Objective:
Empirical support for inclusion of performance validity testing (PVTs) in neuropsychological assessment continues to grow (Sweet et al., 2021). However, considerable validation is still needed to understand the impact of culturally mediated factors on the reliability of current, commonly used PVTs to accurately classify effort among various cultural groups. This study sought to contribute to the literature by examining the utility of several PVTs in a non-clinical, community-dwelling sample in Kampala, Uganda.
Participants and Methods:Participants included 52 residents (25 Female, 27 Male) who were born between 1953-2003 from the Wabigalo community of central Kampala. Individuals were recruited by community leaders and volunteered to participate. All 52 participants were administered the Dot Counting Test (DCT; Boone et al., 2002), Test of Memory Malingering (TOMM; Tombaugh, 1997), and Rey 15-Item Memorization Test (Rey 15; Lezak, 1995). Twenty-five participants also completed Green’s Non-Verbal Medical Symptom Validity Test (NV-MSVT; Green, 2006). Data from three participants was excluded due to suspected memory concerns. Instructions for all tests were translated into Luganda by a professional translator with experience in Luganda and were administered by Luganda-speaking individuals.
Results:Using test manual-derived cut scores, 71.4% (n = 35) participants scored in the invalid range on the DCT, 10.2% (n = 5) produced total combined scores in the invalid range on Rey 15, 6.1% (n = 3) failed TOMM Trial 2, and one participant (4.3%) exceeded cut-offs on Green’s NV-MSVT.
Conclusions:In this non-clinical sample, manual cutoffs for DCT contributed to a high type-1 error rate. These findings suggest that culturally mediated factors may contribute to differences in engagement or performance on DCT. Future studies should explore these factors and continue to examine the utility of widely used tests in diverse samples.

Objectives
To explore the perceptions of parents caring for a child with a life-limiting condition on approaches to communication used by clinicians engaging in routine serious illness communication.
MethodsAn exploratory qualitative design was used, employing a thematic analysis of data derived through semi-structured interviews which presented hypothetical vignettes of serious illness conversations to elicit parental perspectives. Adult parents of children with a life-limiting condition, in a stable phase of care, known to the Neurodevelopment and Disability Department of a tertiary Children’s Hospital in Melbourne, Australia, were purposively sampled to achieve a broad representation of relevant clinical and sociodemographic factors.
ResultsEleven parents (72% female) of children aged 7 months to 18 years participated. Two major themes characterized parental perspectives on serious illness communication: “Approaches clinicians can use to lay the foundation for quality communication” including checking in, validation, aligning with hopes and a commitment to listening and being present; and “Approaches clinicians can use to aid the delivery of information” including honesty and compassion, presenting possibilities, providing a plan, and conveying the clinician’s experience.
Significance of resultsThis study provides novel insights into the perspectives of parents of children with life-limiting conditions that inform how clinicians may best approach serious illness communication. The findings highlight the need for clinicians working in pediatric health care to be cognizant of parents’ needs before and during conversations. Laying the foundation for quality communication is important alongside the approaches outlined that aid in the delivery of information.
Restricting carbohydrates and calories in the treatment of type 2 diabetes: a systematic review of the effectiveness of ‘low-carbohydrate’ interventions with differing energy levels

There are two proven dietary approaches to shift type 2 diabetes (T2D) into remission: low-energy diets (LEDs) and low-carbohydrate diets (LCDs). These approaches differ in their rationale and application yet both involve carbohydrate restriction, either as an explicit goal or as a consequence of reducing overall energy intake. The aims of this systematic review were to identify, characterise and compare existing clinical trials that utilised ‘low-carbohydrate’ interventions with differing energy intakes. Electronic databases CENTRAL, CINAHL, Embase, MEDLINE and Scopus were searched to identify controlled clinical trials in adults with T2D involving low-carbohydrate intake (defined as <130 g carbohydrate/d) and reporting weight and glycaemic outcomes. The initial database search yielded 809 results, of which fifteen studies met the inclusion criteria. Nine out of fifteen studies utilised LCDs with moderate or unrestricted energy intake. Six trials utilised LEDs (<1200 kcal/d), with all except one incorporating meal replacements as part of a commercial weight loss programme. Interventions using both restricted and unrestricted (ad libitum) energy intakes produced clinically significant weight loss and reduction in glycated haemoglobin (HbA1c) at study endpoints. Trials that restricted energy intake were not superior to those that allowed ad libitum low-carbohydrate feeding at 12 and 24 months. An association was observed across studies between average weight loss and reduction in HbA1c at 6, 12 and 24 months, indicating that sustained weight loss is key to T2D remission. Further research is needed to specifically ascertain the weight-independent effects of carbohydrate restriction on glycaemic control in T2D.

