Do I need treatment? Insight and therapy adherence in a group of patients in treatment with aripiprazole long acting injectable

Introduction
Relapse prevention is one of the major goals in psychiatry because it will mark the prognosis and quality of life in patients. In this aspect, plays a very important role insight and adherence to treatment. Some studies show that long acting injectable treatments improve adherence as compared with oral medications.
ObjectiveThe objective of this study is to analyse the degree of insight and adherence to treatment of a group of patients with aripiprazole long acting injectable ALAI treatment.
MethodologyDescriptive study in a group of patients treated with ALAI. Three months of follow-up. To assess the insight we had use the 3 first items of the Unawareness of Mental Disorder SUMD. Adherence to treatment is evaluated by monitoring if patients come to administrate ALAI.
Resultsn = 6, 2 women (33.3%) and 4 men (66.7%). The mean age of the sample was 37 years. Diagnoses were 4 patients with psychotic disorder (66.7%;) 2 with bipolar disorder (33.3%). At the SUMD, mean score for the items consciousness of having a disorder and awareness of the effects of medication was 3, and for the item awareness of the social consequences of mental disorder 4. The mean score for the total punctuation was 10. Five (83.3%) had a 100% of adherence, just 1 patient discontinued de treatment on the third month.
ConclusionsOn our sample adherence, it was almost complete in all patients. To highlight the scores for SUMD which means low insight. A large sample is required to increase the reliability of the study.

There is no suitable vaccine against human visceral leishmaniasis (VL) and available drugs are toxic and/or present high cost. In this context, diagnostic tools should be improved for clinical management and epidemiological evaluation of disease. However, the variable sensitivity and/or specificity of the used antigens are limitations, showing the necessity to identify new molecules to be tested in a more sensitive and specific serology. In the present study, an immunoproteomics approach was performed in Leishmania infantum promastigotes and amastigotes employing sera samples from VL patients. Aiming to avoid undesired cross-reactivity in the serological assays, sera from Chagas disease patients and healthy subjects living in the endemic region of disease were also used in immunoblottings. The most reactive spots for VL samples were selected, and 29 and 21 proteins were identified in the promastigote and amastigote extracts, respectively. Two of them, endonuclease III and GTP-binding protein, were cloned, expressed, purified and tested in ELISA experiments against a large serological panel, and results showed high sensitivity and specificity values for the diagnosis of disease. In conclusion, the identified proteins could be considered in future studies as candidate antigens for the serodiagnosis of human VL.
Diarrhoeagenic Escherichia coli and Escherichia albertii in Brazil: pathotypes and serotypes over a 6-year period of surveillance

Diarrhoeagenic Escherichia coli (DEC) is a leading cause of infectious diarrhoea worldwide. In recent years, Escherichia albertii has also been implicated as a cause of human enteric diseases. This study describes the occurrence of E. coli pathotypes and serotypes associated with enteric illness and haemolytic uremic syndrome (HUS) isolated in Brazil from 2011 to 2016. Pathotypes isolated included enteropathogenic E. coli (EPEC), enteroaggregative E. coli (EAEC), enterotoxigenic E. coli (ETEC), enteroinvasive E. coli (EIEC) and Shiga toxin-producing E. coli (STEC). PCR of stool enrichments for DEC pathotypes was employed, and E. albertii was also sought. O:H serotyping was performed on all DEC isolates. A total of 683 DEC and 10 E. albertii strains were isolated from 5047 clinical samples. The frequencies of DEC pathotypes were 52.6% (359/683) for EPEC, 32.5% for EAEC, 6.3% for ETEC, 4.4% for EIEC and 4.2% for STEC. DEC strains occurred in patients from 3 months to 96 years old, but EPEC, EAEC and STEC were most prevalent among children. Both typical and atypical isolates of EPEC and EAEC were recovered and presented great serotype heterogeneity. HUS cases were only associated with STEC serotype O157:H7. Two E. albertii isolates belonged to serogroup O113 and one had the stx2f gene. The higher prevalence of atypical EPEC in relation to EAEC in community-acquired diarrhoea in Brazil suggests a shift in the trend of DEC pathotypes circulation as previously EAEC predominated. This is the first report of E. albertii isolation from active surveillance. These results highlight the need of continuing DEC and E. albertii surveillance, as a mean to detect changes in the pattern of pathotypes and serotypes circulation and provide useful information for intervention and control strategies.
High-through identification of T cell-specific phage-exposed mimotopes using PBMCs from tegumentary leishmaniasis patients and their use as vaccine candidates against Leishmania amazonensis infection

