Objective:
Needlestick and sharps injury (NSSI) is a common occupational hazard of orthopedic surgery training. The purpose of this study was to examine the incidence and surrounding circumstances of intraoperative NSSI in orthopedic surgery residents and fellows and to examine postexposure reporting.
Design:A 35-question cross-sectional survey.
Setting:The study was conducted by orthopedic surgery residents and faculty at a nonprofit regional hospital.
Participants:The questionnaire was distributed to US allopathic orthopedic surgery residency and fellowship programs; 300 orthopedic surgery trainees participated in the survey.
Results:Of 223 trainees who had completed at least 1 year of residency, 172 (77.1%) sustained an NSSI during residency, and 57 of 63 trainees (90.5%) who had completed at least 4 years sustained an NSSI during residency. The most common causes of NSSI were solid needles, followed by solid pins or wires. The surgical activity most associated with NSSI was wound closure, followed by fracture fixation. The type of surgery most frequently associated with NSSI was orthopedic trauma, followed by hip and knee arthroplasty. Of 177 trainees who had sustained a prior NSSI, 99 (55.9%) failed to report all events to their institution’s occupational health department.
Conclusions:The incidence of NSSI during residency training is high, with >90% of trainees in their fifth year or later of training having received an injury during their training, with a mean of >4 separate events. Most trainees with an NSSI did not report all of their events, which implies that changes are needed in the incident reporting process universally.

Early Jurassic climate change and the radiation of organic-walled phytoplankton in the Tethys Ocean

During the Early Jurassic, cyst-forming dinoflagellates began a long-term radiation that would portend ecological importance of these taxa in the pelagic plankton community throughout the rest of the Mesozoic era. The factors that contributed to the evolutionary success of dinoflagellates are poorly understood. Here we examine the relationship between oceanographic and climatic conditions during the Hettangian–Toarcian interval in relation to the radiation of dinoflagellates and other organic-walled phytoplankton taxa in the Tethys Ocean. Our analysis is based on two data sets. The first includes δ13Ccarb, δ13Corg, total organic carbon (TOC), and quantitative palynological observations derived from the Mochras Core (Wales, U.K.), which spans the complete Early Jurassic. The second is a coupled Mg/Ca and δ18O record derived from analyses of belemnite calcite obtained from three sections in northern Spain, covering the upper Sinemurian to Toarcian. From these two data sets we reconstructed the influence of sea level, trophism, temperature, and salinity on dinoflagellate cyst abundance and diversity in northwest Europe. Our results suggest that organic-walled phytoplankton (acritarchs, prasinophytes, and dinoflagellates) diversity increased through the Early Jurassic. The radiation coincides with a long-term eustatic rise and overall increase in the areal extent of continental shelves, a factor critical to cyst germination. On shorter timescales, we observed short bursts of dinoflagellate diversification during the late Sinemurian and late Pliensbachian. The former diversification is consistent with the opening of the Hispanic Corridor during the late Sinemurian, which apparently allowed the pioneer dinoflagellate, Liasidium variabile, to invade the Tethys from the Paleo-Pacific. A true radiation pulse during the late Pliensbachian, with predominantly cold-water taxa, occurred during sea level fall, suggesting that climate change was critical to setting the evolutionary tempo. Our belemnite δ18O and Mg/Ca data indicate that late Pliensbachian water masses cooled (ΔT ≈ −6°C) and became more saline (ΔS ≈ +2 psu). Cooling episodes during generally warm and humid Early Jurassic climate conditions would have produced stronger winter monsoon northeast trade winds, resulting in hydrographic instability, increased vertical mixing, and ventilation of bottom waters. During the late Pliensbachian, dinoflagellates replaced green algae, including prasinophytes and acritarchs, as primary producers. By producing benthic resting cysts, dinoflagellates may have been better adapted to oxidized ocean regimes. This hypothesis is supported by palynological data from the early Toarcian ocean anoxic event, which was marked by highly stratified anoxic bottom water overlain by low-salinity, warm surface waters. These conditions were advantageous to green algae, while cyst-producing dinoflagellates temporarily disappeared. Our results suggest that the rise in dinoflagellate diversity later in the Jurassic appears to correspond to deep water ventilation as a result of the opening of the Atlantic seaway, conditions that appear to have simultaneously led to a loss of prasinophyte dominance in the global oceans.