The first demonstration of laser action in ruby was made in 1960 by T. H. Maiman of Hughes Research Laboratories, USA. Many laboratories worldwide began the search for lasers using different materials, operating at different wavelengths. In the UK, academia, industry and the central laboratories took up the challenge from the earliest days to develop these systems for a broad range of applications. This historical review looks at the contribution the UK has made to the advancement of the technology, the development of systems and components and their exploitation over the last 60 years.
Assessment of cerebral blood flow in adult patients with aortic coarctation

Background
Survival into adult life in patients with aortic coarctation is typical following surgical and catheter-based techniques to relieve obstruction. Late sequelae are recognised, including stroke, hypertension, and intracerebral aneurysm formation, with the underlying mechanisms being unclear. We hypothesised that patients with a history of aortic coarctation may have abnormalities of cerebral blood flow compared with controls.
MethodsPatients with a history of aortic coarctation underwent assessment of cerebral vascular function. Vascular responsiveness of intracranial vessels to hypercapnia and degree of cerebral artery stiffness using Doppler-derived pulsatility indices were used. Response to photic stimuli was used to assess neurovascular coupling, which reflects endothelial function in response to neuronal activation. Patient results were compared with age- and sex-matched controls.
ResultsA total of 13 adult patients (males=10; 77%) along with 13 controls underwent evaluation. The mean age was 36.1±3.7 years in the patient group. Patients with a background of aortic coarctation were noted to have increased pulse pressure on blood pressure assessment at baseline with increased intracranial artery stiffness compared with controls. Patients with a history of aortic coarctation had less reactive cerebral vasculature to hypercapnic stimuli and impaired neurovascular coupling compared with controls.
ResultsAdult patients with aortic coarctation had increased intracranial artery stiffness compared with controls, in addition to cerebral vasculature showing less responsiveness to hypercapnic and photic stimuli. Further studies are required to assess the aetiology and consequences of these documented abnormalities in cerebral blood flow in terms of stroke risk, cerebral aneurysm formation, and cognitive dysfunction.

Red vent syndrome (RVS) has previously been reported in returning wild Atlantic salmon, Salmo salar from Canada, Iceland, Ireland, Norway and the UK. Affected fish show reddening and swelling in the perianal/vent area, with scale loss, ulceration and bleeding in severe cases. Studies in the UK and elsewhere report the condition to be induced by Anisakis simplex sensu stricto (s.s.) larvae. This parasite, commonly reported in several marine fish species, is typically found in the body cavity and the skeletal muscle, but has recently been reported within the vent tissues of salmon. This latter finding may reflect greater efforts in examining this body portion due to current awareness of the parasite presence in this atypical location. Based on clinical observations, affected fish are classified into three categories according to the severity of the external lesions, but quantification of the vent parasite numbers in relation to the categories, and assessment of the relative importance of this area as a site of infestation, are missing to date. This investigation aimed to provide data on parasite number and distribution in the viscera, skeletal muscle, peduncle and vent area and to confirm the identity of the larvae found in the vent and the viscera. The study showed the perianal/vent region harbours the highest total number of A. simplex larvae per fish and, proportionally to fish biomass, is the most heavily infested body location irrespective of external severity levels of RVS.

