Decentralized clinical trials in the trial innovation network: Value, strategies, and lessons learned

New technologies and disruptions related to Coronavirus disease-2019 have led to expansion of decentralized approaches to clinical trials. Remote tools and methods hold promise for increasing trial efficiency and reducing burdens and barriers by facilitating participation outside of traditional clinical settings and taking studies directly to participants. The Trial Innovation Network, established in 2016 by the National Center for Advancing Clinical and Translational Science to address critical roadblocks in clinical research and accelerate the translational research process, has consulted on over 400 research study proposals to date. Its recommendations for decentralized approaches have included eConsent, participant-informed study design, remote intervention, study task reminders, social media recruitment, and return of results for participants. Some clinical trial elements have worked well when decentralized, while others, including remote recruitment and patient monitoring, need further refinement and assessment to determine their value. Partially decentralized, or “hybrid” trials, offer a first step to optimizing remote methods. Decentralized processes demonstrate potential to improve urban-rural diversity, but their impact on inclusion of racially and ethnically marginalized populations requires further study. To optimize inclusive participation in decentralized clinical trials, efforts must be made to build trust among marginalized communities, and to ensure access to remote technology.

Summary
Introduction
The acknowledgement of state police as important sociopolitical actors is not new (Tankebe, 2008). For many former colonies with complex diasporic histories, state police have always played a critical role in the establishment and maintenance of law and order, and often served as the most visible representation of the character of governance (Bayley, 2005). The contemporary shift in focus to a less confrontational image of police premised on the maintenance of democratic rule is largely viewed as an attempt to learn from past experiences (both local and foreign) and to disassociate from a British colonial policing model grounded in turbulent decolonization processes (Sinclair, 2017). For many countries in the global South, particularly small-island developing states (SIDS), attempts at rebranding police have neither significantly impacted police legitimacy nor positively impacted on community perceptions of police or policing (Trnka, 2011; Watson, 2016; Stamatakis, 2019; Adams, 2020). In such contexts, police legitimacy and accountability are constantly questioned.
The authenticity of the police ‘service’ agenda tends to be underscored by allegations of undemocratic and unlawful policing practices, along with large-scale public distrust and strongly expressed dissatisfaction with police and policing (Jauregui, 2013; Watson, 2016). Though different histories of colonial subjugation have shaped discourses on police and policing practices, several themes centred on questions of police legitimacy, fairness, susceptibility to political influence, lawfulness and monopoly of power remain concurrent throughout existing literature. Attempts at image management, posturing to gain public popularity and rebranding for improved relations with the public suggest either an awareness of a problem with the way police are perceived by the public or an identified need to revise policing strategies for improved service delivery (or both). What is evident is that the model of policing in former colonies remains a work in progress.
This chapter echoes sentiments about the importance of acknowledging colonial undercurrents that continue to inform governance processes which pervade concepts of law-and-order maintenance and enforcement. It draws on examples from Trinidad and Tobago (T&T) to support arguments about the proliferation of problematic policing models, demonstrative of the disconnect between the state and the larger populace.
Associations between clinically diagnosed medical conditions and dietary supplement use: the US military dietary supplement use study

Objective:
This study examined associations between multiple dietary supplement (DS) categories and medical conditions diagnosed by health professionals.
Design:Cross-sectional.
Setting:Volunteers completed an online questionnaire on DS use and demographic/lifestyle factors. Medical diagnoses were obtained from a comprehensive military electronic medical surveillance system and grouped into twenty-four clinically diagnosed medical conditions (CDMC).
Participants:A stratified random sample of US service members (SM) from all military services (n 26 680).
Results:After adjustment for demographic/lifestyle factors (logistic regression), higher risk was found for 92 % (22/24) of CDMC among individual vitamins/minerals users, 58 % (14/24) of CDMC among herbal users, 50 % (12/24) of CDMC among any DS users and 46 % (11/24) of CDMC among multivitamins/multiminerals (MVM) users. Among protein/amino acid (AA) users, risk was lower in 25 % (6/24) of CDMC. For combination products, risk was higher in 13 % (3/24) of CDMC and lower in 8 % (2/24). The greater the number of CDMC, the higher the prevalence of DS use in most DS categories except proteins/AA where prevalence decreased.
Conclusions:Users in many DS categories had a greater number of CDMC, but protein/AA users had fewer CDMC; results for combination products were mixed. These data indicate those with certain CDMC were also users in some DS categories, especially individual vitamins/minerals, herbals and MVM. Data are consistent with the perception that use of DS enhances health, especially in those with CDMC. Protein/AA and combination product users were more likely to be younger, more physically active men, factors that likely reduced CDMC.

