Methods for Improving Screening for Vascular Cognitive Impairment Using the Montreal Cognitive Assessment

Background:
Vascular cognitive impairment (VCI) post-stroke is frequent but may go undetected, which highlights the need to better screen cognitive functioning following a stroke.
Aim:We examined the clinical utility of the Montreal Cognitive Assessment (MoCA) in detecting cognitive impairment against a gold-standard neuropsychological battery.
Methods:We assessed cognitive status with a comprehensive battery of neuropsychological tests in 161 individuals who were at least 3-months post-stroke. We used receiver operating characteristic (ROC) curves to identify two cut points for the MoCA to maximize sensitivity and specificity at a minimum 90% threshold. We examined the utility of the Symbol Digit Modalities Test, a processing speed measure, to determine whether this additional metric would improve classification relative to the MoCA total score alone.
Results:Using two cut points, 27% of participants scored ≤ 23 and were classified as high probability of cognitive impairment (sensitivity 92%), and 24% of participants scored ≥ 28 and were classified as low probability of cognitive impairment (specificity 91%). The remaining 48% of participants scored from 24 to 27 and were classified as indeterminate probability of cognitive impairment. The addition of a processing speed measure improved classification for the indeterminate group by correctly identifying 65% of these individuals, for an overall classification accuracy of 79%.
Conclusions:The utility of the MoCA in detecting cognitive impairment post-stroke is improved when using a three-category approach. The addition of a processing speed measure provides a practical and efficient method to increase confidence in the determined outcome while minimally extending the screening routine for VCI.

This article is about the relationship between the culture of outdoors recreation and the development of progressive politics at the turn of the twentieth century in the United States. It considers the significance of popular outdoors magazines for American culture and politics before focussing in particular on the way in which Caspar Whitney, as editor of Outing magazine, constructed a notion of sportsmanship modelled upon the idealized figure of Theodore Roosevelt – an exemplar, by his reckoning, of the patrician class, and the template for his vision of a progressive citizenship. It was through the notion of sportsmanship that Whitney defined a set of values that would become synonymous with the strain of progressivism known as the New Nationalism, out of which the tradition of reform liberalism emerged in the twentieth century.

This article explores African American armed resistance during the 1917 East St. Louis race riot in the context of black migration and ghetto formation. In particular it considers the significance of the development of the black urban community, composed of an emerging working class and a dynamic, militant and increasingly influential middle class. It was that community which came under attack by white mobs in 1917, and this work illuminates the infrastructure of resistance in the city, showing how African Americans drew upon the resources of the nascent ghetto and older traditions of self-defence to protect their homes and families.

Objective:
To evaluate the influence of dietary fibre on menarche in a cohort of pre-menarcheal girls.
Design:Prospective cohort study.
Setting:Ontario, Canada.
Subjects:Free-living pre-menarcheal girls (n = 637), 6 to 14 years of age.
Methodology:Information on dietary intake, physical activity and date of menarche was collected at baseline and was updated annually by self-administered questionnaires for three years. Cox proportional hazards models were used to evaluate the association between dietary fibre and menarche, adjusting for age at entry to the study and potential confounders.
Results:A higher intake of energy-adjusted dietary fibre was associated with a lower risk of (i.e. a later age at) menarche (relative hazard 0.54, 95% confidence interval (CI) 0.31–0.94 for highest vs. lowest quartile, P for trend = 0.027). At the fibre component level, a higher intake of energy-adjusted cellulose was associated with a lower risk of menarche (relative hazard 0.45, 95% CI 0.26–0.76, P for trend = 0.009).
Conclusions:The findings are consistent with the hypothesis that pre-menarcheal dietary intake can influence menarche.

This study concerns a racial massacre which took place in East St Louis, Illinois on 2 July 1917. The violence erupted during a period of acute industrial unrest, and after significant black migration to the city from the South. These contexts were a focus for Elliott Rudwick's Race Riot, published in 1964, the classic study on this subject. A new approach to the context of industrial conflict is taken in the present work, one which considers the precise timing of the outbreak, and the significance of rumour in the riot. The context of community change is also reconsidered, and the underlying causes of whites' racial hostility are discussed. Close attention is given to the moment of the massacre, and an approach is taken to the social psychology of the collective behaviour of the rioters in order to offer explanations for how an entire community became involved in these atrocities.
