Objective:
Cross-national neuropsychological research is needed to understand the social, economic, and cultural factors associated with cognitive risk and resilience across global aging populations. Memory and language have been shown to be sensitive to age-related cognitive decline and pathological cognitive aging processes and may be more sensitive to subtle cognitive decline than measures of global cognitive function. Thus, we aimed to derive and validate harmonized cognitive domain scores for memory and language across population-based studies in the US and Mexico.
Participants and Methods:Data came from the Health and Retirement Study (HRS) Harmonized Cognitive Assessment Protocol (HCAP) and the Mexican Health and Aging Study (MHAS) Ancillary Study on Cognitive Aging (Mex-Cog). We used confirmatory factor analysis methodology to create statistically co-calibrated cognitive domains of memory and language. We performed differential item functioning (DIF) analysis to evaluate measurement differences across studies, using a cultural neuropsychological approach to identify comparable items across studies (i.e., cross-study anchors). We evaluated harmonized scores by examining their relationship to age and education in each study.
Results:We included 3347 participants from the HRS-HCAP study [Mage=76.6(7.5), 60% female] and 2042 participants from the Mex-Cog study [Mage=68.1(9.0), 59% female]. Education was classified according to the International Standard Classification of Education in the following categories (HRS-HCAP and Mex-Cog, respectively): none or early childhood education: (0.7%; 50.5%), primary education (4.1%; 22.3%), lower secondary education (7.1%; 15.7%), upper secondary education (41.1%; 3.0%), and any college (47.1%; 8.5%). DIF analyses revealed that 5 out of the 7 memory items and 1 out of the 12 language items demonstrated statistical evidence of measurement differences across studies, meaning that these items measured each underlying cognitive construct differently across studies. After adjusting for DIF by not allowing the items with DIF to be cross-study anchors, harmonized memory and language scores showed generally the expected associations with age and education in each study. Increasing age was associated with lower memory (r=-0.40 in HRS-HCAP; r=-0.44 in Mex-Cog) and language (r=-0.31 in HRS-HCAP and r=-0.67 in Mex-Cog) scores. Increasing years of education was associated with better memory and language scores, with mean scores ranging from z=-0.86 and z=-0.29 among those with a primary education or lower to z=0.33 and z=0.90 among those with any college, for HRS-HCAP and Mex-Cog, respectively.
Conclusions:A cultural neuropsychology approach to statistical harmonization facilitates the generation of harmonized measures of cognitive functioning in cross-national studies. Future work can utilize these harmonized cognitive scores to investigate determinants of late-life cognitive decline and dementia in the US and Mexico.

This document is part of the “SHEA Neonatal Intensive Care Unit (NICU) White Paper Series.” It is intended to provide practical, expert opinion, and/or evidence-based answers to frequently asked questions about CLABSI detection and prevention in the NICU. This document serves as a companion to the CDC Healthcare Infection Control Practices Advisory Committee (HICPAC) Guideline for Prevention of Infections in Neonatal Intensive Care Unit Patients. Central line-associated bloodstream infections (CLABSIs) are among the most frequent invasive infections among infants in the NICU and contribute to substantial morbidity and mortality. Infants who survive CLABSIs have prolonged hospitalization resulting in increased healthcare costs and suffer greater comorbidities including worse neurodevelopmental and growth outcomes. A bundled approach to central line care practices in the NICU has reduced CLABSI rates, but challenges remain. This document was authored by pediatric infectious diseases specialists, neonatologists, advanced practice nurse practitioners, infection preventionists, members of the HICPAC guideline-writing panel, and members of the SHEA Pediatric Leadership Council. For the selected topic areas, the authors provide practical approaches in question-and-answer format, with answers based on consensus expert opinion within the context of the literature search conducted for the companion HICPAC document and supplemented by other published information retrieved by the authors. Two documents in the series precede this one: “Practical approaches to Clostridioides difficile prevention” published in August 2018 and “Practical approaches to Staphylococcus aureus prevention,” published in September 2020.

