Stenotrophomonas maltophilia and Achromobacter xylosoxidans are emerging nosocomial, non-glucose fermenting, Gram-negative pathogens. In this nested case-control trial, independent predictors for S. maltophilia infections were hemodialysis and recent antibiotic usage (overall), while recent usage of fluoroquinolones, was independently associated with A. xylosoxidans infections. Infections were independently associated with multiple worse outcomes.

Introduction
The COVID-19 pandemic has had a dramatic impact on the health and social care landscape, both in terms of service provision and citizen need. Responsive, evidence-based research is essential to develop and implement appropriate policies and practices that manage both the pandemic itself, and the impact COVID-19 has on other health and social care issues.
To address this, the Wales COVID-19 Evidence Centre (WCEC) was launched in 2021 with the aim of providing the best available, up-to-date, and relevant evidence to inform health and care decision making across Wales.
MethodsFunded by the Welsh Government, the WCEC comprises of a core team and several collaborating partner organizations, including Health Technology Wales, Wales Centre for Evidence-Based Care, Specialist Unit for Review Evidence Centre, SAIL Databank, Public Health Wales, Bangor Institute for Health & Medical Research in conjunction with Health and Care Economics Cymru, and the Public Health Wales Observatory. Over the last year, WCEC has developed its rapid review processes and methodology informed by best international practice and aims to provide around 50 reviews each year. WCEC works alongside various stakeholder groups from health and social care across Wales, and they form an integral part of the review process, from scoping to knowledge mobilization.
ResultsTo date, the WCEC has produced reviews on a diverse range of COVID-19 topics, including transmission, vaccination uptake (barriers, facilitators and interventions), mental health and wellbeing, as well as face coverings and other preventative interventions. The topics have also covered a wide range of populations, from general public, to healthcare workers, to children. These reviews have been used to inform policy and decision-making, including the Welsh Government’s Chief Medical Officer 21-day COVID-19 reviews.
ConclusionsThe WCEC has brought together multiple specialist centers with a diverse range of skills to produce timely reviews of the most up-to-date research to support decision makers across health and social care. These reviews have informed policy and decision-making across Wales.

Aims and method
A series of eleven patients prescribed intramuscular clozapine at five UK sites is presented. Using routinely collected clinical data, we describe the use, efficacy and safety of this treatment modality.
ResultsWe administered 188 doses of intramuscular clozapine to eight patients. The remaining three patients accepted oral medication. With the exception of minor injection site pain and nodules, side-effects were as expected with oral clozapine, and there were no serious untoward events. Nine patients were successfully established on oral clozapine with significant improvement in their clinical presentations.
Clinical implicationsAlthough a novel formulation in the UK, we have shown that intramuscular clozapine can be used safely and effectively when the oral route is initially refused.

INTRODUCTION:
Ultrasound (US) detects synovitis more accurately than clinical examination (CE) in people with rheumatoid arthritis (RA). This review aimed to investigate the use of US, compared to CE alone, in treatment strategies for RA, and to estimate its potential to be cost-effective in making treatment decisions.
METHODS:A systematic review was conducted of studies: investigating RA treatment response or strategies that compared US with CE-assessed synovitis; and of tapering RA treatment (1). A model was constructed to investigate the potential cost-effectiveness of US in (i) selecting patients suitable for treatment tapering; and (ii) avoiding treatment escalation (2).
RESULTS:Seven prospective cohort studies suggested US-detected synovitis was significantly associated with a treatment response or tapering failure, whereas in most cases clinical examination alone was not. Two randomized controlled trials (RCTs) identified suggested that US added to the Disease Activity Index (DAS)-based treatment strategies but did not significantly improve primary outcomes, but was associated with improved rate of DAS remission. The evidence showed that some patients (proportions varied widely) who had achieved low disease activity could have treatment tapered, with no, or little, short-term harm to the patient.
The model estimated that an average reduction of 2.5 percent in the costs of biological disease-modifying anti-rheumatic drug (bDMARDs) was sufficient to cover the costs of performing US every three months. This value increased to 4 percent and 13 percent for the costs of conventional disease-modifying anti-rheumatic drug (cDMARDs) depending on the assumed regimen.
CONCLUSIONS:Use of US to monitor synovitis could potentially be a cost-effective approach, given that low proportions of patients for whom clinicians consider amending treatment, would need to taper treatment, or remain on therapy without escalation. US could provide clinicians with more confidence in reducing the drug burden. However, there is considerable uncertainty in this conclusion due to lack of robust data relating to key parameters.