Blindness represents an increasing global problem with significant social and economic impact upon affected patients and society as a whole. In Europe, approximately one in 30 individuals experience sight loss and 75% of those are unemployed, a social burden which is very likely to increase as the population of Europe ages. Diseases affecting the retina account for approximately 26% of blindness globally and 70% of blindness in the United Kingdom. To date, there are no treatments to restore lost retinal cells and improve visual function, highlighting an urgent need for new therapeutic approaches. A pioneering breakthrough has demonstrated the ability to generate synthetic retina from pluripotent stem cells under laboratory conditions, a finding with immense relevance for basic research, in vitro disease modeling, drug discovery, and cell replacement therapies. This review summarizes the current achievements in pluripotent stem cell differentiation toward retinal cells and highlights the steps that need to be completed in order to generate human synthetic retinae with high efficiency and reproducibly from patient-specific pluripotent stem cells.

Of the three large ephydrid flies of Yellowstone’s thermal springs, Ephydra thermophila numerically dominates acidic springs because only it can tolerate their water. Alkaline spring effluents are shared by Ephydra bruesi and Paracoenia turbida. E. bruesi is excluded from alkaline high productivity springs, probably because its larval maturation time is long, relative to the short life of patches of larval habitat. Consequently, such springs support only P. turbida. Proportions of the two species in alkaline low-productivity springs are apparently determined by the proportions of available larval food occurring in still versus flowing water. P. turbida is superior in still water, and E. bruesi wins in flowing water. The analysis leads to predictions (i) that E. bruesi should be relatively abundant in low-productivity springs with pulsating flows, and (ii) that E. bruesi adults should prefer ovipositing in low-productivity effluents, while P. turbida adults should be less particular.

Introduction
Ellis – van Creveld syndrome is an autosomal recessive disorder manifest by short-limb dwarfism, thoracic dystrophy, postaxial polydactyly, dysplastic nails and teeth, and an approximately 60% incidence of congenital malformations of the heart. Despite patients with Ellis – van Creveld syndrome being regarded as having a high surgical risk, few data are available regarding their outcomes following surgery for congenital malformations of the heart in the current era.
Materials and methodsIn this retrospective report, we summarise the clinical observations and outcomes of nine infants with Ellis – van Creveld syndrome who underwent surgery for congenital malformations of the heart between 2004 and 2009.
ResultsWe identified 15 patients with Ellis – van Creveld syndrome during the study period; 11 (73%) had haemodynamically significant congenital malformations of the heart warranting surgery. In two of these patients, surgery was not performed. Of the nine patients who underwent surgery, all of whom were infants, eight (89%) had various forms of an atrioventricular septal defect and one patient (11%) had hypoplastic left heart syndrome (mitral and aortic atresia). Among the nine patients who underwent surgery, four (44%) died at a median of 102 days with a range of 25–149 days post-operatively, mostly from respiratory failure. Respiratory morbidity was seen in all surviving patients, of whom three underwent tracheostomy.
ConclusionsSurgery for congenital malformations of the heart can be successful in infants with Ellis – van Creveld syndrome, but mortality is high and post-operative respiratory morbidity should be expected.

Origin of rare Ha-ras alleles: relationship of VTR length to a 5′ polymorphic Xho I site

Amongst the four common Ha-ras alleles in both controls and cancer patients, we detected the presence of a polymorphic Xho I site associated specifically with the 6·6 and 7·7 kb Bam HI fragments but absent from the 7·1 and 8·2 kb alleles, as recently reported by others. We have extended this study and report here, the consistent appearance of this Xho I site in unusual alleles close in size to the two common alleles of 6·6 and 7·7 kb, in control lymphoblastoid DNA samples in a variety of tumor DNAs. Unusual alleles grouped around the 7·1 and 8·2 kb common alleles on the other hand, did not possess the Xho I site. The consistent presence of the Xho I site polymorphism, in the unusual Ha-ras alleles surrounding the 6·6 and 7·7 kb common alleles and its absence in alleles around the 7·1 and 8·2 kb common alleles, suggests that the unusual ones are derived from the corresponding common alleles to which they are closest in size.
Analysis of herbal teas made from the leaves of comfrey (Symphytum officinale): reduction of N-oxides results in order of magnitude increases in the measurable concentration of pyrrolizidine alkaloids

