Evaluation of an online research best practices training for community health workers and promotoras

Introduction:
Community health workers and promotoras (CHW/Ps) have a fundamental role in facilitating research with communities. However, no national standard training exists as part of the CHW/P job role. We developed and evaluated a culturally- and linguistically tailored online research best practices course for CHW/Ps to meet this gap.
Methods:After the research best practices course was developed, we advertised the opportunity to CHW/Ps nationwide to complete the training online in English or Spanish. Following course completion, CHW/Ps received an online survey to rate their skills in community-engaged research and their perceptions of the course using Likert scales of agreement. A qualitative content analysis was conducted on open-ended response data.
Results:104 CHW/Ps completed the English or Spanish course (n = 52 for each language; mean age 42 years SD ± 12); 88% of individuals identified as female and 56% identified as Hispanic, Latino, or Spaniard. 96%–100% of respondents reported improvement in various skills. Nearly all CHW/Ps (97%) agreed the course was relevant to their work, and 96% felt the training was useful. Qualitative themes related to working more effectively as a result of training included enhanced skills, increased resources, and building bridges between communities and researchers.
Discussion:The CHW/P research best practices course was rated as useful and relevant by CHW/Ps, particularly for communicating about research with community members. This course can be a professional development resource for CHW/Ps and could serve as the foundation for a national standardized training on their role related to research best practices.

Objective:
The development of user-friendly nutrition resources for pregnant women seldom involves end-users. This qualitative study used a citizens’ jury approach to determine if our modification of a longstanding, frequently used dietitian-informed diet and diabetes booklet was deemed to be a good healthy eating resource for pregnant women.
Design:Midwives recruited thirteen first-time pregnant women not requiring specialist obstetric care or specialist dietetic advice for any reason. Participants were sent a copy of the modified healthy eating in pregnancy booklet prior to ‘jury day’. Five women were unable to attend the citizens’ jury citing reasons such as early labour. At the jury, five experts presented evidence. Participants adjourned, with an independent facilitator, to ‘deliberate’ as to whether the resource was suitable or not. The verdict was presented, and subsequent discussion was audio-recorded, transcribed and inductively content analysed.
Setting:Southland, New Zealand.
Participants:Pregnant women aged 19–35 years (n 8), of whom half had a household income <$NZ30 000.
Results:The verdict was ‘Yes’; the resource was good. Three themes were derived: communication of health information, resource content and harm reduction in pregnancy. Based on these data, ways to enhance the quality and usability of the booklet were evident.
Conclusions:Citizens’ juries can be used to obtain an independent assessment by end-users of health resources. Our modified diet and diabetes booklet was considered suitable for providing healthy eating advice to pregnant women. Inclusion of end-users’ perspectives is critical for end-user relevant content, comprehension and resource credibility.

Objectives
Systematic characterization of the first psychotic episodes patients may be important in identifying subgroups of patients with a similar course of illness, and may ultimately facilitate diagnosis, treatment, and predicting outcome. The aim of this study was to characterize a population of first psychotic patients in different domains such as psychopathology, prodromal symptoms, drug abuse and socio - demographic variables.
MethodsThis was a retrospective study carried out in the Department of Psychiatry of a large University Hospital (Centro Hospitalar São João, Porto, Portugal). Patients enrolled were admitted in the inpatient unit from 2007 to 2011, had experienced a first psychotic episode, and fulfilled criteria for one of the following diagnosis: schizophrenia, schizoaffective disorder, bipolar disorder, drug induced psychosis, acute and transient psychotic disorders, schizophreniform disorder and psychosis NOS (ICD-10 classification). Data were extracted from clinical records. The analysis focuses on clinical, demographic and social variables.
ResultsThe study is now under statistic evaluation.

