus and can cause associated neurocognitive impairment (HAND)

In study were used clinical-linguistic examination method for determining linguistic markers for mental disorders in HIV patients who did not receive specific antiviral therapy.

As a result, it was found that 50% of HIV-infected patients not suffering from addiction syndromes or surfactant abuse have mental disorders, which are characterized mainly by disorders of adaptive reactions and mild cognitive impairment. 57.7% of HIV-infected people who do not use surfactants showed a high level of social adaptation. For patients who do not use surfactants, and who have undergone a commission examination before starting ART, the leading psychopathological syndromes are anxiety, anxiety-hypochondria. 44.2% of patients did not have obvious psychopathological symptoms.

Structure of emotional experiences was revealed in patients who recently learned about the burden of a serious chronic disease with the corresponding fear of death and self-stigmatization. Identified linguistic markers are additional signs that can be used by physicians and psychiatrists to diagnose both cognitive impairment and emotional impairment in patients with HIV infection.

Metabolic syndrome (MS) is associated with an increased risk of developing a cognitive vascular disorders and dementia.

The associations of cognitive disorders (CD) with components of methabolic syndrome (MS) such as : body mass index, lipid spectrum, arterial hypertension and glucose level (GL) in middle age subjects were study.

The 271 patients with MS according IDF criteria, (aged 30 – 60 years) were examend. Current mild cognitive impairment (MCI) was confirmed by psychodiagnostic interview according to the criteria of ICD-10. All patients passed through: MMSE test, Cognitive Failures Questionnaire, Wechsler memory scale, Symbol Coding and Category Fluency test. Level of blood glucose and plasma indicators of lipid spectrum were assessed in the blood samples with «Abbott» kits. To assess the results the NCEP criteria were used.

All 271 subject were divided into 2 groups, group A – with CD and/or MCI (212 subjects) and the group B -without affective disorders (49 subjects). Using the Mann-Whitney test significantly strong connection between high levels of total cholesterol (TC), cholesterol low density lipoprotein (LDL-C), lipoproteins of very low density (VLDL), the GL and MCI in group A were obtained. Optional subjects with sings of PH, MS and MCI had a fairly high level of VLDL and LDL-C in comparison with subjects without MCI.

The meaning of the relationship between metabolic syndrome and mild cognitive impairments in middle-aged people is in increasing in the level of LDL and VLDL that can provoke MCI in middleage subjects with MS.

To reduce the heterogeneity of depressive GWAS samples it seems relevant to evaluate and compare current instruments for depression phenotyping.

The aim is to evaluate the agreement of DSM criteria and HADS scores in depression phenotyping for population studies.

The self-report data was obtained from 5116 clients (females 50,63%; mean age 36,92±9,82 years, Ме=42; Q1=35, Q3=76) of genetic testing company Genotek Ltd.. The respondents completed an on-line questionnaire with items on social status and biometrics. Depression phenotyping was based on DSM-5 criteria (life-time and current for major and bipolar depression) and HADS (current).

Mean HADS scores were: HADS-A – 6,43±2,9, Ме=8; Q1=6, Q3=18; HADS-D – 4,5±2,83, Ме=6; Q1=4, Q3=17. Abnormal anxiety and depression (≥11 for each subscale) were present in 9% (N–456) and 3,4% (N–174) of respondents, respectively; borderline (8-10) – in 23% (N–1172) and 11,9% (N–592), respectively. The life-time report of major depression according to DSM-5 criteria was 17,6% (N–261) and of bipolar disorder – 8,3% (N–139). Moderate correlations were present for borderline HADS anxiety scores and DSM major depression (0.19, p<.01). Similar correlations of HADS anxiety scores were registered for DSM bipolar depression (0.20, p<.01). Moreover, HADS depression scores did not correlate with any DSM depressive phenotype.

Our study shows significant correlations only for DSM depression criteria and HADS anxiety, but not depression scores. It could indicate the different significance of individual scale items in depression phenotyping and the need for their separate further evaluation.

The psychological stress associated with the COVID-19 pandemic has a complex multifactorial nature.

The aim: to evaluate the level of stress during the COVID-19 pandemic in the Russian-speaking population with(-out) affective disorders in different cities of residence.

