Beta-lactam comprehensive allergy management program in a community medical center

Objective:
The Beta-lactam Comprehensive Allergy Management Program (CAMP) was implemented to facilitate complete beta-lactam allergy history documentation in the electronic medical record (EMR) and increase beta-lactam utilization. The study objective was to assess the rate of complete allergy histories and days of antimicrobial therapy (DOT) before versus after CAMP implementation.
Design:Quasi-experimental study with interrupted time-series analysis.
Setting:Non-teaching, urban, and community medical center within a multi-hospital health system.
Patients:Adult inpatients with a beta-lactam allergy receiving antimicrobial therapy.
Methods:The multidisciplinary CAMP team screened, interviewed, and collected allergy history details of adult inpatients with a beta-lactam allergy receiving antimicrobial therapy starting January 4, 2021. Patients were stratified as high, moderate, or low risk of IgE-mediated allergy and referred to an allergist for skin testing or drug challenge. The EMR was updated with interview details and drug challenge or skin test results. The primary endpoint was rate of complete allergy history documentation before (12/1/18–4/1/19) compared to after (1/4/21–5/1/21) program implementation. The secondary endpoint was days of inpatient beta-lactam therapy. Implementation logistics, de-labeling rate, and antimicrobial therapy changes were evaluated.
Results:The program evaluated 392 individuals, with 184 and 208 patients comprising the pre- and post-intervention groups, respectively. The post-intervention period was associated with an increase of 19.8% in complete allergy histories (0.359 PPc; R2 0.26; p = 0.002) and 9.34 beta-lactam DOT per 1,000-days-present (1.106 PPc; R2 0.194; p = 0.009).
Conclusion:Implementation of a comprehensive beta-lactam allergy management program was associated with higher rates of complete beta-lactam allergy history and beta-lactam use.

The vowel system of the dialect of Scottish Gaelic spoken in Ness, Lewis differs from that of other dialects in several important ways. In particular, several vowels display patterns of allophony that have not been investigated instrumentally and, in some cases, have not been reported before for Scottish Gaelic. This paper documents the Ness system in detail, focusing in particular on the tense–lax opposition in /i e/ and retraction of /a(ː)/ next to velarised consonants. The results of a traditional linguistic fieldwork study are presented first, followed by a detailed acoustic study of nine speakers. The acoustic reality of these allophonic patterns, reflected in F1 and F2 values, is verified statistically using LME modelling. Bimodality in the distribution of tokens in acoustic space, confirmed statistically with Hartigan’s Dip Test, is taken as evidence for the existence of discrete phonological categories (Bermúdez-Otero & Trousdale 2011). It is found that speakers vary as to whether these allophonic oppositions are restricted to the phonetic grammar, or have undergone stabilisation and advanced into the categorical phonology (Bermúdez-Otero 2007, 2015). It is observed that laxing of /i e/ in Ness Gaelic occurs in exactly those contexts where there is a direct transition between the vowel and a following supra-glottal consonant. It is therefore proposed that this tense–lax opposition is grounded in conflicting strategies of contrast enhancement, whereby laxing increases the perceptual distinctiveness of a following consonant by allowing for more distinctive formant transitions, at the expense of the distinctiveness of the vowel itself (Storme 2019).
Metrical structure in Scottish Gaelic: tonal accent, glottalisation and overlength

Scottish Gaelic displays a phonological contrast that is realised in different dialects by means of tonal accent, glottalisation or overlength. In line with existing analyses of similar oppositions in languages such as Swedish, Danish, Franconian and Estonian, I show that this contrast reflects a difference in metrical structure. Using the framework of Stratal Optimality Theory, I argue that this metrical contrast is derived, and results from faithfulness to foot structure that is built regularly at the stem level, but rendered opaque by subsequent phonological processes. Scottish Gaelic therefore represents an intermediate stage in the diachronic development of underlyingly contrastive metrical structure. This analysis successfully accounts for the complex properties of svarabhakti, a process of copy epenthesis that is intimately connected to the phonological contrast in question, and also sheds light upon the relationship between the oppositions of tonal accent, glottalisation and overlength found in various languages of northern Europe.
Connecting the Interstellar Gas and Dust Properties in Distant Galaxies Using Quasar Absorption Systems

Gas and dust grains are fundamental components of the interstellar medium and significantly impact many of the physical processes driving galaxy evolution, such as star-formation, and the heating, cooling, and ionization of the interstellar material. Quasar absorption systems (QASs), which trace intervening galaxies along the sightlines to luminous quasars, provide a valuable tool to directly study the properties of the interstellar gas and dust in distant, normal galaxies. We have established the presence of silicate dust grains in at least some gas-rich QASs, and find that they exist at higher optical depths than expected for diffuse gas in the Milky Way. Differences in the absorption feature shapes additionally suggest variations in the silicate dust grain properties, such as in the level of grain crystallinity, from system-to-system. We present results from a study of the gas and dust properties of QASs with adequate archival IR data to probe the silicate dust grain properties. We discuss our measurements of the strengths of the 10 and 18 μm silicate dust absorption features in the QASs, and constraints on the grain properties (e.g., composition, shape, crystallinity) based on fitted silicate profile templates. We investigate correlations between silicate dust abundance, reddening, and gas metallicity, which will yield valuable insights into the history of star formation and chemical enrichment in galaxies.