Vortex dipoles in a two-dimensional, inviscid flow are obtained by prescribing the profile function relating the vorticity to the stream function. The profile functions used are smooth, and the solutions obtained have a smooth transition from the exterior flow to the interior of the vortex. The dipoles are nearly elliptical, and this relates this work to the ‘supersmooth’ dipoles obtained recently by Kizner & Khvoles (Regular Chaotic Dyn., vol. 9, 2004, pp. 509–518). The solutions found here are obtained by an iterative method for solving the nonlinear partial differential equation for the stream function. This iterative method is both robust and flexible. Solutions are also obtained in a β-plane, and they are shielded, as has also been found in previous work.

A general procedure is presented for computing axisymmetric swirling vortices which are steady with respect to an inviscid flow that is either uniform at infinity or includes shear. We consider cases both with and without a spherical obstacle. Choices of numerical parameters are given which yield vortex rings with swirl, attached vortices with swirl analogous to spherical vortices found by Moffatt, tubes of vorticity extending to infinity and Beltrami flows. When there is a spherical obstacle we have found multiple solutions for each set of parameters. Flows are found by numerically solving the Bragg–Hawthorne equation using a non-Newton-based iterative procedure which is robust in its dependence on an initial guess.

We review different conceptions of inhibitory control that may be relevant to the regulatory problems featured in borderline personality disorder (BPD). These conceptions have often been framed with regard to personality traits of inhibitory control, but can also be related to cognitive measures of response suppression as well as affect regulation. Reactive behavioral inhibition is relatively unstudied in relation to BPD. A substantial amount of literature links executive function problems with BPD, but that literature has not isolated executive response inhibition nor been controlled for other personality disorder symptoms of antisociality, attention-deficit/hyperactivity disorder (ADHD), or depression, anxiety, or posttraumatic symptoms. We therefore conducted a study of this question looking at BPD symptoms in an adult sample with a small number of BPD subjects and other disorders. Results indicated that symptoms of BPD were correlated with response inhibition (measured by stop signal reaction time) even after controlling for the overlap of stop inhibition with ADHD, antisociality, and other Axis II disorder symptoms. We conclude by hypothesizing discrete developmental routes to BPD, based on different mechanism breakdowns, which would be amenable to empirical investigation at the cognitive or trait level of analysis.

The stability of steady inviscid vortex pairs in equilibrium with a circular cylinder is studied by discretizing equations derived from contour dynamics. There are two families of vortices, one with a pair of counter-rotating vortices standing behind the cylinder, which may be thought of as desingularizing the Föppl point vortices, and the other with the vortices standing directly above and below the cylinder. Vortices in the first family are found to be neutrally stable with respect to symmetric perturbations. When asymmetric perturbations are included, there is a single unstable mode and a single asymptotically stable mode. Vortices above and below the cylinder have two modes of instability, one symmetric and the other asymmetric, and likewise two asymptotically stable modes.

Words matter, and they matter most of all in the context in which they are to be read and understood. On July 7, 2005, the New York Times published an opinion piece, “Finding Design in Nature,” purporting to offer “The official Catholic stance on evolution.” The author of that piece, my fellow Catholic Christoph Cardinal Schönborn, got the theology exactly right, but erred dramatically in his take on the science and the politics of the “design” movement as it exists in the United States. Knowing how the Cardinal's words will be misused by the enemies of science in our country, it is important to set the record straight.
As Cardinal Schönborn quite properly points out, the Catholic Church is staunchly opposed to any view of life that would exclude the notion of divine purpose and meaning. In the new century, as he puts it, the church will “defend human reason by proclaiming that the immanent design evident in nature is real.” In response I would echo the words of the Catechism that scientific studies of “the age and development of the cosmos, the development of life-forms and the appearance of man … invite us to even greater admiration for the greatness of the Creator.” Indeed they do.
But the Cardinal is wrong in asserting that the neo-Darwinian theory of evolution is inherently atheistic. Neo-Darwinism, he tells us, is an ideology proposing that an “unguided, unplanned process of random variation and natural selection” gave rise to all life on earth, including our own species. To be sure, many evolutionists have made such assertions in their popular writings on the “meaning” of evolutionary theory. But are such assertions truly part of evolution as it is understood by the “mainstream biologists” of which the Cardinal speaks?