Objectives: Caregivers of youth with heavy prenatal alcohol exposure report impaired communication, which can significantly impact quality of life. Using data collected as part of the Collaborative Initiative on Fetal Alcohol Spectrum Disorders (CIFASD), we examined whether cognitive variables predict communication ability of youth with histories of heavy prenatal alcohol exposure. Methods: Subjects (ages 10–16 years) comprised two groups: adolescents with heavy prenatal alcohol exposure (AE) and non-exposed controls (CON). Selected measures of executive function (NEPSY, Delis-Kaplan Executive Function System), working memory (CANTAB), and language were tested in the child, while parents completed communication ratings (Vineland Adaptive Behavior Scales – Second Edition). Separate multiple regression analyses determined which cognitive domains predicted communication ability. A final, global model of communication comprised the three cognitive models. Results: Spatial Working Memory and Inhibition significantly contributed to communication ability across groups. Twenty Questions performance related to communication ability in the CON group only while Word Generation performance related to communication ability in the AE group only. Effects remained significant in the global model, with the exception of Spatial Working Memory. Conclusions: Both groups displayed a relation between communication and Spatial Working Memory and Inhibition. Stronger communication ability related to stronger verbal fluency in the AE group and Twenty Questions performance in the CON group. These findings suggest that alcohol-exposed adolescents may rely more heavily on learned verbal storage or fluency for daily communication while non-exposed adolescents may rely more heavily on abstract thinking and verbal efficiency. Interventions aimed at aspects of executive function may be most effective at improving communication ability of these individuals. (JINS, 2018, 24, 1026–1037)

A robust collection of mammal, bird, fish, and shellfish remains from an 8,000-year residential sequence at Morro Bay, a small, isolated estuary on the central California coast, shows a strong focus on marine species during the Middle-Late Transition cultural phase (950–700 cal B.P.), which largely coincides with the Medieval Climatic Anomaly (MCA). Previous studies have provided modest evidence for increased fishing and rabbit hunting during the MCA in adjacent regions, but the Morro Bay findings suggest a distinctive marine-focused subsistence refugium during the period of drought. Specifically, the sequence shows striking all-time peaks in marine and estuarine birds, fish NISP/m3, and fish/deer + rabbits during the MCA. Heavy exploitation of fish, aquatic birds, rabbits, and shellfish suggests that the bow and arrow, which seems to have arrived in the area at this time, had little impact on local subsistence strategies.

Visual stress (VS) affects reading in 5–12% of the general population and 31–36% of children with reading disorders. Symptoms include print distortions and visual discomfort when reading, and are exacerbated by fluorescent lighting. Prior research has indicated that VS can also affect proficient readers. We therefore examined levels of visual discomfort in a group of expert readers (n = 24) under both standard and spectrally-filtered fluorescent lighting. Participants rated their awareness of six symptoms of VS under each lighting condition. Under the standard condition, 4(16.7%) of the group recorded moderate to high levels of VS. Differences in symptom levels and reading speed between conditions were analysed using the Wilcoxon Signed Rank Test. Under the filter condition, the group reported less discomfort regarding all six symptoms of VS surveyed. The differences were significant with respect to three of the symptoms (p = .029 - p < .001), with a medium effect size in all of them (r = .31 - r = .46) and total score (p = .007; r = .39). Variations in reading proficiency included significantly fewer self-corrections (p = .019) and total errors (p = .004). Here we present evidence that VS-type symptoms of reading discomfort are not confined to populations with reading difficulties and may also occur in proficient readers, and that simple adaptations to fluorescent lighting may alleviate such symptoms.

Background:
Persons with multiple sclerosis (MS) represent a small segment of the population, but given the progression of the disease, they experience substantial physical, psychosocial and economic burdens.
Objective:The primary aim was to compare demographic characteristics, health status, health behaviours, health care resource utilization and access to health care of the community dwelling populations with and without MS.
Methods:Cross-sectional survey using data from the Canadian Community Health Survey (CCHS 1.1). Adjusted analyses were performed to assess differences between persons with MS and the general population, after controlling for age and sex. Normalized sampling weights and bootstrap variance estimates were used.
Results:Respondents with MS were 7.6 times (95% CI: 5.4, 10.7) more likely to have health-related quality of life scores that reflected severe impairment than respondents without MS. Respondents with MS were 12.2 times (95% CI: 8.6, 17.2) to rate their health as ‘poor’ or ‘fair’ than the general population. Urinary incontinence and chronic fatigue syndrome were 18.7 times (95% CI: 12.5, 28.2) and 21.9 times (95% CI: 11.9, 40.3), more likely to be reported by respondents with MS than those without. Differences between the two populations also existed in terms of health care resource utilization and access and health behaviours.
Conclusion:Large discrepancies in health status and health care utilization existed between persons with MS who reside in the community and the general population according to all indicators used. Health care needs of persons with MS were also not met.

