  • 244 Community perspectives on hospital accountability to equity

  • Objectives/Goals: Our objective is to examine patient and community perspectives on hospital actions that signify accountability to healthcare equity; part of our overall goal is to identify equity measure concepts representative of community perspectives and priorities for future hospital accountability programs. Methods/Study Population: We conducted a qualitative thematic analysis of secondary data – 32 focus group transcripts from our hospital’s Community Health Needs Assessment (CHNA). A tri-annual CHNA is required of nonprofit hospitals to maintain tax exemption. Diverse participants were recruited from our hospital’s large catchment area. Coding focused on responses to 6 pertinent questions. We adapted the National Committee for Quality Assurance, “Health Equity Measurement Framework for Medicaid Accountability” which consists of 5 domains (access, clinical, experience, structure, and social) to guide the development of our a priori coding tree and subsequent analysis. Two coders double-coded 25% of transcripts. The multidisciplinary research team, including community partners, met iteratively to extract and refine themes. Results/Anticipated Results: We organized our analysis by our conceptual framework’s 5 measurement domains. The “access” and “experience” domains were the most salient for participants. We defined “access” by four sub-domains: financial access, physical access, communication access, and navigability; and “experience” by two subdomains: inclusivity and accomodation. Beyond discussing concepts within these measurement domains, participants debated the “scope” of the hospital’s role with regard to healthcare equity. While some did not think “it was the hospitals” responsibility to give people access to good jobs or fair pay, education…, “other participants felt that healthcare involves not just addressing peoples’ physical health but. their housing… because how can someone take care of their health when they are homeless?” Discussion/Significance of Impact: When asked about hospital accountability to healthcare equity, “access” and “experiences” of care are the most salient measurement domains for patients and communities. The “scope” of the hospital’s role is debated. Policy and health system leaders can apply these perspectives to equity measurement initiatives.

  • Living with a blood cancer in later life: The complex challenges and related support needs of adults aged 75 and older

    This study investigated the challenges and support needs of adults aged 75 and older during and after treatment for a blood cancer to aid targeted supportive resource development.

    Methods

    Adults aged 75 and older with a blood cancer participated in in-depth, semi-structured interviews about challenges and unmet support needs. Participants recruited through The Leukemia & Lymphoma Society were (1) in treatment or previously in treatment for a blood cancer at age 75 or older and (2) living in the United States or its territories. A thematic analysis was conducted with findings compared between 2 groups: (1) chronic -living with a chronic blood cancer; (2) acute -living with an acute blood cancer or both an acute and chronic blood cancer.

    Results

    Participants (n = 50) ranged from 75 to 91 years old. Both groups described similar experiences and identified 5 challenges and support needs: (1) socioemotional impact, (2) activities of daily living and instrumental activities of daily living (ADLs/iADLs), (3) uncertainty management, (4) treatment-related stressors, and (5) COVID-19-related strain. Properties for these themes illustrate challenges and support needs, with some differences between groups. For instance, those living with a chronic blood cancer highlighted financial strain with treatment-related stressors, while those with an acute blood cancer focused more on iADLs.

    Significance of results

    Findings inform an agenda for targeted resource development for older adults with a blood cancer nearing the end of the life span. Results demonstrate the need for supportive services and family communication interventions to help patients manage iADLs and navigate socioemotional needs and challenges.

  • Summary

    This chapter explores the relationship between theatrical music, visual record, and audience memory as mediated by a group of Attic vases, mostly dated from the mid- to late sixth century BCE, that show choruses of animals, animal-riders, and/or men wearing animal costumes. I argue for a new interpretation of these sympotic vessels, whereby they are understood as objects that engage and participate in a viewer’s memory of choral performance. I emphasize the referential flexibility of such images of theatrical music-making, which can evoke one specific performance but also, simultaneously, multiple performances across various genres. The vases thus activate a viewer’s cultural repertoire of choreia, which could include his own bodily experience of singing and dancing in a chorus; in doing so, they draw him in as both spectator and performer within their own choral productions.

  • In Greek mythology, the Muses are Memory's daughters. Their genealogy suggests a deep connection between music and memory in Graeco-Roman culture, but how was this connection understood and experienced by ancient authors, artists, performers, and audiences? How is music remembered and how does it memorialize in a world before recording technology, where sound accumulated differently than it does today? This volume explores music's role in the discourses of cultural memory, communication, and commemoration in ancient Greek and Roman societies. It reveals the many and varied ways in which musical memory formed a fundamental part of social, cultural, ritual, and political life in ancient Greek- and Latin-speaking communities, from classical Athens to Ptolemaic Alexandria and ancient Rome. Drawing on the contributors' interdisciplinary expertise in art history, philology, performance studies, history, and ethnomusicology, eleven original chapters and the editors' Introduction offer new approaches for the study of Graeco-Roman music and musical culture.
