Summary
Liaison psychiatry practice is built on a foundation of core ethical principles that apply wherever there are people needing mental healthcare (1). In the UK, liaison psychiatry is practised in accordance with the Human Rights Act 1998 (HRA) (2), the articles of the European Convention on Human Rights (ECHR) and the UN Convention on the Rights of Persons with Disabilities (CRPD).
The development and preliminary evaluation of Cognitive Behavioural Therapy (CBT) for Chronic Loneliness in Young People

Background:
Approximately 10% of young people ‘often’ feel lonely, with loneliness being predictive of multiple physical and mental health problems. Research has found CBT to be effective for reducing loneliness in adults, but interventions for young people who report loneliness as their primary difficulty are lacking.
Method:CBT for Chronic Loneliness in Young People was developed as a modular intervention. This was evaluated in a single-case experimental design (SCED) with seven participants aged 11–18 years. The primary outcome was self-reported loneliness on the Three-Item Loneliness Scale. Secondary outcomes were self-reported loneliness on the UCLA-LS-3, and self- and parent-reported RCADS and SDQ impact scores. Feasibility and participant satisfaction were also assessed.
Results:At post-intervention, there was a 66.41% reduction in loneliness, with all seven participants reporting a significant reduction on the primary outcome measure (p < .001). There was also a reduction on the UCLA-LS-3 of a large effect (d = 1.53). Reductions of a large effect size were also found for parent-reported total RCADS (d = 2.19) and SDQ impact scores (d = 2.15) and self-reported total RCADS scores (d = 1.81), with a small reduction in self-reported SDQ impact scores (d = 0.41). Participants reported high levels of satisfaction, with the protocol being feasible and acceptable.
Conclusions:We conclude that CBT for Chronic Loneliness in Young People may be an effective intervention for reducing loneliness and co-occurring mental health difficulties in young people. The intervention should now be evaluated further through a randomised controlled trial (RCT).

Shallow firn cores, in addition to a near-basal ice core, were recovered in 2018 from the Quelccaya ice cap (5470 m a.s.l) in the Cordillera Vilcanota, Peru, and in 2017 from the Nevado Illimani glacier (6350 m a.s.l) in the Cordillera Real, Bolivia. The two sites are ~450 km apart. Despite meltwater percolation resulting from warming, particle-based trace element records (e.g. Fe, Mg, K) in the Quelccaya and Illimani shallow cores retain well-preserved signals. The firn core chronologies, established independently by annual layer counting, show a convincing overlap indicating the two records contain comparable signals and therefore capture similar regional scale climatology. Trace element records at a ~1–4 cm resolution provide past records of anthropogenic emissions, dust sources, volcanic emissions, evaporite salts and marine-sourced air masses. Using novel ultra-high-resolution (120 μm) laser technology, we identify annual layer thicknesses ranging from 0.3 to 0.8 cm in a section of 2000-year-old radiocarbon-dated near-basal ice which compared to the previous annual layer estimates suggests that Quelccaya ice cores drilled to bedrock may be older than previously suggested by depth-age models. With the information collected from this study in combination with past studies, we emphasize the importance of collecting new surface-to-bedrock ice cores from at least the Quelccaya ice cap, in particular, due to its projected disappearance as soon as the 2050s.

Summary
Setting
This audit is applicable in all areas where detentions under section 136 of the Mental Health Act 1983 are carried out. The audit could be adapted to the legislation of the devolved nations, for example section 118 of the Mental Health (Care and Treatment) (Scotland) Act 2003.
Background
Section 136 of the Mental Health Act is a power that allows police officers to remove a person from a public place to a place of safety if the officer believes that the person is suffering from a mental disorder and is in immediate need of care or control. Police stations have routinely been used as a place of safety in many areas. The Royal College of Psychiatrists (2008) has proposed a national monitoring form for the use of section 136 orders, which has been developed further and has been successfully implemented in three sites and is incorporated in the form used by the Metropolitan Police. The advantage of using the same data-collection form nationally is that it will assist in benchmarking to enable more rapid service improvement.
Standards
Standards were obtained from the Royal College of Psychiatrists’ 2008 report on the use of section 136. Of particular relevance were the following:
ᐅ There should be routine local monitoring of the use of section 136, by a specific group.
ᐅ A mental healthcare setting should be used as a place of safety wherever possible and police custody should be used for this purpose only in exceptional circumstances.
ᐅ An ambulance should be used for conveyance from the public place wherever possible.
ᐅ The approved mental health practitioner should commence the assessment within 3 hours.
ᐅ The first doctor should be approved under section 12(2) of the Mental Health Act.
Method
Data collection
The national monitoring form was used to ensure that the data for the key standards could be compared at both local and national levels. For every section 136 detention, the monitoring form was completed by the detaining police officers and by either the approved mental health practitioner or an approved section 136 nurse.

Use of psychotropic medication among psychiatric out-patients with personality disorder

Aims and method
This survey was conducted to ascertain the use of psychotropic medication in the treatment of patients with a primary diagnosis of personality disorder within a community mental health team. A sample of 113 patients were identified, their notes were reviewed, and details of current medications and diagnoses recorded.
ResultsFour-fifths of patients were prescribed at least one psychotropic medication. The most commonly prescribed medication class was antidepressant, comprising almost half of prescriptions. The total annual cost across 107 patients was £37 000.
Clinical implicationsMedication is commonly prescribed to people with personality disorder but more needs to be known about why prescriptions are started and stopped, what the benefits are, and how these are judged by patients and care teams.

Objective:
To describe the epidemiology, interventions, and molecular typing methods used during the investigation and control of concurrent outbreaks of Serratia marcescens and methicillin-resistant Staphylococcus aureus (MRSA) infections in a neonatal intensive-care unit (NICU).
Setting:A 206-bed women's and infants' hospital with a 48-bed NICU.
Design:A 22-week, prospective, descriptive study of all NICU infants with S marcescens or MRSA infection or colonization. Repetitive polymerase chain reaction (rep PCR) and pulsed-field gel electrophoresis (PFGE), respectively, were applied to the typing of S marcescens and MRSA isolates.
Interventions:Infants with S marcescens or MRSA infection or colonization were placed in isolation; all other infants were cohorted. A multidisciplinary task force implemented education for all hospital and medical staff regarding policies essential for outbreak control. Changes in physical setting and patient contact procedure were required to promote adherence to existing policies.
Results:Two premature infants had S marcescens infection, and five were colonized; rep PCR verified that both invasive and three of five colonizing isolates were related genotypically. Five bacteremic and 10 MRSA-colonized infants were identified; PFGE confirmed that 12 of the isolates had similar electrophoretic patterns. S marcescens infection was eliminated from the NICU 3 weeks after interventions were initiated. MRSA infections also were eliminated, and MRSA colonization fell to below pre-outbreak rates within 8 weeks. Despite a 100% increase in NICU patient days per month during the subsequent 2 years, no further clusters of S marcescens or MRSA infection have occurred.
Conclusions:Concurrent outbreaks of S marcescens and MRSA in an NICU were confirmed by genotyping of strains. Control was achieved by isolation and cohorting of patients and strict adherence to NICU policies and procedures.
