Background
Lewy body dementia, consisting of both dementia with Lewy bodies (DLB) and Parkinson's disease dementia (PDD), is considerably under-recognised clinically compared with its frequency in autopsy series.
AimsThis study investigated the clinical diagnostic pathways of patients with Lewy body dementia to assess if difficulties in diagnosis may be contributing to these differences.
MethodWe reviewed the medical notes of 74 people with DLB and 72 with non-DLB dementia matched for age, gender and cognitive performance, together with 38 people with PDD and 35 with Parkinson's disease, matched for age and gender, from two geographically distinct UK regions.
ResultsThe cases of individuals with DLB took longer to reach a final diagnosis (1.2 v. 0.6 years, P = 0.017), underwent more scans (1.7 v. 1.2, P = 0.002) and had more alternative prior diagnoses (0.8 v. 0.4, P = 0.002), than the cases of those with non-DLB dementia. Individuals diagnosed in one region of the UK had significantly more core features (2.1 v. 1.5, P = 0.007) than those in the other region, and were less likely to have dopamine transporter imaging (P < 0.001). For patients with PDD, more than 1.4 years prior to receiving a dementia diagnosis: 46% (12 of 26) had documented impaired activities of daily living because of cognitive impairment, 57% (16 of 28) had cognitive impairment in multiple domains, with 38% (6 of 16) having both, and 39% (9 of 23) already receiving anti-dementia drugs.
ConclusionsOur results show the pathway to diagnosis of DLB is longer and more complex than for non-DLB dementia. There were also marked differences between regions in the thresholds clinicians adopt for diagnosing DLB and also in the use of dopamine transporter imaging. For PDD, a diagnosis of dementia was delayed well beyond symptom onset and even treatment.

Introduction
Many patients with schizophrenia show major difficulties in performing basic social roles. Occupational and social functioning (and independent living) are considered domains of functioning. Improvement in functioning is recognized as an important aim in treatment guidelines and as an outcome by regulatory agencies.
ObjectivesThe Europe, Middle East and Africa (EMEA) functioning survey canvassed opinions of psychiatrists (and neurologists with psychiatric backgrounds in Germany) across the region.
AimTo ascertain psychiatrists' perceptions of the clinical importance, assessment and management of functioning amongst their patients with schizophrenia.
MethodsThe survey comprised 17 questions and was conducted from March-April 2011, in 39 EMEA countries. Respondents' demographics and their opinions regarding personal and social functioning in patients with schizophrenia were collected.
ResultsResults were obtained from 3610 clinicians. Psychiatrists reported that they perceived more than two in three of their patients show impaired/very poor levels of functioning. 91% believed that personal and social functioning was an important treatment goal for patients with schizophrenia and 90% believed it was an important goal for patients' families. The majority of psychiatrists assess the personal and social functioning of their patient at each visit but 81% determine the level of functioning through clinical interview. To manage personal and social functioning in their patients, 26% of psychiatrists prefer pharmacological interventions while 47% prefer psychosocial interventions.
ConclusionAlthough psychiatrists recognize that functioning is impaired/very poor in patients with schizophrenia, there is still a need to address it as a main treatment goal for patients with schizophrenia.
Glioblastoma Recurrence Versus Treatment Effect in a Pathology-Documented Series

Objective:
Patients diagnosed with glioblastoma (GBM) are treated with surgery followed by fractionated radiotherapy with concurrent and adjuvant temozolomide. Patients are monitored with serial magnetic resonance imaging (MRI). However, treatment-related changes frequently mimic disease progression. We reviewed a series of patients undergoing surgery for presumed first-recurrence GBM, where pathology reports were available for tissue diagnosis, in order to better understand factors associated with a diagnosis of treatment-related changes on final pathology.
Methods:Patient records at a single institution between 2005 and 2015 were retrospectively reviewed. Pathology reports were reviewed to determine diagnosis of recurrent GBM or treatment effect. Survival analysis was performed interrogating overall survival (OS) and progression-free survival (PFS). Correlation with radiation treatment plans was also examined.
Results:One-hundred-twenty-three patients were identified. One-hundred-sixteen patients (94%) underwent resection and seven underwent biopsy. Treatment-related changes were reported in 20 cases (16%). These patients had longer median OS and PFS from the time of recurrence than patients with true disease progression. However, there was no significant difference in OS from the time of initial diagnosis. Treatment effect was associated with surgery within 90 days of completing radiation. In patients receiving radiation at our institution (n = 53), larger radiation target volume and a higher maximum dose were associated with treatment effect.
Conclusion:Treatment effect was associated with surgery nearer to completion of radiation, a larger radiation target volume, and a higher maximum point dose. Treatment effect was associated with longer PFS and OS from the time of recurrence, but not from the time of initial diagnosis.

