  • Summary

    Around 1450 Johannes Gutenberg began to change the literary culture of Europe by using movable type to print books. In 1466 Cicero’s De officiis became the first classical text to be so printed. In 1472 Catullus was printed for the first time by Vindelinus de Spira in Venice. Before the invention of printing, books could circulate only if they were manu scripti (‘copied by hand’). Catullus lived at a time when for the vast majority of literary works these copies were made on rolls of papyrus, to which and to other materials for writing he refers quite often.

    For a work of classical literature to be read in modern times, at least one manuscript copy had to survive until printing brought about more widespread dissemination. This survival depended, so to speak, on the text’s being able to leap over several hurdles.

  • There are some doubts as to whether psychotherapy will remain in the armamentarium of future psychiatrists. Few studies have explored early career psychiatrists’ views and their experience with psychotherapy training. The Early Career Psychiatrists’ Council of the WPA carried out an online survey on training and practice in psychotherapy in 13 European countries in order to assess:

    1. 1) main characteristics of psychotherapy training in the partecipating countries;

    2. 2) organizational and clinical differences of psychotherapy training;

    3. 3) trainees’ satisfaction and confidence in the use of psychotherapy.

    An online survey was conducted through the use of a questionnaire specifically developed for the purposes of this study.Responders were required to collect their opinions on the basis of their own experience. Different aspects of psychotherapy training, such as compulsoriness, payment and supervision, as well as satisfaction with received training and confidence in the use of psychotherapy have been investigated. Results show that training in psychotherapy is mandatory in all countries, except Belgium and France, but most of early career psychiatrists have to pay for it. European trainees are satisfied (70%) with received training, and feel confident to treat patients in psychotherapy settings. Psychodynamic and cognitive-behavioural techniques are more common than systemic, interpersonal, supportive and psychoeducational ones.

    In 3 countries out of 12 it is not compulsory to attend a psychotherapy training, and only psychodynamic and cognitive-behavioural approaches are widely spread in all countries.This survey is a starting point to improve training and practice of psychotherapy across Europe and to enhance early career psychiatrists’ psychotherapeutic skills and knowledge.

  • Summary

    This chapter discusses how and why Dionysius wrote the earliest history of Rome at such length. Dionysius’ own remarks on the writing of history, in both his theoretical treatises and the Roman Antiquities, are examined in order to show how they do much to explain this expansiveness, and two episodes (5.1.1–13.5; 5.52.1–57.4) that may serve as a general illustration of his techniques are analysed. Some suggestions are made as to where in the Roman annalistic tradition Dionysius found the material that allowed him to write at such length (it is argued that the lost history of Gnaeus Gellius was important). The chapter ends with discussion of a battle-scene (8.64.3–65.6) that exhibits characteristic Dionysian or Roman annalistic expansion. Throughout comparison of the Roman Antiquities with Livy’s Ab urbe condita, the only other extended narrative of early Rome that survives, is used to illustrate Dionysius’ techniques.

  • Background

    Although specific phobia is highly prevalent, associated with impairment, and an important risk factor for the development of other mental disorders, cross-national epidemiological data are scarce, especially from low- and middle-income countries. This paper presents epidemiological data from 22 low-, lower-middle-, upper-middle- and high-income countries.

    Method

    Data came from 25 representative population-based surveys conducted in 22 countries (2001–2011) as part of the World Health Organization World Mental Health Surveys initiative (n = 124 902). The presence of specific phobia as defined by the Diagnostic and Statistical Manual of Mental Disorders, fourth edition was evaluated using the World Health Organization Composite International Diagnostic Interview.

    Results

    The cross-national lifetime and 12-month prevalence rates of specific phobia were, respectively, 7.4% and 5.5%, being higher in females (9.8 and 7.7%) than in males (4.9% and 3.3%) and higher in high- and higher-middle-income countries than in low-/lower-middle-income countries. The median age of onset was young (8 years). Of the 12-month patients, 18.7% reported severe role impairment (13.3–21.9% across income groups) and 23.1% reported any treatment (9.6–30.1% across income groups). Lifetime co-morbidity was observed in 60.5% of those with lifetime specific phobia, with the onset of specific phobia preceding the other disorder in most cases (72.6%). Interestingly, rates of impairment, treatment use and co-morbidity increased with the number of fear subtypes.

    Conclusions

    Specific phobia is common and associated with impairment in a considerable percentage of cases. Importantly, specific phobia often precedes the onset of other mental disorders, making it a possible early-life indicator of psychopathology vulnerability.

  • Background:

    Sarcoma of the head and neck is a rare condition that poses significant challenges in management and often requires radical multimodality treatment.

    Objectives:

    This study aimed to analyse current clinical presentation, evaluation, management dilemmas and oncological outcomes.

    Methods:

    Computer records and case notes were analysed, and 39 patients were identified. Variables were compared using Pearson's chi-square test and the log-rank test, while survival outcomes were calculated using the Kaplan–Meier method.

    Results:

    The histopathological diagnosis was Kaposi sarcoma in 20.5 per cent of cases, chondrosarcoma in 15.3 per cent and osteosarcoma in 10.2 per cent. A range of other sarcomas were diagnosed in the remaining patients. The site of disease was most commonly sinonasal, followed by the oral cavity and larynx.

    Conclusion:

    Wide local excision with clear resection margins is essential to achieve local control and long-term survival. There is a need for cross-specialty collaboration in order to accrue the evidence which will be necessary to improve long-term outcomes.

  • In late 2011 the New Zealand Ministry for Primary Industries reported an increase in confirmed laboratory diagnoses of salmonellosis in dairy herds. To identify risk factors for herd-level outbreaks of salmonellosis we conducted a case-control study of New Zealand dairy herds in 2011–2012. In a multivariable analysis, use of continuous feed troughs [adjusted odds ratio (aOR) 6·2, 95% confidence interval (CI) 2·0–20], use of pelletized magnesium supplements (aOR 10, 95% CI 3·3–33) and use of palm kernel meal as a supplementary feed (aOR 8·7, 95% CI 2·5–30) were positively associated with a herd-level outbreak of salmonellosis between 1 July 2011 and 31 January 2012. We conclude that supplementary feeds used on dairy farms (regardless of type) need to be stored and handled appropriately to reduce the likelihood of bacterial contamination, particularly from birds and rodents. Magnesium supplementation in the pelletized form played a role in triggering outbreaks of acute salmonellosis in New Zealand dairy herds in 2011–2012.
