P.087 Early and late exercise intervention after lumbar microdiscectomy reduces low back pain, fear avoidance, and improve neurodynamic mobility

Background: Exercise is commonly recommended to patients following a lumbar microdiscectomy although controversy remains as to the timing and protocols for exercise intervention (early vs late intervention). Our study aimed to evaluate low back pain level, fear avoidance, neurodynamic mobility, and function after early versus later exercise intervention following a unilateral lumbar microdiscectomy. Methods: Forty patients who underwent unilateral lumbar microdiscectomy were randomly allocated to early (Group-1) or later (Group-2) exercise intervention group. The low back pain and fear avoidance were evaluated using Oswestry Low Back Pain Disability Questionnaire, Numeric Pain Rating Scale, and Fear-Avoidance Beliefs Questionnaire. The neurodynamic mobility and function were recorded with Dualer Pro IQ Inclinometer, 50-foot walk test, and Patient-Specific Functional Scale. Measurements were performed before surgery and post-surgery (1-2, 4-6, and 8-10 weeks) after exercise intervention. Results: Both groups showed a significant decrease in low back pain levels and fear avoidance as well as a significant improvement in neurodynamic mobility and function at 4 and 8 weeks post-surgery. No significant difference was detected between the two groups. Conclusions: These findings showed that early exercise intervention after lumbar microdiscectomy is safe and may reduce the low back pain, decrease fear avoidance, and improve neurodynamic mobility and function.

Background: Odontoidectomy for basilar invagination and craniovertebral junction pathology has traditionally been performed using a transoral route. However, the endoscopic endonasal approach to the anterior craniovertebral junction may offer safer and more effective access when compared to transoral approaches. Methods: This study is a retrospective chart review of all adult patients who underwent an endoscopic endonasal odontoidectomy at a single tertiary care center between January 2011 and May 2019. Results: Seventeen patients were included in the study. The median admission age was 67 years (range: 33-84 years) and 65% of the patients were female. One patient (1/17, 6%) had vertebral artery injury which was coiled with no neurological deficits, and 4 patients (4/17, 24%) had intraoperative CSF leaks with no postoperative leak. Fourteen patients (14/17, 82%) were extubated by POD 1. Three patients (3/17, 18%) developed postoperative sinus infections and required antibiotics. Eight patients (8/17, 47%) developed transient postoperative dysphagia. One patient (1/17, 6%) had postoperative epistaxis and one patient (1/17, 6%) had postoperative lower cranial nerve symptoms. The median length of hospital stay was 13 days (range: 2-44 days). Conclusions: Endoscopic endonasal odontoidectomy is a feasible and well-tolerated procedure for anterior decompression of craniovertebral junction, associated with satisfactory patient outcomes and low morbidity.

We analysed associations between exposure to nightlife businesses and severe acute respiratory syndrome coronavirus 2 PCR test results at a tertiary hospital in Tokyo between March and April 2020. A nightlife group was defined as those who had worked at or visited the businesses. We included 1517 individuals; 196 (12.9%) were categorised as the nightlife group. After propensity score matching, the proportion of positive PCR tests in the nightlife group was significantly higher than that in the non-nightlife group (nightlife, 63.8%; non-nightlife, 23.0%; P < 0.001). An inclusive approach to mitigate risks related to the businesses needs to be identified.

Objective
The objective of this study was to explore factors associated with hara-kiri as a method of suicide and suicidal behavior in contemporary Japan.
MethodsA retrospective study was conducted on medical records of 421 patients (174 male; 247 female) who were considered suicidal and treated at the Kitasato University Hospital Emergency Medical Center in Japan between January 2006 and March 2008. We compared hara-kiri and all other methods regarding sociodemographics and clinical features of all suicidal patients.
ResultsInstances of hara-kiri suicide attempt had the highest proportion of males (63%) among all suicide and suicidal behavior. One-way analysis of variance revealed significant differences between hara-kiri and other suicide attempt methods in the age of the suicidal patients. Result of multiple logistic regression analysis indicated that those who attempted hara-kiri suicide were likely to be male, be diagnosed with schizophrenia, survive, and be married.
ConclusionOur findings indicate that hara-kiri as a method of suicide and suicidal behavior remain prevalent in Japan, and the study findings also suggest that both clinical and cultural factors might play a role in hara-kiri as a method of suicide and suicidal behavior.
Analysis of lambing distribution in the Ripollesa sheep breed. I. Development and comparison of circular von Mises models

