A field study was conducted twice in Elizabeth, MS, at on-farm sites in 2010–11 and 2011–12, and twice in 2012–13 at Mississippi State University’s Delta Research and Extension Center in Stoneville, MS, to evaluate glyphosate-resistant (GR) Italian ryegrass control and crop response to fall treatments followed by postemergence herbicide treatments in winter and/or spring. Italian ryegrass was controlled ≥92% and 61% following S-metolachlor and tillage 77 d after fall treatments (DA-FT), respectively. S-metolachlor fall treatment provided 33% greater control than clethodim winter treatment at 21 d after winter treatments (DA-WT). Tillage fall treatment followed by (fb) clethodim winter treatment fb paraquat spring treatment provided similar control (93%) to treatments containing S-metolachlor fall treatment fb a winter or spring herbicide treatment (≥93%) 24 d after spring treatments (DA-ST). Greatest soybean and corn density and yield were also observed following programs containing S-metolachlor fall treatment. Sequential postemergence herbicide treatments were not required to increase corn and soybean density and yield when S-metolachlor was used as a fall treatment. Growers have the best opportunity to maximize GR Italian ryegrass control when S-metolachlor fb a winter or spring herbicide treatment is used.
Bacterial septic arthritis infections associated with intra-articular injection practices for osteoarthritis knee pain—New Jersey, 2017

Background:
In March 2017, the New Jersey Department of Health received reports of 3 patients who developed septic arthritis after receiving intra-articular injections for osteoarthritis knee pain at the same private outpatient facility in New Jersey. The risk of septic arthritis resulting from intra-articular injection is low. However, outbreaks of septic arthritis associated with unsafe injection practices in outpatient settings have been reported.
Methods:An infection prevention assessment of the implicated facility’s practices was conducted because of the ongoing risk to public health. The assessment included an environmental inspection of the facility, staff interviews, infection prevention practice observations, and a medical record and office document review. A call for cases was disseminated to healthcare providers in New Jersey to identify patients treated at the facility who developed septic arthritis after receiving intra-articular injections.
Results:We identified 41 patients with septic arthritis associated with intra-articular injections. Cultures of synovial fluid or tissue from 15 of these 41 case patients (37%) recovered bacteria consistent with oral flora. The infection prevention assessment of facility practices identified multiple breaches of recommended infection prevention practices, including inadequate hand hygiene, unsafe injection practices, and poor cleaning and disinfection practices. No additional cases were identified after infection prevention recommendations were implemented by the facility.
Discussion:Aseptic technique is imperative when handling, preparing, and administering injectable medications to prevent microbial contamination.
Conclusions:This investigation highlights the importance of adhering to infection prevention recommendations. All healthcare personnel who prepare, handle, and administer injectable medications should be trained in infection prevention and safe injection practices.
Corn (Zea mays L.) response to sublethal rates of paraquat and fomesafen at vegetative growth stages

Research was conducted from 2013 to 2015 across three sites in Mississippi to evaluate corn response to sublethal paraquat or fomesafen (105 and 35 g ai ha−1, respectively) applied PRE, or to corn at the V1, V3, V5, V7, or V9 growth stages. Fomesafen injury to corn at three d after treatment (DAT) ranged from 0% to 38%, and declined over time. Compared with the nontreated control (NTC), corn height 14 DAT was reduced approximately 15% due to fomesafen exposure at V5 or V7. Exposure at V1 or V7 resulted in 1,220 and 1,110 kg ha−1 yield losses, respectively, compared with the NTC, but yield losses were not observed at any other growth stage. Fomesafen exposure at any growth stage did not affect corn ear length or number of kernel rows relative to the NTC. Paraquat injury to corn ranged from 26% to 65%, depending on growth stage and evaluation interval. Corn exposure to paraquat at V3 or V5 consistently caused greater injury across evaluation intervals, compared with other growth stages. POST timings of paraquat exposure resulted in corn height reductions of 13% to 50%, except at V7, which was most likely due to rapid internode elongation at that stage. Likewise, yield loss occurred after all exposure times of paraquat except PRE, compared with the NTC. Corn yield was reduced 1,740 to 5,120 kg ha−1 compared with the NTC, generally worsening as exposure time was delayed. Paraquat exposure did not reduce corn ear length, compared with the NTC, at any growth stage. However, paraquat exposure at V3 or V5 was associated with reduction of kernel rows by 1.1 and 1.7, respectively, relative to the NTC. Paraquat and fomesafen applications near corn should be avoided if conditions are conducive for off-target movement, because significant injury and yield loss can result.
Effect of Fall-Applied Residual Herbicides on Rice Growth and Yield

Glyphosate-resistant (GR) Italian ryegrass is one of the most troublesome weeds in Mississippi row crop production. Fall-applied residual herbicide applications are recommended for control of GR Italian ryegrass. However, carryover of residual herbicides applied in fields for rice production can have a negative impact on rice performance. Field studies were conducted in Stoneville, MS, to determine the effects of fall-applied residual herbicides on rice growth and yield. Herbicide treatments included suggested use rates (1×) of clomazone at 840 g ai ha–1, pyroxasulfone 170 g ai ha–1, S-metolachlor 1,420 g ai ha–1, and trifluralin 1,680 g ai ha–1, and two times (2×) the suggested use rates in the fall before rice seeding. Pooled across application rate, pyroxasulfone, S-metolachlor, and trifluralin injured rice to an extent 28% to 36% greater than clomazone 14 d after emergence (DAE). Rice seedling density and height 14 DAE and rice maturity were negatively affected by all fall-applied herbicides except clomazone. Applications at 2× rates reduced rough rice yields in plots treated with pyroxasulfone, S-metolachlor, and trifluralin compared with clomazone. Pyroxasulfone applied at the 2× rate reduced rough rice yield 22% compared with the 1× rate. Rough rice yield was 90% or greater of the nontreated control in plots treated with either rate of S-metolachlor, and these were comparable with rough rice yields from plots treated with both rates of trifluralin and the 1× rate of pyroxasulfone. Early-season injury and reductions in seedling density and height 14 DAE, would preclude even 1× applications of pyroxasulfone, S-metolachlor, and trifluralin from being viable options for residual herbicide treatments targeting GR Italian ryegrass in the fall before rice seeding. Of the herbicides evaluated, only clomazone should be utilized as a fall-applied residual herbicide treatment targeting GR Italian ryegrass before seeding rice.

