Background:
Dementia is an emerging health priority in Australian Aboriginal communities, but substantial gaps remain in our understanding of this issue, particularly for the large urban section of the population. In remote Aboriginal communities, high prevalence rates of dementia at relatively young ages have been reported. The current study is investigating aging, cognitive decline, and dementia in older urban/regional Aboriginal Australians.
Methods:We partnered with five Aboriginal communities across the eastern Australian state of New South Wales, to undertake a census of all Aboriginal men and women aged 60 years and over residing in these communities. This was followed by a survey of the health, well-being, and life history of all consenting participants. Participants were also screened using three cognitive instruments. Those scoring below designated cut-offs, and a 20% random sample of those scoring above (i.e. “normal” range), completed a contact person interview (with a nominated family member) and medical assessment (blind to initial screening results), which formed the basis of “gold standard” clinical consensus determinations of cognitive impairment and dementia.
Conclusion:This paper details our protocol for a population-based study in collaboration with local Aboriginal community organizations. The study will provide the first available prevalence rates for dementia and cognitive impairment in a representative sample of urban Aboriginal people, across city and rural communities, where the majority of Aboriginal Australians live. It will also contribute to improved assessment of dementia and cognitive impairment and to the understanding of social determinants of successful aging, of international significance.

THE IMPACT OF BRIEF MOTIVATIONAL INTERVENTION AT THE START OF AN OUTPATIENT DAY PROGRAMME FOR ALCOHOL DEPENDENCE

Motivational intervention, a strategy to increase motivation for change, was tested as an addition to treatment at the start of an outpatient programme for alcohol dependence. Admissions to the programme were randomly allocated to either a motivational or educational procedure. The three motivational constructs of the SOCRATES-8A Readiness to Change questionnaire (Ambivalence about change, Recognition of problems, Taking Steps to change) were used as measures of motivation for change. At one week post-intervention, motivational participants reported significantly greater levels of problem recognition. The motivational group's post-intervention scores were significantly higher on the Taking Steps scale and significantly lower on the Ambivalence scale. There was no difference on measures of engagement in treatment or drop-out from the outpatient treatment programme. For the population as a whole, a relatively low pre-intervention score on the Ambivalence scale was predictive of drop-out. Results support the efficacy of motivational intervention for decreasing self-reported levels of ambivalence about change and for increasing problem recognition and taking steps towards change. More detailed research with a longer follow-up period is needed to determine whether this has any impact on other vectors of motivation for change, including participation in treatment, outcome expectancy and drinking behaviour.

Summary
This paper proposes an investigation of the acoustical characteristics of the sound of the shakuhachi. We have worked on the hypothesis that a study of this kind would not only provide an exact description of physical phenomena but would also offer a description of the music itself. Our point of departure was the observation that traditional shakuhachi music has shown virtually no interest in architectonic formal relationships; on the other hand, highly structured smaller units – what we have called tone cells – clearly have great musical significance. We set ourselves the task of studying these tone cells as the primary vehicle by which musical meaning is conveyed.
In this ‘world of a single sound’ we found structures whose musical meaning mirrors their physical evolution. As the tone cell stands somewhere between the tone as a physical event and the independent musical phrase, so is our description of its qualities both the representation of physical characteristics and the analysis of music.
The Japanese musicologist Tsuge Gen'ichi has said that in Japanese music ‘there is a deep-seated attitude towards realization of a self-sufficient musical world within the scope of even a single sound’(Tsuge 1981: 110). We hope with this study to contribute to an understanding of this important characteristic of Japanese of music.
Introduction
In Western musicology there is virtually no connection between musical analysis and the acoustical analysis of instrumental sounds. Musical analysis is concerned with the way music is constructed – how it is put together; it is not concerned with individual tones.
Pad-and-Buzzer Training, Dry-Bed Training, and Stop-Start Training in the Treatment of Primary Nocturnal Enuresis

Forty primary nocturnally enuretic children were randomly assigned to one of four experimental conditions over a ten week period. These were (a) standard Pad-and-Buzzer Training (PBT), (b) Stop—Start Training (SST), involving practice in interrupting the flow of urine during micturition, (c) Dry Bed Training (DBT) and (d) Waiting List Control (WLC). Numbers of dry nights were assessed during a 14 days pre-treatment baseline period, and again during 14 days at the end of treatment and at 12 weeks follow-up. At the end of treatment the proportions of subjects in each condition achieving 14 consecutive dry nights were: PBT 44.4%, SST 16.6%, DBT 50% and WLC 0%. Each of the three treatments produced more dry nights than the WLC, but did not significantly differ from one another. The results were discussed in terms of their generalizability and in the context of superior results previously reported for DBT.
Compliance with Relaxation Training: the Effect of Providing Information

In order to test its effect on compliance with recommended practice of relaxation training, patients in anxiety management training groups were provided with additional information about the procedure and its rationale. After an introductory training session all subjects were provided with audiotaped relaxation instructions to use for three weeks until the second group meeting. Additional information was provided either as an audiotaped message at the end of the relaxation instructions (“Message”) or as a printed leaflet (“Leaflet”). In a 2 × 2 factorial experiment 32 subjects were randomly assigned to one of four conditions: (1) Message, (2) Leaflet, (3) Message and Leaflet, (4) Basic Programme alone. Neither manipulation had an effect on self-reported frequency of practice during the three weeks (which was high), but both increased subjects' knowledge of the treatment and also their satisfaction with it. Subjects receiving the message were significantly less likely to drop out of treatment during the remainder of the 16-weeks group programme. Compliance was predicted by subjects' perceptions of the severity of their condition but not by their knowledge, satisfaction or state anxiety.