The objective was to determine the longitudinal associations between callous-unemotional (CU) and oppositional defiant (OD) behaviors from the first to fourth grades for Spanish children. Four possible outcomes were evaluated: (a) CU behaviors in the first grade predict increases in OD behaviors in the fourth grade, controlling for OD behaviors in the first grade; (b) OD behaviors in the first grade predict increases in CU behaviors in the fourth grade, controlling for CU behaviors in the first grade; (c) both unique effects are significant; and (d) neither unique effect is significant. A longitudinal panel model with two latent variables (CU and OD behaviors), three sources (mothers, fathers, teachers), and two occasions (spring of the first and fourth grades) was used to evaluate the four possibilities among 758 (54% boys) first grade and 469 (53% boys) fourth grade Spanish children. For mother-, father-, and teacher-reports, OD behaviors in the first grade predicted increases in CU behaviors in the fourth grade, after controlling for CU behaviors in the first grade, whereas CU behaviors in the first grade did not predict increases in OD behaviors in the fourth grade, after controlling for OD behaviors in the first grade. OD behaviors thus conferred independent vulnerability to increases in CU behaviors 3 years later among young children.
Posterior Cerebellar Volume and Executive Function in Young Adults With Congenital Heart Disease

Objectives: As the number of adolescents and young adults (AYAs) surviving congenital heart disease (CHD) grows, studies of long-term outcomes are needed. CHD research documents poor executive function (EF) and cerebellum (CB) abnormalities in children. We examined whether AYAs with CHD exhibit reduced EF and CB volumes. We hypothesized a double dissociation such that the posterior CB is related to EF while the anterior CB is related to motor function. We also investigated whether the CB contributes to EF above and beyond processing speed. Methods: Twenty-two AYAs with CHD and 22 matched healthy controls underwent magnetic resonance imaging and assessment of EF, processing speed, and motor function. Volumetric data were calculated using a cerebellar atlas (SUIT) developed for SPM. Group differences were compared with t tests, relationships were tested with Pearson’s correlations and Fisher’s r to z transformation, and hierarchical regression was used to test the CB’s unique contributions to EF. Results: CHD patients had reduced CB total, lobular, and white matter volume (d=.52–.99) and poorer EF (d=.79–1.01) compared to controls. Significant correlations between the posterior CB and EF (r=.29–.48) were identified but there were no relationships between the anterior CB and motor function nor EF. The posterior CB predicted EF above and beyond processing speed (ps<.001). Conclusions: This study identified a relationship between the posterior CB and EF, which appears to be particularly important for inhibitory processes and abstract reasoning. The unique CB contribution to EF above and beyond processing speed alone warrants further study. (JINS, 2018, 24, 939–948)

There is limited evidence on the acceptability, feasibility and cost-effectiveness of task-sharing interventions to narrow the treatment gap for mental disorders in sub-Saharan Africa. The purpose of this article is to describe the rationale, aims and methods of the Africa Focus on Intervention Research for Mental health (AFFIRM) collaborative research hub. AFFIRM is investigating strategies for narrowing the treatment gap for mental disorders in sub-Saharan Africa in four areas. First, it is assessing the feasibility, acceptability and cost-effectiveness of task-sharing interventions by conducting randomised controlled trials in Ethiopia and South Africa. The AFFIRM Task-sharing for the Care of Severe mental disorders (TaSCS) trial in Ethiopia aims to determine the acceptability, affordability, effectiveness and sustainability of mental health care for people with severe mental disorder delivered by trained and supervised non-specialist, primary health care workers compared with an existing psychiatric nurse-led service. The AFFIRM trial in South Africa aims to determine the cost-effectiveness of a task-sharing counselling intervention for maternal depression, delivered by non-specialist community health workers, and to examine factors influencing the implementation of the intervention and future scale up. Second, AFFIRM is building individual and institutional capacity for intervention research in sub-Saharan Africa by providing fellowship and mentorship programmes for candidates in Ethiopia, Ghana, Malawi, Uganda and Zimbabwe. Each year five Fellowships are awarded (one to each country) to attend the MPhil in Public Mental Health, a joint postgraduate programme at the University of Cape Town and Stellenbosch University. AFFIRM also offers short courses in intervention research, and supports PhD students attached to the trials in Ethiopia and South Africa. Third, AFFIRM is collaborating with other regional National Institute of Mental Health funded hubs in Latin America, sub-Saharan Africa and south Asia, by designing and executing shared research projects related to task-sharing and narrowing the treatment gap. Finally, it is establishing a network of collaboration between researchers, non-governmental organisations and government agencies that facilitates the translation of research knowledge into policy and practice. This article describes the developmental process of this multi-site approach, and provides a narrative of challenges and opportunities that have arisen during the early phases. Crucial to the long-term sustainability of this work is the nurturing and sustaining of partnerships between African mental health researchers, policy makers, practitioners and international collaborators.
White Matter Integrity Dissociates Verbal Memory and Auditory Attention Span in Emerging Adults with Congenital Heart Disease