Prenatal alcohol exposure and attention-deficit/hyperactivity disorder (ADHD) result in behavioral issues related to poor executive function (EF). This overlap may hinder clinical identification of alcohol-exposed children. This study examined the relation between parent and neuropsychological measures of EF and whether parent ratings aid in differential diagnosis. Neuropsychological measures of EF, including the Delis-Kaplan Executive Function System (D-KEFS), were administered to four groups of children (8–16 years): alcohol-exposed with ADHD (AE+, n=80), alcohol-exposed without ADHD (AE−, n=36), non-exposed with ADHD (ADHD, n=93), and controls (CON, n=167). Primary caregivers completed the Behavior Rating Inventory of Executive Function (BRIEF). For parent ratings, multivariate analyses of variance revealed main effects of Exposure and ADHD and an interaction between these factors, with significant differences between all groups on nearly all BRIEF scales. For neuropsychological measures, results indicated main effects of Exposure and ADHD, but no interaction. Discriminant function analysis indicated the BRIEF accurately classifies groups. These findings confirm compounded behavioral, but not neuropsychological, effects in the AE+ group over the other clinical groups. Parent-report was not correlated with neuropsychological performance in the clinical groups and may provide unique information about neurobehavior. Parent-report measures are clinically useful in predicting alcohol exposure regardless of ADHD. Results contribute to a neurobehavioral profile of prenatal alcohol exposure. (JINS, 2014, 20, 1–13)

Introduction
In Chapter 5, we discussed the normal responses to a variety of noxious stimuli and their modulation by peripheral and central neural mechanisms. This review showed that noxious stimuli preferentially and most commonly activate a set of interconnected structures, namely the insula and secondary (SII) somatosensory cortices, anterior cingulate gyrus and thalamus. Several additional structures are also activated during normal acute pain although somewhat less frequently: the primary (SI) somatosensory cortex, components of the striatum, the cerebellum, premotor cortex, dorsolateral and orbitofrontal regions of the prefrontal cortex, and the medial midbrain in the region of the periaqueductal gray matter.
In this chapter we review the evidence that chronically painful conditions, whether of peripheral or central origin, may alter the nociceptive processing that normally follows the application of noxious or innocuous stimuli (see Chapter 7). In clinical practice and in the interpretation of the results of pain research, the assumption is often made that the perceptual abnormalities sometimes associated with chronic pain states are attributable only to changes occurring at the peripheral or spinal level. Although this assumption may be correct in most instances, functional imaging studies provide evidence to the contrary in some cases. We cannot assume that, in pathological or chronically painful conditions, information ascending through the spinothalamic tract will be processed by the same mechanisms used for acute pain; this has important clinical implications for the management of chronic pain.

Neuropathic pain is pain following a disease or injury to the nervous system, and can be categorized by the location of the causative injury. Chronic pain following injury of the peripheral nervous system, distal to the oligodendroglial cell – Schwann cell junction, can be termed deafferentation pain or peripheral neuropathic pain. Chronic pain “associated with lesions of the CNS” is termed central pain syndrome (Merskey, 1986; Bonica, 1991). There are many situations in which there is injury of both the peripheral and central nervous system, particularly with injuries of the conus medullaris. In this chapter we will consider primate neuropathic pain states, beginning with peripheral neuropathic or deafferentation syndromes, and concluding with central pain syndromes.
In general terms, both central and peripheral chronic pain syndromes have similar characteristics. These include evidence of sensory loss, ongoing pain and pain evoked by stimuli that are not normally painful (allodynia or hyperalgesia). The sensory loss and hypersensitivity are demonstrated by quantitative sensory testing (QST). In addition, a number of primate models have been developed which mimic the sensory abnormalities in patients with neuropathic pain.
Clinical characteristics of peripheral neuropathic pain
The cause of most neuropathies is based on the medical history, supported by laboratory investigations (Casey et al., 1996b). Diabetes is the most common cause of painful neuropathy. Generally, a progressive course suggests an inherited, metabolic or recurrent toxic etiology.

The nature of nociceptors
Nociceptors are sensory receptors that respond to stimuli that are damaging or potentially damaging to tissues (Sherrington,1906). The thresholds for activation of many nociceptors can be reached when stimuli of only moderate or non-damaging intensities are applied, but responses continue to increase as stimulus intensity is progressively increased to a level that produces overt damage. By contrast, other nociceptors respond only to intense stimuli and some may not respond at all, even to the strongest mechanical stimuli, unless they are first sensitized (Lynn and Carpenter, 1982; Meyer et al., 1991; Kress et al., 1992; Davis et al., 1993; Treede et al., 1998). The last mentioned have been called “silent nociceptors” (Schaible and Schmidt, 1985, 1988a, 1988b; Schmidt et al., 1995, 2000). Overall, if we include receptors responding to innocuous warming and cooling of the skin, there may be as many as six receptor classes specific for cooling, warming, noxious heat or cold, destructive mechanical or mixed noxious stimuli in humans and other animals.
Types of nociceptors
Nociceptors can be subdivided according to the tissue in which they are found, the size or conduction velocity of the afferent fiber supplying them and the type of stimulus that activates them. Most experimental studies of nociceptors have been performed on common laboratory animals, especially rodents and cats. Some of the most informative, however, have been made during recordings from peripheral nerves of monkeys or human subjects (reviewed in Willis and Coggeshall, 2004).

