Guidelines for the diagnosis and treatment of neuroangiostrongyliasis: updated recommendations

A subcommittee of the Hawaii Governor's Joint Task Force on Rat Lungworm Disease developed preliminary guidelines for the diagnosis and treatment of neuroangiostrongyliasis (NAS) in 2018 (Guidelines, 2018). This paper reviews the main points of those guidelines and provides updates in areas where our understanding of the disease has increased. The diagnosis of NAS is described, including confirmation of infection by real-time polymerase chain reaction (RTi-PCR) to detect parasite DNA in the central nervous system (CNS). The treatment literature is reviewed with recommendations for the use of corticosteroids and the anthelminthic drug albendazole. Long-term sequelae of NAS are discussed and recommendations for future research are proposed.

The emergence and persistence of conduct problems (CPs) during early childhood is a robust predictor of behavior problems in school and of future maladaptation. In this study we examined the reciprocal influences between observed coercive interactions between children and caregivers, oppositional and aggressive behavior, and growth in parent report of early childhood (ages 2–5) and school-age CPs (ages 7.5 and 8.5). Participants were drawn from the Early Steps multisite randomized prevention trial that includes an ethnically diverse sample of male and female children and their families (N = 731). A parallel-process growth model combining latent trajectory and cross-lagged approaches revealed the amplifying effect of observed coercive caregiver–child interactions on children's noncompliance, whereas child oppositional and aggressive behaviors did not consistently predict increased coercion. The slope and initial levels of child oppositional and aggressive behaviors and the stability of caregiver–child coercion were predictive of teacher-reported oppositional behavior at school age. Families assigned to the Family Check-Up condition had significantly steeper declines in child oppositional and aggressive behavior and moderate reductions in oppositional behavior in school and in coercion at age 3. Results were not moderated by child gender, race/ethnicity, or assignment to the intervention condition. The implications of these findings are discussed with respect to understanding the early development of CPs and to designing optimal strategies for reducing problem behavior in early childhood with families most in need.

There is increasing demand for the implementation of effects-based monitoring and surveillance (EBMS) approaches in the Great Lakes Basin to complement traditional chemical monitoring. Herein, we describe an ongoing multiagency effort to develop and implement EBMS tools, particularly with regard to monitoring potentially toxic chemicals and assessing Areas of Concern (AOCs), as envisioned by the Great Lakes Restoration Initiative (GLRI). Our strategy includes use of both targeted and open-ended/discovery techniques, as appropriate to the amount of information available, to guide a priori end point and/or assay selection. Specifically, a combination of in vivo and in vitro tools is employed by using both wild and caged fish (in vivo), and a variety of receptor- and cell-based assays (in vitro). We employ a work flow that progressively emphasizes in vitro tools for long-term or high-intensity monitoring because of their greater practicality (e.g., lower cost, labor) and relying on in vivo assays for initial surveillance and verification. Our strategy takes advantage of the strengths of a diversity of tools, balancing the depth, breadth, and specificity of information they provide against their costs, transferability, and practicality. Finally, a series of illustrative scenarios is examined that align EBMS options with management goals to illustrate the adaptability and scaling of EBMS approaches and how they can be used in management decisions.

We compared the results of the tuberculin skin test with the results of the QuantiFERON-TB Gold In-Tube (QFT-GIT) assay among 182 low-risk healthcare workers. Overall agreement and specificity were high, but the tests did not agree on positive results. Only 2 of 5 positive QFT-GIT assay results could be confirmed with repeat analyses. Indeterminate results were associated with potential immunosuppression.

Summary
The work of the Moscow poet Aleksandr Velichanskii (1940–90) has hardly been publicly discussed at all, either inside or outside Russia. This is not surprising, because after the poet's remarkable début in 1969 there was no further substantial publication in the USSR until 1988. Velichanskii did not take part in official literary life during the 1970s and 1980s. In the year before his death, Velichanskii's friends took advantage of the new situation in publishing in the USSR and sponsored a series of collections, each with a print run of 3000. These books are as follows: Udel. Izbrannye stikhotvoreniia 1966–1973 (Portion. Selected Poems 1966–1973, 1989); Basta. Rechitativ. 1973–1975 (Basta. Recitative. 1973–1975, 1989); Pomolvka (nesostoiavshiisia roman). 1976–1977 (Betrothal (A Love Affair that Didn't Work). 1976–1977, 1990); Podzemnaia nimfa. 1976–1977 (The Underground Nymph. 1976–1977, 1990); Bezdonnyi cheln. 1982–1983 (The Bottomless Boat. 1982–1983, 1990); Kakhetinskie stikhi. 1985–1986 (Poems from Kakhetia. 1985–1986, 1990); and Vplot′ (Right up to, 1991), which collects lyrics from the last period of Velichanskii's life, 1983–9. All these books were published in Moscow by Prometei, the house attached to the Moscow State Pedagogical University; they all include a statement indicating that private sponsorship was involved. There is only one book published by a major Moscow house, Vremeni nevidimaia tverd′. Stikhotvoreniia (The Invisible Firmament of Time. Poems, Moscow: Sovremennik, 1990), a selection which includes items from almost all the books mentioned earlier.

Among the poets of the Third Emigration are a few who were officially recognized and published in the Soviet Union. The vast majority of them, though, hardly published anything in that country; sometimes because they were denied access and more often because they did not seek publication in the Soviet Union, preferring samizdat or publication abroad. Lev Loseff is a unique figure who falls into neither of these categories. He was a professional journalist and writer for children until he left the Soviet Union in 1976, but, although he was a popular figure in Leningrad's unofficial literary life, he was not known to be a poet. In fact, Loseff began as a serious poet in 1974 at an age—thirty-seven—that by any standards is very late and by Russian standards is something like the beginning of the afterlife: It is the age at which Pushkin was killed.
