We read with interest the recent editorial, “The Hennepin Ketamine Study,” by Dr. Samuel Stratton commenting on the research ethics, methodology, and the current public controversy surrounding this study.1 As researchers and investigators of this study, we strongly agree that prospective clinical research in the prehospital environment is necessary to advance the science of Emergency Medical Services (EMS) and emergency medicine. We also agree that accomplishing this is challenging as the prehospital environment often encounters patient populations who cannot provide meaningful informed consent due to their emergent conditions. To ensure that fellow emergency medicine researchers understand the facts of our work so they may plan future studies, and to address some of the questions and concerns in Dr. Stratton’s editorial, the lay press, and in social media,2 we would like to call attention to some inaccuracies in Dr. Stratton’s editorial, and to the lay media stories on which it appears to be based.

Study Objective
This study aimed to review available disaster training options for health care providers, and to provide specific recommendations for developing and delivering a disaster-response-training program for non-disaster-trained emergency physicians, residents, and trainees prior to acute deployment.
MethodsA comprehensive review of the peer-reviewed and grey literature of the existing training options for health care providers was conducted to provide specific recommendations.
ResultsA comprehensive search of the Pubmed, Embase, Web of Science, Scopus, and Cochrane databases was performed to identify publications related to courses for disaster preparedness and response training for health care professionals. This search revealed 7,681 unique titles, of which 53 articles were included in the full review. A total of 384 courses were found through the grey literature search, and many of these were available online for no charge and could be completed in less than six hours. The majority of courses focused on management and disaster planning; few focused on clinical care and acute response.
ConclusionThere is need for a course that is targeted toward emergency physicians and trainees without formal disaster training. This course should be available online and should utilize a mix of educational modalities, including lectures, scenarios, and virtual simulations. An ideal course should focus on disaster preparedness, and the clinical and non-clinical aspects of response, with a focus on an all-hazards approach, including both terrorism-related and environmental disasters.

Depolymerizable polymers are stimuli-responsive materials triggered to depolymerize rapidly and completely into their constituent monomers on command. Applications include triggerable vehicles for controlled release, restructurable materials, disappearing or sacrificial composites, and lithographic resists. Owing to their widespread utility, significant efforts have aimed to prepare and explore depolymerizable polymers and their corresponding triggers. This “Prospective” highlights advances since their discovery over a half-century ago, discusses methods in their preparation, and presents recent developments in triggered depolymerization. It also surveys applications that harness these polymers’ unique properties, while offering insights into research directions that may contribute to progress in this dynamic field.

Objective
Working within a series of partnerships among an academic health center, local health departments (LHDs), and faith-based organizations (FBOs), we validated companion interventions to address community mental health planning and response challenges in public health emergency preparedness.
MethodsWe implemented the project within the framework of an enhanced logic model and employed a multi-cohort, pre-test/post-test design to assess the outcomes of 1-day workshops in psychological first aid (PFA) and guided preparedness planning (GPP). The workshops were delivered to urban and rural communities in eastern and midwestern regions of the United States. Intervention effectiveness was based on changes in relevant knowledge, skills, and attitudes (KSAs) and on several behavioral indexes.
ResultsSignificant improvements were observed in self-reported and objectively measured KSAs across all cohorts. Additionally, GPP teams proved capable of producing quality drafts of basic community disaster plans in 1 day, and PFA trainees confirmed upon follow-up that their training proved useful in real-world trauma contexts. We documented examples of policy and practice changes at the levels of local and state health departments.
ConclusionsGiven appropriate guidance, LHDs and FBOs can implement an effective and potentially scalable model for promoting disaster mental health preparedness and community resilience, with implications for positive translational impact.(Disaster Med Public Health Preparedness. 2014;8:511-526)

Self-healing polymers have experienced rapid technological advancement over the past seven years. They have moved from a conceptual demonstration to practical application in this time frame and have grown from a single design to a generic paradigm for modern materials development. Potential applications of self-healing polymers are quite broad, including microelectronic substrates and encapsulants, polymeric paints and coatings, structural composites, and biomedical devices. In this article, we focus on polymeric systems that heal in an autonomic fashion, that is, automatically and without human intervention. The types of systems under development and the future of this paradigm in advanced materials are discussed.

