Outcomes after initial heart failure consultation in Fontan patients

Background:
Patients with Fontan failure are high-risk candidates for heart transplantation and other advanced therapies. Understanding the outcomes following initial heart failure consultation can help define appropriate timing of referral for advanced heart failure care.
Methods:This is a survey study of heart failure providers seeing any Fontan patient for initial heart failure care. Part 1 of the survey captured data on clinical characteristics at the time of heart failure consultation, and Part 2, completed 30 days later, captured outcomes (death, transplant evaluation outcome, and other interventions). Patients were classified as “too late” (death or declined for transplant due to being too sick) and/or “care escalation” (ventricular assist device implanted, inotrope initiated, and/or listed for transplant), within 30 days. “Late referral” was defined as those referred too late and/or had care escalation.
Results:Between 7/2020 and 7/2022, 77 Fontan patients (52% inpatient) had an initial heart failure consultation. Ten per cent were referred too late (6 were too sick for heart transplantation with one subsequent death, and two others died without heart transplantation evaluation, within 30 days), and 36% had care escalation (21 listed ± 5 ventricular assist device implanted ± 6 inotrope initiated). Overall, 42% were late referrals. Heart failure consultation < 1 year after Fontan surgery was strongly associated with late referral (OR 6.2, 95% CI 1.8–21.5, p=0.004).
Conclusions:Over 40% of Fontan patients seen for an initial heart failure consultation were late referrals, with 10% dying or being declined for transplant within a month of consultation. Earlier referral, particularly for those with heart failure soon after Fontan surgery, should be encouraged.
SYSTEMATIC ANALYSES OF RADIOCARBON AGES OF COEXISTING PLANKTONIC FORAMINIFERA

We compare radiocarbon (14C) ages of coexisting planktonic foraminifera species from sediment cores VM12-107 and KNR166-2-26JPC from the Equatorial Atlantic Ocean for three time periods (Holocene, Heinrich Stadial 1, last glacial maximum). We find a maximum inter-species difference of 1200 14C yr. On average, the 14C ages deviate by ∼300 yr between Globigerinoides ruber and other species. In most cases, this exceeds the analytical uncertainty range of the measurements and thus renders the choice of species for generating age models as important as sample weight. While modern stratified water-column profiles imply an increase in 14C ages with water depth, we observe an expected parallel increase of 14C ages and δ18O only at VM12-107. The mismatch between 14C ages and δ18O at KNR166-2-26JPC likely results from the effects of bioturbation and the hydrographic setting. The largest difference in 14C ages between mixed-layer versus thermocline-calcifying planktonic foraminifera are observed during Heinrich Stadial 1 despite a decrease in upper-ocean stratification at that time. This difference is likely the result of inconsistent increases in 14C reservoir ages during times of reduced overturning circulation masking the potential of 14C ages of coexisting planktonic foraminifera to reflect the density stratification of the water column.

Electro-Optical Detection of Single Nanoparticles on a Nanopore-Optofluidic Chip

A solid-state nanopore was integrated into an optofluidic sensor chip, liquid-core anti-resonant reflecting optical waveguide (ARROW). The solid-state nanopore worked as a smart gate, which simultaneously provided characteristic electrical signals and controlled the entry of single nanoparticles into the liquid-core channel. The subsequent fluorescence detection further identified the nanoparticles by providing optical signals within a specific wavelength range. In this work, correlated electrical and optical detection of single nanoparticles, H1N1 viruses, and λ-DNA molecules was demonstrated. Different types of particles in a mixture were successfully discriminated. Moreover, the flow velocity in the liquid-core channel was extracted with the help of combined analysis of electrical and optical signals. Enhanced electrical sensitivity using a solid-state nanopore with a thin limiting aperture sculpted by SiO2 deposition was also shown.
