Cardiometabolic disease risk factors are disproportionately prevalent in bipolar disorder (BD) and are associated with cognitive impairment. It is, however, unknown which health risk factors for cardiometabolic disease are relevant to cognition in BD. This study aimed to identify the cardiometabolic disease risk factors that are the most important correlates of cognitive impairment in BD; and to examine whether the nature of the relationships vary between mid and later life.
Methods
Data from the UK Biobank were available for 966 participants with BD, aged between 40 and 69 years. Individual cardiometabolic disease risk factors were initially regressed onto a global cognition score in separate models for the following risk factor domains; (1) health risk behaviors (physical activity, sedentary behavior, smoking, and sleep) and (2) physiological risk factors, stratified into (2a) anthropometric and clinical risk (handgrip strength, body composition, and blood pressure), and (2b) cardiometabolic disease risk biomarkers (CRP, lipid profile, and HbA1c). A final combined multivariate regression model for global cognition was then fitted, including only the predictor variables that were significantly associated with cognition in the previous models.
Results
In the final combined model, lower mentally active and higher passive sedentary behavior, higher levels of physical activity, inadequate sleep duration, higher systolic and lower diastolic blood pressure, and lower handgrip strength were associated with worse global cognition.
Conclusions
Health risk behaviors, as well as blood pressure and muscular strength, are associated with cognitive function in BD, whereas other traditional physiological cardiometabolic disease risk factors are not.
In January 1979, Robert Edwards and Patrick Steptoe delivered a lecture detailing the ten-year clinical and scientific research programme that led to the birth of Louise Brown, the first baby born utilising IVF. This thoroughly-researched book provides both a full annotated transcript of the lecture as well as recorded reminiscences from those who attended, detailing the contemporary understandings of the event. An essay on the lecture's historical context adds fresh insight into the biographies of Edwards and Steptoe and highlights sources from print and broadcast media that have received scant attention in earlier publications. Current and future implications of the advances in IVF since the first procedure are also explored, examining future medical and scientific possibilities as well as ethical issues that may arise. A foreword by Louise Brown herself places this remarkable leap of science in a personal context, one that so many families have since experienced themselves.
Many male prisoners have significant mental health problems, including anxiety and depression. High proportions struggle with homelessness and substance misuse.
Aims
This study aims to evaluate whether the Engager intervention improves mental health outcomes following release.
Method
The design is a parallel randomised superiority trial that was conducted in the North West and South West of England (ISRCTN11707331). Men serving a prison sentence of 2 years or less were individually allocated 1:1 to either the intervention (Engager plus usual care) or usual care alone. Engager included psychological and practical support in prison, on release and for 3–5 months in the community. The primary outcome was the Clinical Outcomes in Routine Evaluation Outcome Measure (CORE-OM), 6 months after release. Primary analysis compared groups based on intention-to-treat (ITT).
Results
In total, 280 men were randomised out of the 396 who were potentially eligible and agreed to participate; 105 did not meet the mental health inclusion criteria. There was no mean difference in the ITT complete case analysis between groups (92 in each arm) for change in the CORE-OM score (1.1, 95% CI –1.1 to 3.2, P = 0.325) or secondary analyses. There were no consistent clinically significant between-group differences for secondary outcomes. Full delivery was not achieved, with 77% (108/140) receiving community-based contact.
Conclusions
Engager is the first trial of a collaborative care intervention adapted for prison leavers. The intervention was not shown to be effective using standard outcome measures. Further testing of different support strategies for prison with mental health problems is needed.
Studying phenotypic and genetic characteristics of age at onset (AAO) and polarity at onset (PAO) in bipolar disorder can provide new insights into disease pathology and facilitate the development of screening tools.
Aims
To examine the genetic architecture of AAO and PAO and their association with bipolar disorder disease characteristics.
Method
Genome-wide association studies (GWASs) and polygenic score (PGS) analyses of AAO (n = 12 977) and PAO (n = 6773) were conducted in patients with bipolar disorder from 34 cohorts and a replication sample (n = 2237). The association of onset with disease characteristics was investigated in two of these cohorts.
Results
Earlier AAO was associated with a higher probability of psychotic symptoms, suicidality, lower educational attainment, not living together and fewer episodes. Depressive onset correlated with suicidality and manic onset correlated with delusions and manic episodes. Systematic differences in AAO between cohorts and continents of origin were observed. This was also reflected in single-nucleotide variant-based heritability estimates, with higher heritabilities for stricter onset definitions. Increased PGS for autism spectrum disorder (β = −0.34 years, s.e. = 0.08), major depression (β = −0.34 years, s.e. = 0.08), schizophrenia (β = −0.39 years, s.e. = 0.08), and educational attainment (β = −0.31 years, s.e. = 0.08) were associated with an earlier AAO. The AAO GWAS identified one significant locus, but this finding did not replicate. Neither GWAS nor PGS analyses yielded significant associations with PAO.
Conclusions
AAO and PAO are associated with indicators of bipolar disorder severity. Individuals with an earlier onset show an increased polygenic liability for a broad spectrum of psychiatric traits. Systematic differences in AAO across cohorts, continents and phenotype definitions introduce significant heterogeneity, affecting analyses.
