Effect of the COVID-19 Pandemic on the Pediatric Emergency Department Flow

Objective:
The aim of this study was to determine how the early stages of the coronavirus disease 2019 (COVID-19) pandemic affected the use of the pediatric emergency department (PED).
Methods:Cross-sectional study of PED visits during January through April, 2016-2020. Data included: total PED visits, emergency severity index (ESI), disposition, chief complaint, age (months), time from first provider to disposition (PTD), and PED length of stay (PED-LOS). P-value <0.01 was statistically significant.
Results:In total, 67,499 visits were reported. There was a significant decrease in PED visits of 24-71% from March to April 2020. Chief complaints for fever and cough were highest in March 2020; while April 2020 had a shorter mean PED-LOS (from 158 to 123 min), an increase of admissions (from 8% to 14%), a decrease in ESI 4 (10%), and an increase in ESI 3 (8%) (P < 0.001). There was no difference in mean monthly PTD time.
Conclusions:Patient flow in the PED was negatively affected by a decrease in PED visits and increase in admission rate that may be related to higher acuity. By understanding the interaction between hospital processes on PEDs and patient factors during a pandemic, we are able to anticipate and better allocate future resources.
AN INTERCOMPARISON PROJECT ON 14C FROM SINGLE-YEAR TREE RINGS

A laboratory intercomparison project was carried out on 20 annually resolved late-wood samples from the Danish oak record. The project included the following three laboratories: (1) the University of Arizona AMS Laboratory, University of Arizona, USA (AA); (2) HEKAL AMS Laboratory, MTA Atomki, Hungary (DeA); and (3) Aarhus AMS Centre (AARAMS), Aarhus University, Denmark (AAR). The large majority of individual data points (96%) lie within ±2σ of the weighted mean. Further assessment of the accuracy associated with the individual laboratories showed good agreement, indicating that consistent and reliable 14C measurements well in agreement with each other are produced at the three laboratories. However, the quoted analytical uncertainties appear to be underestimated when compared to the observed variance of differences from the geometric mean of the samples. This study provides a general quality check of the single-year tree-ring 14C measurements that are included in the new calibration curve.

This article offers the justification for a type of carbon customs union in which countries with a diverse range of high-ambition domestic climate measures would adopt a common external tariff on carbon intensive imports from countries outside the union. We explain why any pragmatic approach to carbon border measures (CBMs) is likely to create problems under prevailing interpretations of the World Trade Organization's (WTO) primary rules. Given the urgency of the climate crisis and the fact that the legality of a CBM – no matter how designed – will be a question of first impression for any WTO dispute panel, WTO consistency should take a backseat to considerations of domestic legal and political feasibility in designing a CBM. Instead of trying to fit a CBM into current understandings of WTO rules, WTO members should renegotiate WTO rules to permit a range of aggressive, but likely trade-restrictive, decarbonization measures. Having said that, we also recognize that WTO negotiations may take longer than optimal from a decarbonization standpoint. We therefore suggest an approach that would allow WTO members to take advantage of flexibilities that existing WTO rules afford.

In 1900, a syndicate of investors used open market purchases and manipulative trading strategies to exploit an ongoing financial crisis at the Third Avenue Railroad Company and stealthily gain control of the company. The acquisition occurred during the first great merger wave in U.S. history and represented the street railway industry’s response to a new technology, namely electrification. The lax regulatory environment of the period allowed operators and insiders to profit handsomely and may have benefited consumers, but possibly harmed some minority shareholders. Our case study illuminates an unusual acquisition, when capital markets were less transparent.
More Rapid 14C Excursions in the Tree-Ring Record: A Record of Different Kind of Solar Activity at About 800 BC?

Two radiocarbon (14C) excursions are caused by an increase of incoming cosmic rays on a short time scale found in the Late Holocene (AD 774–775 and AD 993–994), which are widely explained as due to extreme solar proton events (SPE). In addition, a larger event has also been reported at 5480 BC (Miyake et al. 2017a), which is attributed to a special mode of a grand solar minimum, as well as another at 660 BC (Park et al. 2017). Clearly, other events must exist, but could have different causes. In order to detect more such possible events, we have identified periods when the 14C increase rate is rapid and large in the international radiocarbon calibration (IntCal) data (Reimer et al. 2013). In this paper, we follow on from previous studies and identify a possible excursion starting at 814–813 BC, which may be connected to the beginning of a grand solar minimum associated with the beginning of the Hallstatt period, which is characterized by relatively constant 14C ages in the period from 800–400 BC. We compare results of annual 14C measurements from tree rings of sequoia (California) and cedar (Japan), and compare these results to other identified excursions, as well as geomagnetic data. We note that the structure of the increase from 813 BC is similar to the increase at 5480 BC, suggesting a related origin. We also assess whether there are different kinds of events that may be observed and may be consistent with different types of solar phenomena, or other explanations.

