Psychological predictors of health anxiety and pain in ambulatory presentations in a hospital emergency department

Background:
Health anxiety in attendees of out-patient medical clinics is well established; however, there has been a lack of research into health anxiety within emergency settings.
Aims:This study explored the prevalence of health anxiety in ambulatory presentations in a tertiary emergency department (ED) as well as the factors associated with pain and health anxiety in this setting.
Method:A cross-sectional questionnaire design was used to gather data from adult ED ambulatory attendees across a 4-day sampling period to assess psychological and physical health variables. Number of attendances to ED over the previous 12 months was accessed through healthcare records.
Results:Of the final sample (n = 106), 77% were white British, 54% were male, and 14% presented with severe health anxiety as measured by the Short Health Anxiety Inventory (≥18). Participants with pre-existing health conditions had significantly higher levels of health anxiety (M = 12.36, SE = 1.59) compared with those without (M = 7.79, SE = 0.66). Stepwise multiple regression analyses identified anxiety sensitivity and pain catastrophizing as significant independent predictors of health anxiety, explaining 51% of the variance in health anxiety. Pain catastrophizing was also a significant independent predictor of pain level, accounting for 20% of the variance.
Conclusion:This study provides insight into the prevalence of health anxiety in ED ambulatory presentations and key psychological predictors of health anxiety and pain. This has implications for treatment in an ED setting whereby patients may benefit from referral to medical psychology or mental health services.
Comparison of whole egg v. egg white ingestion during 12 weeks of resistance training on skeletal muscle regulatory markers in resistance-trained men

Eggs are considered a high-quality protein source for their complete amino acid profile and digestibility. Therefore, this study aimed to compare the effects of whole egg (WE) v. egg white (EW) ingestion during 12 weeks of resistance training (RT) on the skeletal muscle regulatory markers and body composition in resistance-trained men. Thirty resistance-trained men (mean age 24·6 (sd 2·7) years) were randomly assigned into the WE + RT (WER, n 15) or EW + RT (EWR, n 15) group. The WER group ingested three WE, while the EWR group ingested an isonitrogenous quantity of six EW per d immediately after the RT session. Serum concentrations of regulatory markers and body composition were measured at baseline and after 12 weeks. Significant main effects of time were observed for body weight (WER 1·7, EWR 1·8 kg), skeletal muscle mass (WER 2·9, EWR 2·7 kg), fibroblast growth factor-2 (WER 116·1, EWR 83·2 pg/ml) and follistatin (WER 0·05, EWR 0·04 ng/ml), which significantly increased (P < 0·05), and for fat mass (WER –1·9, EWR –1·1 kg), transforming growth factor-β1 (WER –0·5, EWR −0·1 ng/ml), activin A (WER –6·2, EWR –4·5 pg/ml) and myostatin (WER –0·1, EWR –0·06 ng/ml), which significantly decreased (P < 0·05) in both WER and EWR groups. The consumption of eggs absent of yolk during chronic RT resulted in similar body composition and functional outcomes as WE of equal protein value. EW or WE may be used interchangeably for the dietary support of RT-induced muscular hypertrophy when protein intake is maintained.
Evidence of temperature depression and hydrological variations in the eastern Sierra Nevada during the Younger Dryas Stade

Sediment records from two lakes in the east-central Sierra Nevada, California, provide evidence of cooling and hydrological shifts during the Younger Dryas stade (YD; ~ 12,900–11,500 cal yr BP). A chironomid transfer function suggests that lake-water temperatures were depressed by 2°C to 4°C relative to maximum temperatures during the preceding Bølling–Allerød interstade (BA; ~ 14,500–12,900 cal yr BP). Diatom and stable isotope records suggest dry conditions during the latter part of the BA interstade and development of relatively moist conditions during the initiation of the YD stade, with a reversion to drier conditions later in the YD. These paleohydrological inferences correlate with similar timed changes detected in the adjacent Great Basin. Vegetation response during the YD stade includes the development of more open and xeric vegetation toward the end of the YD. The new records support linkages between the North Atlantic, the North Pacific, and widespread YD cooling in western North America, but they also suggest complex hydrological influences. Shifting hydrological conditions and relatively muted vegetation changes may explain the previous lack of evidence for the YD stade in the Sierra Nevada and the discordance in some paleohydrological and glacial records of the YD stade from the western United States.
Textural and carbon isotopic evidence of monsoonal changes recorded in a composite-type speleothem from Korea since MIS 5a

Textural and stable isotopic records of a composite-type speleothem from Gwaneum Cave in the eastern part of the Korean peninsula show prominent paleoenvironmental changes since MIS (marine oxygen isotope stage) 5a. Based on 230Th/234U dating, the speleothem experienced growth from 90.9 ± 6.5 ka to 1.2 ± 0.5 ka with several hiatuses. Four growth phases (A, B, C and D) are recognized based on speleothem type and texture. Very irregular and laterally discontinuous growth laminae in Phases B and C indicate that the cave coralloids grew over the stalagmite (Phase A) when the supply of dripping water became limited. Variations within the δ13C time series of Phase A are interpreted as responses to millennial-scale fluctuations of the East Asian monsoon intensity during MIS 5a. The monsoonal interpretation is based on the idea that δ13C values reflect the isotopic composition of soil-derived CO2, which, in turn, should relate to monsoon-driven changes in terrestrial productivity above the cave during the MIS 5a. Our reconstruction reveals that the significant monsoonal changes on the Korean peninsula occurred in conjunction with changes in sea level and/or oceanic circulations during the transition period from MIS 5a to MIS 4.

Drug Interventions Programme: clinical profile of service users v. attendees of standard services

Aims and Method
We conducted a retrospective survey of all cases referred to the Drug Interventions Programme in Hertfordshire for the first 9 months in order to compare them with those referred to one of the community drug and alcohol teams.
ResultsThe Drugs Interventions Programme had significantly more White British clients and clients who had dropped out from previous treatment. Compared with community team clients, the Programme had a higher percentage of clients with an opioid problem (92%), of whom a high percentage also misused other substances (78%) and injected drugs (30%, half of whom shared needles).
Clinical ImplicationsMore chaotic clients who had failed previous treatment have entered treatment with the Drug Interventions Programme.
