Bipolar disorder is a chronic and severe mental health disorder. Early stratification of individuals into subgroups based on age at onset (AAO) has the potential to inform diagnosis and early intervention. Yet, the psychosocial predictors associated with AAO are unknown.
Aims
We aim to identify psychosocial factors associated with bipolar disorder AAO.
Method
Using data from the Bipolar Disorder Research Network UK, we employed least absolute shrinkage and selection operator regression to identify psychosocial factors associated with bipolar disorder AAO. Twenty-eight factors were entered into our model, with AAO as our outcome measure.
Results
We included 1022 participants with bipolar disorder (μ = 23.0, s.d. ± 9.86) in our model. Six variables predicted an earlier AAO: childhood abuse (β = −0.2855), regular cannabis use in the year before onset (β = −0.2765), death of a close family friend or relative in the 6 months before onset (β = −0.2435), family history of suicide (β = −0.1385), schizotypal personality traits (β = −0.1055) and irritable temperament (β = −0.0685). Five predicted a later AAO: the average number of alcohol units consumed per week in the year before onset (β = 0.1385); birth of a child in the 6 months before onset (β = 0.2755); death of parent, partner, child or sibling in the 6 months before onset (β = 0.3125); seeking work without success for 1 month or more in the 6 months before onset (β = 0.3505) and a major financial crisis in the 6 months before onset (β = 0.4575).
Conclusions
The identified predictor variables have the potential to help stratify high-risk individuals into likely AAO groups, to inform treatment provision and early intervention.
An antimicrobial stewardship bundle was implemented in 23 community health system urgent care and primary care clinics to reduce fluoroquinolone prescribing in urinary tract infections. The percentage of urinary tract infection (UTI) visits prescribed a fluoroquinolone subsequently decreased from 17.6% to 3% in urgent care and from 23.8% to 6.8% in primary care.
Although there is growing interest in mental health problems in university students there is limited understanding of the scope of need and determinants to inform intervention efforts.
Aims
To longitudinally examine the extent and persistence of mental health symptoms and the importance of psychosocial and lifestyle factors for student mental health and academic outcomes.
Method
Undergraduates at a Canadian university were invited to complete electronic surveys at entry and completion of their first year. The baseline survey measured important distal and proximal risk factors and the follow-up assessed mental health and well-being. Surveys were linked to academic grades. Multivariable models of risk factors and mental health and academic outcomes were fit and adjusted for confounders.
Results
In 1530 students surveyed at entry to university 28% and 33% screened positive for clinically significant depressive and anxiety symptoms respectively, which increased to 36% and 39% at the completion of first year. Over the academic year, 14% of students reported suicidal thoughts and 1.6% suicide attempts. Moreover, there was persistence and overlap in these mental health outcomes. Modifiable psychosocial and lifestyle factors at entry were associated with positive screens for mental health outcomes at completion of first year, while anxiety and depressive symptoms were associated with lower grades and university well-being.
Conclusions
Clinically significant mental health symptoms are common and persistent among first-year university students and have a negative impact on academic performance and well-being. A comprehensive mental health strategy that includes a whole university approach to prevention and targeted early-intervention measures and associated research is justified.
The Lower Pennsylvanian Hale Formation, which comprises the lower portion of the type Morrowan Series, in northwestern Arkansas, is subdivided into the Cane Hill and overlying Prairie Grove Members. The Cane Hill Member includes interbedded shale and sandstone with a basal conglomerate containing clasts reworked from underlying, truncated Mississippian formations. The Prairie Grove Member is highly variable, but includes sandy biosparite and calcareous sandstone. Highly fossiliferous pebble conglomerate and calcirudite lenses occur sporadically throughout the Hale Formation. Ammonoids and conodonts show that the Cane Hill-Prairie Grove boundary is unconformable.
Several thousand ammonoids collected from more than 100 localities in the Hale Formation show that four ammonoid zones and two subzones are recognizable in the Hale succession, and consequently in the redefined Halian Stage of the Morrowan Series. These are, in ascending order, the Retites semiretia, Quinnites henbesti, Arkanites relictus (including the Arkanites relictus relictus and overlying Cancelloceras huntsvillense Subzones) and Verneuilites pygmaeus Zones. Halian Stage ammonoids are known primarily from northern Arkansas, but an upper Arkanites relictus Zone (Cancelloceras huntsvillense Subzone) ammonoid assemblage occurs in the Primrose Member of the Golf Course Formation in south central Oklahoma.
Conodont-ammonoid associations in the Hale sequence provide a basis for integration of independently based zonal information. Rhachistognathus primus Zone conodonts occur in the Retites semiretia Zone; the Idiognathoides sinuatus Zone ranges through the Quinnites henbesti and Arkanites relictus relictus Subzone. The overlying Cancelloceras huntsvillense Subzone and Verneuilites pygmaeus Zone both contain conodonts of the Neognathodus symmetricus Zone.