Introduction
The diagnosis of psychosis is based on the presence or absence of characteristic symptoms. The presence of such symptoms varies during the course and treatment, raising the question of diagnostic stability after a first psychotic episode.
Aims and objectivesThe aim of this study is to evaluate the diagnostic stability after a first psychotic episode in the long term (five years after the first inpatient admission).
MethodologyA retrospective study that included patients with first psychotic episode between 2007 and 2011 admitted to the inpatient unit of the psychiatry and mental health clinic of São João hospital center, Oporto, Portugal and re-evaluation of the diagnosis after five years.
ResultsWe included 60 patients with a first psychosis episode, 22 of which were drop-outs after five years. Of the 38 patients evaluated, it was possible to see that after 5 years 68.4% (n = 26) maintained the same diagnosis during follow-up. In particular, the diagnosis of schizophrenia was kept in 83.3% of patients after 5 years (n = 15, 18 patients with the diagnosis of schizophrenia after first admission). Diagnosis of acute and transient psychotic disorder and psychosis not otherwise specified were the least stable diagnosis after 5 years.
ConclusionsThe diagnosis after a first psychotic episode has important therapeutic and prognostic implications. The presence of characteristic symptomatology, with periods of partial or total remission between subsequent episodes emphasizes the need for regular monitoring, since this group of patients appears to be more vulnerable to changes in diagnosis over time.

Objectives: Prior research has identified numerous genetic (including sex), education, health, and lifestyle factors that predict cognitive decline. Traditional model selection approaches (e.g., backward or stepwise selection) attempt to find one model that best fits the observed data, risking interpretations that only the selected predictors are important. In reality, several predictor combinations may fit similarly well but result in different conclusions (e.g., about size and significance of parameter estimates). In this study, we describe an alternative method, Information-Theoretic (IT) model averaging, and apply it to characterize a set of complex interactions in a longitudinal study on cognitive decline. Methods: Here, we used longitudinal cognitive data from 1256 late–middle aged adults from the Wisconsin Registry for Alzheimer’s Prevention study to examine the effects of sex, apolipoprotein E (APOE) ɛ4 allele (non-modifiable factors), and literacy achievement (modifiable) on cognitive decline. For each outcome, we applied IT model averaging to a set of models with different combinations of interactions among sex, APOE, literacy, and age. Results: For a list-learning test, model-averaged results showed better performance for women versus men, with faster decline among men; increased literacy was associated with better performance, particularly among men. APOE had less of an association with cognitive performance in this age range (∼40–70 years). Conclusions: These results illustrate the utility of the IT approach and point to literacy as a potential modifier of cognitive decline. Whether the protective effect of literacy is due to educational attainment or intrinsic verbal intellectual ability is the topic of ongoing work. (JINS, 2019, 25, 119–133)

β-Hydroxy-β-methylbutyrate free acid reduces markers of exercise-induced muscle damage and improves recovery in resistance-trained men

The purpose of the present study was to determine the effects of short-term supplementation with the free acid form of β-hydroxy-β-methylbutyrate (HMB-FA) on indices of muscle damage, protein breakdown, recovery and hormone status following a high-volume resistance training session in trained athletes. A total of twenty resistance-trained males were recruited to participate in a high-volume resistance training session centred on full squats, bench presses and dead lifts. Subjects were randomly assigned to receive either 3 g/d of HMB-FA or a placebo. Immediately before the exercise session and 48 h post-exercise, serum creatine kinase (CK), urinary 3-methylhistadine (3-MH), testosterone, cortisol and perceived recovery status (PRS) scale measurements were taken. The results showed that CK increased to a greater extent in the placebo (329 %) than in the HMB-FA group (104 %) (P= 0·004, d= 1·6). There was also a significant change for PRS, which decreased to a greater extent in the placebo (9·1 (sem 0·4) to 4·6 (sem 0·5)) than in the HMB-FA group (9·1 (sem 0·3) to 6·3 (sem 0·3)) (P= 0·005, d= − 0·48). Muscle protein breakdown, measured by 3-MH analysis, numerically decreased with HMB-FA supplementation and approached significance (P= 0·08, d= 0·12). There were no acute changes in plasma total or free testosterone, cortisol or C-reactive protein. In conclusion, these results suggest that an HMB-FA supplement given to trained athletes before exercise can blunt increases in muscle damage and prevent declines in perceived readiness to train following a high-volume, muscle-damaging resistance-training session.