Circular data originates in a wide range of scientific fields and can be analyzed on the basis of directional statistics and special distributions wrapped around the circumference. However, both propensity to transform non-linear to linear data and complexity of directional statistics limited the generalization of the circular paradigm in the animal breeding framework, among others. Here, we generalized a circular mixed (CM) model within the context of Bayesian inference. Three different parametrizations with different hierarchical structures were developed on basis of the von Mises distribution; moreover, both goodness of fit and predictive ability from each parametrization were compared through the analyses of 110 116 lambing distribution records collected from Ripollesa sheep herds between 1976 and 2017. The naive circular (NC) model only accounted for population mean and homogeneous circular variance, and reached the lowest goodness-of-fit and predictive ability. The CM model assumed a hierarchical structure for the population mean by accounting for systematic (ewe age and lambing interval) and permanent environmental sources of variation (flock-year-season and ewe). This improved goodness of fit by reducing both the deviance information criterion (DIC; −2520 units) and the mean square error (MSE; −12.4%) between simulated and predicted lambing data when compared against the NC model. Finally, the last parametrization expanded CM model by also assuming a hierarchical structure with systematic and permanent environmental factors for the variance parameter of the von Mises distribution (i.e. circular canalization (CC) model). This last model reached the best goodness of fit to lambing distribution data with a DIC estimate 5425 units lower than the one for NC model (MSE reduced 13.2%). The same pattern revealed when models were compared in terms of predictive ability. The superiority revealed by CC model emphasized the relevance of heteroskedasticity for the analysis of lambing distribution in the Ripollesa breed, and suggested potential applications for the sheep industry, even genetic selection for canalization. The development of CM models on the basis of the von Mises distribution has allowed to integrate flexible hierarchical structures accounting for different sources of variation and affecting both mean and dispersion terms. This must be viewed as a useful statistical tool with multiple applications in a wide range of research fields, as well as the livestock industry. The next mandatory step should be the inclusion of genetic terms in the hierarchical structure of the models in order to evaluate their potential contribution to current selection programs.

Seasonal reproduction patterns are typically observed in small ruminants and are a major limitation for production efficiency in most meat- and dairy-type production systems. Indeed, selection for reduced seasonality could be an appealing strategy for the small ruminant industry worldwide, although its genetic background has been poorly analyzed. One of the main limitations relied on the availability of appropriate analytical tools to cope with the circular (i.e. year-round) pattern of lambing and kidding data. The recent development of a heteroskedastic circular mixed model provided the statistical tool to go deeply into the knowledge of seasonality in small ruminants. In this study, 26 005 lambing distribution records from 4764 Ripollesa ewes collected in 20 purebred flocks were analyzed. The model accounted for systematic (lambing interval and ewe age), permanent environmental (flock-year-season and ewe) and additive genetic sources of variation influencing both mean and dispersion pattern (i.e. heteroskedasticity). Systematic effects suggested that first-lambing ewes and short lambing intervals delayed lambing date (~30 days) and increased dispersion of the lambing period. Nevertheless, this was partially compensated by ewe age, given that youngest females tended to concentrate the lambing peak. Flock-year-season, permanent ewe and additive genetic sources of variation reached moderate variance components for direct (and residual) effects on lambing distribution, they being 0.119 (0.156), 0.092 (0.132) and 0.195 (0.170) radians2, respectively. Moreover, all 95% credibility intervals were placed far from the null estimate. Covariances between direct and residual effects where high and positive for additive genetic (posterior mean, 0.814) and permanent ewe effects (posterior mean, 0.917), whereas it was not relevant for flock-year-season. Selection for direct additive genetic effects should be able to advance or delay the lambing peak, whereas selection applied on residual additive genetic effects should increase or reduce seasonality (i.e. concentrate or flatten the lambing peak). Moreover, the positive and relevant genetic covariance between direct and residual effects also suggested correlated genetic responses. As example, genetic selection for earlier lambing peaks must also reduce seasonality, whereas selection for narrower lambing seasons may originate a delay in the lambing peak. These results must be viewed as the first attempt to analyze systematic, environmental and genetic sources of variation of lambing distribution within the circular paradigm, they providing a reliable characterization of these effects within the context of an heteroskedastic model.
P.057 A systematic review of the prophylactic antibiotic use in endoscopic endonasal transsphenoidal surgery for pituitary lesions

