Effects of Hazardous Alcohol use on Neurocognition in HIV Positive Individuals

Introduction
There is significant evidence that HIV is brain degenerative and long-term infection can impair cognitive functioning. In South Africa, alcohol remains the dominant substance of abuse and lifetime alcohol dependence has been found to impair memory, executive function and visuospatial capabilities. The individual liability of alcohol and HIV on neurocognitive function have been well demonstrated, however there is relatively little evidence of the potentially aggravating effects of this dual burden on neurocognitive outcomes.
ObjectivesThe present study is ongoing and sought to identify the effects of hazardous alcohol use on neurocognitive functioning in the context of HIV infection.
AimsTo describe the association between HIV and harmful alcohol use on neuropsychological test performance in a cohort of adults in the Western Cape of South africa.
Methodsparticipants (n = 50) were tested using a battery of neuropsychological tests sensitive to the effects of HIV on the brain. Self-reported alcohol use was recorded using the alcohol use identification test (AUDIT). Results The sample consisted of 47 females and 3 males. All participants were HIV-positive and on antiretroviral therapy. A total of 23 (46%) participants reported no alcohol use and 27 (54%) reported drinking alcohol on the AUDIT.
Resultsrevealed a significant difference between groups on the Stroop colour word test, with poorer performance evident among the alcohol users (P = 0.008).
ConclusionAlcohol use in the context of HIV infection contributes to poorer executive function. These preliminary data provide evidence for a synergistic relationship between HIV infection and alcohol use.

Subglacial hydrology plays a key role in many glaciological processes, including ice dynamics via the modulation of basal sliding. Owing to the lack of an overarching theory, however, a variety of model approximations exist to represent the subglacial drainage system. The Subglacial Hydrology Model Intercomparison Project (SHMIP) provides a set of synthetic experiments to compare existing and future models. We present the results from 13 participating models with a focus on effective pressure and discharge. For many applications (e.g. steady states and annual variations, low input scenarios) a simple model, such as an inefficient-system-only model, a flowline or lumped model, or a porous-layer model provides results comparable to those of more complex models. However, when studying short term (e.g. diurnal) variations of the water pressure, the use of a two-dimensional model incorporating physical representations of both efficient and inefficient drainage systems yields results that are significantly different from those of simpler models and should be preferentially applied. The results also emphasise the role of water storage in the response of water pressure to transient recharge. Finally, we find that the localisation of moulins has a limited impact except in regions of sparse moulin density.

Fourteen influenza A(H3N2) field isolates, mainly obtained during the first weeks of the 1982/83 influenza epidemic in The Netherlands, and nine influenza A(H3N2) reference strains were examined by means of haemagglutination inhibition (HI) tests with 23 potyclonal ferret sera. The resulting HI patterns were subjected to various methods of numerical taxonomy using, among others, taxonomic distance and correlation between strains for resemblance coefficients. Marked differences between distance and correlation coefficients were found in strains which differed in avidity only. The field isolates could be divided into four groups in respect of their taxonomic resemblance to the reference strains. The same grouping was found for five of the field isolates by testing these against 200 human sera.

Passive detection of tuberculosis (TB) cases may lead to delay in treatment which may contribute to increased severity of disease and mortality. Active case finding may be an alternative. In a community survey in Cape Town, South Africa, we actively detected 27 bacteriologically positive TB cases and compared those with 473 passively detected TB cases. Seven of 27 (26%) actively detected TB cases did not start treatment within 2 months and were considered initial defaulters. Those who did start treatment had similar treatment success rates as passively detected TB cases (both 80%) (OR 1·01, 95% CI 0·33–3·09). Passively detected cases reported the presence of the symptoms cough (OR 3·72, 95% CI 1·47–9·39), haemoptysis (OR 3·20, 95% CI 1·03–9·93), night sweats (OR 3·35, 95% CI 1·40–7·99), fever (OR 4·28, 95% CI 1·21–15·14), and weight loss (OR 11·14, 95% CI 4·17–29·74) more often than those detected actively. We conclude that although TB cases detected by a community survey are less symptomatic and are prone to a high initial default rate, active case finding can potentially identify a substantial portion of the existing caseload at an earlier stage of disease, thereby reducing the risk of transmission.

AURORA films, which have a Si-O-Si network with –CH3 terminations, were characterized and integrated into Cu single damascene structures. The relatively low carbon concentration (∼ 20%) and the very small pore size (∼ 0.6 nm) found could be advantageous during integration of AURORA. Integration of AURORA into Cu single damascene structures was successfully achieved. Suitable resist strip processes, which are critical for Si-O-C type materials, were developed, resulting in trenches with satisfactory profiles. After a complete single damascene process, a interline dielectric constant value of 2.7 was found for line spacing down to 0.25 µm.

Nitrogen-rich amorphous silicon-nitride films a-Si1−xNx:H were prepared by glow-discharge decomposition of gas mixtures of ammonia and silane. With increasing nitrogen content the spin density, Ns, decreases from 4×1018cm−3 (x = 0.55) to 3×1017cm−3 (x = 0.67). These films were used as a gate dielectric in amorphous silicon TFTs. The TFTs are characterized by measurements of the transfer characteristics and by a transient current spectroscopy (TCS). Nitrogen-rich dielectrics with Ns<1018cm−3 have little influence on the transfer characteristic, however, they tend to have a lower sensitivity to bias stress. Using a device-quality nitride (x = 0.64) the properties of the TFTs were varied in two ways: 1) doping of the a-Si:H film with phosphine or diborane and 2) exposure of the nitride film to an oxygen plasma. The variation of the areal density of defect states, Nd, with Ec-EF suggests that the effective density of interface states, Ni, and the characteristic of undoped TFTs are determined by interface defects of the a-Si:H film. The plasma treatment introduces oxygen into a thin superficial layer of the nitride. By varying the exposure time te it is possible to change the properties of the TFTs continuously from nitride like to oxide like.