Diagnostic accuracy and confusability analyses: an application to the Diagnostic Interview for Genetic Studies

The dominant, contemporary paradigm for developing and refining diagnoses relies heavily on assessing reliability with kappa coefficients and virtually ignores a core component of psychometric practice: the theory of latent structures. This article describes a psychometric approach to psychiatric nosology that emphasizes the diagnostic accuracy and confusability of diagnostic categories. We apply these methods to the Diagnostic Interview for Genetic Studies (DIGS), a structured psychiatric interview designed by the NIMH Genetics Initiative for genetic studies of schizophrenia and bipolar disorder. Our results show that sensitivity and specificity were excellent for both DSM-III-R and RDC diagnoses of major depression, bipolar disorder, and schizophrenia. In contrast, diagnostic accuracy was substantially lower for subtypes of schizoaffective disorder – especially for the DSM-III-R definitions. Both the bipolar and depressed subtypes of DSM-III-R schizoaffective disorder had excellent specificity but poor sensitivity. The RDC definitions also had excellent specificity but were more sensitive than the DSM-III-R schizoaffective diagnoses. The source of low sensitivity for schizoaffective subtypes differed for the two diagnostic systems. For RDC criteria, the schizoaffective subtypes were frequently confused with one another; they were less frequently confused with other diagnoses. In contrast, the DSM-III-R subtypes were often confused with schizophrenia, but not with each other.

Oocysts of the Weybridge and Houghton strains of Eimeria maxima and a fresh field isolate were similar and measured on average 30·9 × 22·4 μm. The Weybridge strain and the field isolate produced similar pathogenic effects in 6-week-old chickens, high doses causing 50–80% mortality and severe weight loss. The Houghton strain was slightly less pathogenic and few birds died but more oocysts were produced. With each strain, a single dose gave complete immunity to a light challenge with the homologous strain. Two or 3 immunizing doses gave complete immunity to a heavy homologous challenge but were not sufficient to protect against heterologous challenge.

Conductors with controlled grain boundaries: An approach to the next generation, high temperature superconducting wire

Much of the conductor development effort in the last decade has focused on optimizing the processing of (Bi, Pb)2Sr2Ca2Cu3Ox oxide-powder-in-tube conductors and (Bi, Pb)2Sr2CaCu2O8 (Bi-2212) and TlBa2Ca2Cu3Ox thick film conductors. It is demonstrated that in each of these conductors, critical current densities are dictated by the grain boundary misorientation distributions (GBMD's). Percolative networks of low-angle boundaries with fractions consistent with the active cross-sectional area of the conductor exist in each of these conductors. Further enhancements in the properties require increased numbers of small-angle grain boundaries. Given the processing methods used to fabricate these materials, no clear route employing a simple modification of the established processing method is apparent. To address this need, conductors with controlled or predetermined GBMD's are necessary. Development of biaxial texture appears to be the only possible way to increase the number of small-angle boundaries in a practical and controllable manner. We summarize in this paper recent results obtained on epitaxial superconducting films on rolling-assisted-biaxially-textured-substrates (RABiTS). This technique uses well established, industrially scalable, thermomechanical processes to impart a strong biaxial texture to a base metal. This is followed by vapor deposition of epitaxial buffer layers (metal and/or ceramic) to yield structurally and chemically compatible surfaces. Epitaxial YBa2Cu3O7–δ films grown using laser ablation on such substrates have critical current densities exceeding 106 A/cm2 at 77 K in zero field and have a field dependence similar to epitaxial films on single crystal ceramic substrates. Deposited conductors made using this technique offer a potential route for the fabrication of the next generation high temperature superconducting (HTS) wire capable of carrying high currents in high magnetic fields and at elevated temperatures.

A new technique, referred to as rolling-assisted-biaxially-textured-substrates (RABiTS) has recently been proposed by Goyal et al., to fabricate long-range, biaxially textured substrates with suitable metal/ceramic surfaces for epitaxial growth of electronic devices [1]. Using standard thermomechanical processing, long lengths of flexible, biaxially oriented substrates with smooth surfaces (rms∼50nm) are obtained [1]. Epitaxial metal and/or oxide layers which can serve both as a chemical as well as a structural buffer are deposited on the biaxially textured metal. The metal with a suitable set of epitaxial, multi-layers comprises the substrate which is expected to have many potential applications, including superconductivity, photovoltaics and ferroelectrics [1].
When high temperature superconductors are deposited on a set of epitaxial multilayers on biaxially textured RABiT substrates, extremely high critical current densities, approaching 106 A/cm2 at 77K have been obtained [2].