Mechanical circulatory support is an invaluable tool in the care of children with severe refractory cardiac and or pulmonary failure. Two forms of mechanical circulatory support are currently available to neonates, infants, and smaller children, namely extracorporeal membrane oxygenation and use of a ventricular assist device, with each technique having unique advantages and disadvantages. The intra-aortic balloon pump is a third form of mechanical support that has been successfully used in larger children, adolescents, and adults, but has limited applicability in smaller children. In this review, we discuss the current experiences with extracorporeal membrane oxygenation and ventricular assist devices in children with cardiac disease.
A variety of forms of mechanical circulatory support are available for children with cardiopulmonary dysfunction refractory to conventional management. These devices require extensive resources, both human and economic. Extracorporeal membrane oxygenation can be effectively used in a variety of settings to provide support to critically-ill patients with cardiac disease. Careful selection of patients and timing of intervention remains challenging. Special consideration should be given to children with cardiac disease with regard to anatomy, physiology, cannulation, and circuit management. Even though exciting progress is being made in the development of ventricular assist devices for long-term mechanical support in children, extracorporeal membrane oxygenation remains the mainstay of mechanical circulatory support in children with complex anatomy, particularly those needing rapid resuscitation and those with a functionally univentricular circulation.
As the familiarity and experience with extracorporeal membrane oxygenation has grown, new indications have evolved, including emergent resuscitation. This utilization has been termed extracorporeal cardiopulmonary resuscitation. The literature supporting emergent cardiopulmonary support is mounting. Reasonable survival rates have been achieved after initiation of support during active compressions of the chest following in-hospital cardiac arrest. Due to the limitations of conventional circuits for extracorporeal membrane oxygenation, some centres have developed novel systems for rapid cardiopulmonary support.
Many centres previously considered a functionally univentricular circulation to be a contraindication to extracorporeal membrane oxygenation, but improved results have been achieved recently with this complex subset of patients. The registry of the Extracorporeal Life Support Organization recently reported the outcome of extracorporeal life support used in neonates for cardiac indications from 1996 to 2000. Of the 740 neonates who were placed on extracorporeal life support for cardiac indications, 118 had hypoplastic left heart syndrome. There was no significant difference in survival between these patients and those with other defects. It is now common to use extracorporeal membrane oxygenation to support patients with a functionally univentricular circulation, and reasonable survival rates are to be expected.
Although extracorporeal membrane oxygenation has become a standard of care for many paediatric centres, its use is limited to those patients who require only short-term cardiopulmonary support. Mechanical ventricular assist devices have become standard therapy for adults with cardiac failure refractory to maximal medical management. Several devices are readily available in the United States of America for adults, but there are fewer options available to children. Over the last few years, substantial progress has been made in paediatric mechanical support. Ventricular assist devices are being used with increasing frequency in children with cardiac failure refractory to medical therapy for primary treatment as a long-term bridge to recovery or transplantation. The paracorporeal, pneumatic, pulsatile “Berlin Heart” ventricular assist device is being used with increasing frequency in Europe and North America to provide univentricular and biventricular support. With this device, a patient can be maintained on mechanical circulatory support while extubated, being mobilized, and feeding by mouth.
Mechanical circulatory support should be anticipated, and every attempt must be made to initiate support “urgently” rather than “emergently”, before the presence of dysfunction of end organs or circulatory collapse. In an emergency, these patients can be resuscitated with extracorporeal membrane oxygenation and subsequently transitioned to a long-term ventricular assist device after a period of stability.
Resource Utilization Among Patients With Sepsis Syndrome