The proposed mechanism for Reducing Emissions from Deforestation and Degradation (REDD+) offers significant potential for conserving forests to reduce negative impacts of climate change. Tanzania is one of nine pilot countries for the United Nations REDD Programme, receives significant funding from the Norwegian, Finnish and German governments and is a participant in the World Bank’s Forest Carbon Partnership Facility. In combination, these interventions aim to mitigate greenhouse gas emissions, provide an income to rural communities and conserve biodiversity. The establishment of the UN-REDD Programme in Tanzania illustrates real-world challenges in a developing country. These include currently inadequate baseline forestry data sets (needed to calculate reference emission levels), inadequate government capacity and insufficient experience of implementing REDD+-type measures at operational levels. Additionally, for REDD+ to succeed, current users of forest resources must adopt new practices, including the equitable sharing of benefits that accrue from REDD+ implementation. These challenges are being addressed by combined donor support to implement a national forest inventory, remote sensing of forest cover, enhanced capacity for measuring, reporting and verification, and pilot projects to test REDD+ implementation linked to the existing Participatory Forest Management Programme. Our conclusion is that even in a country with considerable donor support, progressive forest policies, laws and regulations, an extensive network of managed forests and increasingly developed locally-based forest management approaches, implementing REDD+ presents many challenges. These are being met by coordinated, genuine partnerships between government, non-government and community-based agencies.

Aims and Method
To explore user perspectives on the content and delivery of the undergraduate curriculum in psychiatry. the study design was qualitative and used focus groups. Four focus groups were run with a total of 28 participants.
ResultsThe key finding was that participants were clearer about the attitudes they felt students should convey than they were about the skills and knowledge required.
Clinical ImplicationsService user perspectives on the content of the undergraduate psychiatry curriculum need to be considered as curricula are developed.

Midget, a new dwarfing gene in the house mouse, was found in a line (NS) selected for small body size. Backcrosses to, and heterozygous matings of NS mice gave midget offspring in the expected 1:1 and 1:3 ratios. Backcrosses to two other lines of mice selected for small body size resulted in fewer midget offspring than expected and in a reduced phenotypic expression of the gene, though matings between these midget offspring gave all midget progeny.
A characteristic check in growth occurred between 15 and 21–24 days of age, and midgets rarely exceeded 10 g. at 6 weeks of age. They were 1·2 g. lighter than NS litter-mates at 3 weeks, and 3·6–4·4 g. lighter at 6 weeks. Fostering or transferring 3½-day-old midget embryos to the uteri of large mothers slightly increased the growth-rate, though the characteristic growth check still persisted. Transfer as embryos did not influence the midget phenotype.
Midget males were of near-normal fertility. The females were semi-sterile due to irregularity or absence of oestrous cycles, failure to ovulate, or the death of embryos before implantation. Each of these factors, which were also found in NS mice, could be remedied by the administration of exogenous hormones.
The genetic factors underlying the expression of midget are discussed, and the physiological traits associated with this and with other dwarfing genes are compared.

The fertility of two unrelated strains of mice (strains N and C) which had both been selected for large and small body size has been studied.
The fertility of pairs of mice in the large or small lines of strain C was unimpaired by selection. In strain N, some of the pairs in the large and small lines, but not in the control line, were sterile. Sterility in the large line was due to the low libido of the males, and not to female infertility. Sterility in the small line was probably due to hypo-functioning of the anterior pituitary of some females: the oestrous cycle was delayed or absent, some mice failed to ovulate after mating, and a high proportion of those mating had no implanted embryos at 12 days' gestation. Oestrus and ovulation could be induced in mice of the small line of strain N by exogenous gonadotrophins, and the proportion of mice with implanted embryos was considerably increased by progesterone supplements.
The number of eggs found after natural mating was considerably higher in large mice than in small mice, and was significantly correlated with body weight in the small line of strain C only. The egg-number/body-weight ratio was higher in the lines of strain C than in those of strain N, though the ratios were similar when carcass fat was subtracted from total body weight.
The amount of endogenous follicle-stimulating hormone secreted by the mice of the five lines was estimated by inducing ovulation with various amounts of exogenous gonadotrophins, and comparing the number of eggs found after each dose with the mean number ovulated after natural mating. Estimates of the amount of follicle-stimulating hormone secreted by mice of strain C were higher than those for mice of strain N.
Differences in the rates of growth and in the numbers of eggs ovulated after natural mating indicate a higher level of pituitary activity in strain C than in strain N.