White matter disruptions have been identified in individuals with congenital heart disease (CHD). However, no specific theory-driven relationships between microstructural white matter disruptions and cognition have been established in CHD. We conducted a two-part study. First, we identified significant differences in fractional anisotropy (FA) of emerging adults with CHD using Tract-Based Spatial Statistics (TBSS). TBSS analyses between 22 participants with CHD and 18 demographically similar controls identified five regions of normal appearing white matter with significantly lower FA in CHD, and two higher. Next, two regions of lower FA in CHD were selected to examine theory-driven differential relationships with cognition: voxels along the left uncinate fasciculus (UF; a tract theorized to contribute to verbal memory) and voxels along the right middle cerebellar peduncle (MCP; a tract previously linked to attention). In CHD, a significant positive correlation between UF FA and memory was found, r(20)=.42, p=.049 (uncorrected). There was no correlation between UF and auditory attention span. A positive correlation between MCP FA and auditory attention span was found, r(20)=.47, p=.027 (uncorrected). There was no correlation between MCP and memory. In controls, no significant relationships were identified. These results are consistent with previous literature demonstrating lower FA in younger CHD samples, and provide novel evidence for disrupted white matter integrity in emerging adults with CHD. Furthermore, a correlational double dissociation established distinct white matter circuitry (UF and MCP) and differential cognitive correlates (memory and attention span, respectively) in young adults with CHD. The Murchison Widefield Array (MWA) is one of three Square Kilometre Array Precursor telescopes and is located at the Murchison Radio-astronomy Observatory in the Murchison Shire of the mid-west of Western Australia, a location chosen for its extremely low levels of radio frequency interference. The MWA operates at low radio frequencies, 80–300 MHz, with a processed bandwidth of 30.72 MHz for both linear polarisations, and consists of 128 aperture arrays (known as tiles) distributed over a ~3-km diameter area. Novel hybrid hardware/software correlation and a real-time imaging and calibration systems comprise the MWA signal processing backend. In this paper, the as-built MWA is described both at a system and sub-system level, the expected performance of the array is presented, and the science goals of the instrument are summarised.

Perennial ryegrass evaluation schemes categorize varieties into three maturity (early, intermediate and late) and two ploidy (diploid and tetraploid) groups, and compare the relative yield, persistence and nutritive quality of varieties within these groups. The present study compared these groups for herbage yield, dry matter (DM) concentration and, using near infrared reflectance spectroscopy (NIRS), four quality characteristics (in vitro content of digestible dry matter (CDDM), water-soluble carbohydrate (WSC) and crude protein (CP) concentrations, and buffering capacity). A total of 1208 plots were sown across 5 years in Irish Recommended List trials and then harvested 6–7 times in each of 2 harvest years. This also allowed an assessment of the effect of sward age. Maturity group had no significant effect on annual herbage yield, quality or DM concentration except for in vitro CDDM (P<0·01) but differed significantly for in vitro CDDM (P<0·01), WSC concentration (P<0·01) and buffering capacity (P<0·05) at the first silage harvest. Tetraploid swards had greater annual herbage yields (P<0·001), in vitro CDDM values (P<0·001) and WSC concentrations (P<0·01), but lower CP and DM concentrations (P<0·001) than diploids. Swards in their first full year produced an additional 5·17 t/ha DM (P<0·001) and had a higher (P<0·01) WSC concentration at the second silage harvest than in their second year, but did not differ significantly for in vitro CDDM and WSC, CP or DM concentrations. The present study showed that differences exist in yield, nutritive quality and ensilability indices between maturity and ploidy groups. These observations justify their assessment in variety comparative trials and facilitates particular groups being selected for individual farming systems to increase efficiency.

The Proto-I experiment for a proof-of-principle experiment is described resulting in a beam divergence for ions of only 14mrad at 1·5 MV—420 KA operation and 6 KA/cm2 anode current density before focusing. This is a better beam divergence than required for Sandia National Laboratories' PBFA I and II installations, for which scaling is discussed. Ion source development is reported and the PBFA II experiment described for 30 MV operation with short pulses (10–15 nsec) of lithium ions. Pulses of 50 nsec width and 130 KJ energy have been achieved from the prototype module for PBFA II.

