Variable ranking and selection with random forest for unbalanced data

When one or several classes are much less prevalent than another class (unbalanced data), class error rates and variable importances of the machine learning algorithm random forest can be biased, particularly when sample sizes are smaller, imbalance levels higher, and effect sizes of important variables smaller. Using simulated data varying in size, imbalance level, number of true variables, their effect sizes, and the strength of multicollinearity between covariates, we evaluated how eight versions of random forest ranked and selected true variables out of a large number of covariates despite class imbalance. The version that calculated variable importance based on the area under the curve (AUC) was least adversely affected by class imbalance. For the same number of true variables, effect sizes, and multicollinearity between covariates, the AUC variable importance ranked true variables still highly at the lower sample sizes and higher imbalance levels at which the other seven versions no longer achieved high ranks for true variables. Conversely, using the Hellinger distance to split trees or downsampling the majority class already ranked true variables lower and more variably at the larger sample sizes and lower imbalance levels at which the other algorithms still ranked true variables highly. In variable selection, a higher proportion of true variables were identified when covariates were ranked by AUC importances and the proportion increased further when the AUC was used as the criterion in forward variable selection. In three case studies, known species–habitat relationships and their spatial scales were identified despite unbalanced data.

Background
Studying phenotypic and genetic characteristics of age at onset (AAO) and polarity at onset (PAO) in bipolar disorder can provide new insights into disease pathology and facilitate the development of screening tools.
AimsTo examine the genetic architecture of AAO and PAO and their association with bipolar disorder disease characteristics.
MethodGenome-wide association studies (GWASs) and polygenic score (PGS) analyses of AAO (n = 12 977) and PAO (n = 6773) were conducted in patients with bipolar disorder from 34 cohorts and a replication sample (n = 2237). The association of onset with disease characteristics was investigated in two of these cohorts.
ResultsEarlier AAO was associated with a higher probability of psychotic symptoms, suicidality, lower educational attainment, not living together and fewer episodes. Depressive onset correlated with suicidality and manic onset correlated with delusions and manic episodes. Systematic differences in AAO between cohorts and continents of origin were observed. This was also reflected in single-nucleotide variant-based heritability estimates, with higher heritabilities for stricter onset definitions. Increased PGS for autism spectrum disorder (β = −0.34 years, s.e. = 0.08), major depression (β = −0.34 years, s.e. = 0.08), schizophrenia (β = −0.39 years, s.e. = 0.08), and educational attainment (β = −0.31 years, s.e. = 0.08) were associated with an earlier AAO. The AAO GWAS identified one significant locus, but this finding did not replicate. Neither GWAS nor PGS analyses yielded significant associations with PAO.
ConclusionsAAO and PAO are associated with indicators of bipolar disorder severity. Individuals with an earlier onset show an increased polygenic liability for a broad spectrum of psychiatric traits. Systematic differences in AAO across cohorts, continents and phenotype definitions introduce significant heterogeneity, affecting analyses.

Summary
Neonates frequently suffer from life threatening infections. Immaturity of the immune system increases the vulnerability to infection, and the preterm and term neonatal immune system has specific deficiencies relative to that of an older child or adult [1, 2]. During pregnancy, the physical barrier of the placenta and the maternal immune system protect the developing human fetus from infection. However, maternal infections such as rubella, almost eradicated in developed nations through vaccination [3], or the zika virus, an emerging pathogen [4, 5], can ravage the developing embryo and fetus, leading to life-long disabilities. Furthermore, immaturity of natural barrier systems such as skin, bronchial epithelium and the lining of the gastrointestinal tract compound the weaknesses of the immune system of the premature infant [6, 7]. The importance of interactions between the developing immune system, epithelial barriers, and the microbiome to protect the preterm neonate from infection and promote health is increasingly recognized [8, 9]. Ethical and political concerns limit our ability to study the embryological development of the human immune system to the same depth [10, 11].
Attention, vigilance, and visuospatial function in hospitalized elderly medical patients: relationship to delirium syndromal status and motor subtype profile

