Background: The epidemiology of extended-spectrum cephalosporin-resistant Enterobacterales (ESCrE) in hospitalized patients in low- and middle-income countries (LMICs) is poorly described. Although risk factors for ESCrE clinical infection have been studied, little is known of the epidemiology of ESCrE colonization. Identifying risk factors for ESCrE colonization, which can predispose to infection, is therefore critical to inform antibiotic resistance reduction strategies. Methods: This study was conducted in 3 hospitals located in 3 districts in Botswana. In each hospital, we conducted ongoing surveillance in sequential units hospitalwide. All participants had rectal swabs collected which were inoculated onto chromogenic media followed by confirmatory testing using MALDI-TOF MS and VITEK-2. Data were collected via interview and review of the inpatient medical record on demographics, comorbidities, antibiotic use, healthcare exposures, invasive procedures, travel, animal contact, and food consumption. Participants with ESCrE colonization (cases) were compared to noncolonized participants (controls) using bivariable and multivariable analyses to identify risk factors for ESCrE colonization. Results: Enrollment occurred from January 15, 2020, to September 4, 2020, and 469 participants were enrolled. The median age was 42 years (IQR, 31–58) and 320 (68.2%) were female. The median time from hospital admission to date of sampling was 5 days (IQR, 3–12). There were 179 cases and 290 controls (ie, 38.2% of participants were ESCrE colonized). Independent risk factors for ESCrE colonization were a greater number of days on antibiotic, recent healthcare exposure, and tending swine prior to hospitalization. (Table). Conclusions: ESCrE colonization among hospitalized patients was common and was associated with several exposures. Our results suggest prior healthcare exposure may be important in driving ESCrE. The strong link to recent antibiotic use highlights the potential role of antibiotic stewardship interventions for prevention. The association with tending swine suggests that animal husbandry practices may play a role in community exposures, resulting in colonization detected at the time of hospital admission. These findings will help to inform future studies assessing strategies to curb further emergence of hospital ESCrE in LMICs.

Marine-terminating glaciers lose mass through melting and iceberg calving, and we find that meltwater drainage systems influence calving timing at Helheim Glacier, a tidewater glacier in East Greenland. Meltwater feeds a buoyant subglacial discharge plume at the terminus of Helheim Glacier, which rises along the glacial front and surfaces through the mélange. Here, we use high-resolution satellite and time-lapse imagery to observe the surface expression of this meltwater plume and how plume timing and location compare with that of calving and supraglacial meltwater pooling from 2011 to 2019. The plume consistently appeared at the central terminus even as the glacier advanced and retreated, fed by a well-established channelized drainage system with connections to supraglacial water. All full-thickness calving episodes, both tabular and non-tabular, were separated from the surfacing plume by either time or by space. We hypothesize that variability in subglacial hydrology and basal coupling drive this inverse relationship between subglacial discharge plumes and full-thickness calving. Surfacing plumes likely indicate a low-pressure subglacial drainage system and grounded terminus, while full-thickness calving occurrence reflects a terminus at or close to flotation. Our records of plume appearance and full-thickness calving therefore represent proxies for the grounding state of Helheim Glacier through time.

OBJECTIVES/GOALS: To develop feasible screening methods for activity of the enzyme Glucose-6-phosphate dehydrogenase (G6PD) with point of care applicability. METHODS/STUDY POPULATION: Current knowledge establishes the relevance of G6PD as a critical therapeutic determinant for effective antimalarial therapy due to the occurrence of mutations that lead to post-treatment severe adverse effects. We present our findings on development of cost effective point-of-care screening methodologies to ascertain G6PD deficiency. RESULTS/ANTICIPATED RESULTS: Using Patient Cohort Explorer and data from the Department of Pathology, we established the prevalence of G6PD deficiency at the University of Mississippi Medical Center, Jackson, MS as high as 11.8% (African-American males in all population, n = 2518). Next, for selection of potential target groups, we set up a protocol for recruitment of volunteers based on ethnic background, parental ethnicity, and medical history. G6PD activity was evaluated using point of care methods [Trinity Biotech test or CareSTART Biosensor], and Gold Standard quantitative spectrophotometric assay (LabCorp). Determinations in >20 subjects have showed comparable concordance. If used with a conservative interpretation of the signal, the Trinity Biotech test showed superior potential for use in the field relative to the CareSTART Biosensor. DISCUSSION/SIGNIFICANCE OF IMPACT: We established the prevalence of G6PD deficiency in our medical center. We have also setup tests for point-of-care assessment of G6PD. Pending evaluation of the relative tests performance, we will be in position to screen individuals and select them for a prospective clinical trial to evaluate the safety of antimalarial agents on scope of G6PD deficiency.

