Risk factors and outcomes associated with external ventricular drain infections

Background:
Insertion of an external ventricular drain (EVD) is a common neurosurgical procedure which may lead to serious complications including infection. Some risk factors associated with EVD infection are well established. Others remain less certain, including specific indications for placement, prior neurosurgery, and prior EVD placement.
Objective:To identify risk factors for EVD infections.
Methods:We reviewed all EVD insertions at our institution from March 2015 through May 2019 following implementation of a standardized infection control protocol for EVD insertion and maintenance. Cox regression was used to identify risk factors for EVD infections.
Results:479 EVDs placed in 409 patients met inclusion criteria, and 9 culture-positive infections were observed during the study period. The risk of infection within 30 days of EVD placement was 2.2% (2.3 infections/1,000 EVD days). Coagulase-negative staphylococci were identified in 6 of the 9 EVD infections). EVD infection led to prolonged length of stay post–EVD-placement (23 days vs 16 days; P = .045). Cox regression demonstrated increased infection risk in patients with prior brain surgery associated with cerebrospinal fluid (CSF) diversion (HR, 8.08; 95% CI, 1.7–39.4; P = .010), CSF leak around the catheter (HR, 21.0; 95% CI, 7.0–145.1; P = .0007), and insertion site dehiscence (HR, 7.53; 95% CI, 1.04–37.1; P = .0407). Duration of EVD use >7 days was not associated with infection risk (HR, 0.62; 95% CI, 0.07–5.45; P = .669).
Conclusion:Risk factors associated with EVD infection include prior brain surgery, CSF leak, and insertion site dehiscence. We found no significant association between infection risk and duration of EVD placement.

Three-dimensional magnetic reconnection in particle-in-cell simulations of anisotropic plasma turbulence

We use three-dimensional (3-D) fully kinetic particle-in-cell simulations to study the occurrence of magnetic reconnection in a simulation of decaying turbulence created by anisotropic counter-propagating low-frequency Alfvén waves consistent with critical-balance theory. We observe the formation of small-scale current-density structures such as current filaments and current sheets as well as the formation of magnetic flux ropes as part of the turbulent cascade. The large magnetic structures present in the simulation domain retain the initial anisotropy while the small-scale structures produced by the turbulent cascade are less anisotropic. To quantify the occurrence of reconnection in our simulation domain, we develop a new set of indicators based on intensity thresholds to identify reconnection events in which both ions and electrons are heated and accelerated in 3-D particle-in-cell simulations. According to the application of these indicators, we identify the occurrence of reconnection events in the simulation domain and analyse one of these events in detail. The event is related to the reconnection of two flux ropes, and the associated ion and electron exhausts exhibit a complex 3-D structure. We study the profiles of plasma and magnetic-field fluctuations recorded along artificial-spacecraft trajectories passing near and through the reconnection region. Our results suggest the presence of particle heating and acceleration related to small-scale reconnection events within magnetic flux ropes produced by the anisotropic Alfvénic turbulent cascade in the solar wind. These events are related to current structures of the order of a few ion inertial lengths in size.
A case-control study to identify risk factors for acute salmonellosis in New Zealand dairy herds, 2011–2012

In late 2011 the New Zealand Ministry for Primary Industries reported an increase in confirmed laboratory diagnoses of salmonellosis in dairy herds. To identify risk factors for herd-level outbreaks of salmonellosis we conducted a case-control study of New Zealand dairy herds in 2011–2012. In a multivariable analysis, use of continuous feed troughs [adjusted odds ratio (aOR) 6·2, 95% confidence interval (CI) 2·0–20], use of pelletized magnesium supplements (aOR 10, 95% CI 3·3–33) and use of palm kernel meal as a supplementary feed (aOR 8·7, 95% CI 2·5–30) were positively associated with a herd-level outbreak of salmonellosis between 1 July 2011 and 31 January 2012. We conclude that supplementary feeds used on dairy farms (regardless of type) need to be stored and handled appropriately to reduce the likelihood of bacterial contamination, particularly from birds and rodents. Magnesium supplementation in the pelletized form played a role in triggering outbreaks of acute salmonellosis in New Zealand dairy herds in 2011–2012.

Among 29 multiply affected pedigrees, age of onset among parents and offspring was examined for evidence of both anticipation and genomic imprinting. The results do not suggest that either mechanism is operating in schizophrenia, but only very large effects can be confidently excluded.

A susceptibility locus for schizophrenia in the ‘pseudo-autosomal’ region has been proposed on the basis of the reported excess of sex-chromosome aneuploidies (e.g. XXY and XXX) among patients with schizophrenia and the finding that schizophrenic sib-pairs are more often of the same than of the opposite sex. This hypothesis has been tested in 83 sibships with two or more siblings fulfilling Research Diagnostic Criteria for schizophrenia or schizoaffective disorder. Alleles at the pseudo-autosomal telomeric locus DXYS14, which is unlinked with sex, were analysed using the method of affected sib-pairs. Affected sibs shared alleles at DXYS14 more frequently than expected by random Mendelian assortment, supporting genetic linkage between DXYS14 and schizophrenia.