In the current study, phage-exposed mimotopes as targets against tegumentary leishmaniasis (TL) were selected by means of bio-panning cycles employing sera of TL patients and healthy subjects, besides the immune stimulation of peripheral blood mononuclear cells (PBMCs) collected from untreated and treated TL patients and healthy subjects. The clones were evaluated regarding their specific interferon-γ (IFN-γ) and interleukin-4 (IL-4) production in the in vitro cultures, and selectivity and specificity values were calculated, and those presenting the best results were selected for the in vivo experiments. Two clones, namely A4 and A8, were identified and used in immunization protocols from BALB/c mice to protect against Leishmania amazonensis infection. Results showed a polarized Th1 response generated after vaccination, being based on significantly higher levels of IFN-γ, IL-2, IL-12, tumour necrosis factor-α (TNF-α) and granulocyte-macrophage colony-stimulating factor (GM-CSF); which were associated with lower production of specific IL-4, IL-10 and immunoglobulin G1 (IgG1) antibodies. Vaccinated mice presented significant reductions in the parasite load in the infected tissue and distinct organs, when compared with controls. In conclusion, we presented a strategy to identify new mimotopes able to induce Th1 response in PBMCs from TL patients and healthy subjects, and that were successfully used to protect against L. amazonensis infection.
Topical tamoxifen in the therapy of cutaneous leishmaniasis

The aims of the present work were to test the effect of tamoxifen administered topically and the therapeutic efficacy of tamoxifen and pentavalent antimonial combinations in an experimental model of cutaneous leishmaniasis. BALB/c mice infected with a luciferase expressing line of Leishmania amazonensis were treated with topical tamoxifen in two different formulations (ethanol or oil-free cream) as monotherapy or in co-administration with pentavalent antimonial. Treatment efficacy was evaluated by lesion size and parasite burden, quantified through luminescence, at the end of treatment and 4 weeks later. Topical tamoxifen, formulated in ethanol or as a cream, was shown to be effective. The interaction between tamoxifen and pentavalent antimonial was additive in vitro. Treatment with combined schemes containing tamoxifen and pentavalent antimonial was effective in reducing lesion size and parasite burden. Co-administration of tamoxifen and pentavalent antimonial was superior to monotherapy with antimonial.
Upwelling signals in radiocarbon from early 20th-century Peruvian bay scallop (Argopecten purpuratus) shells

We quantified Δ14C, δ18O, and δ13C cycles along ontogeny within four bay scallop (Argopecten purpuratus) shells collected from Callao Bay, Salaverry, and Sechura Bay, Peru following the 1907–1908 non-El Niño years and the 1925–1926 El Niño. Δ14C and δ13C generally covary; Δ14C and δ18O vary inversely. Simultaneous decreases in Δ14C and increases in δ18O in non-El Niño shells are followed by constant Δ14C and gradually decreasing δ18O, which we interpret as evidence for discrete marine upwelling events followed by warming of the initially cold upwelled water. Upwelling changes from El Niño events are detectable with difficulty in mollusk shell Δ14C.

The science of extra-solar planets is one of the most rapidly changing areas of astrophysics and since 1995 the number of planets known has increased by almost two orders of magnitude. A combination of ground-based surveys and dedicated space missions has resulted in 560-plus planets being detected, and over 1200 that await confirmation. NASA's Kepler mission has opened up the possibility of discovering Earth-like planets in the habitable zone around some of the 100,000 stars it is surveying during its 3 to 4-year lifetime. The new ESA's Gaia mission is expected to discover thousands of new planets around stars within 200 parsecs of the Sun. The key challenge now is moving on from discovery, important though that remains, to characterisation: what are these planets actually like, and why are they as they are?
In the past ten years, we have learned how to obtain the first spectra of exoplanets using transit transmission and emission spectroscopy. With the high stability of Spitzer, Hubble, and large ground-based telescopes the spectra of bright close-in massive planets can be obtained and species like water vapour, methane, carbon monoxide and dioxide have been detected. With transit science came the first tangible remote sensing of these planetary bodies and so one can start to extrapolate from what has been learnt from Solar System probes to what one might plan to learn about their faraway siblings. As we learn more about the atmospheres, surfaces and near-surfaces of these remote bodies, we will begin to build up a clearer picture of their construction, history and suitability for life.
The Exoplanet Characterisation Observatory, EChO, will be the first dedicated mission to investigate the physics and chemistry of Exoplanetary Atmospheres. By characterising spectroscopically more bodies in different environments we will take detailed planetology out of the Solar System and into the Galaxy as a whole.
EChO has now been selected by the European Space Agency to be assessed as one of four M3 mission candidates.