The NeuroStar Outcomes Registry
ObjectiveNeuroStar transcranial magnetic stimulation (TMS) is an effective acute treatment for patients with major depressive disorder (MDD). In order to further understand use of the NeuroStar in a clinical setting, Neuronetics has established a patient treatment and outcomes registry to collect and analyze utilization information on patients receiving treatment with the NeuroStar.
MethodsIndividual NeuroStar providers are invited to participate in the registry and agree to provide their de-identified patient treatment data. The NeuroStar has an integrated electronic data management system (TrakStar) which allows for the data collection to be automated. The data collected for the registry include Demographic Elements (age, gender), Treatment Parameters, and Clinical Ratings. Clinical assessments are: Clinician Global Impression - Severity of Illness (CGI-S) and thePatient Health Questionnaire 9-item (PHQ-9). De-identified patient data is uploaded to Registry server; an independent statistical service then creates final data reports.
ResultsOver 500 patients have entered the NeuroStar Outcomes Registry since Sept 2016. Mean patient age: 48.0 (SD±16.0); 64% Female. Baseline PHQ-9, mean 18.8 (SD±5.0.) Response/Remission Rate, PHQ-9: 61%/33% CGI-S: 78%/59%.
ConclusionsFor the initial 500 patients in the Outcomes Registry, approximately 2/3 patients achieve respond and 1/3 patients achieve remission with an acute course of NeuroStar. These treatment outcomes consistent with NeuroStar open-label study data (Carpenter, 2012). The TrakStar data management system makes large scale data collection feasible. The NeuroStarOutcomes Registry is ongoing, and expected to reach 6000 outpatients from more than 47 clinical sites in 36 months.

Effects of Starch Encapsulation on Clomazone and Atrazine Movement in Soil and Clomazone Volatilization

The effects of formulation on clomazone volatilization and transport through soil were studied. After 22 days of leaching under unsaturated flow in 49-cm long intact soil cores, greater clomazone movement was observed in Plainfield sand than in Cisne silt loam or Drummer silty clay loam soils. Soil clomazone concentrations resulting in injury to oats occurred throughout Plainfield soil cores but were restricted to the upper 14 cm of Cisne and Drummer soils. In addition, clomazone was detected in the leachate from Plainfield soil only. In a similar study with Plainfield sand cores, clomazone was less mobile than atrazine; encapsulation of the herbicides in starch granules did not affect clomazone movement but greatly decreased atrazine movement from the soil surface. Similarly, starch encapsulation did not affect bioavailability of clomazone but did reduce bioavailability of atrazine. In a laboratory study with continual air flow, volatilization of clomazone applied to the soil surface was reduced by encapsulation in starch and starch/clay granules. Clomazone volatilization was not affected by soil water content within a range of 33 to 1500 kPa water tension. Following soil saturation with water, clomazone volatilization from both liquid and granular formulations increased. Granule size appeared to have a greater impact than granule composition on clomazone volatilization.
Effects of Paraquat and Alachlor on Peanut (Arachis hypogaea) Growth, Maturity, and Yield

Studies were conducted in 1988, 1989, and 1992 in Plains, GA to measure effects of paraquat and alachlor on ‘Florunner’ peanut. Peanut treated with paraquat (0.14 kg ai/ha) plus alachlor (3.4 kg ai/ha) applied at vegetative emergence (VE), or paraquat plus alachlor VE followed by paraquat 28 days after emergence (DAE) were compared with a nontreated control. Both herbicide treatments reduced peanut foliage biomass at 65 DAE in 1989 and 1992. Herbicide treatments did not affect foliage biomass 90 DAE in 1988 and 122 DAE in 1989, but paraquat plus alachlor followed by paraquat reduced foliage biomass at 122 DAE in 1992. Pod biomass, measured at 90 and 65 DAE in 1988 and 1992, respectively, was reduced by herbicides. However, pod biomass did not differ among treatments 122 DAE in 1989 and 1992. Percent reflectance from the peanut canopy measured no effects from herbicides in 1988. However, in 1989 and 1992 herbicides applied sequentially reduced peanut canopy development. Peanut treated with a single herbicide and sequentially took longer to mature. Once optimum maturity was reached, peanut yields were not reduced.
Soil microbial and nematode communities as affected by glyphosate and tillage practices in a glyphosate-resistant cropping system