Objectives:
To determine the relative quantities of two hepatotoxic pyrrolizidine alkaloids, symphytine and echimidine, in teas prepared from comfrey leaves (Symphytum officinale), and to determine the potential contribution of the N-oxide forms of these alkaloids to levels of the parent alkaloids.
Design:Comfrey leaves were purchased from three commercial sources and used to prepare tea in a manner consistent with the methods used by consumers. An extraction scheme was devised for extraction of the alkaloids, and a gas chromatographic method was developed to quantify the two major alkaloids, symphytine and echimidine. Recognising that the N-oxide derivatives of these alkaloids have also been identified in comfrey preparations, chemical reduction was applied to determine the total quantities of the alkaloids as free bases and as N-oxide derivatives.
Results:The concentration of symphytine and echimidine varied considerably between teas prepared from leaves purchased from the different vendors of plant material. Moreover, a much higher concentration of symphytine was found in the tea when steps were included to reduce N-oxides prior to analysis. The treatment of pure symphytine with hot water did not generate the N-oxide derivative de novo.
Conclusions:Since the pyrrolizidine alkaloids are known to be hepatotoxic, consumption of herbal teas made from comfrey leaves may be ill-advised. The concentration of pyrrolizidine alkaloids in such teas may be underestimated substantially unless the concentration of N-oxides is taken into consideration.
Stereological analysis of celery leaves infected by Septoria apiicola

Changes caused by Septoria apiicola during the development of leaf spot disease in celery were quantified stereologically in susceptible cvs, a partially resistant parental wild celery line, fully resistant parsley and lovage. Thick sections, cut in vertical orientation, were stained and using a camera lucida, were superimposed on a lattice grid for stereological analysis. The volume of each component within the leaf space was calculated using the Cavalieri method.
In healthy tissue volume fractions of intercellular space, palisade layers and, to a lesser extent, collenchyma, vascular tissue and oil ducts were greater in the more resistant genotypes. This coincided with a reduction in the volume fraction of mesophyll tissue. Estimates of absolute volume fraction were similar. In both resistant and susceptible celery the principal response to infection was a substantial decrease in the volume fractions and absolute volumes of palisade and mesophyll tissue. Volume fractions of mesophyll and palisade tissues declined by 97% in susceptible lesions and by 98% in resistant lesions. The fungus was not observed in xylem or phloem cells. No appreciable changes were observed in either collenchyma or oil duct tissues in response to infection in resistant and susceptible genotypes. Infection had no effect on the epidermis or stomata and these remained intact until late in the infection cycle, when pycnidia erupted through the epidermis of the upper and lower leaf surfaces.
No hyphae were found in leaf sections of lovage or parsley, and fungal growth was severely restricted in all resistant material bred from wild celery. In wild celery lines, S. apiicola did not produce any, or produced very few, pycnidia and the volume fraction of vegetative hyphae was lower than that in susceptible cvs, where the fungus proliferated extensively within the lesions and many pycnidia were produced. This accounted for 30–50% of the fungal material in lesions on susceptible celery plants. The amount of fungus (21%) was less in lesions on resistant celery and in this material 96% of the fungus was present as vegetative hyphae. In lesions formed on susceptible celery 26% of the fungal tissue was reproductive and of that 11% was conidia whereas in resistant celery, where pycnidia were present, these structures were morphologically abnormal and did not contain conidia. Stereological analysis demonstrated clear differences in plant and pathogen response during infection of resistant and susceptible celery by S. apiicola.
SOI Structures Formed Using Line-Source Electron Beam Recrystallization

Recrystallization of silicon-on-insulator (SOI) films using a line-source electron beam is described. This unique heat source can continuously emit several kilowatts of - 5 keV electrons into a beam 150 mm in length and - 2.5 mm in width, an exposure area which allows processing of 100 mm substrates in a single pass. An attractive aspect of this beam is the ability to control the beam profile, which in turn allows one to influence the thermal gradients present during recrystallization. Using a tightly focussed beam to recrystallize the SOI layer results in a film whose physical properties are generally attributed to films grown with a high thermal gradient at the solidifying liquid-solid interface (highly branched subboundaries with a maximum spacing of - 20 microns and several degrees of angular mismatch.) By reducing the gradient at the growth interface it is possible to achieve unbranched sub-boundaries with over 70 micron spacing and less than 0.5 degrees of out-of-plane tilt misalignment.