The Hale ammonoid succession has few, if any, species in common with the type Namurian of Europe, but numerous genera are common to both sequences and the generic successions coincide and are equivalent in degree of development. The Retites semiretia Zone is equivalent to the Reticuloceras circumplicatile Zone (R1a); the Quinnites henbesti and lower Arkanites relictus Zones correspond to some portion of the R2b–R2c interval; and the upper Arkanites relictus Zone and the Verneuilites pygmaeus Zone correlate to Zone G1. The Retites semiretia Zone correlates to the lower Reticuloceras-Baschkortoceras Genozone (Nm2b1) of the upper Namurian in the south Urals; the Quinnites henbesti Zone is equivalent to some portion of the Nm2b2–3 intervals of this zone; the lower Arkanites relictus Zone is equivalent to the lower Bilinguites-Cancelloceras Genozone (Nm2c1) and the upper Arkanites relictus and Verneuilites pygmaeus Zones correspond to the uppermost interval (Nm2c2) in the south Urals sequence.
Systematic descriptions of biostratigraphically significant Halian taxa, including Reticuloceras tiro Gordon, R. wainwrighti Quinn, Retites semiretia McCaleb, Arkanites relictus relictus (Quinn, McCaleb, and Webb), A. relictus redivivus n.subsp., Quinnites n.gen. (type species Q. henbesti (Gordon)), Q. textum (Gordon), Bilinguites eliasi n.sp., Cancelloceras huntsvillense n.sp., and Verneuilites pygmaeus (Mather) are also presented.
The term mild cognitive impairment has been associated with a varying degree of clinical utility and controversy. The concept has been introduced to try and define a pre-dementia period associated with underlying neurodegenerative pathology and a higher likelihood of the person developing a dementia syndrome. As scientific understanding improves then the definition of MCI rightly adapts, meaning that the MCI concept is prone quite rightly to frequent evolution. We consider that we are a long way away from the concept having evolved to a point where it can be embedded with confidence in clinical practice as a diagnosis but should remain as a term primarily for use in research.
Greenhouse and laboratory studies were conducted to determine the degree of dominance of the monogenic sulfonylurea herbicide resistance trait in diploid sugarbeet by comparing the response of homozygous and heterozygous resistant sugarbeet to primisulfuron, thifensulfuron, and chlorimuron on the whole plant and acetolactate synthase (ALS) enzyme level. Progeny tests suggested that the monogenic sulfonylurea herbicide resistance was semidominant. Subsequently, heterozygous resistant (R-1) and homozygous resistant (R-2) sugarbeet lines were sprayed with increasing rates of primisulfuron, thifensulfuron, and chlorimuron, and herbicide rates required for 50% growth reduction (GR50) were determined. GR50 values were also determined for homozygous susceptible sugarbeet lines (S-1 and S-2). The GR50 values indicated that the R-2 sugarbeet was 377, 269, and 144 times more resistant to primisulfuron, thifensulfuron, and chlorimuron, respectively, than susceptible S-2 sugarbeet. In contrast, R-1 sugarbeet was only 107, 76, and 57 times more resistant to primisulfuron, thifensulfuron, and chlorimuron, respectively, than S-1 sugarbeet, indicating at least a twofold difference in the magnitude of resistance between homozygous resistant and heterozygous resistant sugarbeet lines. ALS enzyme activity analysis were consistent with whole plant results. Thus, based on these two, maximum crop resistance can be obtained by developing homozygous resistant cultivars.
Greenhouse and laboratory studies were conducted to determine the extent of cross-resistance of chlorsulfuron-resistant sugarbeet (CR1-B) to other herbicides that inhibit acetolactate synthase (ALS) and to determine the physiological basis of resistance. Cross-resistance to metsulfuron, imazaquin, and imazethapyr was not evident, while only marginal cross-resistance was observed to triasulfuron, DPX-L5300, and nicosulfuron. CR1-B was moderately resistant to chlorsulfuron and chlorimuron and was highly cross-resistant to thifensulfuron and primisulfuron. Further greenhouse studies demonstrated that CR1-B was not significantly injured by thifensulfuron and primisulfuron applied at or exceeding the field use rate. Studies with 14C-primisulfuron showed that differential absorption or metabolism of primisulfuron could not account for the observed resistance. ALS enzyme assays showed that the CR1-B ALS enzyme activity was 66, 26, and 13 times less sensitive to chlorsulfuron, thifensulfuron, and primisulfuron inhibition, respectively, compared to ALS enzyme extracted from sensitive sugarbeets. An altered ALS enzyme, which is less sensitive to sulfonylurea herbicide inhibition, appears to be the physiological basis of resistance.
Free Expression and Democracy takes on the assumption that limits on free expression will lead to authoritarianism or at least a weakening of democracy. That hypothesis is tested by an examination of issues involving expression and their treatment in countries included on The Economist's list of fully functioning democracies. Generally speaking, other countries allow prohibitions on hate speech, limits on third-party spending on elections, and the protection of children from media influences seen as harmful. Many ban Holocaust denial and the desecration of national symbols. Yet, these other countries all remain democratic, and most of those considered rank more highly than the United States on the democracy index. This book argues that while there may be other cultural values that call for more expansive protection of expression, that protection need not reach the level present in the United States in order to protect the democratic nature of a country.