Centuries of Marine Radiocarbon Reservoir Age Variation Within Archaeological Mesodesma Donacium Shells from Southern Peru

Mollusk shells provide brief (<5 yr per shell) records of past marine conditions, including marine radiocarbon reservoir age (R) and upwelling. We report 21 14C ages and R calculations on small (∼2 mg) samples from 2 Mesodesma donacium (surf clam) shells. These shells were excavated from a semi-subterranean house floor stratum 14C dated to 7625 ± 35 BP at site QJ-280, Quebrada Jaguay, southern Peru. The ranges in marine 14C ages (and thus R) from the 2 shells are 530 and 170 14C yr; R from individual aragonite samples spans 130 ± 60 to 730 ± 170 14C yr. This intrashell 14C variability suggests that 14C dating of small (time-slice much less than 1 yr) marine samples from a variable-R (i.e. variable-upwelling) environment may introduce centuries of chronometric uncertainty.

The possibility of referring to the medical literature to support technology assessments in perinatal care was studied by examining more than 40,000 references contained in the Index Medicus of Latin America for the period 1984–88. A total of 1,074 references addressed perinatal technologies, but with irregular distribution. Information on effectiveness, safety, costs, and social impact of perinatal technologies was extremely limited. These results led to the conclusion that immediate action needs to be taken to create the necessary conditions for health technology assessment in Latin America.
Seasonal Variations in Peruvian Marine Reservoir Age from Pre-Bomb Argopecten Purpuratus Shell Carbonate

Marine upwelling along coastal Peru can be intense and variable, making radiocarbon dating marine and coastal systems complex. Historical and proxy records of upwelling along coastal Peru are few, and long-lived species such as corals do not grow in the cold coastal waters. Mollusk shell carbonate, however, can record both the magnitude of the local marine reservoir correction, ΔR, and of seasonal oscillations in the ventilation age of coastal waters. If large, these seasonal oscillations would complicate radiocarbon dating of marine organisms. To examine this possibility, we sampled for δ13C, δ18O, and 14C content a set of pre-bomb Argopecten purpuratus shells collected from coastal Peru during 1908 and 1926. Intrashell variations of up to 216 14C yr were noted, but these were not consistently correlated with seasonal changes in δ18O or δ13C. Only an 11 yr difference was observed in the weighted average ΔR of Callao Bay shells collected during normal (1908) and El Niño (1926) years. Despite the intrashell 14C variation noted, weighted average ΔR values from all 3 sample sites and from normal and El Niño years all overlap at 1 σ. We report ΔR values of 183 ± 18 and 194 ± 23 yr from Callao Bay (12°4′S), 165 ± 24 yr from Salaverry (8°14′S), and 189 ± 23 yr from Sechura Bay (5°45′S).
Methods of carcass classification based on subjective assessments of carcass fatness and of carcass conformation: effect of sex on the prediction of tissue composition in carcasses of sucking lambs

Forty-eight sucking lambs (26 males and 22 females) of the Manchega breed were used in this work. Lambs were slaughtered at 10, 12 and 14 kg live weight. Carcass degree of fatness was assessed by three assessors from colour photographs of the carcasses, using the European Union scale for light lambs (EU), the Colomer-Rocher method (CF) and another new scale developed by our group (sucking lambs scale, SL). Carcass conformation was assessed according to the Colomer-Rocher method (CC). Fatness and conformation scales were divided to give 0·25 points in each interval. Left half-carcasses were jointed and dissected into lean, fat and bone. Muscle proportion, bone proportion and whole fat proportion (obtained by addition of every fat depot: subcutaneous fat, intermuscular fat, inguinal fat and kidney knob and channel fat) were determined. Male and female carcasses showed a very different tissue composition: both groups had statistically significant differences (P < 0·05) in 20 out of 25 tissue composition variates. Males showed a higher number of variates that were well correlated with assessors’ scoring than females. Correlation coefficients between most of the tissue composition variates and assessors’ scores were higher in males. Furthermore, the number of variates that were significantly correlated with assessors’ scores was lower in females. Therefore, subjective methods for the estimation of carcass degree of fatness and of carcass conformation are poor predictors of tissue composition in female sucking lamb carcasses.