Background: The benefit of prophylactic antibiotic use in endoscopic endonasal transsphenoidal surgery (EETS) for pituitary lesions is controversial. Many surgeons administer antibiotics perioperatively not based on clear guidelines but “to be safe”. Methods: A systematic review using PRISMA guidelines was performed to assess the efficacy of perioperative antibiotic use to prevent infectious complications in patients undergoing EETS. Inclusion criteria: randomized controlled trials, systematic reviews, observational studies, and case series. Data extracted: study design, year of publication, sample size, surgery type, perioperative antibacterial treatment (antibiotic, dose, and duration), number of patients with 30-days post-operative meningitis and/or sinusitis. End points: rates of meningitis and sinusitis post-EETS. Results: A total of 280 articles were identified. Four observational studies met inclusion criteria. Based on GRADE score these studies were considered low in quality. 633 patients were -included in those studies. The most common antibiotics used were cefazolin and ceftazidime. The rate of infection ranged from 0.5% to 3.1 % for meningitis as the most common infection. Conclusions: The need to use antibiotic(s) perioperatively is not clear in patients with pituitary lesions undergoing EETS. Randomized control trials are needed to evaluate the efficacy of prophylactic antibiotic use in patients with pituitary lesions undergoing EETS.

Learning Task Strategies in Robotic Assembly Systems

This paper presents a practical method for generating task strategies applicable to chamferless and high-precision assembly. The difficulties in devising reliable assembly strategies result from various forms of uncertainty such as imperfect knowledge on the parts being assembled and functional limitations of the assembly devices.
In order to cope with these problems, the robot is provided with the capability of learning the corrective motion in response to the force signal through iterative task execution. The strategy is realized by adopting a learning algorithm and is represented in a binary tree-type database. To verify the effectiveness of the proposed algorithm, a series of experiments are carried out under simulated real production conditions. The experimental results show that sensory signal-to-robot action mapping can be acquired effectively and, consequently, the assembly task can be performed successfully.
Experimental and numerical studies of an eastward jet over topography

Motivated by the phenomena of blocked and zonal flows in Earth's atmosphere, we conducted laboratory experiments and numerical simulations to study the dynamics of an eastward jet flowing over wavenumber-two topography. The laboratory experiments studied the dynamical behaviour of the flow in a barotropic rotating annulus as a function of the experimental Rossby and Ekman numbers. Two distinct flow patterns, resembling blocked and zonal flows in the atmosphere, were observed to persist for long time intervals.
Earlier model studies had suggested that the atmosphere's normally upstream- propagating Rossby waves can resonantly lock to the underlying topography, and that this topographic resonance separates zonal from blocked flows. In the annulus, the zonal flows did indeed have super-resonant mean zonal velocities, while the blocked flows appear subresonant. Low-frequency variability, periodic or irregular, was present in the measured time series of azimuthal velocity in the blocked regime, with dominant periodicities in the range of 6–25 annulus rotations. Oscillations have also been detected in zonal states, with smaller amplitude and similar frequency. In addition, over a large region of parameter space the two flow states exhibited spontaneous, intermittent transitions from the one to the other.
We numerically simulated the laboratory flow geometry in a quasi-geostrophic barotropic model over a similar range of parameters. Both flow regimes, blocked and zonal, were reproduced in the simulations, with similar spatial and temporal characteristics, including the low-frequency oscillations associated with the blocked flow. The blocked and zonal flow patterns are present over wide ranges of forcing, topographic height, and bottom friction. For a significant portion of parameter space, both model flows are stable. Depending on the initial state, either the blocked or the zonal flow is obtained and persists indefinitely, showing the existence of multiple equilibria.