The article deals with the practice of phenomenological archaeological fieldwork, which is concerned with sensory experience of landscapes and locales. Phenomenological approaches in archaeology have cast light on aspects of past human experience not addressed by traditional archaeological methods. So far, however, they have neither developed explicit methodologies nor a discussion of methodological practice and have laid themselves open to accusations of being ‘subjective’ and ‘unscientific’. This article describes and explores three experiments in phenomenological archaeology developed in the context of the Tavoliere–Gargano Prehistory Project and carried out on Neolithic settlement sites of the type known as villaggi trincerati. Our aims are both to develop explicit methods for this type of fieldwork and to combine phenomenology with other more traditional approaches, such as those concerned with technological, economic and environmental aspects of landscapes and sites. Our work also differs from other phenomenological archaeology in its concern with familiar, everyday experience and domestic contexts, rather than exceptional, special experience in ritual contexts. We consider how our particular approach might be used to further understandings of past lives.
Immucillin-H binding to purine nucleoside phosphorylase reduces dynamic solvent exchange

The rate and extent of hydrogen/deuterium (H/D) exchange into purine nucleoside phosphorylase (PNP) was monitored by electrospray ionization mass spectrometry (ESI-MS) to probe protein conformational and dynamic changes induced by a substrate analogue, products, and a transition state analogue. The genetic deficiency of PNP in humans is associated with severe T-cell immunodeficiency, while B-cell immunity remains functional. Inhibitors of PNP have been proposed for treatment of T-cell leukemia, to suppress the graft-vs.-host response, or to counter type IV autoimmune diseases without destroying humoral immunity. Calf spleen PNP is a homotrimer of polypeptide chains with 284 amino residues, molecular weight 31,541. Immucillin-H inhibits PNP with a Kd of 23 pM when only one of the three catalytic sites is occupied. Deuterium exchange occurs at 167 slow-exchange sites in 2 h when no catalytic site ligands are present. The substrate analogue and product prevented H/D exchange at 10 of the sites. Immucillin-H protected 32 protons from exchange at full saturation. When one of the three subunits of the homotrimer is filled with immucillin-H, and 27 protons are protected from exchange in all three subunits. Deuterium incorporation in peptides from residues 132–152 decreased in all complexes of PNP. The rate and/or extent of deuterium incorporation in peptides from residues 29–49, 50–70, 81–98, and 112–124 decreased only in the complex with the transition state analogue. The peptide-specific H/D exchange demonstrates that (1) the enzyme is most compact in the complex with immucillin-H, and (2) filling a single catalytic site of the trimer reduces H/D exchange in the same peptides in adjacent subunits. The peptides most highly influenced by the inhibitor surround the catalytic site, providing evidence for reduced protein dynamic motion caused by the transition state analogue.

The fourth season of the Fezzan Project continued the interdisciplinary approaches of previous seasons. Geographical and environmental work focused principally in sampling sediments for scientific dating and with integrating ground observation with remote sensing data. Excavations continued at Old Germa, where the site has now reached Garamantian levels. In a separate development, the tentative identification has been made of an early mosque at the site, in an area adjacent to the G1 excavation trench. Substantial results were gained from work aimed at enhancing the important data recorded by Charles Daniels in his earlier excavations and survey in the Wadi al-Hayat. The enhancement of the Daniels' survey archive was integrated with completion of the wider prospection being undertaken by the new project. This survey included fieldwalking, standing building survey, analysis of the foggara irrigation systems and recording of rock art scenes. Finds work comprised the finalisation of a pottery type series for the Germa area, the study of small finds from the recent survey work, botanical analysis and completion of lithics recording. A programme of radiocarbon dating is now being undertaken to improve the phasing of sites and monuments. The first two volumes of final reports are now in preparation.

The Fezzan Project is investigating the last 10,000 years of human settlement, landscape evolution and climatic change in the Germa region in southern Libya. The second season in February–March 1998 comprised interdisciplinary research in archaeology and geography, centred around excavation and survey work carried out at the site of Old Germa. To date, three phases of mud brick buildings have been partially explored. In addition, wider geomorphological study and archaeological survey and fieldwalking were carried out elsewhere in the Germa/Twesh oasis and around el-Hatiya. Numerous sites were discovered, including a new hillfort of Zinchecra type and several valley centre ‘villages’ of Garamantian/Roman date. Artefactual studies were carried out on pottery and lithics, animal bones and seeds. Further work on the subterranean irrigation features, the foggaras, have confirmed their pre-Islamic origins.
