Exchange: A Signature Pedagogy for American Studies in the UK

What does it mean to teach American studies in UK higher education? We teach “American” content in our classes, modules, and degree programmes, but do we also conduct our teaching in ways specific to American studies? Lee Shulman describes a “signature pedagogy” as “the forms of instruction that leap to mind when we first think about the preparation of members of particular professions,” encompassing both the types of teaching we conduct in our classrooms and the assumptions and values that underpin that practice. Initial conversations about the concept of a signature pedagogy for American studies in the UK were held in a session organized by Lydia Plath on behalf of the Teaching American Studies Network at the Digital BAAS Conference in 2021, in the midst of a global pandemic that required all of us to reflect on our teaching practice. Building on that earlier conversation, this exchange was conducted between February and July 2022. The discussion explores American studies pedagogical approaches through the assumptions, beliefs, and values that underpin our teaching; the challenges of multi- and interdisciplinarity; and questions of identity, inclusion, and national context.
Novel Negative Pressure Procedural Tent Reduces Aerosolized Particles in a Simulated Prehospital Setting

Background/Objective:
The coronavirus disease 2019 (COVID-19) pandemic has challenged the ability of Emergency Medical Services (EMS) providers to maintain personal safety during the treatment and transport of patients potentially infected. Increased rates of COVID-19 infection in EMS providers after patient care exposure, and notably after performing aerosol-generating procedures (AGPs), have been reported. With an already strained workforce seeing rising call volumes and increased risk for AGP-requiring patient presentations, development of novel devices for the protection of EMS providers is of great importance.
Based on the concept of a negative pressure room, the AerosolVE BioDome is designed to encapsulate the patient and contain aerosolized infectious particles produced during AGPs, making the cabin of an EMS vehicle safer for providers. The objective of this study was to determine the efficacy and safety of the tent in mitigating simulated infectious particle spread in varied EMS transport platforms during AGP utilization.
Methods:Fifteen healthy volunteers were enrolled and distributed amongst three EMS vehicles: a ground ambulance, an aeromedical-configured helicopter, and an aeromedical-configured jet. Sodium chloride particles were used to simulate infectious particles and particle counts were obtained in numerous locations close to the tent and around the patient compartment. Counts near the tent were compared to ambient air with and without use of AGPs (non-rebreather mask, continuous positive airway pressure [CPAP] mask, and high-flow nasal cannula [HFNC]).
Results:For all transport platforms, with the tent fan off, the particle generator alone, and with all AGPs produced particle counts inside the tent significantly higher than ambient particle counts (P <.0001). With the tent fan powered on, particle counts near the tent, where EMS providers are expected to be located, showed no significant elevation compared to baseline ambient particle counts during the use of the particle generator alone or with use of any of the AGPs across all transport platforms.
Conclusion:Development of devices to improve safety for EMS providers to allow for use of all available therapies to treat patients while reducing risk of communicable respiratory disease transmission is of paramount importance. The AerosolVE BioDome demonstrated efficacy in creating a negative pressure environment and workspace around the patient and provided significant filtration of simulated respiratory droplets, thus making the confined space of transport vehicles potentially safer for EMS personnel.

Background/Objective:
The coronavirus disease 2019 (COVID-19) pandemic has created challenges in maintaining the safety of prehospital providers caring for patients. Reports have shown increased rates of Emergency Medical Services (EMS) provider infection with COVID-19 after patient care exposure, especially while utilizing aerosol-generating procedures (AGPs). Given the increased risk and rising call volumes for AGP-necessitating complaints, development of novel devices for the protection of EMS clinicians is of great importance.
Drawn from the concept of the powered air purifying respirator (PAPR), the AerosolVE helmet creates a personal negative pressure space to contain aerosolized infectious particles produced by patients, making the cabin of an EMS vehicle safer for providers. The helmet was developed initially for use in hospitals and could be of significant use in the prehospital setting. The objective of this study was to determine the efficacy and safety of the helmet in mitigating simulated infectious particle spread in varied EMS transport platforms during AGP utilization.
Methods:Fifteen healthy volunteers were enrolled and distributed amongst three EMS vehicles: a ground ambulance, a medical helicopter, and a medical jet. Sodium chloride particles were used to simulate infectious particles, and particle counts were obtained in numerous locations close to the helmet and around the patient compartment. Counts near the helmet were compared to ambient air with and without use of AGPs (non-rebreather mask [NRB], continuous positive airway pressure mask [CPAP], and high-flow nasal cannula [HFNC]).
Results:Without the helmet fan on, the particle generator alone and with all AGPs produced particle counts inside the helmet significantly higher than ambient particle counts. With the fan on, there was no significant difference in particle counts around the helmet compared to baseline ambient particle counts. Particle counts at the filter exit averaged less than one despite markedly higher particle counts inside the helmet.
Conclusion:Given the risk to EMS providers by communicable respiratory diseases, development of devices to improve safety while still enabling use of respiratory therapies is of paramount importance. The AerosolVE helmet demonstrated efficacy in creating a negative pressure environment and provided significant filtration of simulated respiratory droplets, thus making the confined space of transport vehicles potentially safer for EMS personnel.
Timing and extent of early marine oxygen isotope stage 2 alpine glaciation in Skagit Valley, Washington