Field experiments were conducted at Ashland Bottoms in northeastern Kansas and at Hays in western Kansas in 2001, 2002, and 2003 to determine the response of soil microbial and nematode communities to different herbicides and tillage practices under a glyphosate-resistant cropping system. Conventional herbicide treatments were a tank mixture of cloransulam plus S-metolachlor plus sulfentrazone for soybean and a commercially available mixture of acetochlor and atrazine for corn. Glyphosate was applied at 1.12 kg ai ha−1 when weeds were 10 or 20 cm tall in both corn and soybean. Soil samples were collected monthly at Ashland Bottoms during the growing period for soil microbial biomass (SMB) carbon determination. In addition, substrate-induced respiration (SIR) and BIOLOG substrate utilization were determined at the end of the growing season each year at Ashland Bottoms, and nematode populations were determined at the beginning and the end of the growing season at both sites. Direct effects of glyphosate rates on soil microbial and nematode communities were also studied in a controlled environment. Values for SMB carbon, SIR, and BIOLOG substrate utilization were not altered by glyphosate. Nematode community response to the glyphosate treatment was similar under both conventional tillage and no-till environments. Total nematode densities were similar with the glyphosate and conventional herbicide treatments. SMB carbon and BIOLOG substrate utilization did not differ between tillage treatments. Nematode densities were greater under conventional tillage than in the no-till system. This study showed that soil health when glyphosate was applied in a glyphosate-resistant cropping system was similar to that of cropping systems that used conventional herbicides.

Two radiocarbon excursions (AD 774–775 and AD 993–994) occurred due to an increase of incoming cosmic rays on a short timescale. The most plausible cause of these events is considered to be extreme solar proton events (SPE). It is possible that there are other annual 14C excursions in the past that have yet to be confirmed. In order to detect more of these events, we measured the 14C contents in bristlecone pine tree-ring samples during the periods when the rate of 14C increase in the IntCal data is large. We analyzed four periods every other year (2479–2455 BC, 4055–4031 BC, 4465–4441 BC, and 4689–4681 BC), and found no anomalous 14C excursions during these periods. This study confirms that it is important to do continuous measurements to find annual cosmic-ray events at other locations in the tree-ring record.

In 1899, James Keene, a prominent bear, and Roswell Flower, a well-known bull, both attempted to manipulate the share price of Brooklyn Rapid Transit (BRT), a young commuter railway company. Flower and Keene were stock ‘operators’, who used pools of cash from like-minded investors to push share prices higher or lower. In their efforts to garner profits, BRT operators claimed insider status, planted rumors in the press, used leverage to accumulate large positions, manipulated borrowing costs and camouflaged trades. The events of 1899 can shed light on current market dynamics, and we draw parallels between the predatory trading strategies used in 1899 and those of today.

Open science is a new concept for the practice of experimental laboratory-based research, such as drug discovery. The authors have recently gained experience in how to run such projects and here describe some straightforward steps others may wish to take towards more openness in their own research programmes. Existing and inexpensive online tools can solve many challenges, while some psychological barriers to the free sharing of all data and ideas are more substantial.

Metallic glass fluid flow during welding using self-propagating reactive multilayer foils

We use a fluid mechanics model to analyze glass fluid flow during the welding of bulk metallic glasses with reactive multilayer foils acting as local heat sources. The resulting welded joints were shear tested, and fracture surfaces were analyzed by optical microscopy. Fracture surfaces of failed metallic glass joints show distinct regions of metal-metal veins that indicate effective metallurgical bonding. We observe a monotonic increase in the failure strength of the joints with the fraction of the joint composed of such veins. For the strongest joint tested (shear strength of 420 MPa), nearly 60% of the fracture surface is comprised of metal-metal veins. We have developed a qualitative fluid mechanics explanation of the welding process, in which shear stresses (due to pressure applied during joining) push the reactive foil from the joint interface and create the metal-metal veins. The welding process is more effective at higher joining pressure and greater foil thickness, leading to increased joint strength.

We provide a general overview of lek behavior in insects. Initially, we draw a distinction between substrate–based and aerial (swarming) male mating aggregations. In general, males in substrate–based groups defend territories, wait for arriving females, and perform courtship prior to mating. By contrast, males in swarms typically exhibit no intrasexual aggression before female arrivals and grasp approaching females for immediate mating without courtship. Also, compared with swarms, substrate–based aggregations tend to be small, and males are more likely to produce long–range signals to attract females. We examine the relative importance of intra– and intersexual selection in substrate–based groups with (1) male aggression but no courtship, (2) male courtship but no aggression, and (3) both male courtship and aggression. The final category, groups most closely resembling ‘classical’ lek species, receives most attention; data from drosophilid and tephritid fruit flies are presented to show interspecific differences in the influence of male aggression and female choice on male mating success. Few data are available that address the evolutionary origins of lek behavior in insects. Among male–initiated hypotheses, it is most likely that males cluster at transit ‘hotspots’ where large numbers of females are likely to pass. There is no empirical support for the idea that clustering increases signal effectiveness and hence female arrivals on a per male basis. In addition, data regarding predation and group size do not consistently reveal reduced risks with increasing group size.