We provide a brief account of the life and work of Jules Parrot, a significant figure in French paediatrics, about whom almost nothing has been written. We focus on his work relating to congenital syphilis, specifically reporting on the examination of a collection of bones taken at autopsy from children with congenital syphilis. The collection of bones was brought to London in 1879 by Parrot to illustrate a talk that he gave before the Pathological Society of London. Subsequently, it travelled a circuitous route to the Royal Free Hospital pathology collection, where it remained until we (GC and TW) ‘discovered’ it. The bones represent the largest assemblage of material from cases of congenital syphilis in the UK and they are important as they clearly demonstrate the skeletal lesions associated with congenital syphilis and are now irreplaceable. The bones have been identified to anatomical element and have come from a minimum of eight children, both foetuses and neonates covering the period 30–50 weeks post-conception. Radiological and micro computerised tomography examinations were carried out and three-dimensional models printed at twice life size. The models are durable and can be handled with impunity by students and others wishing to familiarise themselves with the skeletal changes shown.
Individuals with a borderline personality disorder (BPD) suffer from a constellation of rapidly shifting emotional, interpersonal, and behavioral symptoms. The menstrual cycle may contribute to symptom instability among females with this disorder.
Methods
Fifteen healthy, unmedicated females with BPD and without dysmenorrhea reported daily symptoms across 35 days. Urine luteinizing hormone and salivary progesterone (P4) were used to confirm ovulation and cycle phase. Cyclical worsening of symptoms was evaluated using (1) phase contrasts in multilevel models and (2) the Carolina Premenstrual Assessment Scoring System (C-PASS), a protocol for evaluating clinically significant cycle effects on symptoms.
Results
Most symptoms demonstrated midluteal worsening, a perimenstrual peak, and resolution of symptoms in the follicular or ovulatory phase. Post-hoc correlations with person-centered progesterone revealed negative correlations with most symptoms. Depressive symptoms showed an unexpected delayed pattern in which baseline levels of symptoms were observed in the ovulatory and midluteal phases, and exacerbations were observed during both the perimenstrual and follicular phases. The majority of participants met C-PASS criteria for clinically significant (⩾30%) symptom exacerbation. All participants met the emotional instability criterion of BPD, and no participant met DSM-5 criteria for premenstrual dysphoric disorder (PMDD).
Conclusions
Females with BPD may be at elevated risk for perimenstrual worsening of emotional symptoms. Longitudinal studies with fine-grained hormonal measurement as well as hormonal experiments are needed to determine the pathophysiology of perimenstrual exacerbation in BPD.
Speech disfluencies can guide the ways in which listeners interpret spoken language. Here, we examined whether three-year-olds, five-year-olds, and adults use filled pauses to anticipate that a speaker is likely to refer to a novel object. Across three experiments, participants were presented with pairs of novel and familiar objects and heard a speaker refer to one of the objects using a fluent (“Look at the ball/lep!”) or disfluent (“Look at thee uh ball/lep!”) expression. The salience of the speaker's unfamiliarity with the novel referents, and the way in which the speaker referred to the novel referents (i.e., a noun vs. a description) varied across experiments. Three- and five-year-olds successfully identified familiar and novel targets, but only adults’ looking patterns reflected increased looks to novel objects in the presence of a disfluency. Together, these findings demonstrate that adults, but not young children, use filled pauses to anticipate reference to novel objects.
Herbicides inhibit biochemical and physiological processes or both with lethal consequences. The target sites of these small molecules are usually enzymes involved in primary metabolic pathways or proteins carrying out essential physiological functions. Herbicides tend to be highly specific for their respective target sites and have served as tools to study these physiological and biochemical processes in plants (Dayan et al. 2010b).
Ten sites near the Snake River Plain have consistent differences in their climatic histories. Sites at low elevation reflect the “early Holocene xerothermic” of the Pacific Northwest, whereas most climatic chronologies at high elevation indicate maximum warmth or aridity somewhat later, ca. 6000 yr ago. This elevational contrast in climatic histories is duplicated at three sites from the central Snake River Plain. For sites in such close proximity, the different chronologies cannot be explained by changes in atmospheric circulation during the late Quaternary. Rather, the differences are best explained by the autecology of the plants involved and the changing seasonal climate. The seasonal climatic sequence predicted by multiple thermal maxima explains the high- and low-elevation chronologies. During the early Holocene, maximum insolation and intensified summer drought in July forced low-elevation vegetation upward. However, moisture was not a limiting factor at high elevation, where vegetation moved upward in response to increased length of growing season coincident with maximum September insolation 6000 yr ago.
Downy brome (cheatgrass) is a highly successful, exotic, winter annual invader in semi-arid western North America, forming near-monocultures across many landscapes. A frequent but poorly understood phenomenon in these heavily invaded areas is periodic ‘die-off’ or complete stand failure. The fungal pathogen Pyrenophora semeniperda is abundant in cheatgrass seed banks and causes high mortality. To determine whether this pathogen could be responsible for stand failure, we quantified late spring seed banks in die-off areas and adjacent cheatgrass stands at nine sites. Seed bank analysis showed that this pathogen was not a die-off causal agent at those sites. We determined that seed bank sampling and litter data could be used to estimate time since die-off. Seed bank patterns in our recent die-offs indicated that the die-off causal agent does not significantly impact seeds in the persistent seed bank.
We present new, deep VLA 327 MHz, GALEX Far-UV, and Hα images of the inner ~50 kpc of Centaurus A. We find the structure identified by Morganti et al. 1999 as a possible “large scale jet” is part of a knotty, linear feature within a broader region of diffuse radio emission. The linear feature is coincident with a narrow ribbon of Far-UV and Hα emission that extends 6-35 kpc from the galaxy core, as well with a similar ridge of soft X-ray emission. The Far-UV image also shows that a strong starburst is occurring in the central dusty disk, with a star-formation rate of ~ 2M⊙ yr−1. We suggest that the various peculiar phenomena seen to the NE of the galaxy can be explained by a wind from the starburst disk, enhanced by energy input from the AGN.