Objective:
To assess the resource utilization associated with sepsis syndrome in academic medical centers.
Design:Prospective cohort study.
Setting:Eight academic, tertiary-care centers.
Patients:Stratified random sample of 1,028 adult admissions with sepsis syndrome and all 248,761 other adult admissions between January 1993 and April 1994. The main outcome measures were length of stay (LOS) in total and after onset of sepsis syndrome (post-onset LOS) and total hospital charges.
Results:The mean LOS for patients with sepsis was 27.7 ± 0.9 days (median, 20 days), with sepsis onset occurring after a mean of 8.1 ± 0.4 days (median, 3 days). For all patients without sepsis, the LOS was 7.2 ± 0.03 days (median, 4 days). In multiple linear regression models, the mean for patients with sepsis syndrome was 18.2 days, which was 11.0 days longer than the mean for all other patients (P < .0001), whereas the mean difference in total charges was $43,000 (both P < .0001). These differences were greater for patients with nosocomial as compared with community-acquired sepsis, although the groups were similar after adjusting for pre-onset LOS. Eight independent correlates of increased post-onset LOS and 12 correlates of total charges were identified.
Conclusions:These data quantify the resource utilization associated with sepsis syndrome, and demonstrate that resource utilization is high in this group. Additional investigation is required to determine how much of the excess post-onset LOS and charges are attributable to sepsis syndrome rather than the underlying medical conditions.
Fracture and Fatigue Behavior of a Self-Healing Polymer Composite

Inspired by biological systems, in which damage triggers an autonomous healing response, a polymer composite material that can heal itself when cracked has been developed. The material consists of an epoxy matrix composite, which utilizes embedded microcapsules to store a healing agent and an embedded catalyst. This paper investigates issues of fracture and fatigue consequential to the development and optimization of this new class of materials. When damage occurs, the propagating crack ruptures the microcapsules, which releases healing agent into the crack plane. Polymerization of the healing agent is triggered by contact with exposed catalyst, which bonds the crack faces closed. The efficiency of crack healing is defined as the ability of a healed sample to recover fracture toughness. Healing efficiencies of over 90% have been achieved. Embedded microcapsules significantly increase the fracture toughness and reduce the fatigue crack propagation rate of epoxy. Fracture mechanisms for neat epoxy and epoxy with embedded microcapsules are presented.

The design of inorganic and organic solids with novel structures and properties has long been the object of materials research. The classical examples of porous materials (i.e., involving reversible guest passage) are zeolites. First discovered in the mid-1700s, zeolites are hydrated, crystalline aluminosilicates that organize into stable, discrete frameworks. Basic structures employ tetrahedral atoms (silicon or aluminum) bridged by oxygen atoms, where each oxygen is shared between two metalloid tetrahedra. Resulting covalent lattices can be neutral or negatively charged (as a result of bridging oxides) and often employ alkali metal or alkalineearth counterions. As a consequence of this ordered structure, zeolites both benefit from and are limited by their highly geometrical nature: their rigid structures are inherently robust, yet they are difficult to process. In addition, while natural and unnatural zeolites have been characterized, harsh synthetic conditions are common to both and lead to limitations in design and processability. Even so, these impediments have not prevented constructing a myriad of architectures on zeolite host lattices. Strengths and weaknesses aside, zeolites demonstrate a major objective of materials chemistry: the ability to manifest macroscopic physical properties based on embedded microscopic structure.

This issue of MRS Bulletin is devoted to the subject of supramolecular materials. The term supramolecular is widely used to describe the intentional use of noncovalent interactions to bring about a desired arrangement of molecules. As the articles in this issue illustrate, this type of molecular engineering can provide structural control on the nanoscale and beyond, broadly impacting the properties of the resulting materials. The goal is to create a collection of molecules in which the whole possesses characteristics that are different and unattainable from the individual components.

Phenylacetylene (PA) dendrimer labeled with perylene (See Fig 1) is discovered to exhibit temperature-dependent emission spectra in certain organic solvents over the temperature range of 20-65°C. The monomer signal is increasing rapidly when temperature increases, while the excimer signal decreases slowly. Models of excimer formation and weakly associated pairs (M+M) dissociation dynamics are included, and the equilibrium constants at different temperatures are calculated. This behavior suggests potential applications in fluorescence-based thermometry.
