Soc., to appear] have shown that the integer sequence $(x_n)_{n\geq 1}$ defined by $x_{n+2}=\phi (x_{n+1})+\phi (x_{n})+k$, where $x_1,x_2\geq 1$, $k\geq 0$ and $2 \mid k$, is bounded by $4^{X^{3^{k+1}}}$, where $X=(3x_1+5x_2+7k)/2$. We improve this result by showing that the sequence $(x_n)$ is bounded by $2^{2X^2+X-3}$, where $X=x_1+x_2+2k$.

  • Purpose:

    This study compared the plan dosimetry between the intensity-modulated radiation therapy (IMRT) and field-in-field (FIF) technique for head-and-neck cancer using the Elekta Monaco treatment planning system (TPS).

    Materials and methods:

    A total of 20 head-and-neck cancer patients were selected in this study. IMRT and FIF plans for the patients were created on the Monaco TPS (ver. 5.11.02) using the 6-MV photon beam generated by the Elekta Synergy linear accelerator. The dose–volume histograms, maximum doses, minimum doses, mean doses of the target volumes and organs-at-risk (OARs), conformity index (CI), homogeneity index (HI) and monitor units (MUs) were determined for each IMRT and FIF plan. All IMRT plans passed the patient-specific quality assurance tests from the 2D diode array measurements (MatriXX Evolution System, IBA Dosimetry, Germany).

    Results:

    The results showed that the dose distribution to the target volumes of IMRT plans was better than FIF plans, while the dose (mean or max dose) to the OAR was significantly lower than FIF plan, respectively. IMRT and FIF resulted in planning target volume coverage with mean dose of 71·32 ± 0·76 and 73·12 ± 0·62 Gy, respectively, and HI values of 0·08 ± 0·01 (IMRT) and 0·19 ± 0·06 (FIF). The CI for IMRT was 0·98 ± 0·01 and FIF was 0·97 ± 0·01. For the spinal cord tolerance (maximum dose < 45 Gy), IMRT resulted in 39·85 ± 2·04 Gy compared to 41·37 ± 2·42 Gy for FIF. In addition, the mean doses to the parotid grand were 27·27 ± 7·48 and 48·68 ± 1·62 Gy for the IMRT and FIF plans, respectively. Significantly more MUs were required in IMRT plans than FIF plans (on average, 846 ± 100 MU in IMRT and 467 ± 41 MU in FIF).

    Conclusions:

    It is concluded that the IMRT technique could provide a better plan dosimetry than the FIF technique for head-and-neck patients.

  • As someone who has spent most of his career writing about others, i find it odd to see myself, or what passes for me, being written about by others, especially academics, the people with whom I am most familiar. Who is this “Viet Thanh Nguyen” that they are discussing? Is he the one whose name appears as such on the cover of the Vietnamese translation of his short story collection, The Refugees, instead of as Nguyên Thanh Vie t, his Vietnamese name? Has “Viet Thanh Nguyen” disabled, in himself, the authenticity that most Vietnamese people hold dear and that he has been reminded of constantly throughout his life, symbolized in his name and his mother tongue? More important and urgent to him, is “Viet Thanh Nguyen,” in being discussed in such a manner, already dead?

  • Summary

    This chapter talks about three texts which establish the social and literary heterogeneity within the contested terrains of Chinatown and its literature. The two best-known Chinese American depictions of San Francisco's Chinatown from the 1950s are Jade Snow Wong's memoir Fifth Chinese Daughter and C. Y. Lee's novel The Flower Drum Song. Along with C. Y. Lee, Wong was one of a group of ethnic writers and artists whose efforts to promote America's influence abroad were valued, so long as they asserted and embodied the presence of opportunity for minorities. Lee's nuanced treatment of food, dialects, space and Chinese politics marks Chinatown and its representations as contested terrain. Hsi-Tseng Tsiang is arguably the first Chinese American novelist to publish in English. Poet, novelist, playwright, actor and activist, Tsiang combined formal experimentation and strategic appropriation from both Chinese and English literature with a lifelong commitment to left-wing activism. Tsiang's novel, And China Has Hands, indicates American capitalism and Japanese imperialism.

  • The line between a contact zone and a comfort zone is a thin one. I can imagine that line drawn in many ways, but the color line of which W. E. B. DuBois spoke is the one that concerns Michael Omi and Howard Winant. In Racial Formation in the United States, when they declare that “race has no fixed meaning” (71), they mean that this color line is always being redrawn, by different hands and differing hearts. It is this possibility of racial transformation, not just racial formation, that makes Omi and Winant's theory powerful and compelling. With transformation in mind, we can conceive of racial formation as another version of Mary Louise Pratt's contact zone, where friction sparks unpredictable futures and where, by implication, our racial present may not look like our racial past. At the same time, racial formation is also reformation, affirming identities with which we feel at ease. In these comfort zones of inherited identities, we encourage others and are encouraged ourselves to toe the (color) line. These racial tendencies, toward change and constancy, innovation and cliché, are evident in both politics and culture. Although Omi and Winant's sociology of race is not concerned with literature, their insightful model of racial formation is manifest in literary culture, in aesthetic form and in literary institutions: the publishing industry, the literary marketplace, and the department of literature, where racial politics are always present.

  • Background:

    Few studies have been conducted in Vietnam on the epidemiology of healthcare-associated infections or antimicrobial use. Thus, we sought to determine the prevalence of and risk factors for surgical-site infections (SSIs) and to document antimicrobial use in surgical patients in a large healthcare facility in Vietnam.

    Methods:

    We conducted a point-prevalence survey of SSIs and antimicrobial use at Cho Ray Hospital, Ho Chi Minh City, a 1,250-bed inpatient facility. All patients on the 11 surgical wards and 2 intensive care units who had surgery within 30 days before the survey date were included.

    Results:

    Of 391 surgical patients, 56 (14.3%) had an SSI. When we compared patients with and without SSIs, factors associated with infection included trauma (relative risk [RR], 2.65; 95% confidence interval [CI95], 1.60 to 4.37; P < .001), emergency surgery (RR, 2.74; CI95, 1.65 to 4.55; P < .001), and dirty wounds (RR, 3.77; CI95, 2.39 to 5.96; P < .001). Overall, 198 (51%) of the patients received antimicrobials more than 8 hours before surgery and 390 (99.7%) received them after surgery. Commonly used antimicrobials included third-generation cephalosporins and aminoglycosides. Thirty isolates were identified from 26 SSI patient cultures; of the 25 isolates undergoing antimicrobial susceptibility testing, 22 (88%) were resistant to ceftriaxone and 24 (92%) to gentamicin.

    Conclusions:

    Our data show that (1) SSIs are prevalent at Cho Ray Hospital; (2) antimicrobial use among surgical patients is widespread and inconsistent with published guidelines; and (3) pathogens often are resistant to commonly used antimicrobials. SSI prevention interventions, including appropriate use of antimicrobials, are needed in this population.
