Implementation of SARS-CoV-2 Monoclonal Antibody Infusion Sites at Three Medical Centers in the United States: Strengths and Challenges Assessment to Inform COVID-19 Pandemic and Future Public Health Emergency Use

Monoclonal antibody therapeutics to treat coronavirus disease (COVID-19) have been authorized by the US Food and Drug Administration under Emergency Use Authorization (EUA). Many barriers exist when deploying a novel therapeutic during an ongoing pandemic, and it is critical to assess the needs of incorporating monoclonal antibody infusions into pandemic response activities. We examined the monoclonal antibody infusion site process during the COVID-19 pandemic and conducted a descriptive analysis using data from 3 sites at medical centers in the United States supported by the National Disaster Medical System. Monoclonal antibody implementation success factors included engagement with local medical providers, therapy batch preparation, placing the infusion center in proximity to emergency services, and creating procedures resilient to EUA changes. Infusion process challenges included confirming patient severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) positivity, strained staff, scheduling, and pharmacy coordination. Infusion sites are effective when integrated into pre-existing pandemic response ecosystems and can be implemented with limited staff and physical resources.
Linking genes, circuits, and behavior: network connectivity as a novel endophenotype of externalizing

Background
Externalizing disorders are known to be partly heritable, but the biological pathways linking genetic risk to the manifestation of these costly behaviors remain under investigation. This study sought to identify neural phenotypes associated with genomic vulnerability for externalizing disorders.
MethodsOne-hundred fifty-five White, non-Hispanic veterans were genotyped using a genome-wide array and underwent resting-state functional magnetic resonance imaging. Genetic susceptibility was assessed using an independently developed polygenic score (PS) for externalizing, and functional neural networks were identified using graph theory based network analysis. Tasks of inhibitory control and psychiatric diagnosis (alcohol/substance use disorders) were used to measure externalizing phenotypes.
ResultsA polygenic externalizing disorder score (PS) predicted connectivity in a brain circuit (10 nodes, nine links) centered on left amygdala that included several cortical [bilateral inferior frontal gyrus (IFG) pars triangularis, left rostral anterior cingulate cortex (rACC)] and subcortical (bilateral amygdala, hippocampus, and striatum) regions. Directional analyses revealed that bilateral amygdala influenced left prefrontal cortex (IFG) in participants scoring higher on the externalizing PS, whereas the opposite direction of influence was observed for those scoring lower on the PS. Polygenic variation was also associated with higher Participation Coefficient for bilateral amygdala and left rACC, suggesting that genes related to externalizing modulated the extent to which these nodes functioned as communication hubs.
ConclusionsFindings suggest that externalizing polygenic risk is associated with disrupted connectivity in a neural network implicated in emotion regulation, impulse control, and reinforcement learning. Results provide evidence that this network represents a genetically associated neurobiological vulnerability for externalizing disorders.

The vapor deposition methods of planar magnetron sputtering and electron-beam evaporation are used to synthesize materials with nanostructured morphological features that have ultra-high surface areas with continuous open porosity at the nanoscale. These nanostructured membranes are used in a variety of fuel cells to provide electrode and catalytic functions. Specifically, stand alone and composite nickel electrodes for use in thin film solid-oxide, and molten carbonate fuel cells are formed by sputter deposition and electron beam evaporation, respectively. Also, a potentially high-performance catalyst material for the direct reformation of hydrocarbon fuels at low temperatures is deposited as a nanostructure by the reactive sputtering of a copper-zinc alloy using a partial pressure of oxygen at an elevated substrate temperature.

Fuel cells have gained renewed interest for applications in portable power since the energy is stored in a separate reservoir of fuel rather than as an integral part of the power source, as is the case with batteries. While miniaturized fuel cells have been demonstrated for the low power regime (1-20 Watts), numerous issues still must be resolved prior to deployment for applications as a replacement for batteries. As traditional fuel cell designs are scaled down in both power output and physical footprint, several issues impact the operation, efficiency, and overall performance of the fuel cell system. These issues include fuel storage, fuel delivery, system startup, peak power requirements, cell stacking, and thermal management. The combination of thin-film deposition and micro-machining materials offers potential advantages with respect to stack size and weight, flow field and manifold structures, fuel storage, and thermal management. The micro-fabrication technologies that enable material and fuel flexibility through a modular fuel cell platform will be described along with experimental results from both solid oxide and proton exchange membrane, thin-film fuel cells.

Thin film, solid-oxide fuel cells (TFSOFCs) synthesized from an electrolyte and conductive material are developed using photolithographic patterning and physical vapor deposition. The anode layer must enable combination of the reactive gases, be conductive to pass the electric current, and provide mechanical support to the electrolyte and cathode layers. The microstructure and morphology desired for the anode layer should facilitate generation of maximum current density from the fuel cell. For these purposes, the parameters of the deposition process and post-deposition patterning are developed to optimize a continuous porosity across the anode layer. The anode microstructure is characterized using scanning electron microscopy and the power ouput generated through current-voltage measurement.

Deposition into micron-sized holes is known to produce cone shapes as supported on substrates. Potential uses for the cones include field-forming devices as field ionizers and field emission cathodes. The application of such devices include flat panel displays and flash x-ray tubes. Process iterations to closely space arrays of sharp cones have been extensively documented during the past two decades using the physical vapor deposition method of evaporation. Sputter deposition is well known as a method to fill holes and trenches but has only recently been demonstrated as an alternative method to produce field emission cathodes. In a further reduction in size, we have been successful in demonstrating the ability to deposit a cone shape into a cavity with a 300nm diameter hole. Through comparison to the results of electron-beam evaporative deposition, a sputter deposited nanocone appears to be suitable for use as a field emission cathode.
Interface Controlled Amorphization of Crystalline Ni/Ti Multilayers

Solid-State Amorphization (SSA) of crystalline interfaces is observed in the Ni/Ti multilayer system. The amorphization reaction nucleates at location(s) of crystallographic disorder, i.e. the multilayer interfaces. Microstructural analyses reveal the sputter-deposited growth structure to be epitaxial with semi-coherent interfaces. Strain energy originating from interface lattice distortions varies as a function of the multilayer repeat spacing. Therefore, the interfacial energy effects the onset conditions for SSA. Differential thermal analysis is used to measure the critical temperature Tc, to the nucleation of the SSA, which is found to vary with the Ni/Ti multilayer pair spacing.
