A Philosophy of Playing Drum Kit

The author is a drummer with experience in a variety of musical genres and contexts, with emphasis on rock and related styles. This auto ethnographic Element presents the author's philosophy of playing drum kit. The text explains how playing drum kit matters to this musician and may resonate with others to whom making music matters in similar ways. The Element contains audio files of music in which the author plays drum kit in the ensemble settings described. There are photos of the author's drums and of him drumming. Based on June Boyce-Tillman's non-religious model of holistic spirituality and Tim Ingold's notion of correspondences, the author describes how playing drum kit enables him to experience transcendence – the magical nexus at which Materials, Construction, Values/Culture and Expression meet. Each of these domains, and the magic derived from their combination, is illustrated through examples of the author's live and recorded musical collaborations.
Infection control and the prevalence, management and outcomes of SARS-CoV-2 infections in mental health wards in London, UK: lessons learned from wave 1 to wave 2

Background
Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) disease (COVID-19) has high morbidity and mortality in older adults and people with dementia. Infection control and prevention measures potentially reduce transmission within hospitals.
AimsWe aimed to replicate our earlier study of London mental health in-patients to examine changes in clinical guidance and practice and associated COVID-19 prevalence and outcomes between COVID-19 waves 1 and 2 (1 March to 30 April 2020 and 14 December 2020 to 15 February 2021).
MethodWe collected the 2 month period prevalence of wave 2 of COVID-19 in older (≥65 years) in-patients and those with dementia, as well as patients’ characteristics, management and outcomes, including vaccinations. We compared these results with those of our wave 1 study.
ResultsSites reported that routine testing and personal protective equipment were available, and routine patient isolation on admission occurred throughout wave 2. COVID-19 infection occurred in 91/358 (25%; 95% CI 21–30%) v. 131/344, (38%; 95% CI 33–43%) P < 0.001 in wave 1. Hospitals identified more asymptomatic carriers (26/91; 29% v. 16/130; 12%) and fewer deaths (12/91; 13% v. 19/131; 15%; odds ratio = 0.92; 0.37–1.81) compared with wave 1. The patient vaccination uptake rate was 49/58 (85%).
ConclusionsPatients in psychiatric in-patient settings, mostly admitted without known SARS-CoV-2 infection, had a high risk of infection compared with people in the community but lower than that during wave 1. Availability of infection control measures in line with a policy of parity of esteem between mental and physical health appears to have lowered within-hospital COVID-19 infections and deaths. Cautious management of vulnerable patient groups including mental health patients may reduce the future impact of COVID-19.
Validating a risk assessment tool in United Kingdom and Irish paediatric cardiac catheterisation practice

Background:
No established risk prediction tool exists in United Kingdom and Irish Paediatric Cardiology practice for patients undergoing cardiac catheterisation. The Catheterisation RISk score for Paediatrics is used primarily in North American practice to assess risk prior to cardiac catheterisation. Validating the utility and transferability of such a tool in practice provides the opportunity to employ an already established risk assessment tool in everyday practice.
Aims:To ascertain whether the Catheterisation RISk score for Paediatrics assessment tool can accurately predict complications within United Kingdom and Irish congenital catheterisation practice.
Methods:Clinical and procedural data including National Institute for Cardiovascular Outcomes Research derived outcome data from 1500 patients across five large congenital cardiology centres in the United Kingdom and Ireland were retrospectively collected. Catheterisation RISk score for Paediatrics were then calculated for each case and compared with the observed procedural outcomes. Chi-square analysis was used to determine the relationship between observed and predicted events.
Results:Ninety-eight (6.6%) patients in this study experienced a significant complication as qualified by National Institute for Cardiovascular Outcomes Research classification. 4% experienced a moderate complication, 2.3% experienced a major complication and 0.3% experienced a catastrophic complication resulting in death. Calculated Catheterisation RISk score for Paediatrics scores correlated well with all observed adverse events for paediatric patients across all CRISP categories. The association was also transferable to adult congenital heart disease patients in lower Catheterisation RISk score for Paediatrics categories (CRISP 1–3).
Conclusion:The Catheterisation RISk score for Paediatrics score accurately predicts significant complications in congenital catheterisation practice in the United Kingdom and Ireland. Our data validated the Catheterisation RISk score for Paediatrics assessment tool in five congenital centres using National Institute for Cardiovascular Outcomes Research-derived outcome data.