Summary
In Section 3.2, I focus on the development of semantics, which began as a collection of methodological tools for applying formal languages to philosophical problems. In Section 3.3, I trace the origins of contemporary debates about the nature of propositional content. In Section 3.4, I give a brief history of philosophical work on speech acts and pragmatics, emphasizing the origins of current debates in conflicting threads of Wittgenstein’s writing.
Use of airborne infection isolation in potential cases of pulmonary tuberculosis

Objective:
To identify risk factors of patients placed in airborne infection isolation (AII) for possible pulmonary tuberculosis (TB) to better predict TB diagnosis and allow more judicious use of AII.
Methods:Case-control, retrospective study at a single tertiary-care academic medical center. The study included all adult patients admitted from October 1, 2014, through October 31, 2017, who were placed in AII for possible pulmonary TB. Cases were defined as those ultimately diagnosed with pulmonary TB. Controls were defined as those not diagnosed with pulmonary TB. Those with TB diagnosed prior to admission were excluded. In total, 662 admissions (558 patients) were included.
Results:Overall, 15 cases of pulmonary TB were identified (2.7%); of these, 2 were people living with human immunodeficiency virus (HIV; PLWH). Statistical analysis was limited by low case number. Those diagnosed with pulmonary TB were more likely to have been born outside the United States (53% vs 13%; P < .001) and to have had prior positive TB testing, regardless of prior treatment (50% vs 19%; P = .015). A multivariate analysis using non–US birth and prior positive TB testing predicted an 18.2% probability of pulmonary TB diagnosis when present, compared with 1.0% if both factors were not present.
Conclusions:The low number of pulmonary TB cases indicated AII overuse, especially in PLWH, and more judicious use of AII is warranted. High-risk groups, including those born outside the United States and those with prior positive TB testing, should be considered for AII in the appropriate clinical setting.

Couder & Fort (Phys. Rev. Lett., vol. 97, 2006, 154101) demonstrated that when a droplet walking on the surface of a vibrating bath passes through a single or a double slit, it is deflected due to the distortion of its guiding wave field. Moreover, they suggested the build-up of statistical diffraction and interference patterns similar to those arising for quantum particles. Recently, these results have been revisited (Andersen et al., Phys. Rev. E, vol. 92 (1), 2015, 013006; Batelaan et al., J. Phys.: Conf. Ser., vol. 701 (1), 2016, 012007) and contested (Andersen et al. 2015; Bohr, Andersen & Lautrup, Recent Advances in Fluid Dynamics with Environmental Applications, 2016, Springer, pp. 335–349). We revisit these experiments with a refined experimental set-up that allows us to systematically characterize the dependence of the dynamical and statistical behaviour on the system parameters. The system behaviour is shown to depend strongly on the amplitude of the vibrational forcing: as this forcing increases, a transition from repeatable to unpredictable trajectories arises. In all cases considered, the system behaviour is dominated by a wall effect, specifically the tendency for a drop to walk along a path that makes a fixed angle relative to the plane of the slits. While the three dominant central peaks apparent in the histograms of the deflection angle reported by Couder & Fort (2006) are evident in some of the parameter regimes considered in our study, the Fraunhofer-like dependence of the number of peaks on the slit width is not recovered. In the double-slit geometry, the droplet is influenced by both slits by virtue of the spatial extent of its guiding wave field. The experimental behaviour is well captured by a recently developed theoretical model that allows for a robust treatment of walking droplets interacting with boundaries. Our study underscores the importance of experimental precision in obtaining reproducible data.