Experiments were conducted to determine the effect of heating field peas (Pisum sativum) on the N balance and urine, serum and plasma composition of growing pigs. In the first experiment, four diets containing raw field peas (cv. Wirrega) or field peas heated to 150° (cv. Wirrega), 165° (cv. Wirrega) or 150° (cv. Dundale) for 15 min respectively were formulated to contain 1·15 g ileal digestible N/MJ digestible energy (DE) and 036 g ileal digestible lysine/MJ DE in a sugar-based diet. Digestibility estimates were based on those for the Dundale cultivar of field peas used in previous experiments. Total urine and faeces collection from eight pigs was conducted over two 7 d collection periods with a 7 d diet change-over period. Serial blood sampling from the external jugular vein was conducted on the final day of each collection period. There was no significant difference (P > 0·05) in the N balance or apparent biological value of the field-pea treatments. Pigs fed on diets containing peas heated to 150° (cv. Wirrega) or 165° (cv. Wirrega) had a significantly lower (P < 0·01) daily output of urea and uric acid in the urine, and depressed serum protein and serum urea concentrations. Plasma lysine concentration and daily urine lysine output were not significantly different (P > 0·05) in pigs fed on heated peas. Protein excretion in the urine of pigs fed on diets containing peas heated to 165° increased 3–7 times (depending on estimation technique) the level observed in pigs fed on diets containing raw peas. A second experiment was conducted to determine the apparent ileal digestibility of N and amino acids in cv. Wirrega field peas. This study revealed that N digestibility (0·44) and lysine digestibility (0·35) in peas heated to 165° were significantly lower than the cv. Dundale estimates (0·57 and 0·62 respectively) used in diet formulations. The depressed serum and urine variables in pigs fed on heated peas were attributed to overestimation of digestibility. The results exemplify the fact that it is not possible to draw general conclusions as to the effects of heat on any particular protein concentrate. Variability in N balance experiments and problems associated with urine analysis are suggested as likely reasons for the current study not reflecting poor utilization of ileal digestible lysine from heat-treated field peas. Despite considerable variation in the results, it is possible that a large proportion of non-utilizable amino acids in heated field peas may be excreted from the pig via the urine in the form of a protein.
Tc-δ Relations in YB2Cu3O7-δ Thin Films: Effects of Oxygen Pressure During Growth

The Tc dependence on oxygen content was measured for YBa2Cu3O7-δ films grown with a variety of techniques (solid phase epitaxy, laser ablation, off-axis sputtering, co-evaporation) at oxygen pressures p(O2) ranging from 1.0 atm to 0.1 mTorr. Dissimilar dependences resulted for each film type, with Tc either increasing or decreasing for small increments in δ from maximum oxygen occupancy. Varying systematically with p(O2) during growth, the deviations are attributed to competing effects from hole-doping lattice defects (most likely on the Y-site) on the carrier density of the CuO2 planes and basal plane oxygen capacity, respectively, giving rise to overdoping or underdoping after low temperature oxidation in 1.0 atm of oxygen.
Anisotropic Electrical Resistivity of YBCO/PBCO Superlattice Films Grown on Miscut Substrates

Anisotropie electrical resistivity is studied in epitaxial superlattice films of YBa2Cu3O7-δ/PrBa2Cu3O7-ε grown in situ by laser ablation on SrTiO3 surfaces aligned slightly away from the [100] direction. Layer thicknesses of each compound range from two to eight atomic cells. Electrical resistivity of these superlattice films always shows a peak at some cryogenic temperature that decreases with increasing PrBa2Cu3O7-ε layer thickness. Clear evidence is seen of vortex pinning or of supercurrent blocking by the step edges or by twin boundaries. The ratio of electrical resistivities for current directions parallel and perpendicular to these boundaries shows a large step-like change just above the superconductive critical temperature. The possibility is discussed that the Kosterlitz-Thouless transition explains this anisotropy change.

Laser deposition is widely used to fabricate thin-films of the YBa2Cu3O7−δ superconductor due to its ease of operation and its reproducibility. The films thus formed do not require post-deposition annealing to be superconducting. The processing route involves many variables which would be expected to exert an influence on the early stages of the film growth and thereby the final microstructure. The parameters which have been varied are the laser energy, laser pulse repetition rate and laser wavelength. The films have been examined using transmission electron microscopy (TEM) in order to determine the film orientation, composition and microstructure.