The data obtained from an online survey of 4803 Russian-speaking respondents (age over 18) from March 30 to May 18, 2020. The survey included social, demographic and the history of affective disorders data. The anxiety distress level was evaluated with the Psychological Stress Measure (PSM-25).

Individuals from sub-cohort of Russian cities with populations less than one million had higher stress levels (M=135.39) compared to Moscow (M=129.47; p=0.003) or St.-Petersburg (M=126.63; p<0.001). However, stress scores in respondents with a history of affective disorders correspond to the average stress level according to PSM-25. Respondents without affective disorders from St.-Petersburg reported lower stress levels (M=92.88) than respondents from Moscow (M=100.47; p<0.001) and Russian cities with less than one million population (M=98.4; p<0.001). Average stress scores from St.-Petersburg and other Russian cities show a low level of stress on PSM-25, which indicates psychological adaptation. Stress scores from Moscow have borderline values between low and medium levels.

Our study showed that the city of residence and affective disorders status significantly affect stress levels in Russian population. These factors could be further used in individual psychological support strategies.

Compulsory admission procedures of patients with mental disorders vary between countries in Europe. The Ethics Committee of the European Psychiatric Association (EPA) launched a survey on involuntary admission procedures of patients with mental disorders in 40 countries to gather information from all National Psychiatric Associations that are members of the EPA to develop recommendations for improving involuntary admission processes and promote voluntary care.

The survey focused on legislation of involuntary admissions and key actors involved in the admission procedure as well as most common reasons for involuntary admissions.

We analyzed the survey categorical data in themes, which highlight that both medical and legal actors are involved in involuntary admission procedures.

We conclude that legal reasons for compulsory admission should be reworded in order to remove stigmatization of the patient, that raising awareness about involuntary admission procedures and patient rights with both patients and family advocacy groups is paramount, that communication about procedures should be widely available in lay-language for the general population, and that training sessions and guidance should be available for legal and medical practitioners. Finally, people working in the field need to be constantly aware about the ethical challenges surrounding compulsory admissions.

Current D and A were confirmed by psychodiagnostic interview according to the criteria of ICD-10. All patients passed through: MMSE test, Wechsler memory scale, symbol coding and category Fluency test, scales HADS, HAM-A. Inflammatory markers included CRP, IL-6, IL-1 and TNF-α. Subjects were divided into group A–with D and/or A (139) and group B–without affective disorders (132). Using Mann–Whitney test significant connection between presence of MCI and high levels of inflammation is associated with simultaneous presence affective disorders. High correlations in subjects with A/D were between IL-6, IL-1 and MCI. In-group B, there was no significant correlations between inflammatory markers and MCI.

There is link between affective disorders and levels of inflammatory markers. Increased levels of IL-6 and IL-1 provoke co-morbidity of MCI and depression or anxiety.

There is a lack of attention on generalized anxiety disorder (GAD) in the psychiatrists’ education programs in Russia. The consequence of this is difficult to estimate because of insufficiency of the GAD epidemiology in Russia.

Are estimation of the comparative prevalence of diagnosis of GAD among other anxiety and stress related disorders; psychiatrists’ knowledge about GAD and theirs therapeutic approaches.

The invitations to survey were sent by e-mail to members of the Russian Society of Psychiatrists; 888 psychiatrists took part in the survey. Twenty-six percent of them worked in inpatient departments, 43% – in outpatient departments, 15% – in somatic services, 17% – researchers and university professors.

A total of 83% of respondents have diagnosed GAD at least once during last year. Most often GAD was diagnosed by psychiatrists of somatic services. Mixed anxiety and depressive disorder was diagnosed in 2.5 times more often than GAD; adjustment disorders – in 2.1 times. Doctors have noted that among their patients with other mental disorders 26% have chronic anxiety, but most of doctors do not establish the comorbid diagnosis of GAD for these patients. Only a quarter of doctors consider that detachment of GAD from other anxiety disorders is based on the features of etiology and pathogenesis. In the treatment of GAD together with SSRIs, SNRIs, and pregabalin prescribing, doctors often prescribe benzodiazepines, atypical anxiolytics (hydroxyzine, buspirone) and low-potency antipsychotics (alimemazine, chlorprothixene).

Increasing attention to GAD in the psychiatrists’ education programs may improve diagnosis and treatment of this disorder in Russia.