Background:
The early and effective detection of neurocognitive disorders poses a key diagnostic challenge. We examined performance on common cognitive bedside tests according to differing delirium syndromal status and clinical (motor) subtypes in hospitalized elderly medical inpatients.
Methods:A battery of nine bedside cognitive tests was performed on elderly medical inpatients with DSM-IV delirium, subsyndromal delirium (SSD), and no delirium (ND). Patients with delirium were compared according to clinical (motor) subtypes.
Results:A total of 198 patients (mean age 79.14 ± 8.26) were assessed with full syndromal delirium (FSD: n = 110), SSD (n = 45), and ND (n = 43). Delirium status was not associated with differences in terms of gender distribution, age, or overall medication use. Dementia burden increased with greater delirium status. Overall, the ability to meaningfully engage with the tests varied from 59% for the Vigilance B test to 85% for Spatial Span Forward test and was lowest in patients with FSD, where engagement ranged from 32% for the Vigilance B test to 77% for the Spatial Span Forwards test. The ND group was distinguished from SSD group for the Months of the year backwards, Vigilance B, global VSP, Clock Drawing test, and Interlocking Pentagons test. The SSD group was distinguished from the FSD group by Vigilance A, Spatial Span Forward, and Spatial Span Backwards. Regarding differences among motor subtypes in terms of percentage engagement and performance, the No subtype group had higher ratings across all tests. Delirious patients with no subtype had significantly lower scores on the DRS-R98 than for the other three subtype categories.
Conclusions:Simple bedside tests of attention, vigilance, and visuospatial ability are useful in distinguishing neurocognitive disorders, including SSD from other presentations.

Mycobacterium simiae Outbreak Associated With a Hospital Water Supply

Objective:
Mycobacterium simiae is found primarily in the southwestern United States, Israel, and Cuba, with tap water as its suspected reservoir. Our institution saw an increase in M. simiae isolates in 2001. An investigation into possible contaminated water sources was undertaken.
Design:Environmental cultures were performed from water taps in the microbiology laboratory, patient rooms, points in the flow of water to the hospital, and patients' homes. Patient and environmental M. simiae were compared by PFGE.
Setting:Military treatment facility in San Antonio, Texas.
Patients:All patients with cultures positive for M. simiae between January 2001 and April 2002. Medical records were reviewed.
Results:M. simiae was recovered from water samples from the hospital, patients' home showers, and a well supplying the hospital. A single PFGE clone was predominant among water isolates (9 of 10) and available patient isolates (14 of 19). There was an association between exposure to hospital water and pulmonary samples positive for the clonal M. simiae strain (P = .0018). Only 3 of 22 culture-positive patients met criteria for M. simiae pulmonary disease. Of them, two had indistinguishable M. simiae strains from tap water to which they were routinely exposed.
Conclusions:This represents an outbreak of M. simiae colonization with one nosocomial infection. It is only the second time that M. simiae has been recovered from hospital tap water and its first presentation in municipal water. This study raises issues about the need and feasibility of eliminating or avoiding exposure to M. simiae from tap water.

Kenyan runners, and especially those originating from the Kalenjin tribe, have dominated international middle- and long-distance running for over 40 years, prompting significant interest in the factors contributing to their success. Proposed explanations have included environmental factors, psychological advantage and favourable physiological characteristics, which may be genetically conferred or environmentally determined. Running is inherent within local Kenyan tradition and culture, and the Kenyan way of life, which involves many outdoor activities and pastimes in addition to mostly unfavourable living conditions, is conducive to enhanced distance running performance. Despite economic deprivation, Kenya has produced world and international running champions repeatedly over the past few decades; these champions have become role models for the younger generations, who take up running in the hope of a better future for themselves. Favourable environmental conditions such as altitude, diet and anthropometry, in addition to the motivational and socio-economic factors mentioned above, have all been proposed as possible reasons for the unsurpassed achievements of Kenyan distance runners. However, the fact that the majority of internationally successful runners originate from a small tribe that accounts for approximately 3% of the total Kenyan population also points to a possible genetic component. Whether this is subject to influence from other co-factors, such as altitude or training effects acquired during childhood, remains as yet unresolved.

After initial widespread use of private cars under the “common road” concept of early railways, railroad-owned freight cars predominated from the 1840's through the 1860's, except for a short-lived boom in cars owned by “fast freight” lines. From this time on, however, the percentage of private cars has increased as railroads refused to build specialized freight cars because of high initial costs, rapid technological obsolescence, outside pressure, and managerial shortsightedness.
