Summary
An 11-month-old girl was brought into the A&E. She had been left in the bath with her older sister for what the parents reported was approximately 10 minutes. When their father returned to the bathroom, he found the baby submerged, blue and making no respiratory effort.

Summary
When children become critically ill or suffer trauma, they are usually brought to their local hospital, where local teams are responsible for assessment, initial management and stabilisation of the sick child. Tracheal intubation is carried out, in the majority of cases, by local teams, rarely comprising paediatric specialist anaesthetists.

Summary
A previously fit and well 13-year-old girl was referred to the paediatric retrieval service for transfer and request for admission to PICU. She had been found collapsed by police approximately 4 hours after she had messaged a friend saying she was suicidal. Her friend raised the alarm to her parents and subsequently to the police. The friend was able to identify her location by her phone for the police. The child had taken a deliberate mixed overdose of her father’s antihypertensive medication. She has no history of mental health problems and the current situation was felt to be due to school stressors (potential bullying and academic pressure).

Summary
Critical thinking and problem-solving, clinical reasoning, self-reflection and self-awareness are valued attributes of the contemporary nurse. These skills are essential for the provision of safe and competent person-centred care to clients with ever-increasing acuity and multiple, often complex comorbidities. This chapter focuses on critical thinking, clinical reasoning and reflective practice, and personal documentation using e-portfolios, along with strategies to assist beginning nurses in the development of these specific skills, which should be honed, practised and adapted to everyday clinical practice. The chapter also assists the professional nurse to develop methods to demonstrate their personal and professional development through the use of e-portfolios.

Toxoplasma gondii, a zoonotic parasite of global importance, infects all endothermic vertebrates, with extensive health implications. The prevalence of this parasite is seldom monitored in wildlife. Here, a semi-aquatic species, the Eurasian otter (Lutra lutra) was used as a model to assess the potential effect of climate, land cover and biotic factors on T. gondii seroprevalence in British wildlife. The Sabin–Feldman cytoplasm-modifying dye test identified T. gondii antibodies in 25·5% of blood samples from otters found dead, mainly as road kill, in England and Wales, between 2004 and 2010. Otters in the east of England were more likely to be infected with T. gondii than those in western regions. Land cover and temperature are key determinants of T. gondii infection risk, with more infection in arable areas and lower infection where temperatures are higher. The probability of T. gondii infection increased with host age, reflecting cumulative exposure with time, but there was no association between T. gondii seroprevalence and cause of host death.

Background.
Despite an increasing understanding of the importance of near-patient surfaces in the transmission of healthcare-associated pathogens, there remains a need to define the relative clinical effectiveness of disinfection interventions
Design.A serial 2-phase evaluation of the clinical effectiveness of 2 surface disinfectants.
Setting.A general acute care hospital.
Methods.A unique system for quantifying bioburden reduction while monitoring the possible impact of differences in cleaning thoroughness was used to compare the clinical effectiveness of a traditional quaternary ammonium compound (QAC) and a novel peracetic acid/hydrogen peroxide disinfectant (ND) as part of terminal room cleaning.
Results.As a result of QAC cleaning, 93 (40%) of 237 cleaned surfaces confirmed by fluorescent marker (DAZO) removal were found to have complete removal of aerobic bioburden. During the ND phase of the study, bioburden was removed from 211 (77%) of 274 cleaned surfaces. Because there was no difference in the thoroughness of cleaning with either disinfectant (65.3% and 66.4%), the significant (P > .0001) difference in bioburden reduction can be attributed to better cleaning efficacy with the ND.
Conclusions.In the context of the study design, the ND was 1.93 times more effective in removing bacterial burden than the QAC (P > .0001). Furthermore, the study design represents a new research paradigm in which 2 interventions can be compared by concomitantly and objectively analyzing both the product and process variables in a manner that can be used to define the relative effectiveness of all disinfection cleaning interventions.