Lattice polarities and film qualities of GaN grown by rf-MBE were investigated concentrating on the use of different buffer layer processes at the initial stage. Direct clarifying by coaxial impact collision ion scattering spectra technique, together with RHEED and chemical wet etching, were applied to identify the lattice polarity of GaN films. XRD rocking curve and photoluminescence results showed that the qualities of GaN films with Ga-polarity were dramatic improved compared to those with N-polarity. Hall effect measurement results indicated that the mobility of the Ga-face film was increased to one order higher (568 cm2/Vs in maximum at room temperature) than that of N-face one.
Dietary modifications of the biliary bile acid glycine: taurine ratio and activity of hepatic bile acid-CoA:amino acid N-acyltransferase (EC 2.3.1) in the rat

Effects of dietary manipulations on the biliary bile acid glycine: taurine (G: T) ratio and the activity of hepatic bile acid-Co A: amino acidN-acyltransferase (EC 2.3.1) in the post-mitochondrial fraction of liver homogenates were examined in the rat. The G:T ratio in rats fed on the diet containing 100 g pectin/kg (2.18) was markedly higher than that in the animals fed on the diet containing 100 g cellulose/kg (0.09). The diets containing either 10 g cholesterol/kg or 5 g sodium cholate/kg, especially the latter, also increased the G:T ratio (0.77 and 2.33 respectively) compared with a control diet free of these steroids (0.34). When the saturating concentrations of taurine (20 mM) and glycine (100 mM) were the substrates, dietary pectin relative to cellulose significantly increased the activity of both taurine- and glycine-dependent bile acid-CoA: amino acidN-acyltransferase, but neither dietary bile acid nor cholesterol influenced it. In spite of the marked difference in the G:T ratio among the rats given various types of experimental diet, the bile acid-CoA: amino acid N-acyltransferase reaction produced taurine- but little glycine-conjugated bile acid when both taurine and glycine coexisted at physiological concentration ranges in the assay media. Dietary manipulations modified the hepatic taurine concentrations and the changes were inversely correlated with those in the G: T ratio. However, hepatic concentration of taurine (1.67–4.82 μmol/g) in rats given various types of experimental diet was comparable with or even higher than the reported Michaelis constant (Km) value ofN-acyltransferase for this compound (0.8–2.5 mM). In contrast, glycine concentrations (1.81–2.58 μmol/g) were much lower than the Km value for this amino acid (35–40 mM) under various dietary conditions. Thus, neither the substrate specificity of the bile acid conjugation enzyme nor the alteration in the hepatic concentration of taurine or glycine accounted for the change in the G: T ratio in the present study.

The feeding habits and infestation of the bitterleaf weevil, Lixus camerunus Kolbe, were investigated in Rivers State, Nigeria. Vernonia amygdalina was the only species preferred for feeding and oviposition by adult L. camerunus, among the species (Talinum triangulare (Jacq.) Wild, Telfairia occidenlalis Hooker fil, Amaranthus hybridus L., Solanum melongena, Capsicum sp., Chromalaena odorata (Eupatorium odoralum) L., Emilia praetermissa L.) associated with V. amygdalina in farms, that were offered. There was a significant correlation between dry weight of foliage consumed and weight of males and females.
Initial tunnelling direction by larvae was upwards, but on the death of shoots, larvae reversed direction and moved downwards along old tunnels. However, newly-emerged larvae in leaf petioles, tunnelled downwards to the main branch, before pursuing an upward course. In addition to main tunnels, there were also branch tunnels. There was a significant negative correlation between length of tunnel and stem diameter. There was a significant negative correlation between gall/stem diameter ratio and stem diameter.
L. camerunus is widely distributed in the State.
