Aboriginal Elders in Australia are recognised as having an important role as community leaders and cultural knowledge holders. However, the effects of colonisation and institutional racism mean Elders also experience significant social and economic disadvantage and poor health outcomes. There has been a systemic lack of attention to the worldviews and priorities of Aboriginal people as they age. In this article, we detail the findings of a qualitative study using a localised Aboriginal Elder-informed methodology that involved interviews and focus groups with 22 Aboriginal Elders in the remote town of Walgett on what ageing well means to them. This study was undertaken as part of a long-term partnership between a unique community-controlled Elders organisation and a university. The findings illuminate the barriers and enablers to ageing well for Aboriginal people in Walgett and elsewhere, and demonstrate the value to research, policy and service delivery of listening to and learning from Elders, centring Indigenous knowledges and worldviews, and bringing a more holistic conceptualisation of wellbeing to the understanding of what it means to age well.
Effectiveness of a peer-led adolescent mental health intervention on HIV virological suppression and mental health in Zimbabwe: protocol of a cluster-randomised trial

Background
Adolescents living with HIV (ALHIV) experience a high burden of mental health disorder which is a barrier to antiretroviral therapy adherence. In Zimbabwe, trained, mentored peer supporters living with HIV (Community Adolescent Treatment Supporters – CATS) have been found to improve adherence, viral suppression and psychosocial well-being among ALHIV. The Friendship Bench is the largest integrated mental health programme in Africa. We hypothesise that combining the CATS programme and Friendship Bench will improve mental health and virological suppression among ALHIV compared with the CATS programme alone.
MethodsWe will conduct a cluster-randomised controlled trial in 60 clinics randomised 1:1 in five provinces. ALHIV attending the control arm clinics will receive standard CATS support and clinic support following the Ministry of Health guidelines. Those attending the intervention arm clinics will receive Friendship Bench problem-solving therapy, delivered by trained CATS. Participants with the signs of psychological distress will be referred to the clinic for further assessment and management. The primary outcome is HIV virological failure (≥1000 copies/ml) or death at 48 weeks. Secondary outcomes include the proportion of adolescents with common mental disorder symptoms (defined as Shona Symptom Questionnaire (SSQ-14) score ≥8), proportion with depression symptoms (defined as Patient Health Questionnaire (PHQ-9) score ≥11), symptom severity (mean SSQ-14 and PHQ-9 scores) and EQ-5D score for health-related quality of life.
ConclusionsThis trial evaluates the effectiveness of peer-delivery of mental health care on mental health and HIV viral load among ALHIV. If effective this intervention has the potential to be scaled-up to improve these outcomes.

Phenomenological archaeologists and GIS scholars have turned much attention to visibility—who can see whom, and what can be seen—across ancient landscapes. Visible connections can be relatively easy to identify, but they present challenges to interpretation. Ancient peoples created intervisible connections among sites for purposes that included surveillance, defense, symbolism, shared identity, and communication. In the American Southwest, many high places are intervisible by virtue of the elevated topography and the open skies. The Chaco phenomenon, centered in northwestern New Mexico between A.D. 850 and 1140, presents an ideal situation for visibility research. In this study, we use GIS-generated viewsheds and viewnets to investigate intervisible connections among great houses, shrines, and related features across the Chacoan landscape. We demonstrate that a Chacoan shrine network, likely established during the mid-eleventh century, facilitated intervisibility between outlier communities and Chaco Canyon. It is most likely that the Chacoans created this network to enable meaningful connections for communication and identity. We conclude that the boundaries of the Chaco phenomenon are defined in some sense by intervisibility.

Background: Health care professionals are recommended to deliver patient-centered care in dementia; however, guidance and training on how to do this in practice is currently lacking. The aim of this study was to develop and evaluate pragmatically an educational intervention for old age psychiatrists to promote patient-centered care in their consultations with people with dementia and their carers.
Methods: We used a range of methods to (i) identify the theoretical components of patient-centered care (literature review) and (ii) observe actual practice (video recording of 53 consultations between old age psychiatrists and people with dementia and their family carers). We also interviewed participants from (ii) including 7 old age psychiatrists, 25 people with dementia and 44 carers. From this we developed a workshop for old age psychiatrists and piloted and evaluated it. Pre- and post-workshop questionnaires were completed; the latter included an assessment of planned and subsequent behavior change by participants.
Results: The educational workshop, attended by 41 old age psychiatrists, focused on how best to structure the consultation and the most effective communication skills to use in consultation with people with dementia. Three months after the workshop, 59% had made one or more changes to the structure of their consultations, 71% had used new communication skills and 56% had reflected further on their practice.
Conclusions: We developed an educational intervention with both a theoretical and empirical basis. The workshops resulted in many changes to self-reported practice; whether this was noticeable to patients and carers requires further study.

