Addressing personal protective equipment (PPE) decontamination: Methylene blue and light inactivates severe acute respiratory coronavirus virus 2 (SARS-CoV-2) on N95 respirators and medical masks with maintenance of integrity and fit

Objective:
The coronavirus disease 2019 (COVID-19) pandemic has resulted in shortages of personal protective equipment (PPE), underscoring the urgent need for simple, efficient, and inexpensive methods to decontaminate masks and respirators exposed to severe acute respiratory coronavirus virus 2 (SARS-CoV-2). We hypothesized that methylene blue (MB) photochemical treatment, which has various clinical applications, could decontaminate PPE contaminated with coronavirus.
Design:The 2 arms of the study included (1) PPE inoculation with coronaviruses followed by MB with light (MBL) decontamination treatment and (2) PPE treatment with MBL for 5 cycles of decontamination to determine maintenance of PPE performance.
Methods:MBL treatment was used to inactivate coronaviruses on 3 N95 filtering facepiece respirator (FFR) and 2 medical mask models. We inoculated FFR and medical mask materials with 3 coronaviruses, including SARS-CoV-2, and we treated them with 10 µM MB and exposed them to 50,000 lux of white light or 12,500 lux of red light for 30 minutes. In parallel, integrity was assessed after 5 cycles of decontamination using multiple US and international test methods, and the process was compared with the FDA-authorized vaporized hydrogen peroxide plus ozone (VHP+O3) decontamination method.
Results:Overall, MBL robustly and consistently inactivated all 3 coronaviruses with 99.8% to >99.9% virus inactivation across all FFRs and medical masks tested. FFR and medical mask integrity was maintained after 5 cycles of MBL treatment, whereas 1 FFR model failed after 5 cycles of VHP+O3.
Conclusions:MBL treatment decontaminated respirators and masks by inactivating 3 tested coronaviruses without compromising integrity through 5 cycles of decontamination. MBL decontamination is effective, is low cost, and does not require specialized equipment, making it applicable in low- to high-resource settings.
Temporal effects of maternal psychological distress on child mental health problems at ages 3, 5, 7 and 11: analysis from the UK Millennium Cohort Study

Background
Psychological distress is common among women of childbearing age, and limited longitudinal research suggests prolonged exposure to maternal distress is linked to child mental health problems. Estimating effects of maternal distress over time is difficult due to potential influences of child mental health problems on maternal distress and time-varying confounding by family circumstances.
MethodsWe analysed the UK Millennium Cohort Study, a nationally representative sample with data collected throughout childhood. Adopting a marginal structural modelling framework, we investigated effects of exposure to medium/high levels of maternal psychological distress (Kessler-6 score 8+) on child mental health problems (Strengths and Difficulties Questionnaire borderline/abnormal behaviour cut-off) using maternal and child mental health data at 3, 5, 7 and 11 years, accounting for the influence of child mental health on subsequent maternal distress, and baseline and time-varying confounding.
ResultsPrior and concurrent exposures to maternal distress were associated with higher levels of child mental health problems at ages 3, 5, 7 and 11 years. For example, elevated risks of child mental health problems at 11 years were associated with exposure to maternal distress from 3 years [risk ratio (RR) 1.27 (95% confidence interval (CI) 1.08–1.49)] to 11 years [RR 2.15 (95% CI 1.89–2.45)]. Prolonged exposure to maternal distress at ages 3, 5, 7 and 11 resulted in an almost fivefold increased risk of child mental health problems.
ConclusionsPrior, concurrent and, particularly, prolonged exposure to maternal distress raises risks for child mental health problems. Greater support for mothers experiencing distress is likely to benefit the mental health of their children.

Summary
The first year of life is increasingly regarded as a ‘critical’ stage of a child's development and of emerging family relationships (Carnegie, 1994). The most frequently studied early indicators of child development include biological factors such as illness at birth, low birthweight and physical disability. In this chapter, we focus on indicators of early child development as measured by developmental delays in gross motor and fine motor development, as well as the development of communicative gestures. The questions used to identify milestones in these domains of development are set out in Table 6.1. Gross motor skills include sitting up, standing and walking; fine motor skills reflect ability to use hands and fingers; communicative gestures include smiling, waving goodbye and other signals to other people. Motor development is generally considered to be an important indicator of the overall rate and level of development during the early months and years after birth (Gesell, 1973; Illingworth, 1975). It influences the nature and sequence in which certain perceptual and cognitive abilities unfold, and the lack of motor competence may hinder the development of those abilities (Bushnell and Boudreau, 1993). There are rapid improvements in locomotor and manipulative skills during the first year of life while gestural language develops at around 9 or 10 months when babies begin to use gestures to ask for things (Bates et al, 1987; Bee, 1994).
Child development is assessed by a set of functional skills or age-specific tasks, called developmental milestones, that most children can do at a certain age range. The study of child development in the MCS is based on a socioecological model of human development (Bronfenbrenner, 1979; Sameroff, 1983). Such models underscore the role of multiple influences on children's development, and they conceptualise human development as the active interaction between a child and its context or environment. Developmental capacities, therefore, are thought to be shaped by the child's interactions with their parents and the wider context of which both are part.
Plan of this chapter
The aim of this chapter is to investigate the relationship between the child's development and the family environment, examining the relative influences of child, family and environmental factors in shaping individual development during the first year of life.
Synthesis and Nonlinearity of Triene Chromophores Containing the Cyclohexene Ring Structure

A series of conjugated donor-acceptor trienes in which the central double bond is incorporated into an unsaturated isophorone, verbenone or chromone ring has been synthesized. In each case, the donor group consists of an amine and an aromatic or heterocyclic ring system, and the acceptor is the dicyanomethylidene group. The nonlinear optical properties of each of the compounds has been measured and correlated with its structure. The dipole moments and molecular hyperpolarizabilities of these compounds, like those of other conjugated polyenes, are large enough to be used as the active components of electro-optic polymers. Unlike other donor-acceptor polyenes, however, these compounds exhibit the thermal stability required for such applications.