Background
Continuity of care is considered by patients and clinicians an essential feature of good quality care in long-term disorders, yet there is general agreement that it is a complex concept. Most policies emphasize it and encourage systems to promote it. Despite this, there is no accepted definition or measure against which to test policies or interventions designed to improve continuity. We aimed to operationalize a multi-axial model of continuity of care and to use factor analysis to determine its validity for severe mental illness.
MethodA multi-axial model of continuity of care comprising eight facets was operationalized for quantitative data collection from mental health service users using 32 variables. Of these variables, 22 were subsequently entered into a factor analysis as independent components, using data from a clinical population considered to require long-term consistent care.
ResultsFactor analysis produced seven independent continuity factors accounting for 62.5% of the total variance. These factors, Experience and Relationship, Regularity, Meeting Needs, Consolidation, Managed Transitions, Care Coordination and Supported Living, were close but not identical to the original theoretical model.
ConclusionsWe confirmed that continuity of care is multi-factorial. Our seven factors are intuitively meaningful and appear to work in mental health. These factors should be used as a starting-point in research into the determinants and outcomes of continuity of care in long-term disorders.

We report recent advances in the development of conjugated polymer device structures. We have fabricated vertical microcavity structures in which the optical path distance is tunable through control of the orientation of a liquid crystal layer, and demonstrate cavity tuning over 59 nm. We show that this structure allows convenient matching of cavity modes to the freespace emission spectrum from poly(r henylene vinylene), PPV. We have investigated the operation of electrically-excited polymer LEDs with short pulse operation, and have achieved current densities above 103 A/cm2 and peak brightnesses of up to 5 × 106 cd/m 2. We have measured the optical absorption due to the injected carriers under these drive conditions, and find an absorption band near 1.6 eV which we attribute to the presence of polarons. This allows measurement of the injected charge density, from which we deduce that carrier mobilities are strongly enhanced at high fields, reaching values of 4 x 10−2 cm2/Vsec for PPV diodes.

We have developed an amino-functional silicone resin to toughen epoxies which, when prereacted with the epoxy function in resins, undergoes in-situ phase separation during final epoxy curing. SEM analyses of the morphology of fracture surfaces of MY720- DDS, Epon 828-DDS and other epoxy matrices modified with the silicone resin showed rough surfaces with the formation of very uniform <10 μm spheres. Silicon and sulfur elemental distribution mapping showed silicon rich spheres embedded in an epoxy matrix. We report cavitation, particle debonding and pull-out, and an increases in fracture surface area as possible modes of toughening. Silicone modified materials give improvements in slow strain rate G1c fracture toughness measurements of 250–400%, similar to carboxy terminated polybutadiene-acrylonitrile copolymer (CTBN) modifiers, but with a much smaller flexural modulus loss. The Tg of the modified epoxy matrices are maintained, moisture resistance is improved, and flammability is reduced.

We have measured the infrared (IR) reflectance and Raman spectra of electron doped and undoped Nd2CuO4 and Pr2CuO4. Utilizing the results from doped ceramics and undoped ab plane oriented single crystals, we observe and assign all the IR active phonons (3A2u + 4Eu) predicted by group theory. We have also observed the Raman active modes involving atom motion along the c axis. We compare our results to those found in the related hole superconductors based upon La2CuO4.

Capacitance transient spectroscopy has been applied to identify deep levels associated with heteroepitaxial GaAs grown on silicon. Results from p+n diode test structures reveal creation of the electron trap EL2 and a high density of hole states in the bandgap. This is the first reported observation of hole traps in MBE GaAs on Si. The activation behavior of the electron and hole signal peaks fits the signature of two different charge states associated with EL2, a native point defect complex seen in MOCVD, VPE, and bulk-grown GaAs, but not usually observed in MBE grown GaAs. Interstingly, the spectra seen show many similarities with earlier deep level transient spectroscopy (DLTS) observations on plastically deformed GaAs.

The physical study of planets and satellites is probably one of the more active fields of research of the second half of this century. This is due to space exploration by spacecraft, but also to the use of modern detectors, of large ground-based telescopes, and of powerful computers by active researchers. Planetary research (or planetology) is a pluridisciplinary domain, which requires not only the competence of astronomers, but also of geophysicists, of mineralogists, of climatologists, of biologists, of chemists, of physicists, of “pure„ mathematicians, and many other scientists. Many results are at the boundary of those of other commissions such as the 15, 20, 7, 19, 33, 40, 44, 49 and 51 ones. The study of the main results obtained during this last triennum shows a perfect complementarity between space and ground-based observations. It should be arbitrary to separate space and ground-based scientists. The have the same goal and they study the same objects. Quite often, the same individuals use both techniques, depending on the most efficient one for the problem under study. It is remarkable to see that space data collected more than ten years ago are still analysed in connection with ground-based observations. The same remarks can apply for ground-based data. In addition to that, new theoretical models, new numerical simulations and new laboratory experiments have ben recently developed. They all contribute to a better understanding of planets and satellites physics.