Background
Earlier work on the protective role of social support in onset and course of depressive disorder suggested that its provision might be relevant for outcome.
AimsTo evaluate volunteer befriending as an intervention among women with chronic depression in inner London.
MethodA randomised controlled trial, with awaiting list control design, with outcome measured as Present State Examination (PSE)-based full or partial remission after one year.
ResultsA statistically significant effect upon remission was found for befriending. Other treatments monitored naturalistically did not relate to remission, nor did initial duration of chronic episode or comorbidity, but there was an association with initial PSE score. The findings are discussed in relation to standard drug-trial results and to another befriending intervention with the elderly.
ConclusionsThe results are not unpromising. Additional trials with less restricted intake conditions, and in more naturalistic general practice settings, might confirm volunteer befriending as a useful adjunct to current treatments.

Background
Volunteer befriending promoted remission of chronic depression when clinical and other treatment variables were controlled.
AimsTo examine the role of other psychosocial factors relevant for outcome.
MethodFactors measured at baseline interview were examined in multivariate analyses along with psychosocial factors occurring during follow-up, such as ‘fresh-start’ experiences and new severe events and difficulties.
ResultsFresh-start experiences and a standard attachment style were found to enhance chances of remission, with new severe stressors and markedly poor coping strategies liable to prevent it, with volunteer befriending continuing to play a role.
ConclusionsThe positive result reported in the preceding paper is unlikely to be an artefact. However, fresh-start experiences, absence of new severe stressors and standard attachment style were more important predictors of remission. This knowledge might profitably be incorporated into the evaluation of existing treatments.

Summary
All cells transport proteins across membranes, but the complexity of protein traffic in plant cells is especially striking because of the variety of organelle types involved. Many proteins are inserted, during translation, into the lumen of the endoplasmic reticulum, after which they are transported via the endomembrane system to the Golgi apparatus, vacuole, protein bodies or plasma membrane. Other proteins are transported posttranslationally into glyoxysomes, mitochondria and plastids. In each case, the protein is synthesised with an appropriate signal which ensures targeting to the correct organelle, and a number of studies have attempted to define the characteristics of these targeting signals (reviewed by Bennett & Osteryoung, 1991; Robinson, 1991).
In terms of protein transport events, the biogenesis of the chloroplast is particularly complex, primarily owing to the architecture of the organelle. The chloroplast is bounded by a double-membrane envelope, between whose membranes is a soluble phase, the functions of which are presently obscure. Within the organelle is the soluble stromal phase (site of CO2 fixation, amino acid synthesis and many other key reactions) and the extensive internal thylakoid membrane. The thylakoid network also encloses a further soluble phase, usually termed the thylakoid lumen. Thus, the chloroplast comprises in total three distinct membranes and three discrete soluble phases. Most of the proteins located in each of these organellar compartments are encoded by nuclear genes, synthesised in the cytosol, and transported into the organelle. Clearly, therefore, chloroplast biogenesis requires both the specific, efficient targeting of a large number proteins into the organelle, and the operation of intraorganellar ‘sorting’ mechanisms to distribute imported proteins to their correct destinations.

Total glossectomy is the most crippling of all oral cavity resections. Speech problems are unavoidable and post-operative aspiration is often severe. Two patients are presented who have had tongue reconstructions following near-total glossectomies. In each case a quilted, split-skin grafted pectoralis major muscle flap was used. The speech and swallowing results of these two patients are compared with two patients who underwent a hemiglossectomy and total glossectomy respectively without tongue reconstruction. Only if the suprahyoid and extrinsic tongue muscles are preserved can an active oral phase of swallowing and usable speech be regained by tongue reconstruction.