Deflection missions to near-Earth asteroids will encounter non-negligible uncertainties in the physical and orbital parameters of the target object. In order to reliably assess future impact threat mitigation operations such uncertainties have to be quantified and incorporated into the mission design. The implementation of deflection demonstration missions offers the great opportunity to test our current understanding of deflection relevant uncertainties and their consequences, e.g., regarding kinetic impacts on asteroid surfaces. In this contribution, we discuss the role of uncertainties in the NEOTωIST asteroid deflection demonstration concept, a low-cost kinetic impactor design elaborated in the framework of the NEOShield project. The aim of NEOTωIST is to change the spin state of a known and well characterized near-Earth object, in this case the asteroid (25143) Itokawa. Fast events such as the production of the impact crater and ejecta are studied via cube-sat chasers and a flyby vehicle. Long term changes, for instance, in the asteroid's spin and orbit, can be assessed using ground based observations. We find that such a mission can indeed provide valuable constraints on mitigation relevant parameters. Furthermore, the here proposed kinetic impact scenarios can be implemented within the next two decades without threatening Earth's safety.

Cost-effective extension strategies are needed to promote widespreadadoption of agricultural technologies in developing countries. Integratedpest management (IPM) practices, for example, can offer economic, health,and environmental benefits but remain largely underused. This studyevaluates the current IPM dissemination program implemented by theBangladesh Department of Agricultural Extension and uses a linearprogramming model to examine alternative strategies to improve IPM adoption.Results suggest that technology transfer programs may increase their impactby reallocating funding from intensive but costly interpersonalcommunication methods (i.e., farmer field schools) to less intensive methods(i.e., mass media and field days) that reach broader audiences.

We present the results of a theoretical investigation of droplets walking on a rotating vibrating fluid bath. The droplet’s trajectory is described in terms of an integro-differential equation that incorporates the influence of its propulsive wave force. Predictions for the dependence of the orbital radius on the bath’s rotation rate compare favourably with experimental data and capture the progression from continuous to quantized orbits as the vibrational acceleration is increased. The orbital quantization is rationalized by assessing the stability of the orbital solutions, and may be understood as resulting directly from the dynamic constraint imposed on the drop by its monochromatic guiding wave. The stability analysis also predicts the existence of wobbling orbital states reported in recent experiments, and the absence of stable orbits in the limit of large vibrational forcing.

We present the results of an experimental investigation of a droplet walking on the surface of a vibrating rotating fluid bath. Particular attention is given to demonstrating that the stable quantized orbits reported by Fort et al. (Proc. Natl Acad. Sci., vol. 107, 2010, pp. 17515–17520) arise only for a finite range of vibrational forcing, above which complex trajectories with multimodal statistics arise. We first present a detailed characterization of the emergence of orbital quantization, and then examine the system behaviour at higher driving amplitudes. As the vibrational forcing is increased progressively, stable circular orbits are succeeded by wobbling orbits with, in turn, stationary and drifting orbital centres. Subsequently, there is a transition to wobble-and-leap dynamics, in which wobbling of increasing amplitude about a stationary centre is punctuated by the orbital centre leaping approximately half a Faraday wavelength. Finally, in the limit of high vibrational forcing, irregular trajectories emerge, characterized by a multimodal probability distribution that reflects the persistent dynamic influence of the unstable orbital states.

Collaboration is used by the US National Security Council as a means to integrate inter-federal government agencies during planning and execution of common goals towards unified, national security. The concept of collaboration has benefits in the healthcare system by building trust, sharing resources, and reducing costs. The current terrorist threats have made collaborative medical training between military and civilian agencies crucial.
This review summarizes the long and rich history of collaboration between civilians and the military in various countries and provides support for the continuation and improvement of collaborative efforts. Through collaboration, advances in the treatment of injuries have been realized, deaths have been reduced, and significant strides in the betterment of the Emergency Medical System have been achieved. This review promotes collaborative medical training between military and civilian medical professionals and provides recommendations for the future based on medical collaboration.