The norm in fitness to practise proceedings (FTPP) is that where sanctions might be imposed procedural fairness requires a court-like hearing. This paper questions that paradigm, using empirical research to focus on the FTPP to which social workers must account. Procedural fairness is a multi-faceted legitimising concept used to justify the design of decision-making processes. With FTPPs, the major justification is an ‘instrumentally’ focused model of procedural fairness which prioritises making decisions that look right, a goal which is delivered in the context of social work. But other justifications for procedural fairness are inadequately fulfilled, with in particular a ‘dignitarian’ respect not achieved due to the high levels of non-attendance by registrant social workers. Further, procedural fairness as ‘public accountability’ is undermined due to the relative lack of engagement of FTPPs with the perspective of the social work community. These findings hint that in the context of a poorly organised and resource-poor profession other hybrid forms of FTPP might have a stronger claim to procedural fairness than the court-like model.

This articles sketches the history, vision and implementation of the ElectroAcoustic Resource Site 2 (www.ears2.dmu.ac.uk), also known as the EARS pedagogical site. EARS 2 was originally intended to act as a miniature version of the original EARS site (www.ears.dmu.ac.uk), created specifically for young people, thus a site introducing vocabulary and relevant publications. However, after careful consideration, it proved much more valuable to create an entire pedagogical environment introducing primarily, although not exclusively, children aged 11 to 14 to electroacoustic music in terms of its concepts, repertoire and creative practice. The Compose with Sounds software (www.cws.dmu.ac.uk) package has been specifically developed for this initiative and is introduced in this article along with the EARS 2 project. The contextual discussion focuses on the issue of making innovative music accessible to inexperienced audiences, and includes a discussion of the project goal of enhancing learning through technology behind this new initiative.

This paper examines how the relationships between the factors (predisposing, enabling and illness) of the 1973 Andersen framework and service use are influenced by changes in the caring role in older women of the 1921–26 cohort of the Australian Longitudinal Study on Women's Health. Outcome variables were the use of three formal community support services: (a) nursing or community health services, (b) home-making services and (c) home maintenance services. Predictor variables were survey wave and the following carer characteristics: level of education, country of birth, age, area of residence, ability to manage on income, need for care, sleep difficulty and changes in caring role. Carer changes were a significant predictor of formal service use. Their inclusion did not attenuate the relationship between the Andersen framework factors and service use, but instead provided a more complete representation of carers' situations. Women were more likely to have used support services if they had changed into or out of co-resident caring or continued to provide co-resident care for a frail, ill or disabled person, needed care themselves, and reported sleep difficulties compared with women who did not provide care. These findings are important because they indicate that support services are particularly relevant to women who are changing their caring role and who are themselves in need of care.

Despite its inclusion as a basic component of the cognitive-behavioural treatment for panic disorder, the effects of the provision of information about panic anxiety have not been separately assessed. This study compared information-giving with self-monitoring of panic alone, on panic-related variables and negative affect for 40 people with panic disorder. In comparison to self-monitoring alone, information-giving together with self-monitoring was associated with reductions in anticipatory fear of panic and negative affect, but not panic frequency. These results suggest that provision of information about panic anxiety is an important component of the cognitive-behavioural approach to panic disorder in reducing anxious and depressive affect. The other cognitive-behavioural components of this intervention are required to impact upon specific panic parameters.