The Cambridge Companion to Socrates is a collection of essays providing a comprehensive guide to Socrates, the most famous Greek philosopher. Because Socrates himself wrote nothing, our evidence comes from the writings of his friends (above all Plato), his enemies, and later writers. Socrates is thus a literary figure as well as a historical person. Both aspects of Socrates' legacy are covered in this volume. Socrates' character is full of paradox, and so are his philosophical views. These paradoxes have led to deep differences in scholars' interpretations of Socrates and his thought. Mirroring this wide range of thought about Socrates, this volume's contributors are unusually diverse in their background and perspective. The essays in this volume were authored by classical philologists, philosophers and historians from Germany, Francophone Canada, Britain and the United States, and they represent a range of interpretive and philosophical traditions.

Summary
Whenever a problem of sufficient magnitude and complexity presents itself, a standard twenty-first century response among policy makers is to blame the media. And why not? The press is clearly pervasive, supposedly influential, seemingly unaccountable, and, best of all, singularly inept at defending itself against attack. So when assigning responsibility for the current transatlantic divide, the news media present a tempting target. Have they quietly and ruthlessly been sowing the seeds of mistrust between Europe and the United States? Do they have an interest in fanning the embers of transatlantic anger, perhaps to build circulation or indulge the jingoism of some moss-backed proprietor? Have they been conspiring, from opposite sides of the ocean, to cook up this nasty, newsworthy conflict?
The short answer is possibly yes, to a limited degree, but not enough to matter. The news media, as we shall see, are too diverse and feeble to impose their will on the rest of us. Other questions seem more pertinent: Why do Americans and Europeans appear to hold such widely differing versions of what is happening in the world? Is the press to blame? There is no short answer to these questions. The long, complicated explanation has much to do with the war in Iraq. This is no surprise. But well before the United States arrived in Baghdad, there were factors driving U.S. and European opinion apart.

Summary
Whether or to what degree Plato's Republic is a utopian work depends on what one means by “utopia,” a word with a notoriously wide range of meanings. I shall simply stipulate that by “utopia” I mean a description of an imagined society put forward by its author as better than any existing society, past or present. The limiting case (relevant to the Republic) is the portrayal not just of a better society but of the best society. I call a “mere utopia” a description of an ideal society meant or recognized by its author to be an impossible society - a society in some sense better than any historical society, but which could never actually exist.
Socrates calls his ideal city “Callipolis.” Did Plato think Callipolis was realizable? Did he really believe that the city he portrays in the Republic is the best human society? As with so much else in Plato, scholars disagree. One mainstream interpretation, which fits most easily the surface of the text and which I share, is that Plato intends the society described in the Republic to be a utopia that is not a mere utopia. Socrates paints a picture in words of the best human society, one difficult but not impossible to realize.
Compared with classical Athens, or indeed with any society existing today, the conditions in Callipolis are extraordinary. Communism of property, abolition of the family, tight control of every aspect of life by the philosophical ruling class, maximal “unity” of thought and feeling among the citizens – features such as these have led critics from Aristotle onward to criticize Callipolis as so contrary to human nature as to be both impossible and undesirable.

The topic of degrees of being in Aristotle is almost universally ignored. A very few scholars do discuss the topic or make use of it in passing. This situation mightbe explained by a scholarly consensus that (a) Aristotle did have a doctrine ofdegrees of being, but (b) this doctrine is too uninteresting to be worth much discussion. Conversation with a number of scholars from several countries has convinced me, however, that a rather different consensus lies behind the current silence. It turnsout that many experts in the subject deny that Aristotle believed in degrees of being.No one, to my knowledge, has defended this denial in print. But the reason forsilence is not that the topic is dull, but that it is scandalous. Both defenders andopponents of the view that Aristotle had a doctrine of degrees of being tend, in conversation, to find the topic embarrassing. Our contemporary metaphysical prejudices areso opposed to degrees of being that people find themselves unable to make anysense of such a doctrine. As a result, one group of scholars is embarrassed on Aristotle's behalf at the suggestion that he might have held such a senseless doctrine.Another group, less sure of where the philosophical truth lies, finds itself in theinterpretative embarrassment of being unable to explain and motivate the doctrine. So bothgroups avoid the subject.

It is shown that the gamma distribution is the unique mixing distribution of exponentials that leads to a linearly increasing mean residual lifetime function. A corollary of this result is that regardless of the mixing process the Pareto distribution is the unique aggregate distribution mixture with this property.
