Background:
There is limited published data from long-term pediatric bipolar clinical trials with which to guide appropriate treatment decisions. Long-term efficacy and safety of aripiprazole was investigated in this patient population.
Methods:296 youths, ages 10-17 year-old with a DSM-IV diagnosis of bipolar I disorder were randomized to receive either placebo or aripiprazole (10mg or 30mg) in a 4-week double-blind trial. Completers continued assigned treatments for an additional 26 weeks (double-blind). Efficacy endpoints included mean change from baseline to week 4 and week 30 on the Young Mania Rating Scale; Children's Global Assessment Scale, Clinical Global Impressions-Bipolar version severity scale, General Behavior Inventory, Attention Deficit Hyperactivity Disorders Rating Scale, and time to discontinuation. Tolerability/safety assessments included incidence and severity of AEs, blood chemistries and metabolic parameters.
Results:Over the 30-week course of double-blind treatment, aripiprazole (10 mg and 30 mg) was superior to placebo as early as week 1 (p< 0.002) and at all scheduled visits from week 2 through week 30 on mean change from baseline in the Y-MRS total score (p<.0001; all visits). Significant improvements were observed on multiple endpoints including the CGAS, GBI, CGI-BP, ADHD-RS-IV total score, time to discontinuation, and response and remission rates. The 3 most common AEs were somnolence, extrapyramidal disorder, and fatigue. Mean change in body weight z-scores over 30 weeks was not clinically significant.
Conclusions:Over 30-weeks of treatment, both doses of aripiprazole were superior to placebo in the long term treatment of pediatric bipolar patients. Aripiprazole was generally well tolerated.

Rubella haemagglutination inhibiting (HAI) antibody titres were determined in 54 seronegative women and 31 naturally immune women after vaccination and revaccination with Wistar RA27/3 strain rubella vaccine administered by the intranasal or subcutaneous routes or revaccination with the Cendehill strain administered subcutaneously. In addition, HAI antibody titres were determined in 46 seronegative schoolgirls after vaccination with the Cendehill strain and revaccination with the RA27/3 strain.
All seronegative women vaccinated with the RA27/3 strain developed antibody, peak titres being reached 6 weeks after vaccination. Six months after vaccination with the Cendehill strain, 45 (98%) of the 46 seronegative girls had developed antibody, but 11 (24%) had not reached their peak titre by 6 weeks, suggesting a slower response than that elicited by the RA27/3 strain. Revaccination did not induce significant antibody responses in the seronegative women vaccinated 6 months previously with RA27/3 but 4 naturally immune women developed an eightfold increase in antibody. In 10 (22%) of 46 schoolgirls previously vaccinated with the Cendehill strain a significant rise in antibody followed revaccination with RA27/3. These results provide further evidence of the more rapid antibody responses elicited by the RA27/3 vaccine in comparison with the Cendehill vaccine.
An outbreak of natural rubella occurred in 1972 and 97 cases were confirmed serologically. The clinical disease was more common in older school-children and in adults. More males than females were affected in the 11–15 age group, the sex ratio being 18:12; this may be explained by the routine vaccination of girls of this age group as part of the national programme which began in 1970. The significance of the persistence of high HAI antibody titres after natural infection and the effect of the epidemic on the serological status of the population are discussed.

the feasibility of percutaneous intramuscular functional electrical stimulation (p-fes) in children with cerebral palsy (cp) for immediate improvement of ankle kinematics during gait has not previously been reported. eight children with cp (six with diplegia, two with hemiplegia; mean age 9 years 1 month [sd 1y 4mo; range 7y 11mo to 11y 10mo]) had percutaneous intramuscular electrodes implanted into the gastrocnemius (ga) and tibialis anterior (ta) muscles of their involved limbs. stimulation was provided during appropriate phases of the gait cycle in three conditions (ga only, ta only, and ga/ta). immediately after a week of practice for each stimulation condition, a gait analysis was performed with and without stimulation. a significant improvement in peak dorsiflexion in swing for the more affected extremity and dorsiflexion at initial contact for the less affected extremity were found in the ga/ta condition. clinically meaningful trends were evident for improvements in dorsiflexion kinematics for the more and less affected extremities in the ta only and ga/ta conditions. the results suggest that p-fes might immediately improve ankle kinematics in children with cp.

This book collects together multidisciplinary chapters on the most important problems that arise at mid-ocean ridges. The mid-ocean ridge system is the longest continuous feature of the earth's surface, at which the great majority of ocean floor is created, and volumes of seafloor volcanism vastly exceed those on land. It provides the means for much of the heat loss from the interior of the earth. Chemicals as well as heat are extracted by flowing seawater through the rocks of the seafloor, leading to spectacular areas of hydrothermal venting, affecting ocean chemistry and global climate. The chapters range from studies of the mantle and magma generation within it, through tectonics of mid-ocean ridges, to the physical, chemical and biological dynamics of hydrothermal systems. The book will be of importance to specialists and researchers wishing to become informed of the latest developments in the science of mid-ocean ridges. It will prove especially useful for new scientists entering the field.

Summary
The mid-ocean ridge system is the longest continuous feature of the Earth's surface and is where all of the ocean floor is created with volumes of seafloor volcanism vastly exceeding that of any type of volcano on land. It provides the means for much of the heat loss from the interior of the Earth, a large part of which is carried by flowing sea water that moves through the rocks of the seafloor. Chemicals as well as heat are extracted by this process leading to spectacular areas of hydrothermal venting on the seabed at the ridge axis with seafloor sulphide deposits and biological communities. The process affects (some say controls) ocean chemistry and global climate.
In recent years, the recognition that a major coordinated effort was needed in order to understand the fundamental dynamical processes operating at mid-ocean ridges led the scientific community to mount a series of major national and international programmes. The major UK component of this effort has been the British Mid-Ocean Ridge Initiative (BRIDGE) which was established as a NERC Community Research Project, and many other countries have active programmes in this area of science. For this reason a Royal Society Discussion Meeting was organized by the editors and this was held in London on 6 and 7 March 1996. Much of the work presented at the Discussion Meeting has been the product of these initiatives. In addition, a poster session held concurrently with the meeting highlighted achievements of the BRIDGE programme.

It would be senseless presumption in me to speak in this place of the interest of any branch of history studied for its own sake; nor have I interpreted the invitation which I have received through your Secretary as implying any wish that I should do so. I have rather understood the term ‘national’ as expressing a wish that I should speak of the lessons of national importance which are to be sought for in the history of our navy, ‘the wall and fence of the kingdom.’