Summary
In this autoethnographic essay, the author – a drummer – describes how he derives meaning from playing the drum kit. He presents accounts of playing drums both alone and in the context of an original rock band. Drawing from existing scholarship on aesthetic experience and meaning in music making, the author argues that while he plays drums often in a state of flow, it may be unhelpful to construe this – as others have done – as music making for its own sake. Rather than positioning his drumming as autotelic or intrinsically worthwhile, the author explains how he plays for the fulfilment derived therefrom, as part of a life lived in search of eudaimonia – flourishing both individually and as part of a community. Drumming in these contexts is, the author argues, a locus of spirituality, understood through the lenses of embodiment, authenticity, and personal agency as a form of success. Playing drums – for this drummer – provides a connection to, and a window into, his soul.
Supporting Older Co-Resident Carers of Older People – The Impact of Care Act Implementation in Four Local Authorities in England

The Care Act 2014 provides for ‘parity of esteem’ between people with social care needs and carers. This is achieved by extending the principles of prevention and wellbeing to carers; reinforcing carers’ right to an assessment; and setting national eligibility criteria for access to carer support services. This article reports on research that examined the impact of these changes on older co-resident carers of older people in four English local authorities. Findings are described in relation to four key themes: organisational arrangements and rationale; the aims of carer support and preventative services; design and delivery of carer assessment; and barriers to assessment and support. The findings of the research suggest that, despite the new statutory requirements, underlying contradictions and tensions in local authorities’ relationship with carers, and efforts to support them, remain unresolved.

Summary
CRITICAL DISCOURSE SURROUNDING the use of digital technology within archaeology has tended to portray the discipline as a consumer of preexisting technology and has rarely seen itself as an agent of technological development or innovation (Huggett, 2015). Increasingly, however, as barriers to participation in software development have fallen, archaeology has played a small but significant role in the development of digital technology (Wilson and Edwards, 2015; Beale and Reilly, in press). Of equal significance to technological development has been the focus within archaeology upon the development of digital practice in response to the specific social, cultural and political conditions within which archaeological work takes place (Jeffrey et al., 2015; Perry and Beale, 2015).
These developments have been of particular significance within community archaeology where archaeological practice can be a nexus for identity formation and the recasting of the relationship between people and places (Smith, 2006). The shape of practice, and the role of technology within it, can have a powerful influence on the experience of the archaeologist. The meaning, function and perceived value of technology are dictated to a significant degree by the conditions of use and the preconceptions of users. This is complicated still further by the asymmetrical power relationships which have been an endemic feature of the archaeological research community and which have resulted in the privileging of specific ways of working and thinking about archaeological practice (Collins, 1999; Conkey, 2005; Hamilakis, 2007). As we move towards the development and recognition of increasingly diverse research communities it is imperative that we understand technology not only in terms of perceived (abstract) affordances but in terms of their use and their impact within specific social, political and cultural scenarios.
In this chapter we describe the latest developments in the Re-reading the British Memorial project, a University of York-based research project which aims to enable the use of open source and low-cost technologies by community groups involved in the archaeological and historical study of burial space in the United Kingdom. This project responds to the urgent need to re-engage communities in the use and management of burial space. A range of social and environmental factors have created a situation in which burial spaces are at risk as heritage assets and as community resources (Ray et al., 2014; Jones, 2016; Mytum et al., 2015).

Musicians are acknowledged to lead complex working lives, often characterised as portfolio careers. The higher music education research literature has tended to focus on preparing students for rich working lives and multiple identity realisations across potential roles. Extant literature does not address the area of work-life balance, which this paper begins to explore, as the authors seek to better understand potential challenges around combining music graduates' multivariate ambitions, commitments and identities as musicians in the world. Rich data are presented, following interviews with professional musicians in London, UK, discussing health, portfolio careers and family. The authors conclude that more research is required to gain a deeper understanding of work-life balance for musicians, and that pedagogical approaches in higher music education could more effectively help students to prepare for their futures in a more holistic way.