Summary
Almost from the moment On the Origin of Species was published in 1859, the opponents of evolution have fought a long, losing battle against their Darwinian foes. Today, like a prizefighter in the late rounds losing badly on points, they've placed their hopes on one big punch – a single claim that might smash through the overwhelming weight of scientific evidence to bring Darwin to the canvas once and for all. Their name for this virtual roundhouse right is “Intelligent Design.”
In the last several years, the Intelligent Design (ID) movement has attempted to move against the standards of science education in several American states, most famously in Kansas and Ohio (Holden 1999; Gura 2002). The principal claim made by adherents of this view is that they can detect the presence of “Intelligent Design” in complex biological systems. As evidence, they cite a number of specific examples, including the vertebrate blood clotting cascade, the eukaryotic cilium, and most notably, the eubacterial flagellum (Behe 1996a; Behe 2002).
Of all these examples, the flagellum has been presented so often as a counterexample to evolution that it might well be considered the “poster child” of the modern anti-evolution movement. Variations of its image (Figure 5.1) now appear on web pages of anti-evolution groups such as the Discovery Institute, and on the covers of “Intelligent Design” books such as William Dembski's No Free Lunch (Dembski 2002a).

Pluton emplacement in arcs: a crustal-scale exchange process

Buddington (1959) pointed out that the construction of large crustal magma chambers involves complex internal processes as well as multiple country rock material transfer processes (MTPs), which reflect large horizontal, vertical and temporal gradients in physical conditions. Thus, we have attempted to determine the relative importance of different magmatic and country rock MTPs at various crustal depths, and whether country rock MTPs largely transport material vertically or horizontally, rather than seeking a single model of magma ascent and emplacement.
Partially preserved roofs of nine plutons and in some cases roof–wall transitions with roof emplacement depths of 1·5–11 km were mapped. During emplacement, these roofs were not deformed in a ductile manner, detached or extended by faults, or significantly uplifted. Instead, sharp, irregular, discordant contacts are the rule with stoped blocks often preserved immediately below the roof, even at depths of 10 km. The upper portions of these magma chambers are varied, sometimes preserving the crests of more evolved magmas or local zones of volatile-rich phases and complex zones of dyking and magma mingling. Magmatic structures near roofs display a wide variety of patterns and generally formed after emplacement. Transitions from gently dipping roofs to steep walls are abrupt. At shallow crustal levels, steep wall contacts have sharp, discordant, stepped patterns with locally preserved stoped blocks indicating that the chamber grew sideways in part by stoping. Around deeper plutons, an abrupt transition (sometimes within hundreds of metres) occurs in the country rock from discordant, brittle roofs to moderately concordant, walls deformed in a ductile manner defining narrow structural aureoles. Brittle or ductile faults are not present at roof–wall joins.
Near steep wall contacts at shallow to mid-crustal depths (5–15 km), vertical and horizontal deflections of pre-emplacement markers (e.g. bedding, faults, dykes), and ductile strains in narrow aureoles (0·1–0·3 body radii) give a complete range of bulk strain values that account for 0–100% of the needed space, but average around 30%, or less, particularly for larger batholiths. A lack of far-field deflection of these same markers rules out significant horizontal displacement outside the aureoles and requires that any near-field lateral shortening is accommodated by vertical flow. Lateral variations from ductile (inner aureole) to brittle (outer aureole) MTPs are typically observed. Compositional zoning is widespread within these magma bodies and is thought to represent separately evolved pulses that travelled up the same magma plumbing system. Magmatic foliations and lineations commonly cross-cut contacts between pulses and reflect the strain caused either by the late flow of melt or regional deformation.
Country rocks near the few examined mid- to deep crustal walls (10–30 km) are extensively deformed, with both discordant and concordant contacts present; however, the distinction between regional and emplacement-related deformation is less clear than for shallower plutons. Internal sheeting is more common, although elliptical masses are present. Lateral compositional variations are as large as vertical variations at shallower depths and occur over shorter distances. Magmatic foliations and lineations often reflect regional deformation rather than emplacement processes.
The lack of evidence for horizontal displacement outside the narrow, shallow to mid-crustal aureoles and the lack of lateral or upwards displacement of pluton roofs indicate that during emplacement most country rock is transported downwards in the region now occupied by the magma body and its aureole. The internal sheeting and zoning indicate that during the downwards flow of country rock, multiple pulses of magma travelled up the same magma system. If these relationships are widespread in arcs, magma emplacement is the driving mechanism for a huge crustal-scale exchange process.