The clinical descriptions of cordotomy played a major role in elucidating the function and the anatomy of the human spinothalamic tract (STT) (Chapter 1). There are a number of other examples of surgical interventions which have informed our understanding of the pain system. In particular, the pain-related role of the cingulate gyrus is suggested by imaging studies and by the effect of cingulotomy on experimental pain (Rainville et al., 1997; Gildenberg, 2004). Similarly the role of the motor cortex in these systems has suggested the effects of stimulation on activity throughout the pain system (Brown and Barbaro, 2003; Brown, 2004; Peyron et al., 2007). The purpose of this chapter is to examine these surgical interventions in terms of the anatomy and function of structures involved in these interventions. The inclusion of procedures in this chapter is arbitrary and many other such procedures which might have been included have been excluded.
Cordotomy and myelotomy
Percutaneous cordotomy produces relief of pain by interrupting the transmission of signals in the STT from below the level of intervention (Tasker, 1988; Tasker, 2004). The anterolateral quadrant of the spinal cord has long been recognized as the location of the STT (Chapter 1). Recent findings indicate that the dorsal column system also has an important role in visceral nociception (Nauta et al., 1997; Willis et al., 1999). The STT terminates in the primate thalamus, brainstem and other structures such as the hypothalamus and amygdala whereas the dorsal column system terminates in the dorsal column nuclei (Newman et al., 1996).

Introduction
Before the introduction of computerized tomographic (CT) brain imaging, studying human brain mechanisms of pain was largely limited to clinical reports and the post-mortem analysis of brain lesions. Although this approach provided important information and established the background for current investigations, these studies were usually limited by clinical descriptions of each patient's condition. Somatosensory psychophysics seldom included studies of pain and even then it was not possible to relate these observations to brain function or physiology. Because the living brain was invisible (except in the neurosurgery operating suite), research on pain mechanisms focused almost exclusively on the peripheral nervous system.
Brain CT scans introduced the opportunity to apply quantitative sensory testing to the study of living patients with visible, localized brain lesions and to begin to test hypotheses about functional localization and brain mechanisms of pain. The introduction of functional imaging by positron emission tomography (PET) and magnetic resonance imaging (MRI; fMRI) launched a new investigational paradigm into the study of pain mechanisms. Now it is possible to go well beyond the lesion analysis method and to relate human experience, in this case using somatosensory psychophysics, directly to a surrogate measure of activity in groups of neurons at the level of visible, localized brain structure. Since the early 1990s, the number and technical sophistication of functional brain imaging studies, including those related to pain, has increased at a rate that makes it almost impossible to incorporate the results into a conceptual framework.

Introduction
It is well known that much of the sensory input to the central nervous system can be modulated by centrifugally organized control systems that originate in the central nervous system (Head and Holmes, 1911; Hagbarth, 1960). The control mechanisms can be excitatory or inhibitory processes that may occur in the periphery or within the central nervous system. Inhibition can be at pre- and/or postsynaptic sites (Fig. 6.1(I)). Presynaptic inhibition at the first central synapse of a sensory pathway has the potential advantage of being able to reduce sensory input prior to wide dissemination of that sensory input within the central nervous system through the activation of interneuronal networks and multiple ascending pathways, for example, in the spinal cord (Schmidt, 1973; see Chapter 3).
Pre- and postsynaptic inhibition can have somewhat different effects on the stimulus-response curves of second-order sensory neurons, as shown in Fig. 6.1(II). Postsynaptic inhibition involves inhibitory postsynaptic potentials that sum with excitatory postsynaptic potentials (Fig. 6.1(IIA)). If there is a linear summation, the stimulus-response curve will be shifted to the right in a parallel fashion (Carstens et al., 1980). However, if the IPSP is generated in a membrane area near that in which the EPSP is generated, the excitatory current may be shunted and the slope of the stimulus-response curve reduced, causing a reduction in the gain of synaptic transmission (Fig. 6.1(IIB)). A similar reduction in gain can be produced by presynaptic inhibition.