This article describes the Early Psychosis Prevention and Intervention Centre (EPPIC) — an innovative and comprehensive service that attempts to address the needs of older adolescents and young adults experiencing their first episode of psychosis. Some of the key theoretical concepts and philosophical tenets which underpin the service delivery model are adumbrated. Furthermore, the historical antecedents of the current service, and the “successive approximations” approach taken in developing the same, are delineated. Given the evolutionary nature of the program, the perceived future needs and developments are stipulated.

Objective.
To better understand hospital infection control practices in Ethiopia.
Design.A cross-sectional evaluation of healthcare worker (HCW) knowledge, attitudes, and practices about hand hygiene and tuberculosis (TB) infection control measures.
Methods.An anonymous 76-item questionnaire was administered to HCWs at 2 university hospitals in Addis Ababa, Ethiopia. Knowledge items were scored as correct/incorrect. Attitude and practice items were assessed using a Likert scale.
Results.In total, 261 surveys were completed by physicians (51%) and nurses (49%). Fifty-one percent of respondents were male; mean age was 30 years. While hand hygiene knowledge was fair, self-reported practice was suboptimal. Physicians reported performing hand hygiene 7% and 48% before and after patient contact, respectively. Barriers for performing hand hygiene included lack of hand hygiene agents (77%), sinks (30%), and proper training (50%) as well as irritation and dryness (67%) caused by hand sanitizer made in accordance with the World Health Organization formulation. TB infection control knowledge was excellent (more than 90% correct). Most HCWs felt that they were at high risk for occupational acquisition of TB (71%) and that proper TB infection control can prevent nosocomial transmission (92%). Only 12% of HCWs regularly wore a mask when caring for TB patients. Only 8% of HCWs reported that masks were regularly available, and 76% cited a lack of infrastructure to isolate suspected/known TB patients.
Conclusions.Training HCWs about the importance and proper practice of hand hygiene along with improving hand sanitizer options may improve patient safety. Additionally, enhanced infrastructure is needed to improve TB infection control practices and allay HCW concerns about acquiring TB in the hospital.

We examined interventions to optimize piperacillin-tazobactam use at 4 hospitals. Interventions for rotating house staff did not affect use. We could target empiric therapy in only 35% of cases. Because prescribing practices seemed to be institution specific, interventions should address attitudes of local prescribers. Interventions should target empiric therapy and ordering of appropriate cultures.
Epidemiology of Device-Associated Infections Related to a Long-Term Implantable Vascular Access Device

Objective:
To examine risk factors for, and determine the incidence of, device-associated infections among patients with an implantable vascular access device.
Setting:Grady Health System, including a 1,000-bed, inner-city, public, teaching hospital and human immunodeficiency virus (HIV), oncology, and sickle cell clinics in Atlanta, Georgia.
Patients:123 consecutive patients who received a PASPort implantable venous access device between January 1 and June 30, 1995.
Design:Retrospective cohort study with follow-up ≥1 year following device implantation.
Results:Underlying illnesses included HIV infection in 66 patients (median CD4 count, 24.4 cells/mm3), malignancy in 51, and sickle cell disease in 6. Mean age of patients was 43.7 years, 50% were male, and 74% were black. Thirty-one (25%) of 123 patients developed a primary or device-associated bloodstream infection (BSI), and 3 of the 31 patients experienced two separate episodes of infection. The overall rate of infection was 1.23 primary BSIs per 1,000 device days. Patients with cancer had a lower rate of infection than those with HIV infection, but the difference was not statistically significant (0.96 vs 1.50 BSIs/1,000 device days; relative risk, 0.58; 95% confidence interval, 0.27-1.26). Subgroup analysis of patients with different malignancies indicated that infection rates differed according to type of cancer, and there was a trend for heterogeneity across the different cancer strata (P=.06). Gram-positive pathogens accounted for 60% of the pathogens recovered. Six (19%) of 31 patients who developed an infection did so within the first 14 days after implantation. In 11 (32%) of the 34 BSIs, the port required removal; two patient deaths were attributed to device-associated bacteremias (0.072 deaths/ 1,000 device days).
Conclusions:Approximately one fourth of patients who had a vascular access device implanted developed a primary BSI, but the overall infection rate (per 1,000 device days) was relatively low, even among those with HIV infection. Primary BSI rates in patients with vascular access devices appeared to differ according to the specific underlying illness.

Between August and May-the butter-making season in New Zealand-the average vitamin Dcontent of the butterfat varied between 10 and 64 i.u./100g and during the corresponding months in Great Britain (February-November) the values were between 15 and 39 i.u./100g. At the beginning and end of the season the values in both countries were similarly low but in mid-season New Zealand values were up to twice those for Great Britain. Within each country the vitamin D values were highly correlated with hours of sunshine and, for both countries, with total radiation, which includes the diffuse radiation in ‘skyshine’ and also allows for variation in sunshine intensity. The higher content of vitamin D in New Zealand butter in mid-season was attributed to a higher level of total radiation.