Twenty-two new radiocarbon ages from Skagit valley provide a detailed chronology of alpine glaciation during the Evans Creek stade of the Fraser Glaciation (early marine oxygen isotope stage (MIS) 2) in the Cascade Range, Washington State. Sediments at sites near Concrete, Washington, record two advances of the Baker valley glacier between ca. 30.3 and 19.5 cal ka BP, with an intervening period of glacier recession about 24.9 cal ka BP. The Baker valley glacier dammed lower Skagit valley, creating glacial Lake Concrete, which discharged around the ice dam along Finney Creek, or south into the Sauk valley. Sediments along the shores of Ross Lake in upper Skagit valley accumulated in glacial Lake Skymo after ca. 28.7 cal ka BP behind a glacier flowing out of Big Beaver valley. Horizontally laminated silt and bedded sand and gravel up to 20 m thick record as much as 8000 yr of deposition in these glacially dammed lakes. The data indicate that alpine glaciers in Skagit valley were far less extensive than previously thought. Alpine glaciers remained in advanced positions for much of the Evans Creek stade, which may have ended as early as 20.8 cal ka BP.
Net Returns from Feeding Cull Beef Cows: The Influence of Initial Body Condition Score

The impact of initial body condition scores on net returns from retaining beef cull cows for delayed marketing was investigated in a three-year experiment. Cows were retained either on native grass pasture or in a low-input dry lot setting. Net returns are examined across five alternative marketing periods, including culling. Sensitivity of net returns to changes in retention cost is also examined. Although a native grass pasture system was generally more profitable than a low-input dry lot system, thin and medium cows were typically more profitable than cows with higher initial body condition score regardless of the feeding system.

Objective:
To examine the extent to which the strategies recommended by the National Foundation for Infectious Diseases (NFID)-Centers for Disease Control and Prevention (CDC) co-sponsored workshop, Antimicrobial Resistance in Hospitals: Strategies to Improve Antimicrobial Use and Prevent Nosocomial Transmission of Antimicrobial-Resistant Microorganisms, have been implemented and the relationship between the degree of implementation and hospital culture, leadership, and organizational factors.
Design:Survey.
Setting:A representative sample of U.S. hospitals stratified by teaching status, bed size, and geographic region.
Participants:Infection control professionals.
Results:Surveyed hospitals had implemented strategies to optimize the use of antimicrobials and to detect, report, and prevent transmission of antimicrobial-resistant microorganisms. Multivariate analyses found that hospitals with a greater degree of implementation of the NFID–CDC strategic goals were more likely to have management support, education of staff, and interdisciplinary groups specifically to address these issues; they were also more likely to engage in benchmarking on broader quality of care indicators.
Conclusions:Most surveyed hospitals had implemented some measures to address the NFID–CDC recommendations; however, hospitals need to do much more to improve antimicrobial use and to increase their efforts to detect, report, and control the spread of antimicrobial resistance. A supportive hospital administration must foster a culture of ongoing support, education, and interdisciplinary work groups focused on this important issue to successfully accomplish these goals.

Background:
Antimicrobial resistance is a growing clinical and public health crisis. Experts have recommended measures to monitor antimicrobial resistance; however, little is known regarding their use.
Objective:We describe the use of procedures to detect and report antimicrobial resistance in U.S. hospitals and the organizational and epidemiologic factors associated with their use.
Methods:In 2001, we surveyed laboratory directors (n = 108) from a random national sample of hospitals. We studied five procedures to monitor antimicrobial resistance: (1) disseminating antibiograms to physicians at least annually, (2) notifying physicians of antimicrobial-resistant infections, (3) reporting susceptibility results within 24 hours, (4) using automated testing procedures, and (5) offering molecular typing. Explanatory variables included organizational characteristics and patterns of antimicrobial resistance for oxacillin-resistant Staphylococcus aureus, vancomycin-resistant enterococci, quinolone-resistant Escherichia coli, and extended-spectrum beta-lactamase-producing Klebsiella species. Generalized estimating equations accounting for the correlation among outcomes at the facility level were used to identify predictors of the five outcomes.
Results:Use of the procedures ranged from 85% (automated testing) to 33% (offering molecular typing) and was related to teaching hospital status (OR, 3.1; CI95, 1.5–6.5), participation of laboratory directors on the infection control committee (OR, 1.7; CI95, 1.1–2.8), and having at least one antimicrobial-resistant pathogen with a prevalence greater than 10% (OR, 2.2; CI95, 1.4–3.3).
Conclusion:U.S. hospitals underutilize procedures to monitor the spread of antimicrobial resistance. Use of these procedures varies and is related to organizational and epidemiologic factors. Further efforts are needed to increase their use by hospitals.
Carers' and Users' Expectations of Services – User version (CUES–U): A new instrument to measure the experience of users of mental health services

Background
No existing instrument measures all or even most of the issues considered important by users of mental health services.
AimsTo develop and test a self-assessment instrument to enable users of mental health services to rate their experience across the range of domains that they consider to be important.
MethodRelevant domains were identified and a new instrument was drafted and field tested to examine its psychometric properties.
ResultsThe 17-item, self-rated Carers' and Users' Expectations of Services – User version (CUES–U) appears acceptable to most service users. Its items have reasonable test–retest reliability and a ‘total CUES–U score’ correlates significantly with a total score of the Health of the Nations Outcome Scales (Spearman's ρ=0.42; P<0.01).
ConclusionsThe development and testing of CUES–U suggest that it might be feasible to apply a self-rated measure of the expectations and experience of users of mental health services.
