Personality disorder coverage, prevalence, and convergence: do the DSM-5's two models of personality disorder identify the same patients?

Background
Research on the Alternative DSM-5 Model for Personality Disorders (AMPD) in DSM-5's Section-III has demonstrated acceptable interrater reliability, a largely consistent latent structure, substantial correlations with theoretically and clinically relevant measures, and evidence for incremental concurrent and predictive validity after controlling for DSM-5's Section II categorical personality disorders (PDs). However, the AMPD is not yet widely used clinically. One clinician concern may be caseness – that the new model will diagnose a different set of PD patients from that with which they are familiar. The primary aim of this study is to determine whether this concern is valid, by testing how well the two models converge in terms of prevalence and coverage.
MethodParticipants were 305 psychiatric outpatients and 302 community residents not currently in mental-health treatment who scored above threshold on the Iowa Personality Disorder Screen (Langbehn et al., 1999). Participants were administered a semi-structured interview for DSM-5 PD, which was scored for both Section II and III PDs.
ResultsConvergence across the two PD models was variable for specific PDs, Good when specific PDs were aggregated, and Very Good for ‘any PD.’
ConclusionsResults provide strong evidence that the AMPD yields the same overall prevalence of PD as the current model and, further, identifies largely the same overall population. It also addresses well-known problems of the current model, is more consistent with the ICD-11 PD model, and provides more complete, individualized characterizations of persons with PD, thereby offering multiple reasons for its implementation in clinical settings.

Summary
Diagnosis and classification for mental disorder are in flux. This transition has downstream consequences on the nature of clinical assessment in research and treatment settings. We begin this chapter by describing the prevailing categorical rubrics, which are the predominant guide to clinical assessment worldwide. These systems, despite their popularity, suffer from serious defects, which have prompted the development of alternate frameworks for conceptualization and assessment of psychopathology. We focus the remainder of the chapter on two prominent contenders to supplement, and perhaps eventually supplant, traditional categorical models. The Hierarchical Taxonomy of Psychopathology is an empirically derived system of the phenotypic dimensions of psychopathology and the Research Domain Criteria represent a biologically oriented approach to understanding risk processes implicated in mental disorder. We describe the promise and challenges facing these two emerging systems, and we speculate about how they will shape the future of clinical assessment.

Britain's pre-Conquest past and its culture continues to fascinate modern writers and artists. From Henry Sweet's Anglo-Saxon Reader to Seamus Heaney's Beowulf, and from high modernism to the musclebound heroes of comic book and Hollywood, Anglo-Saxon England has been a powerful and often unexpected source of inspiration, antagonism, and reflection. The essays here engage with the ways in which the Anglo-Saxons and their literature have been received, confronted, and re-envisioned in the modern imagination. They offer fresh insights on established figures, such as W.H. Auden, J.R.R. Tolkien, and David Jones, and on contemporary writers such as Geoffrey Hill, Peter Reading, P.D. James, and Heaney. They explore the interaction between text, image and landscape in medieval and modern books, the recasting of mythic figures such as Wayland Smith, and the metamorphosis of Beowulf into Grendel - as a novel and as grand opera. The early medieval emerges not simply as a site of nostalgia or anxiety in modern revisions, but instead provides a vital arena for creativity, pleasure, and artistic experiment.

The nosological organisation of DSM – IV and ICD–10 does not capture the empirical structure of the mood and anxiety disorders. Instead, they form a broad group of ‘internalising’ disorders with two subclasses: distress disorders and fear disorders. This empirical structure should form the basis for revised taxonomies in DSM–V and ICD–11.
Multiple measures, methods, and moments: a factor-analytic investigation of change in depressive symptoms during acute-phase cognitive therapy for depression

Background. Researchers and clinicians use a variety of measures to assess depressive symptoms, including clinician reports [for example the 17-item Hamilton Rating Scale for Depression (HRSD-17) and the 30-item Inventory for Depressive Symptomatology – Clinician Report (IDS-C-30)]; and patient reports [for example, the Beck Depression Inventory (BDI) and the 30-item Inventory for Depressive Symptomatology – Self-Report (IDS-SR-30)]. Although their concurrent convergent validity is well established, the degree to which these measures reflect the same pattern and level of change during treatment is unclear.
Method. We assessed depressive symptoms with two clinician (HRSD-17, IDS-C-30) and two patient (BDI, IDS-SR-30) reports on 14–15 occasions in 127 out-patients with recurrent major depressive disorder treated with 20 sessions of acute-phase cognitive therapy.
Results. Factor analyses revealed that time was the major source of variation in depressive symptom severity and change, with distinct ‘early’ and ‘late’ assessment factors, regardless of measure or rater (patient v. clinician). Average symptom severity decreased rapidly on all measures until mid-treatment, after which gradual decreases continued through the end of treatment. Effect sizes for pre- to post-treatment change were large for all measures (d=1·9–2·2), although there were significant differences among measures at some time-points.
Conclusions. The HRSD-17, BDI, IDS-C-30 and IDS-SR-30 reflect essentially the same symptom severity and change constructs during acute-phase cognitive therapy. Clinicians and researchers using one of these measures may estimate scores on the others with the provided common-factor conversions.
